The Italian version of ABA2: a new test for the assessment of apraxia of speech.

BACKGROUND: Apraxia of speech (AOS) is a motor speech disorder defined by impaired planning and programming of movements necessary for accurate speech production. Assessment of AOS is typically qualitative, with limited standardized tools available for comprehensive evaluation. OBJECTIVE: The Apraxia Battery for Adults - Second Edition (ABA-2) is a widely used English-language instrument that includes subtests assessing diadochokinetic rate, increasing word length, limb and oral apraxia, latency, and pronunciation time for polysyllabic words and repeated trials, as well as an articulation cha

Using the DIVA model to better understand speech motor disorders.

The Directions Into Velocities of Articulators (DIVA) model provides a comprehensive neurocomputational framework for understanding speech production and speech motor disorders. This theoretical model describes three main control processes: feedforward control, auditory feedback control, and somatosensory feedback control. By mapping these processes to specific brain regions and neural circuits, DIVA offers insights into the mechanistic basis of various speech disorders and provides a foundation for developing targeted therapeutic interventions. This paper reviews the DIVA model's architecture

Multimodal Assessment of Consciousness with Brain-Computer Interfaces and Artificial Intelligence: From Acquired Brain Injury to Neurodegenerative Disease.

The assessment of consciousness has been shaped largely by research on acquired disorders of consciousness after acute or chronic brain injury, but similar problems of unreliable behavioral expression increasingly arise in neurodegenerative disease. This translational overlap is especially relevant when preserved cognition, awareness, or intentionality cannot be reliably expressed because of severe motor impairment, fluctuating arousal, cognitive decline, aphasia, apraxia, or impaired cooperation. In neurodegenerative disease, degeneration of arousal systems, large-scale brain networks, cognit

Cortical and network architectures in limb and oral apraxia.

To characterize the distinct network principles underlying limb and oral apraxias, we investigated the structural brain lesions and white matter disconnections associated with limb apraxia (LA), buccofacial apraxia (BFA), and apraxia of speech (AOS) within a single cohort. In 136 patients with acute left-hemisphere ischemic stroke, LA was assessed using pantomime of tool use and communicative gestures, BFA via non-verbal orofacial movements, and AOS through perceptual evaluation of speech production tasks. We employed voxel-based, region-of-interest-based, and connectome-based lesion-symptom m

Dementia as a rare neurological adverse event of Nivolumab in a patient with metastatic renal cell carcinoma.

BackgroundImmune checkpoint inhibitors (ICIs) have transformed cancer therapy but can cause immune-related adverse events (irAEs). Neurological irAEs are rare, and dementia associated with nivolumab has been rarely reported.Case PresentationA 68-year-old female with metastatic clear cell renal cell carcinoma on nivolumab since 2019 presented with progressive cognitive decline, disorientation, dysarthria, and apraxia. Brain MRI showed no metastasis, only parietal-temporal atrophy and a small meningioma. CSF analysis and paraneoplastic antibody panel were normal. Mini-Mental State Examination (M

Juvenile-onset Huntington's disease: a challenging diagnosis.

The clinical presentation of juvenile-onset Huntington's disease differs from the adult-onset variant which adult neurologists are familiar. We report how we eventually arrived at this diagnosis in a patient whose clinical presentation was marked by numerous confounders and distractors for several years. Only when new clinical signs were elicited, in particular oculomotor apraxia, and a detailed timeline of events was reconstituted, was genetic testing performed to confirm the diagnosis of Huntington's disease. We reflect on several of the difficulties we experienced in securing this diagnosis

Addressing Diagnostic Challenges in Acquired Apraxia of Speech: A Preliminary Study to Inform Curriculum Development.

PURPOSE: Acquired apraxia of speech (AOS) frequently co-occurs with aphasia and/or dysarthria, complicating accurate and reliable diagnosis for speech-language pathologists (SLPs). To address this challenge, this preliminary study explored the diagnostic processes and decision making of SLPs during recorded evaluations of persons with aphasia with potential co-occurring AOS and/or dysarthria, with the ultimate goal of informing the development of a training curriculum to improve diagnostic accuracy. METHOD: Eight licensed SLPs participated and viewed recorded videos of eight individuals with

Against the clock: How time of day influences speech-motor learning in adolescents and young adults with and without childhood apraxia of speech.

The long-term implications of childhood apraxia of speech (CAS) beyond childhood are relatively unknown. This research tracks production performance of a non-native vowel contrast following morning and evening training in adolescents and young adults with and without long-standing CAS. 82 participants aged 12-21 years trained in production of Danish vowels [y] and [ø] in either the morning or evening and completed post-training and 12-hour retention probes. Aligned with previous work, we found that for those with typical speech profiles (TP), offline changes were modulated by time of day of tr

Comparing Traditional Motor Speech Practice to Contextualized Speech Practice in Preschoolers With Childhood Apraxia of Speech.

