Predictive value of pre-procedural QTc interval for in-hospital major adverse cardiovascular events following primary percutaneous coronary intervention in acute myocardial infarction.

BACKGROUND: The relationship between pre-procedural corrected QT (QTc) interval and in-hospital major adverse cardiovascular events (MACE) following primary percutaneous coronary intervention (PCI) for acute myocardial infarction (AMI) remains incompletely understood. This study evaluated the predictive value of pre-procedural QTc for in-hospital MACE. METHODS: This single-center retrospective cohort study consecutively enrolled AMI patients undergoing primary PCI. QTc intervals were manually measured from pre-procedural 12-lead electrocardiograms using the Bazett formula, with a prespecified

Hantavirus cardiopulmonary syndrome: an emerging fatal zoonotic infection.

Hantavirus cardiopulmonary syndrome is a potentially fatal respiratory zoonotic infection caused by New World hantaviruses, and it is acquired mostly via inhaling aerosolized excreta from infected rodents. Over the past 30 years, HCPS has expanded beyond its initial foci, and recent events suggest a potential for spread beyond traditionally endemic regions. The disease rapidly progresses from a vague prodrome to acute cardiopulmonary failure with a high fatality rate despite optimum supportive treatment. Pathophysiology involves endothelial failure, increased vascular permeability, acute throm

Unexpected Cardiogenic Shock in a Young Woman: A Case Report.

Recent cardiogenic shock (CS) literature emphasizes early recognition, staging, and treatment to improve survival, highlighting the need for system-wide detection pathways and prompt shock team activation to enable timely intervention. We present a 26-year-old woman initially diagnosed with septic shock who deteriorated despite therapy. Ongoing reassessment led to reclassification as CS, prompting shock team activation and initiation of mechanical circulatory support, ultimately resulting in full recovery. CS is uncommon in young adults and may be misdiagnosed as distributive shock. Repeated e

Known Unknowns: Unpacking Controversies and Uncertainties in the 2025 Guidelines for Management of Acute Coronary Syndromes.

PURPOSE OF REVIEW: The 2025 ACC/AHA/ACEP/NAEMSP/SCAI Guideline for managing patients with acute coronary syndromes (ACS) is the first major update since the 2013-2015 ST-elevation myocardial infarction (STEMI) and non-ST elevated myocardial infarction (NSTEMI) guidelines. It brings both conditions together into a single framework. This review covers the most important new and revised recommendations and points out where evidence is still lacking. RECENT FINDINGS: New findings show that several significant updates are based on recent large-scale trials. The use of bivalirudin with high-dose in

Temporary Right Ventricular Mechanical Circulatory Support Devices in Acute Right Ventricular Failure and Cardiogenic Shock: A Cross-Device Systematic Review and Single-Arm Meta-Analysis.

BACKGROUND: Evidence for temporary right ventricular assist devices (tRVADs) is fragmented. We characterized outcomes among adults receiving temporary support for acute right ventricular (RV) failure or RV-predominant cardiogenic shock. METHODS: MEDLINE, Embase, CENTRAL, and ClinicalTrials.gov were searched from inception to June 2026. Arm-level outcome proportions were pooled on the logit scale using three-level random-effects models, with arms nested within studies. Device-stratified estimates, meta-regression, and sensitivity analyses were examined. RESULTS: We included 106 studies (119 d

A rapidly deteriorating pneumonia: when the clinical course does not fit.

Not all severe CAP is the same. Haemoptysis, cavitation and rapid deterioration should raise suspicion for Panton-Valentine leukocidin-driven disease. Early toxin-targeted therapy and recognition of mixed septic-cardiogenic shock may be key to survival. https://bit.ly/49KWALg.

Contemporary Management of Valvular Cardiogenic Shock Using Transcatheter Valve Interventions.

Valvular cardiogenic shock is an under-recognized, yet potentially reversible cause of hemodynamic collapse associated with high mortality. Rapid recognition, comprehensive hemodynamic assessment, and multidisciplinary Heart Team management are critical for successful treatment. Surgical intervention is often prohibitive because of hemodynamic instability, advanced comorbidities, prior cardiac surgeries, or multiorgan dysfunction. We report a case series of patients with diverse valvular pathologies presenting with cardiogenic shock who were treated with transcatheter valve interventions, incl

Initial V-A ECMO Flow-Index and Its Association With Inflammation, Organ Failure, and Outcomes.

BACKGROUND: Refractory cardiogenic shock remains associated with high mortality despite macrocirculatory stabilization with V-A ECMO. We investigated the association between initial ECMO flow-index, organ dysfunction, inflammatory activation, hemolysis, and clinical outcome. METHODS: In this retrospective single-center study, 553 patients receiving V-A ECMO between 2014 and 2024 were divided into low-flow (< 1.63 L/min/m2, n = 269) and high-flow (≥ 1.63 L/min/m2, n = 284) groups. Patients were additionally stratified according to eCPR status. Multivariable logistic regression was used to asse

Parallel Venovenous and Venoarterial Extracorporeal Membrane Oxygenation for Differential Titration of Cardiopulmonary Support in Combined Severe Cardiac and Respiratory Failure: A Case Report.

