Computational and Experimental Approaches for Development of Therapeutics Agents against Acute Pain from Heliotropium Indicum L.

Traditionally, the Heliotropium indicum plant has been utilized in indigenous medicine to manage different health issues including asthma, coughs, bronchitis, skin disorders, wounds, and rheumatic pains. The purpose is to measure the comparative characteristics of H. indicum (MEHI) in terms of its pain-relieving properties by in vivo testing on mice and to conduct a quantitative phytochemicals analysis using mass spectrometer to screen out the most active compound through computational investigation. The pain-relieving properties of MEHI were analyzed through the nociceptive model, revealing a

Patient-reported outcomes, postoperative pain and pain relief after day-case surgery (POPPY): baseline data from day surgery practice in the UK.

INTRODUCTION: Most patients undergoing elective surgery in the UK are discharged from hospital on the same day. Despite this, there is a lack of UK patient-centred outcome measures relating to quality of recovery, pain and analgesic use. The POPPY study was a UK-wide prospective, observational study measuring short- and longer-term patient-reported outcomes following day-case surgery. This article is the second in a series and describes baseline characteristics to contextualise successive articles which explore outcome data. METHODS: Baseline data were collected from eligible consenting adult

Role of the dentate gyrus of hippocampus on acute pain modulation: Investigating of dopaminergic-opioidergic interactions in pain-related behaviors in the tail-flick test.

Previous studies have shown that injections of opioid and dopamine agonists alone into the dentate gyrus (DG) increase the threshold for acute pain responses. Therefore, this study aimed to investigate whether the opioid and D1-like dopamine receptor (D1R) interact to modulate acute pain in DG. One hundred and forty-seven adult male Wistar rats were cannulated unilaterally in the DG. Separate groups of animals received different doses of SCH23390 (6, 12, and 24 mmol/0.5 μL), a D1R antagonist, before injection of an effective dose of morphine (25 mmol/0.5 μL). In another experiment, animals rec

MetoksyKval: the extent of pre-hospital methoxyflurane administration for acute traumatic pain: focus on economic impact and rationale for use.

BACKGROUND: Effective pain management is critical in pre-hospital care, yet many patients still receive suboptimal treatment. Methoxyflurane, a handheld self-administered analgesic, is increasingly used in European ambulance services. Its effectiveness and safety are established, but the reasons for its use in acute pre-hospital settings have received limited attention. This study aimed to explore patterns of use, effectiveness, and economic implications of methoxyflurane in traumatic pain, with particular focus on decision-making and patient satisfaction. METHODS: This prospective observatio

Modulatory effects of genetic vs. pharmacological HCN4 channel inhibition on stimuli transmission during acute pain.

Acute pain processing emerges from complex interactions among multiple brain regions, with local ion channels critically shaping neuronal communication. To better understand the role of HCN4 channels during acute pain in mice, a genetic brain-specific HCN4-KO was compared with pharmacological inhibition by the selective HCN4 channel blocker EC18. Stimulus-driven BOLD-fMRI measurements using graded peripheral thermal stimulation allowed brain-wide investigation of both discriminative and suppressive processes within ascending and descending pain pathways. Classical BOLD parameters and graph-the

The nonlinear nature of MEP amplitudes during cold-water immersion.

OBJECTIVE: The objective of this study is to investigate whether motor-evoked potentials (MEPs) amplitudes are associated nonlinearly with cold-water immersion-induced experimental pain in muscle. METHODS: The cortical representation area of the right abductor pollicis brevis muscle of 10 subjects was stimulated using a multi-locus transcranial magnetic stimulation device, and the resulting MEPs were recorded with electromyography. The stimulation was delivered with and without the cold-water immersion. A generalized additive model (GAM) was applied to study the association of different param

Symptomatic chikungunya and chronic post-infection arthralgia in a highly endemic setting in Northeastern Brazil, 2018-2019: Clinical characteristics, prevalence and associated factors.

Chikungunya, an Aedes-borne disease, poses a significant global health threat due to its substantial morbidity. The prevalence of symptomatic chikungunya virus (CHIKV) infection and chronic arthralgia, as well as their associated factors, vary geographically and across studies. We estimated the prevalence of these outcomes in a household-based survey conducted in a large northeastern Brazilian city (2018-2019) approximately three years after the city's first CHIKV outbreak (2016). Sociodemographic and clinical data were collected through interviews, and arboviruses serostatus (IgG and/or IgM)

Dehydrocorybulbine blocks the Lyn-mediated MAPK pathway to promote anti-inflammatory polarization of microglia in mice after sevoflurane anesthesia.

Dehydrocorybulbine (DHCB) has demonstrated efficacy in alleviating thermally induced acute pain. The present study sought to study the impact of DHCB on postoperative cognitive dysfunction (POCD). Mice or mouse BV2 cells were exposed to sevoflurane for modeling. Behavioral tests (Morris water maze, Y-maze, and novel object recognition test) and western blot analysis of APP, p-Tau (Thr231), and Tau protein expression in mouse hippocampal tissues were conducted to analyze cognitive impairment. DHCB inhibited M1 polarization of BV2 cells and mediated anti-inflammatory M2 polarization, further all

Decreasing Door-to-Analgesia Time: Increasing Use of Sickle Cell Pain Management Pathway.

