Longitudinal association between dysmenorrhoea in adolescence and chronic pain in adulthood: a UK population-based study.

BACKGROUND: Dysmenorrhoea affects many adolescents and often goes untreated for various sociocultural reasons. Dysmenorrhoea frequently co-occurs with other chronic pain conditions, and adult women with dysmenorrhoea have greater sensory sensitivity compared with controls. We aimed to test the hypothesis that adolescent dysmenorrhoea leads to the development of general chronic pain, including pain outside the pelvis, by estimating the risk of chronic pain in adulthood following the experience of dysmenorrhoea at age 15 years. METHODS: We used data from the Avon Longitudinal Study of Parents a

Interdisciplinary Headache Management.

Patients with headache disorders may benefit from a collaborative, interdisciplinary and/or multidisciplinary approach with involvement from multiple physician specialties, therapists, psychologists, and pharmacists. Each member of the team plays a role in headache diagnosis and management, from medication management to patient education to procedural or surgical intervention. Initiation of an interdisciplinary approach for the patient with headache is crucial, especially if the patient fails initial conservative treatment or suffers from other chronic pain states so as to adequately address a

The Synergistic Effect of Acute Traumatic Brain Injury and Spinal Cord Injury on the Development of Chronic Pain: A Populational Cohort Study on Neurotrauma Patients in Canada.

BACKGROUND: The impact of concomitant traumatic brain injury (TBI) and spinal cord injury (SCI) on the development of problematic chronic pain has never been studied. We hypothesized that concomitant TBI increases the rates of problematic chronic pain outcomes in SCI individuals. OBJECTIVES: To examine the association between concomitant TBI-SCI (as opposed to isolated TBI or SCI) and the development of problematic chronic pain outcomes, including referral to a specialized pain clinic or long-term usage of opioids or other chronic pain medications. METHODS: A retrospective observational coho

Multisensory sensitivity predicts menstrual pain and widespread pain trajectories over 12 months in adolescents with and without primary dysmenorrhea.

A substantial body of evidence points to menstrual pain as a risk factor for chronic pain, yet measures of central sensitization of pain have failed to account for this transition. New research suggests that multisensory sensitivity (MSS) may better characterize those at risk for chronic pain. This study aimed to determine the contribution of MSS to menstrual pain and widespread pain trajectories over 1 year in adolescents with and without primary dysmenorrhea and no other chronic pain conditions at baseline. One hundred forty-one regularly menstruating participants (ages 13-19 years) complete

Minimally important difference and responsiveness to change for numerical rating scale of menstrual pain severity: a psychometric study.

BACKGROUND: Menstrual pain affects 45%-95% of reproductive-age females and increases the risk of other chronic pain conditions. Psychometrically sound measurement tools are essential for advancing research and clinical care in menstrual pain. Numerical rating scales (NRS) are widely used to measure pain severity. However, the minimally important difference (MID) and responsiveness to change of the NRS in the context of menstrual pain are not well understood. Understanding MID and responsiveness to change helps guide the evaluation of treatment efficacy and clinical decision-making. This study

[Pain mechanisms involved in chronic pelvi-perineal pain].

PAIN MECHANISMS INVOLVED IN CHRONIC PELVI-PERINEAL PAIN. Chronic pelvic and perineal pain, like other chronic pain, often is the result of specific organic lesion mechanisms, but can lead, via central and peripheral sensitization mechanisms, to dysfunctions that maintain the pain autonomously. To manage this type of pain in the most appropriate way, it is necessary to understand the mechanisms involved, and therefore the anatomical basis of the region, the neurophysiology of "normal" pain, and finally the various components involved when pain becomes chronic. The latter can become therapeutic

Impaired gastric and urinary but preserved cardiac interoception in women with endometriosis.

