BACKGROUND: Trauma accounts for over 60,000 deaths annually in South Africa and is also responsible for a high proportion of emergency centre (EC) visits. Up to 91% of trauma patients in the EC experience acute pain, underscoring a critical public health concern, particularly in low and middle-income countries (LMICS), where research indicates poor pain management and a paucity of data. This study aimed to describe trauma pain assessment and management practices in a busy Cape Town EC.
METHODS: This single-centre retrospective chart review in a high trauma burden EC collected a convenience sa

Ruptured hemorrhagic ovarian cyst is a potentially life-threatening condition that can lead to significant hemoperitoneum and hypovolemic shock. Imaging plays a crucial role in rapid diagnosis and guiding management. We report the case of a 19-year-old female who presented to the emergency department with acute pain in the lower left quadrant. Ultrasonography and computed tomography revealed a left hemorrhagic cyst with associated moderate hemoperitoneum. The patient underwent laparoscopic cyst wall removal and evacuation of hemoperitoneum, with subsequent uneventful recovery. This case highli

BACKGROUND: Post-procedural pain following percutaneous coronary intervention (PCI) remains a significant clinical challenge, affecting 5% of patients and potentially impairing recovery and quality of life. While pharmacological interventions are commonly used, their side effects highlight the need for non-invasive alternatives. Recent studies demonstrate VR's efficacy in acute pain settings, yet its application in post-PCI pain management remains underexplored.
OBJECTIVES: This study evaluated the efficacy of VR on pain management in patients undergoing PCI. As a secondary objective, we asse

BACKGROUND: Punch excision is gaining recognition in keloid management. Its distinct mechanism from conventional surgery necessitates a re-evaluation of the efficacy and safety when combined with radiotherapy (RT), an area currently lacking evidence.
AIMS: To investigate the clinical efficacy and safety of punch excision combined with RT for treating trunk keloids and to compare the differences between perioperative (RT initiated one day before surgery) and postoperative RT (RT within 24 h after surgery) regimens.
METHODS: A retrospective study was conducted on patients with trunk keloids tr

The aim was to investigate the independent risk factors for acute pain after total knee arthroplasty, and to build a nomogram prediction model accordingly. Data were collected from total knee replacement patients in our hospital from June 2022 to December 2023, and independent risk factors for acute pain after total knee replacement were identified using univariate and multivariate logistic regression analyses, and the corresponding nomograms were established. The performance of the model was evaluated by plotting the working characteristic curves of the subjects and calculating the area under

Hospitalists care for patients with acute pain who have an opioid use disorder and require pain management. Hospitalists may withhold opioids for acute pain due to a variety of reasons. However, adequate pain management in patients with opioid use disorder, including with opioids, is the standard of care. This piece explains how to best manage acute pain in hospitalized patients with opioid use disorder with supportive evidence and discharge planning considerations.

In this letter, the authors evaluate the potential of suzetrigine, a newly U.S. Food and Drug Administration-approved selective NaV1.8 sodium channel inhibitor, as a novel nonopioid alternative for managing moderate-to-severe pain. By targeting peripheral sensory neurons and avoiding the central nervous system, suzetrigine offers a favorable safety profile that bypasses typical opioid-related risks such as respiratory depression, sedation, and tolerance. While clinical trials demonstrate its noninferiority to hydrocodone/acetaminophen in acute postsurgical settings, the authors highlight signi

Specialized cutaneous Schwann cells (scSCs) are a recently identified glial class implicated in cutaneous pain modulation, yet their three-dimensional architecture and role in chronic pain remain unclear. Using tissue optical clearing, we reconstructed the 3D morphology of scSCs, revealing an intricate mesh-like network, with extensive branching penetrating the epidermal layer and establishing close associations with A- and C-fiber primary sensory nerve terminals. Optogenetic activation of scSCs elicited nociceptive reflex behaviors, dependent on concurrent A- and C-fiber activation, but not a

BACKGROUND AND IMPORTANCE: The high prevalence of acute pain in the emergency department (ED) underscores the need for accurate pain assessment to guide effective interventions. A pain assessment tool for ED patients with acute pain should enable clinicians to distinguish those who require analgesia from those who do not. However, a universally accepted definition of the 'need for analgesia' is lacking.
OBJECTIVE: To identify predictors of the need for analgesia in ED patients through a consensus-based interdisciplinary approach.
DESIGN/SETTING AND PARTICIPANTS: A three-stage modified Delphi
