A Phase III triple-blind randomised placebo-controlled trial to evaluate effectiveness of early treatment with pregabalin to reduce pain severity after whiplash road traffic injury: study protocol for the PRioRTI Trial {1}.

BACKGROUND: There are few effective treatments for acute whiplash-associated disorder (WAD) following a road traffic crash. Early clinical features of central sensitisation, for example widespread hyperalgesia, predict poor recovery. Pregabalin's effects on central sensitisation suggest the potential to prevent or modulate these processes after whiplash injury and prevent later chronic pain. Preliminary evidence indicates that pregabalin provided in the Emergency Department and within 96 hours of injury shows promise to prevent chronic pain. This trial aims to definitively evaluate, in patient

Reliability and validity of Japanese version of Neonatal Pain, Agitation, and Sedation Scale for acute pain in infants.

BACKGROUND: To assess the clinical validity and reliability of the Neonatal Pain, Agitation, and Sedation Scale (N-PASS) for acute pain in infants with illnesses treated in the neonatal intensive care unit (NICU). METHODS: Data of 32 infants were collected. NICU nurses measured pain and distress scores using the Japanese version of the N-PASS (henceforth, N-PASS-J) at baseline and during skin puncture. To determine internal consistency, discriminant validity, and convergent validity, we calculated Cronbach's alpha, conducted a one-way repeated measures ANOVA, and examined correlation coeffici

Simulation Topics for Palliative Medicine Specialty Training: A Two-Phase Needs Assessment for Curriculum Development.

BACKGROUND: Simulation-based learning is increasingly integrated into medical education; however, the use of simulation in palliative medicine is generally limited to ad hoc communication skills and pain management. There is a recognized need for a structured approach to a simulation-based curriculum aligned with the unique challenges in complex symptom management and emergencies in palliative medicine specialty training. OBJECTIVES: To identify and prioritize key clinical topics for a simulation curriculum for palliative medicine specialty training. METHODS: Using Kern's six-step approach t

Perioperative Management of Patients on Buprenorphine.

PURPOSE OF THE REVIEW: Our review discusses the pharmacologic profile of buprenorphine, then dives into the current literature regarding buprenorphine's use to treat chronic pain, opioid use disorder, and acute postoperative pain. We aim to focus on how that literature may influence the perioperative management of patients on buprenorphine and prompt reconsideration of buprenorphine as a frontline opioid analgesic. RECENT FINDINGS: Buprenorphine has been used effectively for both acute and chronic pain management, for the treatment of opioid withdrawal, and for the treatment of opioid use dis

Gaining the Upper Hand? Further Evidence of Pain as a Pleasurable Experience and the Unexpected Relationship Between Sadomasochistic Sexual Preference and Chronic Pain.

BACKGROUND: A growing body of research reports positive consequences of acute pain, including emotional self-regulation. Pain as a pleasurable experience has also been regarded in the context of sadomasochistic (SM) interest, albeit quantitative empirical evidence is scarce. Recently, an elevated prevalence of chronic pain (CP) has been reported in SM practitioners; however, in absence of a control group. To contribute to research in the field, we strove to see whether we could replicate the finding and further aimed to identify psychological predictors of SM. METHOD: A total of 617 participa

Understanding the gender pain gap affecting women in healthcare.

Pain is a significant issue for women accessing healthcare services in the UK, with reports showing that they are disproportionally affected by chronic pain compared with men. Not only are women more likely to experience pain than men, but their pain is also often under-managed. This article examines the gender pain gap, which can result in acute and chronic pain in women being treated differently than in men, and describes the physical and psychological effects this can have on individuals. The author also discusses the limited evidence which currently exists regarding the gender pain gap in

Gabapentin as an adjunct to multimodal pain regimens in surgical patients: the GAP placebo-controlled RCT and economic evaluation.

BACKGROUND: Gabapentin is an anticonvulsant medication with a United Kingdom licence to treat partial seizures and neuropathic pain. It is used off-licence for acute pain and is frequently added to multimodal analgesic regimens after surgery to try and reduce opioid use while controlling pain effectively. OBJECTIVE: To test the hypothesis that gabapentin reduces opioid use after major surgery and speeds up recovery, thereby reducing postoperative hospital length of stay compared to standard multimodal analgesia. DESIGN, SETTING AND PARTICIPANTS: The GAP study was a multicentre, blinded, rand

Enhancing anterior shoulder analgesia with a modified PECS II block during shoulder arthroscopy under regional anesthesia alone: a translational anatomical and clinical feasibility study.

BACKGROUND: Regional anesthesia (RA) alone offers important perioperative advantages for shoulder arthroscopy, but conventional interscalene brachial (ISB) and superficial cervical plexus (SCP) blocks may insufficiently cover the anterior shoulder region-especially during biceps, rotator interval, or anterior capsular procedures. This gap likely reflects sensory input from intercostobrachial and pectoral nerves, which are not reliably anesthetized with standard techniques. We therefore hypothesized that a cranially directed modified pectoserratus plane block (PECS II) (second-third intercostal

Strategies for a Rational Use of Opioids in Critical Care Settings.

