A composite inflammatory-renal prognostic score predicts in-hospital mortality in older adults with sepsis.

Sepsis remains a leading cause of mortality among older hospitalized adults. Readily available biomarkers may improve early risk stratification in this vulnerable population. We evaluated the prognostic value of the neutrophil-to-lymphocyte ratio (NLR), C-reactive protein-to-albumin ratio (CAR), and estimated glomerular filtration rate (eGFR), and developed a composite score (NCe) integrating these parameters. This retrospective cohort study included 846 patients aged ≥ 75 years hospitalized with sepsis or septic shock within the Report-AGE project. NLR, CAR, and eGFR were assessed at admissio

Nailfold Capillaroscopy Reveals Peripheral Microcirculatory Trajectories in Septic Shock.

BackgroundSepsis-induced microcirculatory dysfunction may persist despite systemic hemodynamic stabilization, yet longitudinal changes in peripheral microvascular structure and flow during recovery remain insufficiently characterized. We used nailfold capillaroscopy (NFC) to characterize these changes during recovery from septic shock.MethodsIn this prospective two-center study, patients with Sepsis-3 septic shock underwent NFC during shock and after clinician-confirmed hemodynamic stabilization. Baseline NFC analyses required analyzable initial recordings; longitudinal analyses required paire

Associated factors for unfavorable prognosis in neonates with septic shock.

BACKGROUND: Neonatal septic shock represents a life-threatening complication of neonatal sepsis associated with substantial in-hospital mortality. Conventional diagnostic criteria only detect shock at the decompensated stage, which inevitably delays optimal treatment. This study aimed to identify independent predictors associated with adverse outcomes in neonates with septic shock and evaluate the predictive performance of combined early laboratory biomarkers and point-of-care cardiac ultrasound parameters. METHODS: A single-center retrospective cohort study was performed on 98 neonates diagn

The use of capillary refill time In adult critically ill patients: a narrative review on potentials, pitfalls, and perspectives.

The evolution of Capillary refill time (CRT) use in clinical medicine is interesting, transforming a simple bedside observation into a reliable tool for assessing tissue perfusion in critically ill patients. When Henry Beecher and colleagues first systematized CRT measurement in 1947, they recognized its potential as a rapid method to evaluate circulatory status in trauma casualties, establishing a foundational framework that would influence intensive care practice for decades to come. However, clinicians soon discovered that relying on visual inspection alone introduced troubling variability

CS-MORT-6: A mortality score that refines risk within EHR-derived SCAI stages in cardiogenic shock.

BACKGROUND: Cardiogenic shock carries high in-hospital mortality, and the ordinal SCAI classification does not rank risk within a stage. Existing scores generally require imaging, procedural, or support-dependent data. METHODS: We identified 3103 adults with documented cardiogenic shock in MIMIC-IV. Six predictors (lactate, urine output, cardiac arrest, age, blood urea nitrogen, red cell distribution width) were selected by bootstrap LASSO screening and clinical review. CS-MORT-6 was derived as two continuous ridge logistic models (lactate or anion gap) fitted at a 24-h landmark and a 0-to-15

Left Ventricular End-Diastolic Diameter Predicts Clinical Outcomes in Patients Supported With Surgically Implanted Microaxial Flow Pump.

BACKGROUND: This study sought to determine the impact of left ventricular size on clinical outcomes after a microaxial flow pump was surgically implanted. METHODS: In this retrospective cohort study, we included patients who received a surgical microaxial left ventricular assist device from May 2020 to February 2024 at 2 high-volume centers in the United States. Patients were categorized into 2 groups based on their preimplant left ventricular end-diastolic diameter (LVEDD): "small LVEDD," defined as LVEDD ≤5.5 cm, and "large LVEDD," defined as LVEDD >5.5 cm, respectively. End points were all

Acute Effects of Impella 5.5 on Biventricular Filling and Pressure-Volume Area in Patients With Heart Failure Associated Cardiogenic Shock.

Use of Impella 5.5 in patients with decompensated heart failure-associated cardiogenic shock (ADHF-CS) is increasing as a bridge to advanced therapies. The effects of maximal trans-valvular unloading on biventricular filling and pressure-volume area (PVA) in ADHF-CS are poorly understood. Nine patients with medically refractory ADHF-CS underwent Impella 5.5 placement with simultaneous invasive arterial, venous, and biventricular pressure-volume loop analysis. Acutely after Impella 5.5 activation, cardiac output (3.4 ± 0.5 vs. 5.0 ± 0.7 L/min, p = 0.0011), diastolic (67.2 ± 8.0 vs. 80.7 ± 9.7 m

Forty-Eight Hour Lactate Normalization Predicts Favorable Neurologic Recovery and Survival in Venoarterial ECMO for Cardiogenic Shock.

Extracorporeal membrane oxygenation (ECMO) for cardiogenic shock (CS) is associated with high mortality and poor neurological outcomes. Dynamic assessment of metabolic recovery, reflected by lactate normalization, may offer additional prognostic value compared with static severity indices. We retrospectively analyzed 662 consecutive patients from the multicenter RESCUE II registry who remained on venoarterial ECMO for at least 48 hours. Patients were stratified by 48 hour lactate level: 48 hour lactate normalization group (< 2.0 mmol/L, n = 371) and persistent lactate elevation group (≥ 2.0 mm

Associated factors for in-hospital mortality in patients with fulminant myocarditis: a systematic review and meta-analysis.

BACKGROUND: Fulminant myocarditis (FM) progresses rapidly and carries high all-cause in-hospital mortality. Patients in the early phase often develop cardiogenic shock and malignant ventricular arrhythmias, with widely varying mortality rates reported across studies. Existing single-center cohorts yield conflicting prognostic findings with marked cross-cohort heterogeneity, and no standardized meta-analysis with GRADE certainty grading has been published to guide risk stratification. This meta-analysis pooled prognostic associations of all-cause in-hospital mortality in FM to inform acute risk

Enhanced Early Risk Stratification in Cardiogenic Shock Using Machine Learning: A Hierarchical Clustering Analysis.

BACKGROUND: Cardiogenic shock (CS) exhibits marked clinical heterogeneity, limiting the performance of conventional risk stratification tools like the Society for Cardiovascular Angiography and Interventions (SCAI) staging system. Machine learning (ML) has identified reproducible CS phenotypes that improve risk prediction, but their temporal consistency across the progression of shock remains incompletely described. OBJECTIVES: The purpose of this study was to evaluate the temporal evolution of ML-derived CS phenotypes across the first 3 intensive care unit (ICU) days and determine whether ph

Sex Differences in Vasopressor De-escalation With Arginine Vasopressin in Mixed Cardiogenic-Vasodilatory Shock Under VA-ECMO.

BACKGROUNDS: Women with cardiogenic shock (CS) under VA-ECMO face excess mortality, yet the pharmacological basis of this disparity remains poorly understood. Arginine vasopressin (AVP) is increasingly used as a non-adrenergic vasopressor in this setting, but whether sex modifies the hemodynamic response to AVP is unknown. METHODS: In this multicenter retrospective cohort of VA-ECMO-supported CS patients, we assessed whether sex was independently associated with successful vasopressor de-escalation at 48 hours (norepinephrine equivalent dose [NEE] <0.25 µg/kg/min) after AVP initiation.
