Premenarche risk factors for future dysmenorrhoea: a prospective cohort study.

BACKGROUND: Dysmenorrhoea, or pain during menstruation, is common in adolescence and is often dismissed or left untreated. Dysmenorrhoea can interfere with daily functioning and can lead to other chronic pain conditions; however, little is known about the risk factors for dysmenorrhoea. We aimed to characterise premenarche risk factors for the presence and severity of future dysmenorrhoea. METHODS: In this prospective cohort study, we obtained data for female adolescents from the population-based Adolescent Brain Cognitive Development Study (USA) who were premenarchal at baseline (age 9-10 ye

Predictive markers of endometriosis: a future perspective.

Endometriosis is a chronic gynecological disorder affecting up to 10% of women of reproductive age and is characterized by the presence of endometrial-like tissue outside the uterus. The condition is associated with chronic pelvic pain, dysmenorrhea, dyspareunia, and infertility, and remains difficult to diagnose at early stages. Recent evidence suggests that endometriosis shares genetic and molecular pathways with other chronic pain and inflammatory disorders. Identifying reliable biomarkers is therefore essential to improve risk stratification and support earlier evaluation. Multiple domains

The Ability to Relax Muscles in Chronic Pain Conditions in the General Population: A Cross-Sectional Study (The HUNT Pain Examination Study).

BACKGROUND AND PURPOSE: Muscle tension is considered a central pain mechanism in some chronic pain conditions. In clinical practice, muscle tension can be examined as the ability to relax through tests for flexibility and passive movements. However, empirical evidence for the association with chronic pain is limited. We aimed to investigate and compare the ability to relax muscles in individuals with and without chronic pain and to particularly study individuals with Fibromyalgia syndrome (FMS) and Chronic tension-type headache (CTTH) compared to other chronic pain conditions. METHODS: We uti

A multi-ancestry genome-wide association study of dry eye disease and systemic comorbidities.

BACKGROUND: Dry eye disease (DED) affects up to 25% of the adult population, with chronic symptoms of pain and dryness often negatively impacting quality of life. Twin studies indicate a substantial heritability. However, the genetic architecture of DED is largely unknown. METHODS: We devised a case-control algorithm using a combination of ICD-9/10 codes and prescription records and validated its performance through manual chart review, identifying 132,637 DED cases and 352,201 controls across four ancestry groups in the Million Veteran Program biobank. We then performed multi-ancestry genome

Subcortical neuroimaging signature of painful disorders of the trigeminal nerve: a UK Biobank study.

BACKGROUND: Trigeminal neuralgia, classified as a painful disorder of the trigeminal nerve (PDTN), is characterized by brief, electric shock-like or stabbing pain localized mainly to the maxillary and mandibular regions. The severity of these paroxysmal attacks severely impairs quality of life and, in extreme cases, may lead to suicidal ideation. We hypothesized that PDTN shares a common neuroanatomical signature with other chronic pain disorders, while also exhibiting disorder-specific alterations. METHODS: High-resolution T1-weighted structural MRI scans from 85 individuals diagnosed with p

Three decades of research on the association between temporomandibular disorders and headache: A bibliometric analysis.

OBJECTIVES: To perform a bibliometric analysis of the scientific literature published over the past 3 decades on the relationship between temporomandibular disorders (TMDs) and headache, examining scientific production, citation impact, and the conceptual structure and temporal evolution of research topics in this field. BACKGROUND: TMDs are the most common chronic orofacial pain condition, whereas migraine and tension-type headache are among the leading causes of pain and disability worldwide. These conditions frequently co-occur and share underlying pain-related mechanisms, contributing to

PLAIN ENGLISH SUMMARY: Outcome domains are aspects of a condition that matter to patients and clinicians and can be measured to assess treatment effects. For tinnitus, examples include 'tinnitus loudness' and 'ability to concentrate'. This study focuses on the first stage of agreeing which outcome domains should be measured in all clinical trials of tinnitus. Crucially, it involves identifying outcome domains, prior to a voting process. This article describes how we effectively involved patients in that study design process, and reflects on the impact of their input. The study first compiled a

Identification of common genetic and molecular signatures in migraine and comorbid conditions.

BACKGROUND: Migraine is a very common chronic neurological disorder associated with severe disability and significant social burden worldwide. Beyond recurrent headache attacks, increasing evidence indicates that migraine often coexists with a broad range of systemic disorders, forming complex and often bidirectional relationships. These overlapping conditions complicate clinical management and suggest the presence of shared pathophysiological mechanisms extending beyond the central nervous system. MAIN FINDINGS: Epidemiological studies have consistently shown strong associations between migr

A Hidden Pandemic? Abuse of Gabapentinoids: A Brief Review of Recent Studies.