INTRODUCTION: Individuals living with sickle cell disease often experience episodes of acute pain, prompting them to seek care in the emergency department. These patients frequently encounter prolonged wait times and invasive procedures before receiving analgesia. This evidence-based quality improvement project aimed to enhance the timely management of sickle cell-related pain by increasing adherence to an existing pain management pathway. The initiative focuses on promoting the use of intranasal fentaNYL as a first-line analgesic. METHODS: To promote consistent use of the established pathway

Feasibility, efficacy, and safety of virtual reality in reducing pain and anxiety during oocyte retrieval: a randomized controlled trial.

BACKGROUND: Transvaginal ultrasound-guided oocyte retrieval (TVOR) is a critical and essential step in in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI) for the treatment of infertility. However, it is generally considered an uncomfortable and painful procedure. Recent studies have demonstrated the effectiveness of virtual reality (VR) in reducing pain intensity in various acute pain scenarios, including procedural pain. This study aims to evaluate the efficacy and safety of VR interventions in alleviating pain and anxiety among women undergoing TVOR. METHODS: A parallel

Cytokines, Somatosensory Functioning, and Psychological Distress in Post-Breast Cancer Surgery Pain: A longitudinal study.

Approximately 40% of breast cancer (BC) patients experience pain during a curative treatment episode. While the biological impact of surgery and adjuvant therapies is central to pain onset, patients' pain intensity and persistence are further shaped by its interplay with psychosocial factors. Here we explored cytokine levels, somatosensory functioning, and psychological distress as biomarkers for pain in BC patients at one and three months post-surgery. Specifically, we compared these factors in BC patients with and without pain at both time points and examined their associations with pain int

Transdermal Initiation of Buprenorphine Among Hospitalized Persons: A Retrospective Study.

BACKGROUND: Buprenorphine is an effective treatment for opioid use disorder but may be difficult to initiate among hospitalized patients due to concerns about acute pain and withdrawal risks. OBJECTIVE: We sought to evaluate the safety and effectiveness of a novel approach for initiating buprenorphine treatment that did not require patients to first discontinue full-agonist opioids or experience an acute opioid withdrawal syndrome. This approach, referred to as low-dose initiation with opioid continuation (LDI), involved applying a transdermal patch which delivered microgram-level doses of bu

Protectin DX resolves fracture-induced postoperative pain in mice via neuronal signaling and GPR37-activated macrophage efferocytosis.

Protectin DX (PDX) is a member of the superfamily of specialized proresolving mediators and exerts anti-inflammatory actions in animal models; however, its signaling mechanism remains unclear. Here, we demonstrate the analgesic actions of PDX in a mouse model of tibial fracture-induced postoperative pain (fPOP). Intravenous early- and late-phase treatment of PDX (100 ng/mouse) effectively alleviated fPOP. Compared with protectin D1 (PD1)/neuroprotectin D1, DHA, steroids, and meloxicam, PDX provided superior pain relief. While dexamethasone and meloxicam prolonged fPOP, PDX shortened the pain d

Study protocols for the GEPard project: systematic analysis of sex- and gender-specific differences in the efficacy of perioperative pain management - a two-pronged approach.

INTRODUCTION: Postoperative pain is common, with approximately one-third of surgical patients experiencing severe acute pain and 10-20% developing chronic post-surgical pain (CPSP). Evidence shows that female patients are at higher risk of pain after sex non-specific surgery, thus sex- or gender-specific differences in pain treatment efficacy with potential consequences for perioperative pain management are to be expected. Considering the clinical and societal burden of poorly managed postoperative pain, the GEPard project comprises two systematic reviews, GEPard 1: sex- and/or gender-specific

Effects of peripheral nerve block adjuvants with and without local anesthetic on ex vivo rodent nerve A-alpha/beta, A-delta, and C fiber activity.

INTRODUCTION: Preclinical and clinical studies have shown enhanced analgesia when some adjuvant medications are combined with local anesthetics during peripheral nerve (PN) blockade. However, the effects of many adjuvants on individual PN fibers, that is, A-alpha/beta, A-delta, and C, remain poorly characterized. METHODS: Using ex vivo rat sciatic nerve electrophysiology, changes in action potential amplitude (AMP) and area under the curve (AUC) for A-alpha/beta, A-delta, and C fibers were used to determine the half-maximal effective concentrations (EC50) for four adjuvants (dexmedetomidine,

Dispositional factors affecting the analgesic response to imagery and mindfulness suggestions during hypnosis.

Medical hypnosis combined with mindfulness suggestions may improve pain management, but individual benefits are difficult to predict. This study compared the effects of hypnotic induction suggestions and different hypnotic suggestions for analgesia on acute experimental pain, considering hypnotic susceptibility (SHSS:A) and five mindfulness facets (FFMQ). Thirty-four healthy participants were administered phasic electrical stimulations during a Baseline condition, a Hypnotic induction alone condition, and experimental hypnosis conditions involving Imagery and Mindfulness-like suggestions of an

Opioid-sparing by low-dose ketamine for endometriosis surgery. A single-center, prospective, randomized, trial.

Endometriosis affects 10% of women of childbearing age and 25-50% of infertile women worldwide. Although most patients with endometriosis are completely asymptomatic, there is still a significant proportion of patients, whose quality of life is impaired by the disease, resulting in chronic pelvic pain. However, there is a poor correlation between objective findings of significant endometriosis and pain severity. Opioids typically form part of a postoperative analgesic regimen. However, opioid tolerance, adverse effects, contraindications, or a combination of these factors may limit their use.
