The current status of electrical stimulation of the nervous system for the relief of chronic pain.

Neuroaugmentation techniques have been in common use for nearly 30 years. Spinal stimulation is the most important treatment mode for many patients with failed back syndrome. Stimulation can replace or supplement reoperation. Stimulation has been shown to be useful in many other chronic pain states and deserves wider application. However, it is not a technique to be used casually. Effective use requires a thorough understanding of the pain states to be treated, appreciation of the comorbidities that accompany chronic pain, an infrastructure to support the patient and the surgeon, and a dedicat

[Fibromyalgia (generalized tendomyopathy) in expert assessment. Analysis of 158 cases].

MEDAS-agencies are medical institutions within the Swiss Disability Insurance, which specialize in assessing the working capacity of candidates who apply for a disability pension. Degenerative and other chronic pain disorders of the musculoskeletal system form the majority of cases that we investigate. Fibromyalgia is one of our most frequent diagnoses (8.6%). We become involved in cases on average 8.5 years after the first onset of painful symptoms and on average 2.5 years after the patients have ceased to work. Our experience, tells us that fibromyalgia is usually associated with psychologic

Amitriptyline is effective in chronic but not in episodic tension-type headache: pathogenetic implications.

The tricyclic antidepressant, amitriptyline, is an effective drug for the treatment of chronic tension-type headache and for other chronic pain syndromes, but it is also effective in the prophylaxis of an episodic type of headache such as migraine. However, its efficacy in episodic tension-type headache has not yet been clarified. We compared the efficacy of amitriptyline (25 mg/day) in 82 nondepressed patients with either chronic or episodic tension-type headache in an open-label study. Amitriptyline significantly reduced (P < 0.05) frequency and duration of headache as well as analgesic cons

Central nervous system abnormalities in complex regional pain syndrome (CRPS): clinical and quantitative evidence of medullary dysfunction.

OBJECTIVE: Sensory and motor abnormalities are common among patients with complex regional pain syndrome (CRPS). The purpose of the present study was to define and characterize these abnormalities and to develop a hypothesis regarding the area of the central nervous system from which they derive. DESIGN: Data were acquired from study subjects using clinical examination and quantitative assessment of neurological function. Subjects were divided into four groups. CRPS patients were differentiated into two groups based on the presence or absence of sensory deficit on the face to clinical examina

Vulvodynia and its differential diagnoses.

Vulvodynia is the symptom of chronic vulvar discomfort characterized by the patient's complaint of burning and sometimes stinging or rawness. It is a multifactorial problem in which no single etiologic factor or pathogenetic mechanism has been identified. Patients with vulvodynia exhibit several features of neuropathic pain, such as allodynia, hyperalgesia, dysesthesia, and chronic pain in the absence of ongoing noxious stimuli. The treatment of vulvodynia has, in the past, focused on irradicating suspected infective causes, such as human papillomavirus, with varying success. More recently, ap

Experimental pressure pain in patients with complex regional pain syndrome, Type I (reflex sympathetic dystrophy).

Research in animals shows that the levels of neuropathic pain expression is genetically associated with a characteristic response profile to sensory stimuli. The aim of the present investigation was to examine if pressure algometry can identify a specific pain sensitivity profile in patients with complex regional pain syndrome, Type I (reflex sympathetic dystrophy), and to distinguish complex regional pain syndromes from other chronic pain dysfunction syndromes. Pressure pain threshold and pain tolerance measured at the sternum in 17 patients with complex regional pain syndrome, Type I (reflex

Personality assessment of patients with complex regional pain syndrome type I.

OBJECTIVE: There is controversy regarding the importance of psychological/psychiatric factors in the development of the Complex Regional Pain Syndrome (CRPS). Our objective was to determine whether CRPS type I patients were psychiatrically different from other chronic pain patients, with particular attention to personality pathology. DESIGN: A standardized clinical assessment of all major psychiatric categories, including personality disorders, was performed on 25 CRPS type I patients and a control group of 25 patients with chronic low back pain from disc-related radiculopathy. MEASURES: Bot

Personality characteristics of temporomandibular disorder patients using M.M.P.I.

It is generally recognized that psychological factors play an important role in chronic orofacial pain patients. This study analysed psychological profiles of chronic pain patients affected with temporomandibular disorders (TMD), by means of the Minnesota Multiphasic Personality Inventory (MMPI) test. Fifty consecutive TMD patients were examined and were then divided into two subgroups: 1. myofascial pain and 2. temporomandibular joint articular disorders. Sixty-two percent of the whole sample presented pathological MMPI scores. Both subgroups presented similar profiles with alteration of the

Sexual and physical abuse history in subjects with temporomandibular disorders: relationship to clinical variables, pain sensitivity, and psychologic factors.

