Pyogenic Sacroiliitis in Older Adults Presenting With Massive Retroperitoneal Abscess: A Case Report.

Pyogenic sacroiliitis (PSI) is an uncommon form of septic arthritis and is frequently diagnosed late in older adults because initial symptoms are nonspecific and imaging findings may be misattributed to other sources of infection. We report two older patients with PSI complicated by massive retroperitoneal and gluteal abscesses, highlighting the importance of early recognition and, when indicated, timely surgical source control. Case 1 was an 83-year-old woman in whom a conspicuous retroperitoneal abscess and abnormal urinalysis led to suspicion of an intra-abdominal or urinary tract source, d

Extensive Gastric Necrosis and Perforation Secondary to Intragastric Balloon-Related Gastric Outlet Obstruction: A Case Report.

Intragastric balloons (IGBs) are widely used as temporary endoscopic therapy for obesity, but rare complications can be catastrophic. A 55-year-old woman presented with severe epigastric pain, persistent vomiting, and progressive abdominal distension four months after the placement of a 650-mL Orbera intragastric balloon (Apollo Endosurgery, Austin, TX). She was hypotensive and tachycardic and presented with generalized peritonitis. Contrast-enhanced CT revealed marked gastric distension, antropyloric balloon impaction, absent mural enhancement along the lesser curvature, pneumoperitoneum, and

Transcatheter Edge-to-Edge Repair in Advanced Heart Failure and Ventricular Dysfunction: An Evolving Heart Team Strategy.

BACKGROUND: Patients with severe left ventricular (LV) dysfunction and mitral regurgitation are underrepresented in pivotal transcatheter edge-to-edge repair trials and are often ineligible for transplantation or LV assist devices, leaving limited options when guideline-directed medical therapy fails. CASE SUMMARY: We describe 6 patients aged 38 to 82 years with LV ejection fraction 10% to 20% and severe (4+) to torrential (5+) mitral regurgitation, 4 patients presenting in or near cardiogenic shock requiring inotropic, vasopressor, or intra-aortic balloon pump support. All underwent transcat

Antithrombotic strategies and outcomes in acute myocardial infarction-related cardiogenic shock requiring short-term mechanical circulatory support.

BACKGROUND: Cardiogenic shock following acute myocardial infarction (AMI-CS) is associated with high mortality. Short-term mechanical circulatory support (ST-MCS) is frequently used after percutaneous coronary intervention (PCI) to stabilize hemodynamics. However, balancing anticoagulation (AC) and antiplatelet therapy in this setting poses substantial bleeding risk. OBJECTIVES: This study aimed to evaluate real-world antithrombotic strategies and associated ischemic and hemorrhagic complications in AMI-CS patients receiving ST-MCS. METHODS: This retrospective, single-center study included a

Beyond Blood Pressure: The Role of Flow Parameters and Biomarkers of Hypoperfusion in Cardiogenic Shock.

Cardiogenic shock (CS) remains a heterogeneous syndrome with persistently high mortality across acute myocardial infarction, decompensated heart failure, and post-cardiotomy states. Blood pressure, the most commonly used marker of hemodynamic status, is the product of cardiac output and systemic vascular resistance and can be preserved by compensatory vasoconstriction even as flow and tissue perfusion deteriorate. This review examines the evidence for measuring flow-derived hemodynamic parameters and biomarkers of hypoperfusion, rather than relying on pressure alone, to risk-stratify and infor

Suicide Left Ventricle During Lung Transplantation in a Patient With Hereditary Hemochromatosis.

Suicide left ventricle (SLV) is a rare cause of hemodynamic collapse due to dynamic left ventricular outflow tract (LVOT) obstruction, usually after surgical or transcatheter aortic valve replacement. We report a 54-year-old man with hereditary hemochromatosis undergoing bilateral lung transplantation. Following the weaning of venoarterial extracorporeal membrane oxygenation, he developed acute severe hypotension refractory to norepinephrine, vasopressin, and epinephrine. Intraoperative transesophageal echocardiography demonstrated a small left ventricular cavity, systolic anterior motion of t

Mechanical CPR - Assisted Percutaneous Coronary Intervention During Cardiac Arrest: Feasibility and Outcomes.

Cardiac arrest complicating acute coronary syndromes requires immediate coronary reperfusion; however, performing percutaneous coronary intervention (PCI) during active manual cardiopulmonary resuscitation (CPR) presents significant technical challenges. The Lund University Cardiac Arrest System (LUCAS) provides automated mechanical chest compressions and enables uninterrupted CPR during coronary angiography and PCI. Existing evidence is primarily limited to case reports and small observational studies, mechanical CPR has emerged as a practical adjunct in catheterization laboratories where man

Temporary Mechanical Circulatory Support in Acute Myocardial Infarction-Related Cardiogenic Shock: Lessons from Contemporary Randomized Trials.

Background: Acute myocardial infarction-related cardiogenic shock (AMI-CS) remains associated with high mortality despite advances in revascularization, critical care, and temporary mechanical circulatory support (MCS). Recent randomized trials have challenged the concept of routine device-based therapy and highlighted the importance of matching MCS strategy to shock phenotype, timing, and patient risk profile. Methods: We reviewed contemporary randomized clinical trials and prespecified or post hoc analyses evaluating temporary MCS strategies in adult patients with AMI-CS. PubMed was searched

Exploring the triage accuracy of the Manchester Triage System and the Emergency Severity Index in emergency department patients with sepsis: A secondary retrospective analysis.