BACKGROUND: Gabapentin and pregabalin were developed for epilepsy and neuropathic pain. They work via voltage-gated calcium channels and are used for broad-spectrum diagnoses, e.g., epilepsy, neuropathic pain, other chronic pain syndromes, anxiety disorders, alcohol-drug withdrawal syndromes, agitation, insomnia, etc. Especially in a world dealing with the opioid crisis, gabapentinoids were considered safer alternatives to opioid analgesics. METHODS: This review aims to comprehensively search and summarize recent studies concerning the abuse of gabapentinoids published between 2021 and 2022 f

High-Resolution Whole-Genome DNA Methylation Revealed Unique Signatures of Painful Diabetic Neuropathy.

Approximately one out of two patients with diabetes develops diabetic neuropathy; of these, 20% experience neuropathic pain. Risk factors for neuropathic pain are largely unknown; however, DNA methylation was recently associated with neuropathies and degeneration of nerve fibers. The aim of this work was to describe the DNA methylation signature and identify genes associated with neuropathic pain in type 2 diabetes mellitus (T2DM). We discovered distinct DNA methylation signatures that differentiate painful and painless neuropathy phenotypes associated with T2DM and identified genes with poten

Cognitive Functioning in Females with Endometriosis-Associated Chronic Pelvic Pain: A Literature Review.

OBJECTIVE: Cognitive complaints are commonplace among women living with endometriosis-associated chronic pelvic pain (CPP); yet, surprisingly few studies have assessed their cognitive functioning. As an initial step to address the resulting knowledge gap, this review aimed to synthesize the current literature reporting on cognition in females with endometriosis-associated CPP, and due to the poverty of endometriosis studies, to draw on data from female cohorts with other chronic pain conditions to gain potentially relevant insights. METHODS: Three database searches were conducted (Scopus, Pub

Understanding the Pain Experience and Treatment Considerations Along the Spectrum of Peripheral Artery Disease: A Scientific Statement From the American Heart Association.

Peripheral artery disease (PAD) is an atherosclerotic condition that affects a growing number of individuals worldwide, with estimates exceeding 220 million. One of the central hallmarks of PAD is lower extremity pain, which may present as intermittent claudication and atypical leg pain, and, in more severe cases, ischemic rest pain, neuropathic pain, or phantom limb pain in those who underwent amputation. Although the majority of individuals with PAD may experience pain that is chronic in nature, the pathogenesis and phenomenology of pain may differ. Nociceptive, inflammatory, and neuropathic

Definition of chronic postsurgical pain in the ICD-11: a narrative review of implications for anaesthesiology and pain medicine.

Chronic postsurgical pain (CPSP) is associated with reduced health-related quality of life and disability. In some patients, it can result in long-term opioid use even after minor surgery. Epidemiological studies have reported highly varying rates of CPSP, largely because researchers have used different definitions with self-defined cut-offs for pain scores. With the introduction of the 11th revision of the World Health Organisation International Classification of Diseases and Related Health Problems (ICD-11), chronic pain is now recognised as an entity of its own, its biopsychosocial nature i

Poly-symptomatology of chronic multi-canalicular benign paroxysmal positional vertigo: a deductive, inductive, and abductive narrative review.

This narrative review aims to present an overview of the symptomatology of chronic multi-canalicular benign paroxysmal positional vertigo (mc-BPPV) from deductive (what is believed to be known), inductive (what is likely), and abductive (hypothetical) perspectives. The purpose is to recognize these symptoms as expressions of an eventual mc-BPPV when they occur in patients with vestibular migraine, whiplash associated disorders (WAD) and other chronic pain disorders. These symptoms are often considered to be biopsychosocial conditions due to a lack of objective findings, that is, the absence of

Novel findings regarding the role of the endocannabinoid system in pediatric functional gastrointestinal disorders.

INTRODUCTION: Pain constitutes the chief complaint of some functional gastrointestinal disorders (FGIDs). The endocannabinoid (EC) and peroxisome proliferator-activated receptors (PPARs) agonist systems have not been explored as possible contributors. OBJECTIVE: To determine if EC and PPAR agonist abnormalities occur in adolescents with FGID. METHODS: Institutional Review Board approved study compared 33 children (12-18 years) with a FGID to 18 healthy controls (HC). Clinical measures: functional disability inventory and pediatric pain questionnaire (PPQ). Endocannabinoid and PPAR agonist co

Risk of Severe Exacerbation Associated with Gabapentinoid Use in Patients with Chronic Obstructive Pulmonary Disease: A Population-Based Cohort Study.

Evidence on the risk of adverse respiratory outcomes associated with gabapentinoids in patients with chronic obstructive pulmonary disease (COPD) remains limited. Thus, we aimed to assess the risk of severe COPD exacerbation associated with gabapentinoids. We assembled a base cohort of patients aged ≥ 55 years newly diagnosed with COPD between 1993 and 2021 using the UK's Clinical Practice Research Datalink, linked to the Hospital Episode Statistics, and Office for National Statistics datasets.