Recent evidence suggests that a past history of physical and/or sexual abuse is more frequently reported among chronic pain populations; however, the prevalence of reported abuse has not been examined in patients with chronic orofacial pain caused by temporomandibular disorders (TMD). This study compares reported physical/sexual abuse among female TMD subjects recruited from the general population with that of age-matched female control subjects. The association of reported abuse with clinical pain, experimental pain responses, and psychologic variables was examined in the TMD group. Results i

Chronic abacterial prostatitis--a urological chronic pain syndrome?

Chronic abacterial prostatitis has all the hallmarks of a chronic pain syndrome without having been identified as such, either in the urology literature or in the pain literature. This review proposes that this common urological syndrome merits consideration as a 'chronic pain syndrome', as it is commonly defined. The presentation of this syndrome in PAIN results from the unlikely collaboration of a clinical psychologist and a urologist with an interest in exploring unconventional assessment and treatment recommendations for the patients he saw in a university-based Prostatitis Clinic. In addi

Psychological dysfunction in patients with reflex sympathetic dystrophy.

Patients with reflex sympathetic dystrophy (RSD) often present with pain and disability that cannot be explained on the basis of objective physical findings. This has led some to speculate that RSD may be caused or mediated by non-organic factors. Unfortunately, there have been few studies using standardized measures of mood and illness behavior that have compared patients with RSD to patients with other chronic pain disorders. The goal of the present study, therefore, was to compare the pattern of psychological dysfunction in patients with RSD to the pattern of dysfunction in patients with ch

[Patients with cancer-related pain and other chronic pain. Priorities and assessment].

In a survey completed at our hospital, 519 doctors and nurses were asked how pain treatment was estimated whether it received priority, and to what degree patients' pain syndromes were assessed. A total of 473 responded to the questionnaire. In the study cancer-related pain and pain from causes other than cancer were assessed in separate population groups. The responders considered that the staff gave higher priority to patients with cancer-related pain, than to patients with other pain. There was a discrepancy between the physicians' and the nurses' answers to the question whether optimal pai

A guide to the understanding and use of tricyclic antidepressants in the overall management of fibromyalgia and other chronic pain syndromes.

The purpose of this review is to present relatively detailed information on the characteristics of tricyclic antidepressants, mainly amitriptyline hydrochloride and doxepin hydrochloride, for use as an integral part of the safe and effective management of fibromyalgia and, to a lesser extent, other chronic pain syndromes. Data sources include MEDLINE searches in English, relevant reference books and textbooks, my personal database and library, as well as personal clinical experience. I discuss these data with regard to the pharmacologic characteristics, mechanisms of action, adverse effects, a

Psychological differences between reflex sympathetic dystrophy and non-RSD chronic pain patients.

This study examined possible psychological differences between Reflex Sympathetic Dystrophy (RSD) and non-RSD chronic pain patients. Unlike the few previous studies in this area, this study controlled statistically for age and pain duration differences across diagnostic groups, and included a non-RSD limb pain control group. Subjects were a consecutive series of 34 RSD, 50 non-RSD limb pain (Limb), and 165 low back pain (LBP) patients presenting for treatment at the Rush Pain Center. Analyses revealed that RSD patients reported more somatization and phobic anxiety on the Brief Symptom Inventor

Self-care strategies used for acute attack of interstitial cystitis.

OBJECTIVE: To determine the kinds of self-care used by women with interstitial cystitis and to find effective ways to manage symptoms of acute attack. STUDY DESIGN: One hundred thirty-eight women with interstitial cystitis completed a survey indicating how often they used and how effective they found strategies in four physical self-care subdomains (medication, treatment, diet, and body comfort) and three psychologic self-care subdomains (cognitive/stress reduction, distraction, and help-seeking). RESULTS: Descriptions of symptoms during an "acute attack" are more consistent with symptoms de

Pet ownership and prophylaxis of headache and chronic pain.

The belief that having household pets promotes good health is ubiquitous among Americans. Recent studies support this belief where certain medical conditions are concerned. To investigate the advantages of pet ownership in the prophylaxis of headache and other chronic pain conditions, we queried 62 patients suffering from such diseases about whether they owned pets and whether children and other adults shared their households. We similarly queried a control group of 38 patients with various conditions not involving headache or chronic pain. We found that statistically, the experimental group a

Personality characteristics of patients with temporomandibular disorders: diagnostic and therapeutic implications.

One hundred ten patients complaining of nondental orofacial pain of more than 3 months' duration were psychologically evaluated with the Minnesota Multiphasic Personality Inventory. Four distinct personality profiles were found: psychophysiologic reaction, in 52% of the patients; depressed reaction, in 11% of the patients; defensive reaction, in 12% of the patients; and "no diagnosis" (normal), in 24% of the patients. These results indicate that chronic temporomandibular pain patients present personality characteristics that are similar to those of other chronic pain patients according to the
