The Hemodynamic Trap: Fetal-Type Posterior Cerebral Artery and Occult PICA Aneurysms.

Isolated intraventricular hemorrhage (IVH) is an uncommon presentation of ruptured aneurysms, often misdiagnosed as hypertensive hemorrhage. Distal posterior inferior cerebellar artery (PICA) aneurysms can easily be missed on computed tomography angiography (CTA) due to their small size and skull-base artifacts. A 48-year-old female presented with a thunderclap headache. Initial imaging showed isolated IVH without subarachnoid hemorrhage. A primary CTA was read as negative, noting only a suspected plaque near the foramen magnum. However, multiformat reconstructions revealed a right-sided fetal

Association between central sensitization inventory and taste alterations in patients with burning mouth syndrome: a cross-sectional study.

BACKGROUND: Burning mouth syndrome (BMS) is a chronic condition characterized by a burning sensation in the mouth without observable lesions, lasting over two hours daily for more than three months. In this study, we aimed to examine whether central sensitization inventory (CSI) scores differ between primary and secondary BMS (SBMS), arising from identifiable causes, and to explore their associations with taste thresholds and pain characteristics. METHODS: This study was conducted between February 2022 and September 2025 and included 30 patients with BMS, 30 patients with SBMS, and 30 healthy

The Hidden Trap: Tolosa-Hunt Syndrome Masking Entrapped Great Occipital Nerve-A Case of Headache Surgery.

Headache syndromes may coexist with peripheral nerve entrapment, which can obscure the true pain generator and delay effective treatment. Tolosa-Hunt syndrome is a rare, painful ophthalmoplegia syndrome characterized by unilateral orbital pain and cranial nerve palsy, often showing a marked response to corticosteroids. We report the case of a 56-year-old woman with a long history of migraine who developed recurrent right-sided orbital pain, maxillary pain, and ophthalmoplegia. She was diagnosed with Tolosa-Hunt syndrome by an experienced neurologist after skull magnetic resonance imaging and c

Tolosa-Hunt Syndrome: A Diagnostic Challenge and the Complexities of Long-Term Corticosteroid Management.

Tolosa-Hunt syndrome (THS) is an uncommon cause of painful ophthalmoplegia resulting from idiopathic granulomatous inflammation involving the cavernous sinus, superior orbital fissure, or orbit. Its diagnosis remains challenging because early manifestations may be nonspecific and its clinical and radiological features overlap with several vascular, infectious, inflammatory, and neoplastic conditions. We report the case of a 59-year-old woman with type 2 diabetes mellitus, arterial hypertension, dyslipidemia, and post-thyroidectomy hypothyroidism who initially presented to her family physician

Internal Jugular Compression Syndrome Because of Compression Between the Styloid Process and C1 Transverse Process: Clinical Presentation, Diagnosis, and Management.

BACKGROUND AND OBJECTIVES: Internal jugular vein (IJV) compression at the skull base represents a rare cause of cerebral venous outflow obstruction and may mimic Eagle's syndrome. Unlike classic Eagle's syndrome, this condition results from venous compression between the hypertrophic C1 transverse process and the styloid process, producing symptoms such as pulsatile tinnitus, headache, and cognitive disturbances. We describe our experience highlighting the diagnostic protocol and management strategies. METHODS: We retrospectively reviewed 5 patients with symptomatic IJV compression syndrome.

Migraine comorbidity in epilepsy subtypes: Association with focal versus generalized epilepsy and seizure control-A systematic review and meta-analysis.

Migraine and epilepsy are paroxysmal neurological disorders that frequently co-occur and share common pathophysiological mechanisms. Whether comorbid migraine is associated with epilepsy subtypes (focal vs. generalized) and with seizure control remains poorly understood. This systematic review and meta-analysis aimed to evaluate these associations. This study was conducted in accordance with the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) 2020 guidelines. PubMed and Web of Science databases were searched from inception to April 1, 2026. Studies ascertaining migr

Rare presentation of hydrocephalus occlusus caused by membranous obstruction of the foramina of Magendie and Luschka: a case report.

Fourth ventricle outlet obstruction is a rare cause of obstructive hydrocephalus in children. Although surgical management has been reported, the diagnostic challenges, differential considerations, and therapeutic strategies remain debated. We report a 5-year-old boy with progressive vomiting, severe headache, gait instability, and incontinence. High-resolution magnetic resonance imaging with T2-CISS sequences confirmed a membranous obstruction at the foramina of Magendie and Luschka. The patient underwent microsurgical fenestration via suboccipital craniotomy, with histopathological confirmat

Gut microbial and metabolomic alterations associated with herpetic trigeminal neuralgia: an exploratory multi-omics study.