Endometriosis is a chronic inflammatory gynaecological condition frequently associated with chronic pelvic pain. Visceral hypersensitivity could be present, like in other chronic pain conditions, causing altered levels of interoception. So far, studies have explored interoceptive deficits in chronic pain individuals mainly using questionnaires or cardiac interoceptive accuracy tasks. Here, we explore the cardiac, gastric, and urinary domains to probe interoceptive differences between patients with endometriosis and healthy women. 30 patients and 30 controls underwent three interoceptive tasks

Association of National Area Deprivation Index with dietary patterns and pain-related inflammatory biomarkers in community-dwelling adults with chronic low back pain.

INTRODUCTION: Chronic low back pain (cLBP) is a common health condition associated with substantial personal and economic costs. Recent literature suggests that socioeconomic status (SES) and diet quality may influence its impact. METHODS: The purpose of this study was to examine whether SES, measured via the National Area Deprivation Index (NADI), and diet quality, assessed by the dietary inflammatory index (DII), were associated with proinflammatory cytokine levels and movement-evoked pain outcomes in individuals with cLBP. We hypothesized that individuals with cLBP with lower NADI and DII

Does rehabilitation improve work participation in patients with chronic spinal pain after spinal surgery: a systematic review.

OBJECTIVE: Patients with therapy-refractory chronic spinal pain after spinal surgery experience increased disability, resulting in substantial loss of employment and consequently lower quality of life. Despite findings that rehabilitation improves socio-economic outcomes in other chronic pain conditions, evidence for patients with chronic spinal pain after spinal surgery is limited. A systematic review was conducted to provide an overview of rehabilitation interventions and their effectiveness to improve work participation for patients with chronic spinal pain after spinal surgery. METHODS: M

Decomposing the genetic background of chronic back pain.

Chronic back pain (CBP) is a disabling condition with a lifetime prevalence of 40% and a substantial socioeconomic burden. Because of the high heterogeneity of CBP, subphenotyping may help to improve prediction and support personalized treatment of CBP. To investigate CBP subphenotypes, we decomposed its genetic background into a shared one common to other chronic pain conditions (back, neck, hip, knee, stomach, and head pain) and unshared genetic background specific to CBP. We identified and replicated 18 genes with shared impact across different chronic pain conditions and two genes that wer

Genome-wide association study of dry eye disease reveals shared heritability with systemic comorbidities.

Dry eye disease (DED) affects up to 25% of the adult population, with chronic symptoms of pain and dryness often negatively impacting quality of life. The genetic architecture of DED is largely unknown. Here, we develop and validate an algorithm for DED in the Million Veteran Program using a combination of diagnosis codes and prescription records, resulting in 132,657 cases and 352,201 controls. In a multi-ancestry genome-wide association study, we identify ten significant loci in nine susceptibility regions with largely consistent effects across ancestries, including loci linked to synapse ma

Psychedelic-assisted Therapy as a Promising Treatment for Irritable Bowel Syndrome.

Irritable bowel syndrome (IBS) is prevalent and can be disabling. Many patients remain symptomatic despite behavioral and medical therapies. Psychedelic-assisted therapy (PAT), in which serotonergic agents like psilocybin are administered in a psychotherapeutic context, has shown promise for refractory psychiatric disorders, including major depressive disorder and post-traumatic stress disorder. Emerging evidence suggests PAT may also be beneficial for chronic pain conditions, including fibromyalgia, low back pain, and migraines. IBS is highly comorbid with depression, anxiety, and other chron

Persistent Spinal Pain Syndrome: A Study of Contact Heat-Evoked Potentials.

INTRODUCTION: Persistent spinal pain syndrome (PSPS) type 2 is a chronic condition characterized by low back pain, with or without radiating limb pain, persisting after spinal surgery. The underlying pathophysiology remains unclear, with potential contributions from altered central pain processing mechanisms. OBJECTIVE: This study investigates the role of central sensitization in PSPS using contact heat-evoked potentials (CHEPs), an electrophysiological tool for assessing spinothalamic tract integrity and central pain processing. MATERIALS AND METHODS: The study included 36 healthy controls

Fibromyalgia: one year in review 2025.

