The state of health of patients previously operated on for primary hyperparathyroidism compared with randomized controls.

240 patients operated on for PHPT in the years 1956-79 were compared with 240 sex and age matched randomized controls to examine the present state of health of the patients. Diseases which occurred in both groups were registered to clarify their possible association with PHPT. Recurring urinary tract infections occurred as often in both groups, though renal stones had naturally been more frequent in the PHPT group. Serum creatinine was elevated in 33 PHPT patients and in 9 controls (p less than 0.001), and renal damage was also more severe in the PHPT patients. Malaise and fatigue occurred in

Malaise and fatigue.

Patients used to complain of not being "in the pink". Nowadays they are "bushed", "beat", or "washed out". They used to expect a simple diagnosis and treatment; now both are much more complex.

The early clinical manifestations of Lyme disease.

Lyme disease, caused by a tick-transmitted spirochete, typically begins with a unique skin lesion, erythema chronicum migrans. Of 314 patients with this skin lesion, almost half developed multiple annular secondary lesions; some patients had evanescent red blotches or circles, malar or urticarial rash, conjunctivitis, periorbital edema, or diffuse erythema. Skin manifestations were often accompanied by malaise and fatigue, headache, fever and chills, generalized achiness, and regional lymphadenopathy. In addition, patients sometimes had evidence of meningeal irritation, mild encephalopathy, mi

Familial chronic mononucleosis.

A syndrome of chronic mononucleosis occurred in two members of a family. Symptoms were chronic malaise and fatigue; recurrent upper respiratory tract infections; and mild, variable immune abnormalities. Intermittently positive heterophil titers were present for more than 2 years after acute infectious mononucleosis. Epstein-Barr-virus-specific antibodies were persistently abnormal. In the proband, the R component of the early antigen complex was present for 3 years and she never developed normal antibodies to Epstein-Barr nuclear antigen. Her brother had low to absent Epstein-Barr nuclear anti

Successful treatment of a patient with retractile mesenteritis with prednisone and azathioprine.

A patient with retractile mesenteritis is discussed, who presented with a huge 15 X 15-cm inflammatory mass involving the root of the mesentery. Gradually the patient developed duodenal obstruction for which it was necessary to perform a gastroileostomy. Because of a relentless downhill course with fever, weight loss, general malaise, and fatigue, it was finally decided to treat the patient with prednisone and azathioprine. Upon institution of this treatment there was a steady, progressive clinical amelioration with disappearance of fever, improvement in laboratory findings, and gradual regres

Atypical IgA with hidden light chain.

A 64-year-old Caucasian male was examined following complaints of persistent malaise and fatigue. Electrophoretic analysis of the patient's serum revealed a paraprotein spike which was initially attributed to α chains. However, subsequent reduction and alkylation of the purified protein showed the presence of free light chains of κ type. Further proof of the κ type light chains was obtained from tests with antisera to the paraprotein. This case represents another instance of a `hidden' light chain in a paraprotein, possibly related to the tertiary structure and not a case of α-chain disease as