BACKGROUND: Herpetic trigeminal neuralgia (HTN) is a neuropathic pain disorder caused by varicella-zoster virus invasion of the trigeminal nerve, but its mechanisms remain poorly elucidated. Accumulating evidence implicates gut microbiota dysbiosis and metabolic perturbations in modulating chronic pain via the gut-brain axis, yet their roles in HTN have not been systematically explored. METHODS: Integrated multi-omics analysis was performed on 19 patients with HTN and 18 healthy controls. Fecal metagenomic sequencing characterized gut microbial composition and function. Untargeted metabolomic

A Rare Case of Synchronous Glioblastoma and Central Neurocytoma: An Unreported Occurrence.

Glioblastoma and central neurocytoma are primary central nervous system tumors with distinct radiologic and histopathologic characteristics. This report presents the case of a 39-year-old male with synchronous glioblastoma and central neurocytoma, an exceptionally rare diagnosis. The patient presented with a seven-day history of headache, nausea, and vomiting, prompting neuroimaging that revealed two supratentorial lesions with imaging and pathologic findings that favored independent tumorigenesis. This case highlights the distinctive imaging features of synchronous glioblastoma and central ne

Restlessness in cluster headache: unveiling the role of its key associations and clinical implications from a cohort study.

BACKGROUND: ICHD-3 recognizes restlessness/agitation as an accompanying feature of cluster headache (CH). However, its phenotypic significance within CH remains uncertain. We compared patients with CH with (CHwR) and without restlessness (CHwoR) to characterize the clinical features and symptom burden associated with restlessness. METHODS: We conducted a cross-sectional cohort study of Turkish patients with CH (n = 318) across tertiary neurology clinics, using a standardized clinician-administered questionnaire. We compared patients with CHwR versus CHwoR across clinical domains, applied mult

The cerebellum in migraine: a narrative review of anatomical, imaging, and clinical evidence.

Migraine is usually framed through trigeminovascular pathways and interactions among the brainstem, hypothalamus, and cortex. The cerebellum has received less attention, although human studies place it within circuits that process trigeminal nociception and vestibular signals. This narrative review examines anatomical and experimental work together with imaging, genetic, and clinical studies, giving greatest weight to direct human migraine evidence. Repeated-measures imaging suggests that cerebellar connectivity changes across the migraine cycle, but the cohorts are small. Structural findings

Myelin oligodendrocyte glycoprotein antibody-associated isolated seizures in children: a retrospective single-center case series.

BACKGROUND AND PURPOSE: Isolated seizures represent a distinct, rare, and underrecognized phenotype of myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD). In recent years, documentation of this phenotype has been limited to sporadic reports, with dedicated case series being extremely scarce. This study aimed to describe the clinical characteristics of isolated seizures in pediatric patients with MOGAD. METHODS: We retrospectively reviewed the medical records of pediatric patients diagnosed with MOG antibody-associated isolated seizures between January 2019 and Janua

Potential sphenopalatine ganglion block efficacy as an alternative to epidural bloodpatch for post-dural puncture headache: a monocentric french study.

BACKGROUND: Post-dural puncture headache (PDPH) is a common complication of lumbar puncture caused by cerebrospinal fluid leakage and intracranial hypotension. Epidural blood patch (EBP) is considered the gold standard treatment but remains invasive. Sphenopalatine ganglion block (SPGB), a non-invasive technique, has shown promising results in recent studies, but it was not evaluated as an alternative of EBP. This study aimed to evaluate the effectiveness of SPGB in PDPH by determining whether it could reduce the need for EBP in clinical practice. METHODS: We conducted a prospective monocentr

A foundation-model approach to pediatric headache classification from resting-state fMRI.

Headache is the most common neurological disorder in children and substantially affects quality of life. We investigated whether resting-state functional MRI (rs-fMRI) can support pediatric headache classification using machine learning. We encoded rs-fMRI data using NeuroSTORM, a recent foundation model, and fine-tuned it to distinguish healthy controls from children with headache and subsequently classify headache subtypes. We then compared NeuroSTORM with a standard neuroscience approach that uses functional connectivity (FC) matrices derived from brain activity as predictors. Using 189 rs-
