Prevalence of Voice Disturbances in the Pediatric Craniofacial Patient Population.

OBJECTIVE: To report on the prevalence of voice disturbances in the pediatric craniofacial population and to prove that the pediatric Voice Handicap Index (pVHI) is a useful questionnaire for this unique population. STUDY DESIGN: Case series with chart review. SETTING: Tertiary care center. SUBJECTS: Pediatric patients seen by a pediatric otolaryngologist in a craniofacial clinic. METHODS: Consecutive patients (N = 366) seen by a single pediatric otolaryngologist in a craniofacial clinic from July 2011 to September 2012 were included. Any patient specifically referred for airway problems o

Application of Linear Discriminant Analysis to the Long-term Averaged Spectra of Simulated Disorders of Oral-Nasal Balance.

OBJECTIVE: Acoustic studies of oral-nasal balance disorders to date have focused on hypernasality. However, in patients with cleft palate, nasal obstruction may also be present, so that hypernasality and hyponasality co-occur. In this study, normal speakers simulated different disorders of oral-nasal balance. Linear discriminant analysis was used to create a tentative diagnostic formula based on the long-term averaged spectra (LTAS) of the speech stimuli. MATERIALS AND METHODS: Eleven female participants were recorded while reading nonnasal and nasal speech stimuli. LTASs of the recordings we

Combined repair of lip and palate with pharyngoplasty for late presenting clefts.

A combined lip and palatal repair with an Orticochea sphincter repair was done in nine patients presenting to our cleft team in Nepal and Tibet for the first time after the age of 8 years. This retrospective study assessed the procedure using the outcome measurements of operative morbidity and postoperative nasality and intelligibility of speech. The total median postoperative hospital stay was 2 days (range 1-4). There were no intraoperative or in-hospital complications. One patient developed a small fistula. Speech results were available in only four of the patients. Nasality was mild in two

Pharyngeal flap surgery: long-term outcomes at the University of Iowa.

The pharyngeal flap is the most often used surgical approach to treat the problem of velopharyngeal insufficiency, a common challenge encountered in cleft palate and craniofacial clinics. The authors retrospectively reviewed short-term and long-term measures of children treated with the pharyngeal flap at the University of Iowa Cleft and Craniofacial Center. All patients who underwent pharyngeal flap surgery between January of 1970 and December of 2000, with at least one postoperative speech assessment between 2 and 5 years after the operation, were identified. Both hypernasality and hyponasal

Evaluation of articulation of Turkish phonemes after removable partial denture application.

In this study, the adaptation of patients to removable partial dentures was evaluated related to articulation of Turkish phonemes. Articulation of /t,d.n,l,r/, /g,k/, /b,p,m/ and /s,z,ş,v,f,y,j,h,c/ phonemes were evaluated by three speech pathologists, on records taken from 15 patients before the insertion of a removable partial denture, just after insertion, and one week later. The test consisted of evaluation of phoneme articulation of independent syllables in terms of distortion, omission, substitution, mass effect, hypernasality and hyponasality. Data were evaluated with Cochrane Q. McNema

Changes in speech following maxillary distraction osteogenesis.

OBJECTIVE: The purpose of this study was to describe changes in articulation and velopharyngeal function following maxillary distraction osteogenesis. DESIGN: This is a descriptive, post hoc clinical report comparing the performance of patients before and after maxillary distraction. The independent variable was maxillary distraction while the dependent variables were resonance, articulation errors, and velopharyngeal function. SETTING: The data were collected at a tertiary health care center in Chicago. PATIENTS: The data from pre- and postoperative evaluations of 18 maxillary distraction

Devising an objective nasal vibration test for nasal resonatory disorders.

The present study investigates the clinical applicability of a new device, which objectively measured nasal resonating vibration via piezoelectric vibratory sensor in 10 normal volunteers, 10 patients with definite hypernasality, and 10 nasal polyposis patients. For the assessment of the hypernasality, the ratio of /ng/ to /a/ as well as the ratio of /mama/ to /papa/ passages were used. For the evaluation of hyponasality, the ratio of nasal vibration postinduced to preinduced cul-de-sac resonation was calculated. The control group showed the ratio of /ng/ to /a/ and /mama/ to /papa/ passages t

Relative values of various standardized passages in the nasometric assessment of patients with velopharyngeal impairment.

A series of 155 patients referred for evaluation at the UNC Craniofacial Center was studied to compare nasometric performance data obtained from the Rainbow Passage to information provided when subjects read two other standardized passages (Zoo Passage and Nasal Sentences). Nasometric findings obtained using the three reading passages were compared to aerodynamic estimates of velopharyngeal area and nasal cross-sectional area as well as clinical judgments of hypernasality and hyponasality. The results suggest that the Rainbow Passage does not provide clinically relevant information that cannot

Comparison of Nasometer and listener judgments of nasality in the assessment of velopharyngeal function after pharyngeal flap surgery.

This study examined the relationship between nasalance scores as derived from the Model 6200 Nasometer and listener judgments of perceived nasality for individuals with pharyngeal flaps. Sixteen individuals with pharyngeal flaps read a speech sample consisting of seven sentences for which associated nasalance scores were obtained. In addition, 10 trained listeners were asked to judge the subjects' audiorecorded speech samples for the degree of both hypernasality and hyponasality using two 6-point scales. The mean judges' ratings of hypernasality did not increase systematically with increasing

Correspondence between nasalance scores and listener judgments of hypernasality and hyponasality.

The relationship between nasalance scores and perceptual judgments of hypernasality and hyponasality was examined for 74 subjects (51 with cleft palate and 23 noncleft controls). Twenty-nine of the 51 subjects with cleft palate had received pharyngeal flap surgery. Predictive analyses were performed to assess the sensitivity, specificity, and efficiency of the Nasometer as a screening instrument. The overall relationship between perceptual judgments of hypernasality and nasalance scores was good for the nonflap subjects when a nasalance cutoff score of 26 was used. A sensitivity coefficient of
