Congenital Ocular Motor Apraxia as the First Sign of Joubert Syndrome: A Case Report.

Congenital ocular motor apraxia (COMA) is characterized by an inability to initiate voluntary horizontal saccades, often leading patients to compensate with characteristic head thrusts. While COMA can sometimes present as an isolated ocular finding, it is frequently a manifestation of broader neurodevelopmental disorders. We report the case of a nine-month-old female infant presenting with developmental delay and abnormal eye movements. Examination revealed marked motor developmental delay, hypertonia, truncal arching, and poor head control. Notably, oculomotor apraxia requiring compensatory h

Dystextia due to impairment in spatial operation on a smartphone: A case report.

Smartphone texting has become indispensable in modern life. Although dystextia (texting disability) has been reported, dystextia without aphasia or other cognitive impairment remains rare, and its pathophysiology is not fully understood. In Japanese text entry, the consonant is selected first on the screen, and the vowel is then determined by slide direction. Thus, accurate text input likely requires a sequence of processes, including segmentation of morae into consonants and vowels, followed by the spatial operation corresponding to each. Here, we report a patient with dystextia associated wi

Time to Reconsider the Value of Shunting Procedures for "Idiopathic Normal Pressure Hydrocephalus".

The concept of "normal pressure hydrocephalus" dates back to the description by Hakim and Adams in 1965 of 2 series of patients with enlarged lateral ventricles, high normal cerebral spinal fluid (CSF) pressure and a triad of cognitive impairment, urinary incontinence, and gait apraxia. The validity of this concept is based upon the reversal of its symptoms by a shunting procedure. However, all of these patients had secondary communicating hydrocephalus after previous episodes of meningitis, brain trauma, or subarachnoid hemorrhage. Nevertheless, shunting procedures for "idiopathic normal pres

Cerebrospinal fluid tap test in normal pressure hydrocephalus.

Normal pressure hydrocephalus (NPH) is a common cause of gait apraxia, cognitive impairment and urinary incontinence in older adults and can respond to cerebrospinal fluid (CSF) shunting. It can be challenging to select optimal candidates for CSF shunting given that communicating hydrocephalus is common in the normal ageing population as are the clinical symptoms of NPH. Large-volume lumbar puncture or CSF tap test can identify good candidates for CSF shunting. While neurologists are familiar with and have access to routine lumbar punctures, there are nuances about the CSF tap test for NPH to

Neuropsychological Profile and Cognitive Trajectories of Patients With Biomarker Evidence of Alzheimer Disease or Dementia With Lewy bodies.

BACKGROUND AND OBJECTIVES: Alzheimer disease (AD) and dementia with Lewy bodies (DLB) often coexist, yet the cognitive effect of mixed AD and α-synuclein pathology in vivo remains unclear. We aimed to (1) characterize baseline neuropsychological profiles, (2) assess plasma biomarker associations, and (3) examine longitudinal cognitive trajectories in patients with AD, DLB, and mixed pathology in a memory clinic cohort. METHODS: This was a retrospective longitudinal study including participants from an academic memory clinic cohort with available CSF and blood samples at baseline who completed

Delineating the CTBP1-Related Phenotypic Spectrum: A Review of HADDTS and Atypical Variants.

Hypotonia, Ataxia, Developmental Delay, and Tooth Enamel Defect Syndrome (HADDTS; OMIM #617915) is an ultra-rare autosomal dominant disorder caused by predominantly de novo pathogenic variants in CTBP1, encoding a NAD(H)-dependent transcriptional corepressor. We reviewed all HADDTS cases reported from database inception to July 2026, searching PubMed/MEDLINE, Google Scholar, ClinVar, DECIPHER, OMIM, preprint servers, and the HADDTS Foundation, identifying 25 peer-reviewed cases from at least 11 countries; registries indicate at least 50 known individuals. Rett syndrome: MECP2 biology, multisystem pathophysiology, and the evolving therapeutic landscape.

UNLABELLED: Rett syndrome (RTT) is an X-linked neurodevelopmental disorder arising predominantly from de novo loss-of-function mutations in MECP2, affecting approximately 1 in 10,000 live female births. MECP2 is a dosage-sensitive epigenetic regulator acting through its methyl-CpG-binding (MBD) and transcriptional-repression (TRD) domains; isoform-specific expression and X-chromosome-inactivation mosaicism drive the wide phenotypic variability, with mutation type broadly correlating with severity. Although historically viewed as neuron-centric, RTT is a multisystem disorder: beyond psychomotor

Clinical, biochemical and molecular spectrum of acute neuronopathic type 2 Gaucher disease from India.

BACKGROUND: Gaucher disease (GD) is one of the most common lysosomal storage disorders, and has three clinical subtypes (Type 1, 2 and 3). Of the three subtypes, type 2 GD (GD2), also referred to as acute infantile neuronopathic GD is the most severe type. It is characterized by early and profound central nervous system involvement, and patients succumb to disease before two years of age due to severe neurological deterioration and associated complications. It is the rarest of the three subtypes with a reported proportion of < 1% among all Gaucher subtypes. Furthermore, data from the Indian su

Unravelling the genetic basis of stuttering: GWAS meta-analysis highlights link with rare speech disorders.

