Headache as an Initial Manifestation of Syphilis Infection in a Young Woman: A Case Report and Literature Review.

The incidence of syphilis is increasing rapidly worldwide and has become a public health threat. Although osseous lesions are commonly observed in congenital or tertiary syphilis, some cases of skull involvement have been reported in earlier stages of the disease. A 22-year-old woman with a history of tension headaches presented with a severe, new-onset headache localized to the forehead. Imaging studies revealed multiple osteolytic lesions confined to the skull, and serological tests confirmed cranial syphilitic osteomyelitis in the early stage of syphilis. The patient's condition improved wi

Community-Acquired Resistant Strain of Klebsiella pneumoniae in an Elderly Hispanic Patient in the United States: Causing Klebsiella-Invasive Syndrome With a Primary Liver Abscess and Antibiotic Resistance.

Klebsiella pneumoniae (K. pneumoniae) commonly causes respiratory and urinary tract infections but can also lead to community-acquired liver abscesses, primarily in Asian populations. This condition is rare in the Americas and Europe. The emergence of sulfhydryl reagent variable extended-spectrum beta-lactamase (SHV ESBL) strains complicates the management of these abscesses. A Hispanic female in her late 60s presented with bloody urine after three weeks of foul odor, dry cough, headache, and blurry vision. A CT scan of the abdomen and pelvis showed a 1 cm right-sided ureteral calculus and a 5

Toxic Shock Syndrome Triggered by Thoracic Drain Insertion: A Pediatric Case Report.

INTRODUCTION: Postoperative Staphylococcus and streptococcal toxic shock syndrome (TSS) are associated with significant morbidity and mortality rates. As a result, surgical awareness is required to recognize and treat TSS appropriately. We report a pediatric case of TSS after thoracentesis for a pneumothorax. CASE PRESENTATION: A 14-year-old boy was diagnosed with a right pneumothorax and underwent thoracentesis with a trocar catheter. After 2 days, the patient developed a fever, headache, vomiting, and diarrhea. No obvious contamination of the drain puncture wound was observed. He was diagno

Community-Acquired Legionnaires' Disease Presenting With Gastrointestinal and Neurological Symptoms Without Respiratory Complaints in an Elderly Male.

Legionnaires' disease, caused by Legionella pneumophila (L. pneumophila), is a recognized but often underappreciated cause of community-acquired pneumonia, especially in elderly patients with multiple comorbidities who may present with atypical and predominantly systemic features. We report the case of a 78-year-old man with a history of chronic obstructive pulmonary disease, chronic kidney disease, non-alcoholic fatty liver disease, and hypertension, who presented to the medical assessment unit with fever, acute confusion, diarrhea, vomiting, and headache but notably without any respiratory c

Concurrent invasive disseminated Nocardia farcinica and Candida infections in a patient undergoing long-term glucocorticoid therapy for autoimmune thrombocytopenia: a case report.

BACKGROUND: Nocardia farcinica is a virulent organism known for its high resistance to many antibiotics and its ability to cause disseminated life-threatening infections, particularly in immunocompromised patients or those undergoing organ transplantation. Candida albicans can cause disseminated candidiasis with a mortality rate ranging from 30% to 60%. Cases involving concurrent disseminated N. farcinica and C. albicans, particularly in patients with autoimmune thrombocytopenia, are extremely rare. The presence of two disseminated pathogens complicates both diagnosis and treatment, creating s

A case report of central nervous system infection by Nocardia nova in a patient with systemic lupus erythematosus treated with steroids.

As a rare opportunistic pathogen of Gram-positive bacillus, Nocardia is capable of entering the human body through the respiratory tract and wounds, leading to disseminated infections in the lungs, skin, or systemically. The majority of patients infected with Nocardia exhibit defective or suppressed immune function. Herein, in this report, we present a case of systemic lupus erythematosus (SLE) complicated by a central nervous system infection caused by Nocardia nova. Specifically, a female patient was admitted to our hospital for treatment with a three-day history of headache, nausea, and vom

SLE complicated with Nocardia farcinica bloodstream infection and disseminated nocardiosis: a case report.

