APTX deficiency is associated with altered neuronal differentiation and alternative splicing in patient-derived neurons.

Ataxia with oculomotor apraxia type 1 (AOA1), caused by mutations in the DNA repair protein aprataxin (APTX), leads to progressive neurodegeneration. In this study, we established an AOA1 patient-derived induced pluripotent stem cell (iPSC) and a neuronal differentiation model. We demonstrated that AOA1-derived neurons exhibit neurite morphology and maturation defects correlated with the accumulation of DNA single-strand break (SSB) signals. AOA1-derived neurons showed greater DNA-damage and PAR signals together with lower protein-normalized NAD(H) and ATP after genotoxic exposure. These paral

Lysosomal metabolism in pancreatic cancer and its crosstalk with host physiology.

Transcription factors from the family of Signal Transducers and Activators of Transcription (STAT) are activated by numerous cytokines. Two members of this family, STAT5A and STAT5B (collectively called STAT5), have gained prominence in that they are activated by a wide variety of cytokines such as interleukins, erythropoietin, growth hormone, and prolactin. Furthermore, constitutive STAT5 activation is observed in the majority of leukemias and many solid tumors. Inactivation studies in mice as well as human mutations have provided insight into many of STAT5's functions. Disruption of cytokine

Dressing patterns in healthy individuals and brain-damaged patients: the MIlan Dressing Assessment.

BACKGROUND: The ability to dress appropriately is a fundamental daily living skill that is often impaired by neurological disorders and can manifest as a specific form of apraxia (dressing apraxia). Despite its prevalence and impact on functional independence, standardized and reliable assessment tools for dressing skills remain limited. The aim of this study was to identify dressing patterns in healthy individuals and use them as a reference for introducing a new clinical tool to assess dressing skills. METHODS: We conducted two studies, including seventy healthy participants (mean age = 48.

Lactoferrin concentration in breast milk during the first postpartum year in Lima Norte, Peru.

Bile duct injuries (BDI) tend to be more complex in laparoscopic than in open cholecystectomy procedures, and frequently involve young adults with benign pathologies. The ultimate consequence may be a liver transplantation (LT), making this situation one of the most rare transplant indications. Fatal post-transplant outcome is extreme infrequently reported. Aim of this study is to report on our single-case experience and to review the literature concerning lethal outcome after LT for major BDI following cholecystectomy. A 36-year old obese caucasian woman underwent a laparoscopic cholecystecto

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Increased Serum EphA2 is Associated with Disease Severity in Pediatric Patients with Asthma.

BACKGROUND: Asthma is the most prevalent chronic inflammatory airway disease in children, with increasing incidence and prevalence. Ephrin type-A receptor 2 (EphA2) belongs to the Ephrin (Eph) family. It is predominantly found in bronchial epithelial cells and may play a potential role in mediating airway inflammation in asthma. However, this study aimed to evaluate the association between a novel biomarker, EphA2, in two distinct pediatric asthma populations stratified by disease severity. MATERIALS AND METHODS: Serum levels of interleukins (IL-1β, IL-4, IL-6, IL-8, IL-13), tumor necrosis fa

Cognitive neuropsychological and neuroanatomic predictors of naturalistic action performance in left hemisphere stroke: a retrospective analysis.

Individuals who have experienced a left hemisphere cerebrovascular accident (LCVA) have been shown to make errors in naturalistic action tasks designed to assess the ability to perform everyday activities such as preparing a cup of coffee. Naturalistic action errors in this population are often attributed to limb apraxia, a common deficit in the representation and performance of object-related actions. However, naturalistic action impairments are also observed in right hemisphere stroke and traumatic brain injury, populations infrequently associated with apraxia, and errors across all these po

Clinical-grade intranasal NGF fuels neurological and metabolic functions of Mecp2-deficient mice.

MECP2 deficiency causes a broad spectrum of neuropsychiatric disorders that can affect both genders. Rett syndrome is the most common and is characterized by an apparently normal growth period followed by a regression phase in which patients lose most of their previously acquired skills. After this dramatic period, various symptoms progressively appear, including severe intellectual disability, epilepsy, apraxia, breathing abnormalities and motor deterioration. MECP2 encodes for an epigenetic transcription factor that is particularly abundant in the brain; consequently, several transcriptional

A novel alpha-synuclein G14R missense variant is associated with atypical neuropathological features.

BACKGROUND: Parkinson's disease (PD) affects millions of people worldwide, but only 5-10% of patients suffer from a monogenic form of the disease with Mendelian inheritance. SNCA, the gene encoding for the protein alpha-synuclein (aSyn), was the first to be associated with familial forms of PD and, since then, several missense variants and multiplications of the SNCA gene have been established as rare causes of autosomal dominant forms of PD. AIM AND METHODS: A patient carrying aSyn missense mutation and his family members were studied. We present the clinical features, genetic testing - whol

The clinical and genetic spectrum of paediatric speech and language disorders.

Speech and language disorders are known to have a substantial genetic contribution. Although frequently examined as components of other conditions, research on the genetic basis of linguistic differences as separate phenotypic subgroups has been limited so far. Here, we performed an in-depth characterization of speech and language disorders in 52 143 individuals, reconstructing clinical histories using a large-scale data-mining approach of the electronic medical records from an entire large paediatric healthcare network. The reported frequency of these disorders was the highest between 2 and 5

Cognitive rehabilitation in posterior cortical atrophy.

