In a group of about 1000 patients with chronic pain, approximately 4% had pain of very prolonged duration--25 years or longer. These subjects were similar in general to other chronic pain patients but they did differ significantly in the following: a higher incidence of face and head pain, greater dependency on analgesics and sedatives, more frequent diagnoses of depression, and a greater elevation of nearly all clinical MMPI scales.

Nociceptive flexion reflexes, RIII reflex in particular, have been demonstrated to be a useful tool for pain research in humans, since the threshold of RIII reflex is that of pain. The development of a new scale to measure somatic and autonomic perception, the Modified Somatic Perception Questionnaire or MSPQ is described. A course of 10 daily acupuncture treatments was given to 200 patients who suffered from chronic pain syndromes of at least one year duration and the result assessed at the end of the course of treatment and after an interval of at least 2 months. Treatments were individualized using needling of body loci distally and near the site of pain, and ear acupuncture. Phenytoin was given in full dosage to eight patients with thalamic pain and two others with intractable pain resistant to other forms of treatment. Serum levels were monitored and correlated with dosage levels. Three patients improved markedly, two improved only minimally, two were unchanged, and three were worse. Those patients who had improved noted return of original pain on stopping phenytoin. The results indicate the need for a further study of the drug in thalamic and other chronic pain states.