BACKGROUND: Nocardia farcinica, a gram-positive filamentous bacterium, is predominantly found in individuals with compromised immune systems. Bacteremia caused by Nocardia farcinica is relatively rare. CASE PRESENTATION: A 58-year-old woman who was diagnosed with systemic lupus erythematosus (SLE) for more than three months presented with a fever that persisted for three days. Following comprehensive diagnostic evaluations, including antinuclear and anti-dsDNA antibody tests, electrocardiogram, lung CT, MRI, and cultures of blood and sputum, the patient was diagnosed with Nocardia farcinica b

[MYCETOMA].

Sinonasal mycetoma (fungus ball) is a non-invasive, typically unilateral fungal infection affecting one of the paranasal sinuses. It presents with variable clinical symptoms such as facial pain and unilateral nasal discharge or may be discovered incidentally on imaging. Fungus ball, or Mycetoma, is a saprophytic infection of the paranasal sinuses. Usually, it is an isolated disease of one paranasal sinus, most commonly the maxillary sinus. The sinus obstruction and accumulation of fungi, typically Aspergillus Fumigatus causes a chronic infection which can be asymptomatic or associated with hea

Nocardiosis Unmasked: Therapeutic Challenges of Multiple Abscesses in an Immunocompromised Patient.

Nocardiosis has a predilection for affecting the lungs or occurs as a disseminated disease. Cerebral involvement resulting in multiple brain abscesses is rarely reported in 2% of cases. A 30-year-old woman diagnosed with immunocompromised state six months back presented with headache, multiple episodes of seizures, and progressive right hemiparesis for the past three months. She was initially treated with antibiotics, but in view of non-improvement, an early burr-hole aspiration of the abscess resulted in the correct diagnosis. She was then subsequently treated with antimicrobial therapy and s

Case Report: 18 F-FDG PET/CT for FUO Due to Whipple Disease Infection.

We present the case of a 60-year-old man admitted to the hospital presenting with high FUO (fever of unknown origin), strong headache, face erythematous-desquamative cutaneous lesions, long history of abdominal pain, and diffuse myalgia. He was also previously treated with immunosuppressants and currently managed with corticosteroids for a seronegative rheumatic disease. Given the immunocompromised state, an infective etiology was suspected. The patient was referred, as part of standard clinical practice, to perform an 18 F-FDG PET/CT to identify potential causes of FUO.

Trend of Candida species profiles and their susceptibility to various antifungals in Jakarta, Indonesia.

BACKGROUND AND PURPOSE: Pathogenic fungi, including both true and opportunistic pathogens, pose significant health risks, particularly in immunocompromised individuals. Species, such as Candida albicans, Aspergillus fumigatus, and Cryptococcus neoformans, cause fatal infections and frequently develop resistance to conventional antifungal therapies. Limitations of current antifungal medications, such as drug toxicity, resistance development, and environmental concerns, highlight the urgent need for novel therapeutic strategies. Earthworm extracts, particularly those derived from Eisenia fetida,

Disseminated gonococcal infection developing two days after initial eculizumab administration in a patient with neuromyelitis optica spectrum disorder: A case report and literature review.

Disseminated gonococcal infection (DGI) is a rare complication of Neisseria gonorrhoeae infection. Eculizumab, a complement C5 inhibitor used in conditions such as paroxysmal nocturnal hemoglobinuria, increases the risk of invasive Neisseria spp. infections. Although previous reports have described DGI in eculizumab-treated patients, evidence involving individuals with neuromyelitis optica spectrum disorder (NMOSD) is limited, and all cases occurred weeks to months after treatment initiation. In the present case, the patient received a quadrivalent meningococcal vaccine before receiving eculiz