BACKGROUND: Developmental stuttering affects up to 11% of children globally, with around one-fifth developing a persistent lifelong stutter. Twin and family studies indicate a strong genetic contribution and comorbidity with other heritable traits. Despite efforts to investigate the common genetic architecture of stuttering, much of variation contributing to clinically ascertained stuttering, persistence and recovery remains uncharacterised. METHODS: We performed a genome-wide association study (GWAS) meta-analysis of stuttering across 18 cohorts (6,096 cases, 81,629 controls) of European anc

A Preliminary Investigation of the Prevalence of Motor Speech Impairment Within a Pediatric Outpatient Clinical Setting.

BACKGROUND: Childhood apraxia of speech (CAS) is a motor speech disorder often co-occurring with language impairment and complex neurodevelopmental disorders. An association between autism spectrum disorder (ASD) and CAS has been suggested but not definitely quantified. The reported prevalence of CAS has been variable. The current study sought to quantify prevalence of apraxia as well as comorbid ASD presentation in a pediatric population in a large outpatient hospital setting. METHOD: An electronic medical record (EMR) was used to identify all pediatric clients between the ages of 24 and 48

Pediatric ocular motor abnormalities: clinical features and neurological correlates.

INTRODUCTION: Pediatric ocular abnormalities may indicate underlying neurological or ophthalmological disorders; however, comprehensive analyses remain limited. This study aimed to investigate associations between specific ocular motor abnormalities and their underlying etiologies. METHODS: We retrospectively reviewed pediatric patients with ocular motor abnormalities presenting to our tertiary referral medical center between 2005 and 2025. They were classified by final diagnosis into the neurologic (N group) and ophthalmologic-only (O group) etiology groups. Associations among ocular signs,

Tardive blepharospasm.

BACKGROUND: Tardive syndromes may present with variable movement disorders. Blepharospasm (BSP), while typically idiopathic in origin, may be drug-induced, but there is little information regarding frequency and phenomenology of tardive BSP. OBJECTIVES AND METHODS: We aimed to assess the clinical features of tardive BSP and compare them with patients with idiopathic BSP. The Abnormal Involuntary Movement Scale (AIMS) was used to assess the severity of movements and the Jankovic Rating Scale (JRS) was used to evaluate the severity of BSP. RESULTS: Among 72 consecutive patients with tardive sy

Qualitative Analysis of Constructional Errors in Neurodegenerative Conditions: A Systematic Review.

Background/Objectives: Constructional apraxia (CA) is an impairment in combining simple elements into coherent spatial configurations without basic motor deficits. Although common in neurodegenerative disorders, the qualitative features of visuo-constructional errors and their role in differentiating dementia types remain unclear. This systematic review aimed to synthesize patterns of visuo-constructional errors in dementia and mild cognitive impairment (MCI), exploring distinctive qualitative features associated with different neurodegenerative conditions. Methods: A systematic literature sea

Progressive Neurodegeneration From Anoxic Injury Following Perioperative Cardiac Arrest.

Severe hypoxic-ischemic brain injury following cardiac arrest can result in persistent and evolving neurologic, cognitive, and visual impairment, with later clinical decline that may be difficult to distinguish from superimposed neurodegenerative disease. A prolonged and evolving pattern of neurologic, cognitive, and visual impairment is documented in a woman throughout her 70s and 80s who survived a perioperative cardiac arrest requiring an extended resuscitation interval. Her recovery was complicated by Anton syndrome, prosopagnosia, gait apraxia, chronic microvascular disease with progressi

Diagnostic value of genetic testing in chorea: a retrospective monocentric study.

BACKGROUND: Chorea is a hyperkinetic movement disorder with a broad differential diagnosis, ranging from acute symptomatic causes to slowly progressive neurogenetic diseases. While Huntington's disease (HD) remains the most prevalent hereditary form, numerous other genetic disorders may mimic its clinical presentation. A major diagnostic challenge arises in patients with a seemingly negative family history, which can obscure the suspicion of a genetic etiology. In patients with sporadic chorea, the potential contribution of genetic testing to the diagnostic process has not yet been systematica

Late-Onset Depression Mimicking a Primary Psychiatric Disorder: Diagnostic Pitfalls in Sporadic Creutzfeldt-Jakob Disease.

Human prion diseases are rare neurodegenerative disorders that typically present with rapidly progressive neurological decline. Sporadic Creutzfeldt-Jakob disease (sCJD) is a fatal neurodegenerative disorder characterized by rapidly progressive encephalopathy. Although classically presenting with cognitive decline and myoclonus, early manifestations may be predominantly psychiatric, particularly in older adults, leading to diagnostic delay. We report the case of a 75-year-old woman with no prior psychiatric history who initially developed late-onset depressive symptoms accompanied by behaviora
