Percutaneous dorsal column stimulator for chronic pain control.

This is a retrospective review of 26 patients with chronic intractable pain in which dorsal column stimulation was used as a salvage procedure. On follow-up of 12 to 42.7 months, 21 of the patients had diminished narcotic usage. Seventeen patients subjectively rated their pain relief as good to excellent. Another five patients reported some relief of pain. Two-thirds of the patients reported an increase in their ability to perform daily activities such as walking, stair climbing, and time spent sitting. There are still many technical problems that plague this procedure, as evidenced by a very

Nonsurgical management of pain secondary to peripheral nerve injuries.

Physicians and surgeons dealing with peripheral nerve injuries have long been plagued by the chronic intractable pain which often follows these injuries. Early recognition of the associated problems and prompt intervention by a multidisciplinary team approach to the pain is essential. Although the techniques reviewed here were presented as separate choices, the treatment of the patient should employ a combined approach of good surgical repair, positive encouragement, non-addicting medications, appropriately timed nerve blocks, aggressive physical and occupational therapy, and electrical stimul

Chronic intractable pain: an atypical presentation of multiple sclerosis.

Both acute paroxysmal pains and a variety of chronic pain syndromes have been described in multiple sclerosis. These usually occur in the setting of well-established disease. Although paroxysmal pain has been recognized as a rare first symptom in this disorder, presentation with severe, continuous dysesthesias of long duration has not been previously appreciated. We report the cases of three patients in whom refractory continuous pain signalled the onset of demyelinating disease. These observations further define the clinical spectrum of multiple sclerosis and have implications for the evaluat

Recent advances in the knowledge of mechanisms of intractable pain.

Chronic intractable pain can arise from damage at three separate sites, or a combination of these sites: (a) deep tissue, (b) peripheral nerves, and (c) dorsal roots. At each site there is a complex progression of local and distant effects which are produced by a variety of chemical mediators and neurophysiological events. Damage, no matter at which site, often brings changes far beyond the original lesion. It is only by an understanding of the complex mechanisms involved, and the effects that they cause, that new and rational treatments for pain will be found.

The epidural opioid internalized system.

For several years opioids have been given epidurally with success to control chronic intractable pain. The authors report their 2-year experience with internalization of the epidural catheter and injection port in 18 patients at University Hospital, Saskatoon. In all, 21 devices were used. All patients had metastatic cancer; 16 are now dead. The need for other medications was eliminated with 9 and reduced with 12 devices. In the patients who died, the devices were effective for 82% of their remaining life-span. They were able to spend 46% of this time at home. Blockage of the epidural end of t

Totally implantable spinal cord stimulation for chronic pain: design and efficacy.

Neurostimulators used to treat chronic, intractable pain have evolved from technical developments in pacemaker technology. A totally implantable spinal cord stimulation (SCS) system was designed based on elements of a widely used cardiac pacemaker. This paper reports on the transformation of pacemaker technology for neurostimulation applications and presents results of using this system for the treatment of 90 patients with chronic, intractable pain of the low back and/or legs. Significant reduction in pain levels resulted from use of a totally implantable spinal cord stimulation system. Seven

Neuropeptides and their role in nociception and analgesia.

The authors highlight the potential importance of neuropeptides as clinical tools in the study and treatment of pain. Although many questions remain unanswered, it is encouraging that several clinical and experimental advances have shed new light on the neuropharmacology of nociception and have prompted new hope for more effective treatments of such diverse problems as chronic intractable pain, migraine, and drug dependency. Within the next few years, it is anticipated that further work in this area will lead to better basic and clinical understanding of these important clinical problems.

Pharmacology of enkephalinase inhibitors: animal and human studies.

We have shown that a number of compounds which inhibit the degradation of met-enkephalin can produce naloxone-reversible analgesia in mice. These compounds also potentiate the analgesia produced by acupuncture, foot shock, and transcutaneous nerve stimulation in animals and humans. The potency of their effectiveness as analgesics or potentiators parallels their potency as inhibitors of mouse brain enkephalinase. D-Phenylalanine (DPA), one of these enkephalinase inhibitors, has been used successfully for the management of chronic intractable pain in humans and to potentiate the treatment of man

Deep brain stimulation for alleviating chronic intractable pain.

The authors present their 4-year experience with 18 patients who had deep brain stimulation. Most were referred because of chronic pain of varied etiology. All conventional modalities of management had failed. Both the paraventricular gray matter and the sensory thalamus were target sites. The 18 patients underwent a total of 21 implants. Follow-up ranged from 6 months to 4 years with moderate relief of prestimulation pain in 14 patients (77%). Patients with failed back syndrome secondary to multiple disc operations fared well. Patients with pain secondary to progressive neurologic disorders o

Alterations of dorsal root potential in the course of transganglionic degenerative atrophy.

