Pharmacological and Non-Pharmacological approaches to manage Tension-Type Headache: a Systematic Review.

INTRODUCTION: Tension-Type Headaches (TTH), particularly in its chronic form, constitutes a persistent pain syndrome that can substantially impair physical, cognitive, emotional, and social functioning. Although this is the most common type of headache, there is a lack of clear consensus on the optimal long-term treatment strategy. OBJECTIVES: A systematic review was conducted to evaluate the evidence for the effectiveness of pharmacological and non-pharmacological treatments in patients diagnosed with episodic or chronic TTH, focusing on improvements in headache pain and related clinical out

Dual Minicraniotomy for Resection of Ruptured Multicompartmental Intraventricular Epidermoid: A Technical Note.

Epidermoid tumors arising within the lateral ventricle are rare and pose unique surgical challenges, particularly when the bulk of the lesion involves both the atrium/body and the temporal horn, as these compartments are difficult to access through a single surgical corridor. We describe a dual minicraniotomy technique that provides orthogonal microsurgical corridors to the lateral ventricle body/trigone and temporal horn, facilitating complete resection of ruptured intraventricular epidermoid tumors. A 15-year-old boy presented with a 6-month history of headache, vomiting, and papilledema, an

Multiple Sclerosis During Pregnancy and the Postpartum Period: Diagnostic and Multidisciplinary Challenges in Two Cases.

BACKGROUND: Pregnancy may complicate the recognition and evaluation of new neurological symptoms suggestive of a central nervous system demyelinating disease. Diagnostic uncertainty may arise when clinical progression, neuroimaging findings, and supportive investigations are incomplete or inconclusive. CASE PRESENTATION: We report two pregnant women with progressive focal neurological symptoms and radiological findings suggestive of a demyelinating disease. Case 1 involved a 27-year-old woman with a history of autoimmune spondyloarthritis who developed dizziness, gait instability, asymmetric

Alterations in the interstitial microenvironment across headache phases and clinical phenotypes in migraine: A multimodal diffusion MRI study.

AimProcesses related to glymphatic function and fluid exchange may contribute to migraine pathophysiology. We used multimodal diffusion MRI to investigate perivascular diffusivity and the interstitial microenvironment in migraine and to assess differences across migraine phases and clinical phenotypes.MethodsWe enrolled 120 migraine participants and 52 healthy controls (HC) who underwent 3 T MRI. Participants were classified as episodic migraine (EM, n = 93) or chronic migraine with medication-overuse headache (CM-MOH, n = 25), and by headache status as migraine with headache (MwH, n = 25) or

Non-aneurysmal perimesencephalic subarachnoid hemorrhage in a middle-aged female possibly triggered by Valsalva-like event and uncontrolled hypertension: a case report.

INTRODUCTION: Non-aneurysmal perimesencephalic subarachnoid hemorrhage (NAPMSAH) is a rare subtype of subarachnoid hemorrhage with a generally benign course and accounts for a small proportion of spontaneous SAH cases. Compared with other SAH populations, patients with this condition are more commonly male and less likely to have a history of smoking or hypertension. CASE PRESENTATION: We report a 58-year-old hypertensive woman who presented with sudden, severe headache and repeated vomiting without focal neurological deficits. She also had a history of intermittent vomiting associated with g

The MIND study: Design, feasibility, and baseline characteristics of a smartphone-based migraine cohort.

OBJECTIVES/BACKGROUND: Our objective was to describe the design, feasibility, and baseline characteristics of the Migraine Impact on Neurocognitive Dynamics study, a 30-day smartphone-based, prospective, decentralized, observational cohort study evaluating high-frequency assessment of cognition and symptoms in adults with migraine. Cognitive symptoms are an important component of migraine burden, but they are difficult to measure using single-visit testing or retrospective questionnaires. Repeated smartphone-based assessments may better capture real-world variability in cognition and symptoms.

Plasma amylin levels in migraine and after erenumab treatment: A REFORM study.

OBJECTIVE: To investigate whether plasma amylin concentrations differ between adults with migraine and healthy controls (HCs) and whether baseline or longitudinal amylin levels are associated with treatment response to erenumab. BACKGROUND: Amylin is a calcitonin family peptide closely related to calcitonin gene-related peptide, with overlapping receptor pharmacology, and has been implicated in migraine. Human provocation studies have shown that pramlintide, an amylin analogue, triggers migraine attacks, but it remains unclear whether plasma amylin distinguishes people with migraine from HCs,

Pediatric Brain Tumors: A Clinicopathological Study and Assessment of Caregiver Quality of Life at a Tertiary Neurosurgical Referral Center in Northern India.

BACKGROUND: Pediatric brain tumors are the leading cause of cancer-related death in children, yet in low- and middle-income countries (LMICs) their outcomes are shaped as much by socio-environmental factors as by tumor biology. The child and caregiver form a single therapeutic unit whose combined trajectory is rarely studied together. We analyzed the clinicopathological profile and short-term surgical outcomes of pediatric brain tumor patients alongside the quality of life (QoL) of their caregivers. MATERIALS AND METHODS: In this prospective observational study, 56 consecutively admitted chil

Catecholamine-Secreting Retroperitoneal Paraganglioma: A Case Report and Literature Review.

INTRODUCTION: Paragangliomas are rare neuroendocrine tumors arising from autonomic paraganglia. These tumors have metastatic potential, and histopathological findings alone cannot reliably predict their biological behavior. Clinical manifestations range from the classic triad of headache, diaphoresis, and palpitations to incidental findings in asymptomatic patients. CASE PRESENTATION: A 75-year-old man was incidentally found to have a 5 cm retroperitoneal mass adjacent to the right renal hilum. Biochemical testing confirmed marked catecholamine hypersecretion, and 18F-FDOPA PET/CT demonstrate

Clinical features and early warning factors of symptomatic pulmonary embolism after total joint arthroplasty: a retrospective cohort study.

BACKGROUND: Symptomatic pulmonary embolism (PE) is a severe and life-threatening complication following total joint arthroplasty (TJA). Despite advances in diagnosis, treatment, and prediction, postoperative PE still occurs. This study aimed to investigate the onset time window, initial clinical presentations, and preoperative comorbidity profiles of patients with symptomatic PE after TJA, thereby providing clinical evidence for early identification and perioperative management of post-TJA PE. METHODS: In this retrospective study, patients who underwent TJA at our institution between January