Fibromyalgia (FM) is a chronic syndrome characterised by widespread pain, high prevalence, and a significant impact on quality of life. Despite extensive research, its pathogenesis and treatment remain only partially understood, driving continued investigation throughout 2024. Dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis and sympathetic nervous system has been linked to chronic stress responses and neuroinflammation, with neuroimaging and preclinical studies confirming altered pain and stress processing. Low-grade inflammation and metabolic disturbances, including cytokine im

Transcutaneous auricular nerve stimulation modulates the functional connectivity of the descending pain modulation system and reward network in patients with chronic low back pain.

This study aims to examine the modulatory effects of transcutaneous auricular vagus nerve stimulation (taVNS) on Chronic low back pain (cLBP). 70 cLBP patients were recruited and randomized into taVNS or transcutaneous greater auricular nerve stimulation (tGANS) group. Both interventions were administered by participants themselves after initial training (five times/week for four weeks). Magnetic resonance imaging (MRI) data were collected at baseline and after 4-week interventions. Seed-based static and dynamic functional connectivity (sFC and dFC) were performed to investigate the modulation

Fibromyalgia syndrome-am I an autoimmune condition?

Assessments of serum-autoantibodies in fibromyalgia syndrome (FMS) date back to the 1980s and have yielded inconsistent results. Based on a new passive transfer paradigm, since 2021 causative involvement of immunoglobulin G-mediated autoimmunity in severe FMS has been demonstrated in several studies, which have included UK, Swedish, and Canadian patients. These findings open the path to the development of novel diagnostic and immune-therapeutic approaches. Autoantibody targets and downstream mechanisms and the molecular processes that translate infection-, toxicity-, or stress-triggers into th

Temporal interaction information and laser evoked responses: preliminary results in fibromyalgia patients with small fibers pathology.

OBJECTIVES: We propose a novel application of higher-order information-theoretic measures to assess the temporal interaction information (TII) between laser-evoked potentials (LEPs) and individual pain ratings in healthy subjects and patients with fibromyalgia (FM) affected by small fiber pathology. METHODS: Seventy-nine FM patients, categorized into three groups based on skin biopsy findings (normal innervation n 19, proximal denervation n 53, and both proximal and distal denervation n 7), and 14 control subjects were studied. We used cluster-based permutation tests (p < 0.05) to identify si

Differences Among Veterans With Chronic Overlapping Pain Conditions and Other Chronic Pain: Baseline Results From the LAMP Pain Management Trial.

BACKGROUND: Chronic overlapping pain conditions (COPCs) are a constellation of disorders posited to share an underlying pain mechanism (nociplastic pain). Unfortunately, individuals with COPCs are under-represented in clinical research. The current study aimed to determine COPC prevalence among participants enrolled in a pragmatic clinical trial and compare those with COPCs to those with non-COPCs across several domains. METHODS: Learning to Apply Mindfulness to Pain (LAMP) study baseline data of veterans with chronic pain (N = 811) were utilised. COPC diagnoses were determined using ICD-10 c

How do people with fibromyalgia interpret ambiguous cues in empathy-related healthcare scenarios?

Empathic communication is essential for high-quality healthcare, yet patients must often interpret subtle and ambiguous interpersonal cues during healthcare encounters. In such contexts, prior experiences and cognitive-affective processes may shape interpretations in biased ways. In Fibromyalgia Syndrome (FMS), a frequently stigmatised and 'invisible' condition, such biases may influence how patients perceive unclear messages from healthcare professionals. While interpretation bias for pain-related information has been explored, bias in interpreting social-emotional information has not. This s

The Association Between miRNA-223-3p Levels and Pain Severity in Fibromyalgia Syndrome: A Molecular Approach.

Fibromyalgia Syndrome (FMS) is a chronic syndrome commonly characterized by widespread musculoskeletal pain and fatigue. Current evidence suggests that FMS diagnosis relies on clinical evaluation and patient-reported symptoms. MicroRNAs, which serve as key regulators of gene expression, have been proposed to play a role in the pathogenesis of FMS and other chronic pain syndromes. In this pilot study, miRNA-223-3p expression levels were examined in patients with FMS, and their relationship with pain intensity-assessed using the Visual Analog Scale (VAS)was evaluated. To obtain a broader underst