Posterior cortical atrophy is an uncommon type of dementia often caused by Alzheimer's disease and characterised by progressive loss of visuospatial and perceptual abilities. Although there is no curative treatment, patients may benefit from a range of symptom-based techniques and strategies to address visuospatial deficits and apraxia, and to reduce disability. Specific techniques based on visual and tactile cues, adapted and assistive equipment, environmental modifications and skill training may help people with posterior cortical atrophy continue to carry on activities that are important to

Voluntary Swallowing Initiation Difficulty After Dorsomedial Prefrontal Cortex Damage: A Case Report.

The dorsomedial prefrontal cortex plays a critical role in movement initiation, and damage to this area can impair this function. Here we present the case of an individual who had difficulty with voluntary initiation of liquid swallowing after surgical removal of a glioblastoma from the right dorsomedial prefrontal cortex. This individual had no difficulty swallowing solids, perhaps because of the additional external movement triggers (eg, chewing) involved. Liquid swallowing involves fewer movement triggers and requires a quicker application of force during the oral propulsive phase when liqu

Phonological and speech motor abilities in children with childhood apraxia of speech.

Childhood apraxia of speech (CAS) is a motor speech disorder in which the precision and consistency of speech sounds are impaired due to deficits in motor planning and programming. The literature on CAS suggests that the clinical features of CAS cannot be limited to one level of speech processing and that a more comprehensive understanding of how all levels involved in speech production are part of a complex system is needed. The aim of this study was to investigate the relationship between phonological and speech motor abilities in children with CAS and to determine the extent to which speech

Altered cortical network dynamics during observing and preparing action in patients with corticobasal syndrome.

Corticobasal syndrome (CBS) is characterized not only by parkinsonism but also by higher-order cortical dysfunctions, such as apraxia. However, the electrophysiological mechanisms underlying these symptoms remain poorly understood. To explore the pathophysiology of CBS, we recorded magnetoencephalographic (MEG) data from 17 CBS patients and 20 age-matched controls during an observe-to-imitate task. This task involved observing a tool-use video (action observation), withholding movement upon a Go cue (movement preparation), and subsequently imitating the tool-use action. We analyzed spectral po

The role of the left primary motor cortex in apraxia.

BACKGROUND: Apraxia is a motor-cognitive disorder that primary sensorimotor deficits cannot solely explain. Previous research in stroke patients has focused on damage to the fronto-parietal praxis networks in the left hemisphere (LH) as the cause of apraxic deficits. In contrast, the potential role of the (left) primary motor cortex (M1) has largely been neglected. However, recent brain stimulation and lesion-mapping studies suggest an involvement of left M1 in motor cognitive processes-over and above its role in motor execution. Therefore, this study explored whether the left M1 plays a speci

Word Count Matters: Features of Written Language Production in Progressive Apraxia of Speech with and without Agrammatism.

UNLABELLED: <p>Introduction: Apraxia of speech (AOS) is a motor speech disorder characterized by sound distortions, substitutions, deletions, and additions; slow speech rate; abnormal prosody; and/or segmentation between words and syllables. AOS can result from neurodegeneration, in which case it can be accompanied by the primary agrammatic aphasia (PAA), which when presenting together are called AOS+PAA. AOS can also be the sole manifestation of neurodegeneration, termed primary progressive AOS (PPAOS). Together these form the agrammatic-apractic spectrum disorders. Recent work has shown that

Cortical representation of novel tool use: Understanding the neural basis of mechanical problem solving.

INTRODUCTION: Using tools effectively is a fundamental human ability. Besides the proper recall of semantic knowledge, the application of mechanical problem solving strategies allows one to execute tool-related tasks properly. Past fMRI studies have shown a mainly left-lateralized network, including ventral, ventro-dorsal, and dorso-dorsal streams while using familiar tools with access to semantic information. However, to what degree the network is recruited when applying mechanical problem solving strategies to handle novel tools remains unclear. METHODS: An event-related fMRI study includin

Apraxia Rehabilitation Using Gesture Training and Sequencing Tasks: A Case Report.

This case report describes the effects of combining gesture training and sequencing tasks to improve apraxia and enhance activities of daily living (ADLs) in a patient with left inferior parietal lobule and temporal lobe damage caused by a cardioembolic stroke. The intervention leveraged the patient's preserved semantic knowledge of tools. A male patient in his late 70s presented with significant impairments in gesture execution and action sequencing while retaining semantic knowledge of tool functions. Beginning on day 13 post-onset, the intervention included gesture training with photographs

Monogenic disorders associated with motor speech phenotypes in children and adolescents undergoing clinical exome sequencing.

PURPOSE: Prior studies investigating the genetic architecture of pediatric motor speech disorders (MSDs) have been limited by small sample sizes and an exclusive focus on apraxia. We aimed to identify pathogenic genomic variants associated with MSDs in a large pediatric population referred for exome sequencing (ES). METHODS: We identified pediatric patients with MSDs who had clinical ES between 2012 and 2022. The rate of pathogenic/likely pathogenic (P/LP) findings considered causative of the MSD phenotype was determined and delineated by sex and neurodevelopmental comorbidity. Gene-based bur

aFTLD-U presenting with primary progressive aphasia suggestive of non-fluent type with apraxia of speech.

Atypical frontotemporal lobar degeneration with ubiquitin-positive inclusions (aFTLD-U) is a rare subtype of Frontotemporal Lobar Degeneration. aFTLD-U is reported with a typically young onset behavioral variant of frontotemporal dementia syndrome, more specifically with younger age, predominant hyperorality, obsessive-compulsive features, and severe caudate atrophy. Very few cases have been reported with prominent language impairment with no major behavioral features. We present another such rare case of aFTLD-U presenting with prominent language impairment adding to the phenotypic spectrum,
