Weekend admission and early in-hospital mortality among patients with hematologic malignancies admitted for febrile neutropenia.

INTRODUCTION: Weekend admissions have been associated with worse health outcomes compared with weekday admissions. Our study assessed the association between weekend admission and early in-hospital mortality among patients with hematologic malignancies and febrile neutropenia. We further evaluated whether septic shock mediates this relationship, regardless of length of in-hospital stay. METHODS: We conducted a retrospective analysis of the 2016-2020 NIS database identifying patients with febrile neutropenia and hematologic malignancies using ICD-10 codes. Weekend admission was the primary exp

Balanced Solutions Versus Normal Saline in Pediatric Septic Shock: A GRADE-Assessed Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BackgroundIn pediatric septic shock, early fluid resuscitation is a vital approach to treat this condition. The appropriate isotonic crystalloid to use (balanced salt solution [BSS] vs normal saline [NS]) remains debated. We performed an updated systematic review and meta-analysis to evaluate the safety and efficacy of both isotonic crystalloid solutions in this patient population.MethodsWe systematically searched PubMed, Scopus, Web of Science, and Cochrane Central Register of Controlled Trials (CENTRAL) from inception until May 1, 2026, for randomized controlled trials (RCTs) comparing BSS w

Dynamics of ventriculo-arterial coupling and peripheral tissue perfusion during early resuscitation of septic shock.

BACKGROUND: Left ventriculo-arterial coupling (LVAC), the ratio of arterial elastance to left ventricular end-systolic elastance, is a dimensionless index that depicts the interaction between arterial load and cardiac contractility, reflecting energy transmission from the heart to the proximal arterial tree. Nevertheless, how dynamic variations of LVAC may impact tissue perfusion remains unclear. We evaluated the dynamic relationship between LVAC and capillary refill time (CRT), a peripheral perfusion surrogate, during early resuscitation of septic shock. METHODS: Adult patients were prospect

An early prediction model of septic shock in patients with Escherichia coli bloodstream infection based on machine learning.

BACKGROUND: Escherichia coli bloodstream infection (E. coli BSI) is a global clinical challenge with markedly higher mortality once it progresses to septic shock. Current tools are impractical in primary care, creating an urgent clinical need for accessible, laboratory-based early prediction tools. Thus, we developed and validated an interpretable machine learning model using routine laboratory indicators acquired prior to septic shock to predict septic shock in patients with E. coli BSI. METHODS: A retrospective analysis was conducted on 719 patients with E. coli BSI treated at a tertiary re

When the abdomen misleads: Emphysematous pyelonephritis masquerading as pneumoperitoneum.

A presentation in septic shock with tense abdominal distension and left flank pain, combined with a plain radiograph showing massive subdiaphragmatic free air usually suggests perforation of a hollow viscus. A CT scan obtained before laparotomy was undertaken, showing gas destroying the left renal parenchyma with extension into the pararenal space and rupture through the posterior parietal peritoneum with an intact gastrointestinal tract. Laparotomy was thus avoided; pneumoperitoneum is not always gastrointestinal in origin.

Pressure-volume loop reconstruction of Impella support in severe aortic regurgitation: explaining a hemodynamic paradox.

BACKGROUND: Severe aortic regurgitation (AR) causes left ventricular (LV) volume overload, while transvalvular Impella support may permit regurgitant recirculation. We constructed schematic pressure-volume (PV) loops to illustrate the interaction between ventricular unloading and systemic perfusion in severe AR. METHODS: Conceptual LV PV loops were constructed from established principles of ventricular mechanics to depict progressive AR, cardiogenic shock (CS), and alternative hypothetical responses to Impella support. No patient-specific measurements or numerical simulation were used. RESUL

Mock Circulatory Loop With Autoregulated Renal Circulation Based on Perfusion Pressure for Circulatory Support Devices Evaluation.

INTRODUCTION: Renal hypoperfusion, driven by reduced mean arterial pressure or increased venous congestion, contributes to acute kidney injury in cardiogenic shock. Assessing cardiovascular support strategies for patients with cardiogenic shock requires controlled platforms such as mock circulatory loops (MCLs). However, most MCLs lack renal autoregulation or do not account for venous pressure, risking inaccurate renal flow estimation. Here, we describe how to implement a perfusion pressure-based autoregulated renal circulation within an MCL. METHODS: A renal circulation was integrated into a

Early and Mid-Term Mortality After Transcatheter Aortic Valve Implantation: Five-Year Experience from the KOSUTAVI Study.

BACKGROUND: Although transcatheter aortic valve implantation (TAVI) outcomes are well-documented, predictors of mortality beyond conventional surgical risk scores and valve-related factors in real-world, high-risk cohorts require further elucidation. This study aimed to identify the key determinants of 30-day and 1-year mortality following TAVI, with a focus on both patient-specific and procedure-specific complications. METHODS: The KOSUTAVI study is a single-center, retrospective cohort analysis of 859 consecutive high-risk individuals with severe aortic stenosis undergoing TAVI between 2020

Transvenous Lead Extraction Without Dedicated Surgical Back up in Low-Risk Patients.

BACKGROUND: Transvenous lead extraction (TLE) is associated with major complications that could require immediate surgical rescue. These procedures are performed in an EP lab or hybrid OR with the presence of a surgical team on standby. We have designed an institutional protocol that permits performance of TLE in low-risk situations, in an EP lab without dedicated surgical back-up. OBJECTIVES: To investigate outcomes of TLE in low-risk patients METHODS: We retrospectively identified patients who underwent low-risk TLE at Emory Healthcare between 2010-23. Low-risk TLE were defined as an extrac

[A delayed threat: S. aureus sepsis 4 years post-IUD insertion in a healthy young woman].

BACKGROUND: Intrauterine devices (IUDs) are highly effective long-acting reversible contraceptives with an established safety profile. Infectious complications are rare and usually occur shortly after insertion, typically involving polymicrobial pathogens. Staphylococcus aureus is an uncommon pathogen in this context, but its biofilm-forming ability may contribute to delayed and severe presentations. OBJECTIVES: A healthy 25-year-old primiparous woman developed septic shock four years after the insertion of a levonorgestrel-releasing intrauterine system (Mirena®). She was admitted to the inte

Temporal cytokine trajectories during microaxial flow pump support identify a high-risk inflammatory phenotype in cardiogenic shock.

BACKGROUND: Despite advances in temporary mechanical circulatory support, the mortality rate for patients with cardiogenic shock (CS) remains high. Current shock frameworks rely on hemodynamic parameters and fail to capture ongoing biological injury. Our study aimed to investigate cytokine trajectories during support and their association with clinical outcomes. METHODS: This single-center retrospective cohort study analyzed 113 patients with CS in SCAI stage ≥ C who were treated with microaxial flow pump (mAFP). Serial cytokine levels were measured from the time of mAFP implantation until da

Optimal timing of surgical intervention for post-myocardial infarction ventricular septal rupture: A comprehensive evidence review.

BackgroundVentricular septal rupture (VSR) is a rare but devastating complication of acute myocardial infarction, historically associated with extremely high mortality rates. The optimal timing of surgical repair-balancing the risks of operating on friable, acute necrosis against the dangers of progressive cardiogenic shock during a waiting period-remains a central clinical dilemma.MethodsA systematic narrative review was conducted following PRISMA guidelines. PubMed/MEDLINE, Embase, and Cochrane Library databases were searched from 2000 to 2024 for studies comparing early (≤7 days) versus del
