Effect of bupivacaine combined with morphine intrathecal injection on postoperative recovery quality in patients undergoing pulmonary surgery: a study protocol for a multicentre, randomised, double-blind, controlled trial.

INTRODUCTION: Acute pain following pulmonary surgery can affect the recovery process of patients. The use of intrathecal morphine (ITM) injections offers a long-lasting analgesic effect, but its clinical application remains controversial. This study aims to investigate the impact of combining bupivacaine with ITM injections on the quality of postoperative recovery in patients who have undergone pulmonary surgery. METHODS AND DESIGN: This multicentre, randomised, double-blind, controlled trial will enrol 254 patients undergoing elective lung surgery, who will be randomly assigned in a 1:1 rati

Can Machines Identify Pain Effects? A Machine Learning Proof of Concept to Identify EMG Pain Signature.

This study introduces a machine-learning-based approach for identifying "pain signatures" using electromyography data from volunteers undergoing acute pain. Leveraging the XGBoost algorithm, our method analyzes electromyography features (variance, mean absolute deviation, integral, peak, and entropy) to classify muscle contractions as painful or non-painful. Fifteen participants performed controlled elbow flexion tasks under three conditions: during painful and painless conditions. The results revealed that electromyographic peak and integral activity were key predictors of pain states, with t

Effect of Vitamin D3 on Transected and Crushed Injuries in Rat Sciatic Nerve Healing.

Background: Peripheral nerve injury can happen for a variety of causes. Despite major breakthroughs in microsurgery, nerve repair results are not always sufficient. Methods: Thirty-two Wistar albino rats were split into four groups: primary nerve repair (PNR), PNR with vitamin D3 treatment, nerve crush injury (NCI), and NCI with vitamin D3 treatment. In the PNR + D3 and NCI + D3 groups, 1 mcg/kg of vitamin D3 was given intraperitoneally on days 1, 3, 5, and 7 of the 12-week healing period. Electrophysiological measurements were taken prior to the injury. At 12 weeks after damage, a hot plate t

Therapeutic Use of Cannabis and Cannabinoids: A Review.

IMPORTANCE: Approximately 27% of adults in the US and Canada report having ever used cannabis for medical purposes. An estimated 10.5% of the US population reports using cannabidiol (CBD), a chemical compound extracted from cannabis that does not have psychoactive effects, for therapeutic purposes. OBSERVATIONS: Conditions for which cannabinoids have approval from the US Food and Drug Administration include HIV/AIDS-related anorexia, chemotherapy-induced nausea and vomiting, and certain pediatric seizure disorders. A meta-analysis of randomized clinical trials reported a small but significant

Targeting TRPA1 with Novel Synthetic Compounds Based on Different Scaffolds to Reduce Acute and Chronic Pain.

The transient receptor potential ankyrin 1 (TRPA1) channel is a nonselective cation channel that detects noxious stimuli. Due to its role in acute and chronic pain transmission, interest in this receptor as a potential therapeutic target has grown. Among the natural compounds tested, δ-sanshool proved to be a promising modulator of TRPA1 due to its interaction with specific receptor cysteines. Starting from this polyunsaturated amide, we designed and prepared a small library of derivatives in which different amide heads were introduced and the length of the unsaturated chain was changed. The n

Laparoscopic versus open repair for paediatric inguinal hernia.

RATIONALE: Inguinal hernia is one of the most prevalent paediatric conditions requiring surgical treatment. It can be repaired either by the laparoscopic technique or open surgery. There is a need for a high-quality systematic review with meta-analyses to evaluate the laparoscopic approach versus the open approach for inguinal hernia repair in children, as available evidence is based on different interpretations or calculations of the same RCTs. OBJECTIVES: To compare the benefits and harms of laparoscopic versus open repair in paediatric inguinal hernia. SEARCH METHODS: We searched CENTRAL,

Neurophysiological potentials mediating the influence of social support on pain.

The supportive presence of a significant other can act as an efficient analgesic. However, neurophysiological mechanisms of this effect have not been adequately explored. The current EEG study, using electrical pain stimulation, explored neurophysiological mechanisms mediating the influence of romantic partner's presence on acute pain perceptions. Female participants (N = 41, mean age = 27.44, SD = 7.93) were exposed to painful stimuli in two alternating conditions: a) while receiving support from a male romantic partner via a live camera interaction and b) in the absence of partner support. I

Relationship between cortical electrical responsiveness and changes in regional cerebral oxygenation (rSO2) and return of spontaneous circulation in prolonged cardiac arrest: a multi-center observational study.