Parallel venovenous (VV) and venoarterial (VA) extracorporeal membrane oxygenation (ECMO) can allow separate titration of respiratory and circulatory support in patients with concurrent severe cardiopulmonary failure. A 64-year-old woman with central hypopituitarism developed cytomegalovirus infection with hemophagocytic lymphohistiocytosis, followed by severe Pseudomonas aeruginosa pneumonia and septic cardiomyopathy. Despite VV-ECMO and intra-aortic balloon pump support, refractory cardiogenic shock required the addition of VA-ECMO. After parallel ECMO was initiated, hemodynamics improved ra

Inflammation, Infarct Size and 10-Year Mortality in STEMI Patients Stratified by the ORBI Risk Score: A DANAMI-3 Sub-study.

BACKGROUND: The ORBI Risk Score predicts in-hospital cardiogenic shock after primary percutaneous coronary intervention (PCI) for ST-elevation myocardial infarction (STEMI). Whether high-risk patients also have more systemic inflammation, larger infarcts and higher long-term mortality is not known. METHODS: In a DANAMI-3 sub-study, 1,574 STEMI patients without cardiogenic shock were classified as high (ORBI ≥8, n = 273) or low risk (n = 1,301). Inflammation was assessed by routine C-reactive protein (CRP), leukocyte count and neutrophil-to-lymphocyte ratio (NLR) within 72 hours of PCI, and ad

Fat Emboli Entrapped in the Tricuspid Valve Chordae Following Autologous Fat Grafting: An Echocardiographic-Pathological Correlation.

Fat embolism is a rare but feared complication of liposuction combined with autologous fat grafting (lipofilling). It carries a high mortality rate, predominantly affecting young patients with few or no comorbidities. To date, few guidelines exist, and management remains largely supportive. We report the case of a 40-year-old woman admitted to our center following cardiac arrest during breast lipofilling surgery. Given persistent hemodynamic instability and echocardiographic evidence of acute right ventricular overload with an intracardiac mass, veno-arterial extracorporeal membrane oxygenatio

Drug coated balloon vs. drug eluting stent in calcified de novo coronary lesions: Long term outcomes from the SPARTAN registry.

AIM: Coronary artery calcification presents major challenges during PCI, including device deliverability, suboptimal lesion preparation, impaired stent expansion, increased restenosis, stent thrombosis and mortality. Drug-coated balloons (DCB) offer a stent-less alternative, but evidence in de novo calcified lesions is limited. This study compared long-term clinical outcomes between DCB and drug-eluting stent (DES) strategies in such lesions. METHODS: We retrospectively identified all patients undergoing PCI for operator reported angiographically calcified de novo lesions between January 2015

The Non-invasive Hemodynamic Profile: A Modern Approach to the Evaluation and Management of Acute Decompensated Heart Failure-Related Cardiogenic Shock with Point-of-Care Ultrasound.

Emergency department presentations of acute decompensated heart failure-related cardiogenic shock (ADHF-CS) are increasing and are associated with high short-term mortality. Early diagnosis is challenging due to variable clinical presentation, compensatory physiology, and limited availability of invasive hemodynamic monitoring. Point-of-care ultrasound enables quick, noninvasive assessment of cardiac output, right atrial pressure, and systemic vascular resistance. This review outlines a practical bedside echocardiographic approach to generate a rapid hemodynamic profile in suspected ADHF-CS, f

Extracorporeal cardiopulmonary resuscitation for cardiogenic shock in a Jehovah's witness.

Veno-arterial (V-A) and veno-venous (V-V) extracorporeal membrane oxygenation (ECMO) are frequently employed in the management of cardiopulmonary failure in critically ill patients. Utilization, however, is controversial in the Jehovah's Witness (JW) population as temporary mechanical circulatory support (tMCS) typically is associated with blood loss requiring transfusions. Despite this fact, use of ECMO is still feasible in this population while avoiding blood transfusions and there have been few reported cases in the literature. However, its use as part of cardiopulmonary resuscitation is li

Mechanical circulatory support in hypertrophic cardiomyopathy with cardiogenic shock: A National Inpatient Sample analysis.

BACKGROUND: Cardiogenic shock in patients with obstructive hypertrophic cardiomyopathy (HCM) is highly lethal. Temporary mechanical circulatory support (MCS), including percutaneous ventricular assist devices such as Impella and veno-arterial extracorporeal membrane oxygenation (VA-ECMO), is increasingly used in refractory cardiogenic shock. However, data regarding outcomes in this population remain limited. METHODS: We performed a retrospective cohort study using the National Inpatient Sample from 2016 to 2022. All adult patients with obstructive HCM complicated by cardiogenic shock were inc