OBJECTIVE: To meet the requirement of the Sepsis-3 guidelines for rapid initiation of treatment, emergency department (ED) triage systems must assign patients with suspected sepsis to the highest-priority triage categories. Our aim was to explore how effectively the Manchester Triage System (MTS) and the Emergency Severity Index (ESI) assign these patients to appropriate urgency categories and avoid undertriage. METHODS: A secondary analysis was conducted from a prospective, multicenter observational study (LIFE-POC), which included patients presenting with suspected organ dysfunction in 1 of

Pancreatic Ascites Complicated by Salmonella Enteritidis Peritonitis: A Diagnostic Challenge and the Role of Surgical Source Control.

Pancreatic ascites is an uncommon complication of pancreatic disorders resulting from persistent leakage of pancreatic juice into the peritoneal cavity, most commonly due to disruption of the main pancreatic duct or rupture of a pancreatic fluid collection. Bacterial peritonitis complicating pancreatic ascites is rarely reported. We report the case of a 40-year-old man who presented with massive pancreatic ascites associated with a marked elevation of serum lipase. Analysis of the ascitic fluid strongly supported its pancreatic origin, and conservative management with enteral nutrition and a s

Differences in treatment and outcome of patients with ST- elevation myocardial infarction (STEMI) and non-STEMI in Germany.

BACKGROUND: Although ST-segment elevation myocardial infarction (STEMI) and non-STEMI (NSTEMI) are very similar regarding pathophysiology and clinical treatments, especially NSTEMI comprises a much more heterogenic group of patients and underlying diseases. We therefore aimed to assess the treatments and outcomes of both entities in a large contemporary cohort. METHODS: Patients with STEMI and NSTEMI between 01/01/2010-31/12/2018 were identified from the largest German health insurance (AOK, ≈ 26 million members). Patient demographics, their hospital course, adherence to guideline-directed dr

Prognostic discrimination of SOFA-2 vs. SOFA-1 and incremental information from cardiovascular scoring and vasoactive support intensity in patients with sepsis receiving early vasoactive therapy.

BACKGROUND: Patients with sepsis who require vasoactive support during the first 24 h of intensive care represent a heterogeneous population. It is uncertain whether the Sequential Organ Failure Assessment (SOFA)-2 score improves mortality discrimination compared with the SOFA-1 score and whether observed circulatory support intensity provides complementary prognostic information. METHODS: This multicohort observational study used Medical Information Mart for Intensive Care IV (MIMIC-IV) for a 24-h conditional landmark analysis. Patients alive and still in the ICU beyond 24 h were followed fr

Vascular norepinephrine responsiveness index for early risk stratification in emergency department septic shock: a real-world cohort study from a Latin American sepsis code registry.

BACKGROUND: Septic shock is characterized by heterogeneous vascular responsiveness to norepinephrine. The vascular norepinephrine responsiveness index (VNERi), calculated as diastolic arterial pressure divided by the product of norepinephrine dose and heart rate, has been proposed as a bedside risk-stratification marker. Its performance in routine emergency department care remains uncertain. METHODS: We conducted a single-center retrospective cohort study of adults with septic shock treated with norepinephrine in the emergency department from 2023 to 2025. The index time was the first protoco

The red blood cell distribution width-to-albumin ratio and 28-day mortality in patients with severe cardiogenic shock: a retrospective cohort study based on MIMIC-IV with single-center external validation.

BACKGROUND: Cardiogenic shock (CS) is a critical syndrome with a very high fatality rate, and in-hospital mortality ranges from 27% to 51%. Among the various prognostic approaches, serum biomarkers have attracted considerable attention because they are easy to measure, inexpensive, and can be monitored dynamically. The red cell distribution width-to-albumin ratio (RAR) is a novel composite marker that integrates red blood cell heterogeneity (reflecting inflammation and oxidative stress) with albumin levels (indicating nutritional and anti-inflammatory capacity). It has shown initial promise fo

Extracorporeal Cardiopulmonary Resuscitation as a Bridge to Cardiac Transplantation in Refractory Cardiac Arrest Due to Fulminant Myocarditis: A Case Report.

Extracorporeal cardiopulmonary resuscitation (ECPR) is increasingly used for refractory cardiac arrest, although evidence for a survival benefit remains heterogeneous and appears highly dependent on patient selection and systems of care. We report the case of a previously healthy 36-year-old woman who presented with acute fulminant myocarditis and cardiogenic shock. Following an in-hospital cardiac arrest unresponsive to conventional cardiopulmonary resuscitation, bedside ECPR was initiated using peripheral veno-arterial extracorporeal membrane oxygenation (VA-ECMO) as a bridge to longer-term

Case Report: Necrotizing fasciitis caused by Vibrio vulnificus without recalled seafood or seawater exposure diagnosed by metagenomic next-generation sequencing.

Vibrio vulnificus is a highly virulent marine Gram-negative pathogen that rapidly induces soft tissue necrosis and fatal septic shock, with infections generally linked to raw seafood ingestion or open wound contact with seawater. We report an atypical severe case of V. vulnificus septic shock in a 60-year-old male patient without recalled seafood or seawater exposure and without classic underlying high-risk diseases such as liver cirrhosis and diabetes mellitus. The patient presented with rapidly progressive swelling and subcutaneous ecchymosis of the right lower limb, which quickly deteriorat
