Fever in a patient with mild acute pancreatitis caused by ulinastatin injection: A case report.

RATIONALE: The case describes a patient with mild acute pancreatitis (AP) who developed a persistent high fever during ulinastatin treatment. This highlights the possibility of drug-induced fever, which is a rare but important adverse reaction of ulinastatin and is often overlooked in clinical practice. PATIENT CONCERNS: A 58-year-old male patient with mild AP had a persistent high fever during the treatment with ulinastatin, but there was no evidence of infection. DIAGNOSES: Mild AP, ulinastatin-related drug fever. INTERVENTIONS: The initial conservative treatment for AP included fasting,

Tolerance induction in a drug-induced fever case due to trimethoprim-sulfamethoxazole in a cystic fibrosis patient.

Drug-induced fever is a probably an under/misdiagnosed condition. The onset of drug-induced fever is highly variable among drugs and patients, usually occurring after 7-10 days of treatment, with rapid resolution after discontinuation. However, it sometimes appears at any time during treatment, even after stopping the drug. The estimated prevalence is 10%, and early diagnosis avoids hospitalizations, expensive treatments, and techniques, favoring the sustainability of the Health System. We report a 40-year-old woman diagnosed with Cystic Fibrosis and drug-induced fever due to trimethoprim-sulf

Drug fever induced by Xingnaojing injection: A case report.

RATIONALE: The application of Xingnaojing injection is widespread; however, drug-induced fever associated with this medication is challenging to diagnose. Previous literature rarely reports cases of drug-induced fever resulting from the use of Xingnaojing injection. PATIENT CONCERNS: A 46-year-old male patient, who underwent surgery for cerebral hemorrhage, presented with fever. The condition is considered to be drug fever induced by the administration of Xingnaojing. DIAGNOSES: Considering the potential for drug fever associated with the administration of Xingnaojing injection. INTERVENTIO

Fatal hyperthermia triggered by drug-induced fever in a patient with high-level spinal cord injury: a case report.

We report a rare fatal hyperthermic crisis likely precipitated by drug-induced fever in a muscular, obese patient with chronic high-level spinal cord injury (SCI). Shortly after antibiotic therapy was changed to meropenem, the patient developed severe agitation and hyperthermia exceeding 41 °C, accompanied by autonomic instability. Despite intensive management, including active cooling, intravenous fluids, and vasoactive support, the core temperature remained above 41 °C. Computed tomography revealed a low-density brainstem lesion consistent with infarction. The patient experienced respiratory

β-Lactam-Induced Fever Mimicking Post-caesarean Infection: A Diagnostic Dilemma.

Fever after a caesarean section is often believed to be caused by infections, which usually require an escalation of antimicrobial treatment. Although caesarean delivery can be lifesaving for both mother and baby, the procedure carries risks, with emergency caesarean sections being associated with a higher chance of complications, including infection and haemorrhage, compared to elective caesarean sections.  Drug-induced fever is an important yet often overlooked alternative diagnosis in obstetrics and can be difficult to recognise in patients receiving antibiotics for suspected postoperative

Successful Sequential and Additive Drug Challenge With Rapid Oral Desensitization in Rifampicin-Induced Fever.

Adverse drug reactions to first-line antituberculosis (anti-TB) therapy are common and may necessitate treatment interruption. While drug desensitization is increasingly used for delayed hypersensitivity, evidence supporting outpatient rapid oral desensitization in mild delayed reactions such as drug-induced fever is limited. We report a 63-year-old man with active pulmonary tuberculosis who developed drug-induced fever after commencing standard anti-TB therapy. Sequential and additive drug challenge identified rifampicin as the offending agent. The patient subsequently underwent successful ra

From suspected necrotizing fasciitis to diagnosed drug-induced fever: a diagnostic maze and reflections on anti-infection management in a case of postoperative fever after femoral fracture.

Postoperative fever represents a common clinical challenge in orthopedics, and the differential diagnosis of its etiology directly influences anti-infective decision-making and patient outcomes. In the context of anti-infective therapy, "drug fever" induced by antimicrobial agents themselves can easily be mistaken for uncontrolled or recurrent infection, leading to antibiotic overuse. This article reports the case of a 49-year-old male with a comminuted fracture of the right femur. Preoperatively, he presented with post-traumatic absorption fever. Postoperatively, due to intraoperative observa

Drug-induced fever in post-surgical patients: a systematic review of case reports.

