A 68-year-old left-handed woman with a history of hypertension presented with writing difficulties initially affecting numbers and then progressing to include letters. She described an inability to form the shape of numbers and found her hand drifting towards incorrect keys on her phone despite knowing what she wanted to select. Neurological examination revealed fluent but mildly slowed speech without paraphasic errors, intact comprehension and naming, and mild temporal disorientation, where she often stated the wrong year. Her writing was characterized by hesitation, poor spatial planning, an

Reversible splenial lesion syndrome (RESLES), mild encephalitis/encephalopathy with a reversible splenial lesion (MERS), and cytotoxic lesions of the corpus callosum (CLOCCs) are overlapping clinicoradiological constructs associated with infection, seizures, metabolic disturbance, and drug exposure. We report an 11-year-old right-handed boy with recurrent paroxysmal speech arrest over approximately 24 hours. During the episodes, consciousness and comprehension were preserved; he could vocalize and communicate by typing but could not produce meaningful speech. Transient left upper-limb or gener

The characteristics of structural brain topological network alterations and their correlation with clinical features in posterior cortical atrophy (PCA) remain elusive. This study aims to explore the structural topological network alterations and their correlation with clinical features in PCA. Thirty-four patients with PCA and 34 healthy controls were enrolled in this cross-sectional study and underwent diffusion tensor imaging, structural MRI and neuropsychological assessment. The graph theory method was applied to capture the individual structural properties of the network. Partial correlat

Ataxia with oculomotor apraxia type 2 is a rare autosomal recessive cerebellar ataxia caused by senataxin gene mutations, with minimal published data on anaesthetic management. We report the peri-operative care of a 46-year-old woman with genetically confirmed ataxia with oculomotor apraxia type 2 undergoing laparoscopic cholecystectomy under general anaesthesia. Key challenges included uncertain sensitivity to neuromuscular blocking agents and the potential for respiratory weakness and bulbar dysfunction. Non-technical challenges arose from incomplete recognition and communication of the cond

Obstetric brachial plexus injury (OBPI) has long been considered primarily a peripheral nerve disorder. However, recent studies increasingly suggest central nervous system (CNS) involvement, which may play an important role in shaping clinical outcomes and guiding treatment decisions. This review examines the neurophysiological and neuroimaging evidence pointing to CNS alterations in individuals with OBPI, including impaired motor programming, developmental apraxia, altered interhemispheric dynamics, and reorganization of sensorimotor cortical areas. Techniques such as functional magnetic reso

PURPOSE: The aim of this study was to characterize sources of variability in spelling performance in adolescents and young adults with a history of childhood apraxia of speech (CAS), including linguistic skills, motor-speech sequencing, spelling error types, and residual speech errors. We also examined whether selected endophenotypes differ from peers with a history of speech sound disorder without CAS (SSD-no CAS).
METHOD: Correlations assessed relations among single-word decoding, oral language, phonological awareness, syllable sequencing, and spelling in participants with a history of CAS

A 37-year-old woman with a history of antiphospholipid syndrome (APS) developed a progressive visuospatial impairment, including difficulty tying shoelaces and inserting keys. A neuropsychological assessment revealed simultagnosia, optic ataxia, dressing apraxia, alexia, agraphia, and left hemispatial neglect, mimicking the symptoms of posterior cortical atrophy (PCA). Brain MRI revealed occipital-parietal atrophy and FLAIR hyperintensities in the watershed areas, suggestive of chronic ischemia. Cerebral blood flow SPECT revealed bilateral hypoperfusion in the same regions. We report that chro

Joubert Syndrome (JS) is a rare neurodevelopmental disorder characterized by cerebellar ataxia, oculomotor apraxia, and the characteristic "molar tooth sign" on brain MRI. Niemann - Pick Disease Type C (NPC) is an autosomal recessive lysosomal storage disorder associated with progressive neurological involvement, including ataxia and vertical supranuclear gaze palsy. Although these disorders have distinct genetic and pathophysiological mechanisms, they share overlapping clinical features such as ataxia, oculomotor abnormalities, and developmental delay, which may complicate the diagnostic proc

BACKGROUND: Joubert syndrome (JS) is a rare, predominantly autosomal recessive neurodevelopmental disorder characterized by hypotonia, motor delay, intellectual disability, oculomotor apraxia, and the hallmark "molar tooth sign" on axial view of MRI. JS is genetically heterogeneous, with pathogenic variants identified in more than 40 genes involved in primary cilia function. Among these, CEP290 is one of the most frequently mutated genes.
RESULTS: In this study, we investigated two children-an 11-year-old boy (the proband) and his 5-year-old sister-both presenting with a similar phenotype con

Joubert syndrome (JS) is a rare ciliopathy characterised by the molar tooth sign on brain MRI, cerebellar vermis hypoplasia, hypotonia and developmental delay. We report an elementary school-aged boy initially diagnosed with autism spectrum disorder without intellectual impairment because of early language delay, social communication difficulties and stereotyped behaviours. Developmental screening at 4 years of age using the Denver Developmental Screening Test II demonstrated delays in language and social communication domains. At presentation to our clinic at 8 years of age, cognitive evaluat