PURPOSE: The aim of this study was to compare retention of real-word targets across practice conditions (contextualized vs. motor-only) within a modified integral stimulation treatment for preschoolers with childhood apraxia of speech (CAS). METHOD: A single-subject experimental design with alternating treatments was used with matched target sets randomly assigned to contextualized practice, motor-only practice, or no treatment. Three preschoolers with CAS completed 18 therapy sessions, each consisting of two 25-min blocks: one contextualized practice and one motor-only practice. Order of pra

Speech-music therapy for children with Childhood Apraxia of Speech and intellectual disability: A multiple single-subject design study.

Childhood Apraxia of Speech (CAS) is a rare condition in the general population, but it is more prevalent in children with neurodevelopmental disorders. 75% of children with CAS also have intellectual disability (ID), but these children are rarely included in treatment studies. In this study, the effectiveness of Speech-Music Therapy for Aphasia and Apraxia (SMTA) is evaluated in the treatment of children with CAS+ID. SMTA was evaluated in a multiple-single subject design study, comparing 10 weeks of treatment with two months of no treatment in four participants. They varied in the severity of

Progression of language profiles in primary progressive aphasia: A bicentric longitudinal study.

BACKGROUND: Primary progressive aphasias (PPA) are syndromes characterised by a progressive loss of language functions. To date, four main variants have been identified: semantic (svPPA), non-fluent/agrammatic (nfvPPA), logopenic (lvPPA) and Primary Progressive Apraxia of Speech (PPAoS). Currently, little is known about their clinical course, especially in French-speaking populations, though such data could aid prognosis and guide therapy. OBJECTIVE: To characterise longitudinal changes in the language profiles in PPA using the GRÉMOTS, a French language assessment tool specifically developed

Where does the "ἀφημί" (speechless) in nfvPPA come from?: a case study on phonological retrieval and the potent effect of moraic assistance.

Diagnosing non-fluent/agrammatic variant primary progressive aphasia (nfvPPA) relies primarily on apraxia of speech (AOS) and agrammatism. However, differentiating phonetic-level (articulatory) from phonemic-level (phonological) impairments remains clinically challenging, especially in advanced stages. We report a 65-year-old Japanese male with nfvPPA who had become virtually speechless (ἀφημί). Leveraging the Kana writing system's one-to-one correspondence to morae, we designed a cross-modal experiment to isolate his underlying deficit. Speech production was evaluated across four conditions:

Assessment and Diagnostic Standards of Childhood Apraxia of Speech in Turkish-Speaking Children: A Scoping Review.

BACKGROUND: Childhood apraxia of speech (CAS) is a motor speech disorder for which assessment and diagnostic standards have been largely informed by studies of English-speaking populations. Evidence related to CAS in Turkish-speaking children remains sparse, is published across both Turkish- and English-language sources, and variable in diagnostic rigor. AIMS: This scoping review aimed to map existing evidence on the assessment and diagnostic standards of CAS in Turkish-speaking populations, with a focus on diagnostic approaches, assessment tasks, diagnostic quality, reported clinical feature

Does More Speech Therapy Lead to Better Results in Childhood Apraxia of Speech? A Systematic Review and Meta-Analysis of the Available Evidence.

BACKGROUND: In recent years there has been a significant increase in the number and quality of childhood apraxia of speech (CAS) treatment research published. Much of this research has focussed on establishing treatment efficacy and identifying treatment components. A smaller subset of the literature has looked at treatment intensity, such as comparing therapy delivered twice per week with four times per week, however this literature has not been compared across interventions. AIMS: This systematic review and meta-analysis examines cumulative intervention intensity and its components (dose pe

Speech Characteristics of Childhood Apraxia of Speech in Hebrew-Speaking Children.

Purpose: Childhood apraxia of speech (CAS) is a motor speech disorder characterized by deficits in speech planning and programming. However, current diagnostic frameworks have been developed primarily from studies of English-speaking children, raising questions regarding their applicability to other languages. This study examined the common and diagnostic speech characteristics of Hebrew-speaking children with CAS as perceived by Israeli speech-language pathologists (SLPs). Method: Two hundred Israeli SLPs with experience diagnosing and/or treating children with CAS completed an online questio

Artificial intelligence-based speech error detection to differentiate primary progressive aphasia variants.

Artificial intelligence-based approaches to speech analysis have the potential to assist with objective speech error analysis in aphasia but off-the shelf tools often fail to detect speech errors due to prioritizing 'fluent transcription'. Speech production errors (dysfluencies) are hallmark diagnostic features of the nonfluent and logopenic variants of primary progressive aphasia, yet they can be challenging to detect and characterize even by expert clinicians. This study aimed to evaluate whether the novel automated lightweight Scalable Speech Dysfluency Modeling system, specifically designe