BACKGROUND: Fever is a common postoperative complication, typically caused by aseptic inflammation or infection. However, drug-induced fever (DIF) is an underdiagnosed etiology that should be considered in the differential diagnosis, especially in patients receiving complex medication regimens post-surgery. OBJECTIVES: This systematic review aims to assess the current literature on DIF in post-surgical patients to improve diagnostic accuracy and patient care. DESIGN: Systematic review of case reports and case series. DATA SOURCES AND METHODS: This systematic review was conducted following t

Ciprofloxacin-induced drug fever.

Drug-induced fever is an uncommon but important differential in patients with unexplained fever. We present a case of a woman in her 60s with advanced renal disease and recent total hip arthroplasty who developed fever after an extended course of ciprofloxacin treatment. Extensive investigations, including cultures, imaging and orthopaedic review, failed to identify an infectious source. Although her fever initially resolved by day 2, it recurred on day 19 of treatment. Repeat assessments remained inconclusive. Ciprofloxacin-induced drug fever was suspected, and defervescence occurred within 4

Evaluation of factors associated with drug fever following gemcitabine and nanoparticle albumin-bound paclitaxel treatment for pancreatic cancer.

PURPOSE: Gemcitabine (GEM) + nanoparticle albumin-bound paclitaxel (nab-PTX) is an effective treatment for advanced pancreatic cancer. However, drug fever is a frequently observed adverse effect of GEM, and PTX can also induce symptoms. This study aimed to identify factors for drug fever in the real-world treatment of pancreatic cancer with GEM + nab-PTX. METHODS: We retrospectively assessed patients with pancreatic cancer who received GEM + nab-PTX (n = 386). The primary endpoint was the identification of factor(s) associated with the incidence of drug fever following the first administratio

Suspected Rifampicin-Induced Fever During Periprosthetic Joint Infection Treatment: A Report of 2 Cases.

CASE: Drug-induced fever is a rare adverse reaction to antibiotics. This case report describes 2 cases of suspected rifampicin-induced fever in patients undergoing periprosthetic joint infection treatment. Both patients experienced fever with the absence of inflammatory symptoms and a discrepancy between clinical improvement and persistent fever. Discontinuation of rifampicin led to rapid symptom resolution. CONCLUSION: Drug-induced fever should be considered when fever persists without infectious symptoms and after excluding other causes. Prompt recognition and discontinuation can lead to ra

The Silent Culprit: Drug-Induced Fever Associated With Trimethoprim-Sulfamethoxazole.

Drug-induced fever (DIF) is often misdiagnosed, particularly when it occurs alongside an infection. Its hallmark is the resolution of fever upon drug withdrawal and rapid recurrence upon re-exposure. Although five potential mechanisms have been proposed to explain this phenomenon, they remain poorly understood, making DIF a challenging diagnosis. We report a case of trimethoprim-sulfamethoxazole-induced drug fever, demonstrated by reproducible symptom onset following a drug provocation test, with supportive evidence from positive intradermal and lymphocyte transformation tests. The diagnosis o

Risedronate-Induced Chronic Drug Fever in a Case of Parkinson's Disease.

We present an atypical case of risedronate-induced chronic fever in an 85-year-old woman with Parkinson's disease, with a dosage regimen of 17.5 mg/week. Our patient had been administered an analgesic/antipyretic drug, acetaminophen, at a rate of 600 mg/day for treatment of a vertebral fracture that occurred relatively frequently, which might have masked the fever caused by risedronate. We noted two clinically significant indications. Firstly, blood test results do not necessarily show the cause of risedronate-induced fever, as white blood cell counts and C-reactive protein levels vary. A simp

Record Setting Temperatures in the Setting of SARS-CoV-2 Suggests Dexmedetomidine Drug-Induced Fever: A Case Report.

Patients infected with COVID-19 can develop coinfections or acute respiratory disorder that result in ventilation. Dexmedetomidine is a common medication used to sedate ventilated patients in the intensive care unit and for nonintubated patients prior to a surgical procedure. As a highly selective alpha-2 agonist, dexmedetomidine provides sedation while reducing the need for anxiolytics or opioids. However, previous case reports suggest dexmedetomidine can induce fever in a variety of conditions. The purpose of this case report is to describe a patient who acquired a fever of 42.6°C in the set