BACKGROUND: Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis (anti-NMDAR encephalitis) is one of the most common types of autoimmune encephalitis in children, characterized by neuropsychiatric syndromes such as memory deficits, verbal apraxia and complex movement disorders. Evidence indicates thalamic abnormalities in anti-NMDAR encephalitis, yet existing studies have predominantly examined the thalamus only as an undifferentiated whole, particularly within the pediatric population. This study specifically characterized thalamic abnormalities in pediatric anti-NMDAR encephalitis at subn

BACKGROUND: Asymmetric brow ptosis presents a unique surgical challenge. It has been traditionally managed with full deep-plane release of the forehead retaining ligaments, which often leads to overcorrection or persistent asymmetry. This case series evaluates an approach using titrated asymmetric ligament release and selective fixation to address brow asymmetry while minimizing complications.
METHODS: We conducted a retrospective analysis of 50 consecutive patients (average age: 62.1 years) with preoperative brow asymmetry greater than 2 mm who underwent endoscopic brow lift between 2022 and

BACKGROUND: Developmental/Epileptic encephalopathy with spike and wave activation in sleep (DEE-SWAS) comprises a spectrum of childhood-onset epilepsies, characterized by near-continuous spike-wave discharges during non-REM sleep which can result in cognitive, behavioral, and motor regression. Negative myoclonic status in the context of DEE-SWAS is extremely rare, and management is difficult as conventional anti-seizure medications (ASM) and immunomodulatory therapies often show limited efficacy. Adrenocorticotropic hormone (ACTH), classically used for infantile spasms, has recently been repor

INTRODUCTION: Abemaciclib, a cyclin-dependent kinase 4/6 inhibitor that has the ability to penetrate breast tissue and the blood-brain barrier, has been used as an addition to endocrine therapy in breast cancer. Adverse effects like infections, nausea and vomiting, diarrhoea, fatigue, cytopenia (including neutropenia) have been mentioned quite frequently.
CASE PRESENTATION: Here, we present the first report of a possible temporal association in a patient who has been treated with abemaciclib due to an unresectable breast carcinoma and developed cerebral amyloid angiopathy-related inflammation

A 68-year-old female was admitted to our hospital because of a right-sided headache since one year, an inability to perform housework, and a decrease in speech since March of that year. Upon admission, higher brain dysfunctions, including attention disorder, aphasia, and apraxia, were observed. Contrast-enhanced MRI revealed dural thickening in the bilateral frontal to parietal lobes and FLAIR imaging revealed high signal intensities in the bilateral frontal brain sulcus. Blood tests revealed a high IgG4 level (273 mg/dl). Dural biopsy revealed infiltration of IgG4-positive plasma cells into t

A 66-year-old right-handed woman with corticobasal syndrome (CBS) developed a distinctive unilateral hand dystonia, having previously presented with progressive cognitive and behavioural decline. An MR scan of the brain showed bifrontal cortical atrophy with subtle signal changes involving the left caudate nucleus and putamen. Electroencephalography and cerebrospinal fluid analyses were unremarkable. Over subsequent months, the patient developed worsening right-sided rigidity, limb apraxia, myoclonus and alien limb phenomena. During this progression, a characteristic abnormal hand posture beca

Five different homozygous PTRHD1 mutations, two of them in more than one family, have been reported as responsible for intellectual disability (ID) and parkinsonism. In all 10 families, onset of ID was early childhood, and in parkinsonism, later childhood to the fourth decade. We report on a family with four siblings presenting with mild to moderate ID and mildly ataxic gait without spasticity or hemiparesis that is not consistent with parkinsonism. The signs of parkinsonism such as bradykinesia, tremor, slow to response, dementia, and gait problems appeared in the fourth decade. There was no

Hypertrophic olivary degeneration (HOD) is a rare neurological condition due to hypertrophy of the inferior olivary nucleus (ION), usually due to the disruption of the Guillain-Mollaret triangle (GMT). Here, we studied a patient with primary progressive apraxia of speech (PPAOS), a neurodegenerative disease condition of impaired motor speech production, who developed HOD and Parkinsonian features later in her disease course. We examined the patient's disease course and evaluated various clinical, pathological, and neuroimaging variables. The patient developed motor speech problems at the onset

Clinical syndromes of frontotemporal dementia present both distinct and overlapping neuroimaging abnormalities and, consequently, domains of impairment. The aim of this study was to determine how well covariance patterns from multiple neuroimaging modalities discriminate across clinical frontotemporal dementia syndromes. Four-hundred participants with a clinical frontotemporal dementia diagnosis, including behavioural variant of frontotemporal dementia, semantic variant of primary progressive aphasia, right temporal variant of frontotemporal dementia, primary progressive aphasia, apraxia of sp