BACKGROUND: Ischemic/anoxic brain injury is often assumed to occur within minutes of severe cerebral ischemia. However, emerging evidence suggests brain tissue may be more resilient, with important implications for resuscitation. We hypothesized that during prolonged cardiac arrest, cortical electrical activity may be restorable if cerebral oxygenation thresholds are met and may be associated with return of spontaneous circulation (ROSC). METHODS: This was an ancillary cohort analysis of a multicenter, prospective observational study. Consecutive adult patients with in-hospital cardiac arrest

Acceptance or distraction first while suffering acute pain? Order effects on visual attention to external painful stimuli.

INTRODUCTION: This study examines attention to pain-related facial expressions in healthy individuals during a cold pain induction, as well as the efficacy of acceptance- or distraction-based instructions in modifying attentional patterns during the induction. METHOD: Sixty pain-free participants completed an attentional task to analyse attentional biases towards pain-related facial expressions (Stage 1). Immediately afterwards, they performed a Cold Pressor Task (CPT) to obtain pain-related measures. Then, participants were randomly assigned to either an experimental group (n = 30), which pe

Suzetrigine for Acute Pain in the Emergency Department: A First-in-Class NaV1.8 Blocker.

Suzetrigine is the first selective, orally bioavailable inhibitor of the peripheral voltage-gated sodium channel NaV1.8. In January 2025, it became the first nonopioid analgesic approved by the US Food and Drug Administration in more than 2 decades. We review preclinical data, regulatory filings, and all published phase 2 to 3 randomized, double-blind trials conducted to describe its potential role in emergency department (ED) pain management. The target receptor, NaV1.8, is critical for nociceptor firing yet absent from the central nervous system and cardiac tissue. This means suzetrigine pro

Novel drugs approved by the EMA, the FDA and the MHRA in 2025: A year in review.

In the 2025 novel drug mini-review, one can take a full measure of the ingenuity that underlies current drug design and development, despite the year's smaller harvest (46 novel drugs) compared to 2024 (53) and 2023 (70). 54% of the novel drugs are first-in-class (FIC). The emphasis on proteins/antibodies is maintained (~25% novel drugs in 2025), an industry trend that does not seem to abate. Fewer than half of the novel medicines address major or common disorders. Among the FIC drugs, it is worth mentioning the Nav1.8 channel inhibitor suzetrigine, the first non-opioid approved to palliate ac

Prevalence of Patients With Pain Consulting Physiotherapists (PHYSIODOL Survey). A Nationwide Survey in France.

BACKGROUND: Pain, whether as a symptom or a chronic disease, is the leading cause of medical consultations but there is no data on whether this is also the case in the practice of physiotherapy. OBJECTIVE: To assess the prevalence of patients with pain consulting a physiotherapist in France. METHODS: A questionnaire to be completed online was sent to physiotherapists practicing in France, asking them about the presence of pain in the patients they treated in the previous five days. The questionnaire was sent through representative healthcare organisations and professional societies, from Jan

Efficacy of perioperative intravenous lidocaine infusion on postoperative recovery and analgesia in patients undergoing video-assisted thoracoscopic surgery: study protocol for a randomised, double-blind, placebo-controlled trial.

INTRODUCTION: Despite the minimally invasive nature of video-assisted thoracoscopic surgery (VATS), moderate-to-severe postoperative pain remains frequent and impairs recovery. Intravenous lidocaine possesses multimodal analgesic, antihyperalgesic and anti-inflammatory properties that may improve pain control and functional outcomes, but robust evidence in thoracic surgery is lacking. Moreover, its potential to attenuate neuropathic pain, a key component of chronic post-thoracic pain syndromes, has not been adequately investigated. This trial will determine whether continuous perioperative int

The effect of adding a second multimodal analgesic component to anesthesia regimens on acute postoperative pain outcomes: a systematic review and meta-analysis.

BACKGROUND: Postoperative pain remains a common clinical challenge, affecting patient recovery, satisfaction, and risk of chronic pain. Conventional analgesics often inadequately address the multifactorial nature of postoperative pain, while opioids carry risks of adverse effects and dependence. Multimodal analgesia, combining agents with complementary mechanisms, aims to improve pain control and reduce opioid consumption, but the incremental benefit of adding a second component is unclear. OBJECTIVE: To evaluate whether adding a second multimodal analgesic component to conventional regimens i

Suzetrigine (a NaV1.8 inhibitor) versus placebo for acute postoperative pain: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: NaV1.8 channels, expressed in peripheral nociceptors, mediate sustained pain signaling. Their inhibition offers a potential opioid-sparing strategy for postoperative pain, although efficacy and safety remain incompletely defined. We synthesized randomized evidence for suzetrigine versus placebo in postoperative pain. METHODS: We conducted a Preferred Reporting Items for Systematic Reviews and Meta-Analyses-compliant meta-analysis of randomized controlled trials comparing suzetrigine (a NaV1.8 inhibitor) versus placebo in surgical patients. The primary outcome was 24-hour pain with