Opioids play a central role in pain management and sedation in Intensive Care Units (ICUs), where critically ill patients frequently experience moderate-to-severe pain due to illness and invasive procedures or devices. Uncontrolled pain exacerbates stress responses, contributing to clinical deterioration and adverse outcomes. Although analgesics and sedatives can mitigate these effects, their use must be carefully individualized to avoid complications such as delirium, prolonged mechanical ventilation, and increased mortality. Evidence now shows that excessive or poorly controlled analgosedati

Central Sensitisation After Orthopaedic Trauma: An Overlooked Contributor to Chronic Pain and Functional Disability-A Scoping Review.

Background: Persistent pain following orthopaedic trauma is common, often disproportionate to structural healing, and increasingly interpreted as reflecting centrally mediated pain mechanisms. However, the mechanisms, clinical features, diagnostic approaches, prognostic indicators, and management strategies relevant to trauma-related central sensitisation (CS) remain poorly understood. Objective: To map and synthesise existing evidence on CS following orthopaedic trauma, addressing mechanistic pathways, clinical manifestations, epidemiology, assessment methods, management approaches, and healt

Emerging technologies in acute pain management.

Acute pain is a prevalent clinical concern that impacts millions worldwide and often initiates opioid use. Despite advances in multimodal analgesia, challenges such as opioid dependency, under-treatment, and patient heterogeneity persist. This narrative review aims to present recent evidence on emerging technologies in acute pain management, covering innovations in diagnostics, therapeutics, and drug delivery systems. We searched PubMed and Embase databases and included studies from 2015 to 2025 that presented positive evidence of the investigation of these technologies. The review identifies

Thalamocortical and corticostriatal pathways in the progression from acute to chronic musculoskeletal pain: An fMRI study.

Most people experience acute pain as a temporary condition, while a small subset develops chronic pain. The role of pain-related circuits driving this transition remains unclear. Using UK Biobank data and an independent dataset (OpenPain), we analyzed MRI scans of participants with acute musculoskeletal pain (n=160), categorizing them based on those who recovered (AMPR) and those who developed chronic pain (CMPO) later. A machine learning model was applied to predict follow-up outcomes in two independent validation cohorts. AMPR participants showed increased functional connectivity (FC) betwee

The Effects of Lidocaine Patches on Post-Radiofrequency Ablation Pain in Patients With Hepatocellular Carcinoma.

BACKGROUND AND AIM: Radiofrequency ablation (RFA) is a standard treatment for patients with early-stage hepatocellular carcinoma (HCC). However, analgesics are often insufficient in mitigating post-RFA pain. The lidocaine patch is being investigated as an alternative for acute pain relief, though it has not yet been used for post-RFA pain. This study aims to evaluate the effect of lidocaine patches on post-RFA pain for patients with HCC. METHODS: Patients undergoing RFA treatment were randomly assigned to an intervention group, receiving lidocaine patches (LP group), or a control group, recei

A narrative review on fascial plane blocks - Part B: Clinical applications, practical considerations, and future directions.

Fascial plane blocks (FPBs) have rapidly expanded from regional anaesthesia adjuncts to versatile analgesic tools across perioperative, emergency, and chronic pain settings. Building upon the anatomical and mechanistic foundations outlined in Part A, this Part B translates theory into practice by presenting a region-wise, surgery specific, and scenario-focused synthesis of FPB applications. We outline evidence based block selection for thoracic, abdominal, hip, pelvic, and limb procedures, while integrating their roles in trauma care, rib and clavicle fractures, and acute pain management. Association between tailored prescriber training and buprenorphine treatment for opioid use disorder among emergency medicine clinicians.

BACKGROUND: Buprenorphine, despite effectiveness as a medication for opioid use disorder (MOUD), remains underutilized in emergency departments (EDs), with few scalable, statewide programs to improve prescribing. This study evaluated whether a tailored, emergency medicine-specific training offered by the Overdose Prevention Engagement Network (OPEN) across Michigan increased buprenorphine prescribing. METHODS: This pre-post cohort study compared 9-month periods before vs. after training using IQVIA dispensing data linked by National Provider Identifier (NPI) to participating clinicians. Eligi

The association between pain and diagnosis-specific long-term sickness absence at different levels of body mass index - a register-linked cohort study.

BACKGROUND: Chronic (≥ 3 months) pain and obesity (body mass index [BMI] ≥ 30 kg/m2) are both associated with long-term sickness absence (LTSA). This study aimed to examine the contribution of acute (< 3 months) and chronic pain to total and diagnosis-specific LTSA (> 10 working days) at different levels of BMI among young and early midlife employees. METHODS: Helsinki Health Study questionnaire data covering 19–39-year-old employees were collected in 2017 (n = 4091). Pain was classified as no pain, acute pain, or chronic pain. BMI was classified as healthy weight (BMI 18.5–24.9 kg/m2), overwe

Suzetrigine, a novel nonopioid analgesic for acute pain.

PURPOSE OF REVIEW: The clinical utility of many analgesics is limited by their nonselective action, which yields desired analgesia at the cost of significant off-target effects across multiple organ systems. Developing non-opioid therapies that act selectively on peripheral nociceptive pathways remains a key goal for achieving effective and safer pain management. RECENT FINDINGS: Suzetrigine, a highly selective inhibitor of the voltage-gated sodium channel subtype 1.8 (NaV1.8), represents a novel class of nonopioid analgesics that recently granted the Food and Drug Administration approval for