Alterations in the dorsal root potential (DRP) which was evoked by stimulation of the common peroneal nerve of the rat, have been studied in the course of transganglionic degenerative atrophy (TDA) of primary sensory terminals in the upper dorsal horn. TDA was induced by perineural application of Vinca alkaloids around the sciatic nerve. In 9 to 30 days after this treatment, latency of DRP increased, whereas its amplitude and duration decreased. In this period, no C fibre response could be elicited. As a possible mechanism underlying the alterations of DRP, the functional consequences of atrop

[Epidural spinal cord stimulation: its efficacy and mechanisms].

Pain-alleviating effects of epidural spinal cord stimulation (ESCS) were assessed in patients with chronic intractable pain including cancer pain, and some aspects concerning its mechanisms were discussed. The temporary ESCS with percutaneously inserted electrodes was employed in 105 patients, and the implantable systems for long-term use in 19 patients. The ESCS had satisfactory effects especially in patients suffering from cancer pain, causalgia, facial and nape pain. The conditioning stimuli applied to the cervical dorsal cord exhibited interactions with the segmentally evoked spinal cord p

Cross-validation of the Back Pain Classification Scale with chronic, intractable pain patients.

The current study investigated the utility of the Back Pain Classification Scale with chronic, intractable low back pain and headache patients. Subjects consisted of 50 chronic low back pain and 50 chronic headache patients referred to a university based Pain Center for evaluation. Data indicated that subjects were a representative sample of severely intractable chronic pain patients typically referred for Pain Center evaluation and treatment. Each was administered the Back Pain Classification Scale as part of a thorough physical and psychological/behavioral evaluation. All subjects were class

Neuropathologic lesions and CSF morphine concentrations during chronic continuous intraspinal morphine infusion. A clinical and post-mortem study.

Seven patients with chronic intractable pain due to cancer were given chronic intraspinal narcotic administration (CINA) and subsequently underwent post-mortem examination. All deaths were unrelated to CINA. Two of these patients were found to have clinically unsuspected posterior column degeneration. Both patients had had epidural catheters placed, and one had received prior radiotherapy to ports which included parts of the spinal cord. In retrospect, it is impossible to ascertain whether the degeneration occurred before or after infusion of morphine began. Review of the potential causes for

Non-organic chronic intractable pain: a comparative study.

This report compares patients with non-organic chronic intractable (NOCI) pain with those having chronic pain associated with diagnosed physical illness though it was insufficient to explain the severity and nature of pain. Samples were selected after strict screening procedures. The results reveal that NOCI pain is more often seen in those who are younger, unmarried, with higher education and belonging to a professional group of occupation. Chronic pain associated with physical illness is reported more by elderly, married, those with lesser education and housewives. However, only slight diffe

Thalamic stimulation for chronic pain.

Electrical stimulation of the thalamus was found to be effective for the treatment of chronic intractable pain of various etiologies. Diencephalic recordings revealed spontaneous focal electrical discharge patterns with relatively low activation thresholds. Localization of the optimum thalamic discharge site is used as a guide for the electrode implant to be used for chronic stimulation. The thalamic sites which most frequently displayed the low threshold spontaneous focal discharge activity were the CM-Pf complex and related intralaminar nuclear structures. Generation of diencephalic pain-rev

Combined electrical stimulation of the periaqueductal gray matter and sensory thalamus.

11 patients with chronic intractable pain of at least 3 years' duration underwent a morphine infusion test, the results of which suggested a syndrome of superimposed somatogenic and neurogenic pain components. They then underwent stereotactic implantation of a dual-channel brain stimulation system with two brain electrodes, one in the left periaqueductal gray matter (PAG) and the other in the sensory thalamus contralateral to the neurogenic pain. Using this system, all patients have obtained excellent simultaneous relief of both pain components (follow-up 12-36 months). The findings support a

Psychiatric aspects of chronic intractable pain.

Two hundred patients with chronic intractable pain have been evaluated in order to study the clinical characteristics of pain and associated psychiatric illnesses. The commonest site of pain was reported to be head and face, usually dull in nature. Almost 75% of patients reported continuous pain. A great majority (40%-80%) had some psycho-social problem or other problem resultant from the chronic pain. 72% patients had identifiable psychiatric illness, commonest being neurotic depression and anxiety states. The common symptoms reported on the Present State Examination (PSE) were worrying (77%)
