Explainable Machine Learning Identifies Red Blood Cell Distribution Width as a Key Prognostic Biomarker in Cardiogenic Shock.

Cardiogenic shock (CS) is a life-threatening syndrome characterized by severe pump failure and systemic hypoperfusion, with persistently high mortality. Red blood cell distribution width (RDW) has been associated with adverse outcomes in several cardiovascular conditions, but its prognostic role in CS, particularly for long-term mortality and machine learning-based risk stratification, remains incompletely defined. We aimed to investigate the association between RDW and mortality in patients with CS and to evaluate its value in interpretable machine learning models for 1-year mortality predict

Prognostic value of the atherogenic index of plasma combined with lactate-to-albumin ratio for in-hospital mortality in patients with acute myocardial infarction complicated by cardiogenic shock.

BACKGROUND: Acute myocardial infarction complicated by cardiogenic shock (AMICS) carries high in-hospital mortality. We evaluated the combined prognostic value of the Atherogenic Index of Plasma (AIP), reflecting lipid metabolism, and the Lactate-to-Albumin Ratio (LAR), reflecting tissue hypoperfusion and inflammation, in AMICS. METHODS: This retrospective cohort included 207 consecutive patients with AMICS between January 2023 and January 2025, classified as survivors (n = 174) or non-survivors (n = 33). Admission clinical, laboratory, echocardiographic, and treatment variables were collecte

Optimal Surgical Timing After Postinfarction Ventricular Septal Rupture: A Narrative Review With a Proposed Heart Team Decision Algorithm.

Postinfarction ventricular septal rupture is a rare, frequently lethal mechanical complication of acute myocardial infarction, and the optimal timing of repair remains controversial. Early surgery is often unavoidable in refractory shock but is technically hazardous because necrotic, friable myocardium provides poor suture support. Delayed repair after tissue maturation is associated with lower operative mortality, although this apparent benefit is substantially inflated by survivorship and immortal time bias. Mechanical circulatory support, including intra-aortic balloon counterpulsation, ven

Socioeconomic Disparities in Patients With Cardiogenic Shock Treated With Extracorporeal Support.

Venoarterial extracorporeal membrane oxygenation (VA-ECMO) is a resource-intensive therapy that can be used in severe cardiogenic shock (CS). Socioeconomic factors are known determinants of cardiovascular outcomes, yet their association with mortality among patients receiving mechanical circulatory support remains unclear. We conducted a retrospective cohort study using the Healthcare Cost and Utilization Project National Inpatient Sample, including adult hospitalizations with CS receiving VA-ECMO between 2018 and 2022. Exposure was Zone Improvement Plan (ZIP) code-level median household incom

Type 1 Kounis Syndrome Presenting as Coronary Pseudostenosis During Status Asthmaticus.

BACKGROUND: Kounis syndrome is an allergic acute coronary syndrome that may produce multivessel coronary vasospasm, angiographically mimicking obstructive coronary artery disease without atherosclerosis. CASE SUMMARY: A 46-year-old atopic man admitted for status asthmaticus developed inferior ST-segment elevation myocardial infarction, complete heart block, ventricular fibrillation, and cardiogenic shock on hospital day 3. Emergent angiography revealed multivessel vasospasm with apparent stenoses up to 90%. Cardiac magnetic resonance showed ischemic injury without myocarditis. Repeat angiogra

Individualized Or Conventional Transfusion Strategy During Veno-Arterial Extracorporeal Membrane Oxygenation Support (ICONE Study): Rationale and Design of a Prospective Multicenter Randomized Controlled Trial.

BACKGROUND: Red blood cell (RBC) transfusion practices during veno-arterial extracorporeal membrane oxygenation (VA-ECMO) are heterogeneous, and the benefit - harm balance remains uncertain. Current recommendations rely on predefined hemoglobin thresholds and do not provide criteria for individualized transfusion. As the primary goal of RBC transfusion is to optimize oxygen delivery (DO₂) to meet oxygen consumption (VO₂), a strategy guided by a surrogate of DO₂/VO₂ mismatch - central venous oxygen saturation (ScvO₂)- may be more appropriate than fixed liberal transfusion thresholds. METHODS:

Disentangling objects' contextual associations from perceptual and conceptual attributes using time-resolved neural decoding.

IMPORTANCE: Currently, mortality risk for patients with ST-segment elevation myocardial infarction (STEMI) treated with primary percutaneous coronary intervention (PCI) with an uncomplicated postprocedure course is low. Less is known regarding the risk of in-hospital ventricular tachycardia (VT) and ventricular fibrillation (VF). OBJECTIVE: To evaluate the risk of late VT and VF after primary PCI for STEMI. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included adults aged 18 years or older with STEMI treated with primary PCI between January 1, 2015, and December 31, 2018, identified

Association of Renal Impairment Severity with Surgical Outcomes in Patients with Infective Endocarditis.

INTRODUCTION: This study aimed to assess the association of renal impairment (RI) severity on short and mid-term outcomes in patients undergoing cardiac surgery for infective endocarditis (IE). METHODS: Patients undergoing cardiac surgery for IE between January 2010 and October 2022 were included. They were stratified based on preoperative renal function into four groups: Normal (N: Creatinine clearance (CrCl) >85 mL/min), moderate RI (M: CrCl 51-85 mL/min), severe RI (S: CrCl ≤50 mL/min), and haemodialysis-dependent (H). Each group was compared with group N. Survival analysis was performed u

Variation in mechanical circulatory support use for acute myocardial infarction cardiogenic shock.

BACKGROUND: Cardiogenic shock (CS) is a leading cause of mortality following acute myocardial infarction (AMI). Some patients may require intra-aortic balloon pump (IABP) or percutaneous ventricular assist device (PVAD) placement; however, there is a paucity of standardised algorithms to guide the deployment of each device. The present study evaluated interhospital variation in the use of IABP and PVAD for AMI CS and identified institutional factors associated with hospital-level device preference. METHODS: All non-elective adult hospitalisations entailing AMI and CS were identified within th

Impact of high-grade atrioventricular block on outcomes in patients with acute myocardial infarction.

AIMS: Contemporary data on the impact of high-grade atrioventricular blocks (HAVB) in acute myocardial infarction (AMI) are scarce. Therefore, we investigated this in the Swiss national AMI registry (AMIS Plus) cohort. METHODS AND RESULTS: We included all AMI patients enrolled from January 2005 to September 2024 with available ECG information. Primary outcome was in-hospital mortality; secondary outcomes included in-hospital major adverse cardiac or cerebrovascular events (MACCE) defined as cardiogenic shock, stroke, re-infarction or death. Among 50 279 AMI patients, 747 (1.5%) patients prese

EACTS Expert Consensus Document on protected cardiac surgery: pre-emptive use of temporary mechanical circulatory support in adult cardiac surgery patients at high risk for perioperative low cardiac output syndrome.

Perioperative low cardiac output syndrome (LCOS) remains a significant complication in adult cardiac surgery, contributing to substantial morbidity, prolonged intensive care, and increased mortality. Its incidence is expected to rise further due to the evolving complexity of referred surgical patients, often characterised by advanced age, multiple comorbidities, challenging anatomy, and impaired haemodynamics. Despite advances in pharmacological and perioperative care, outcomes for high-risk patients have not shown significant improvement, prompting interest in temporary mechanical circulatory

DanGer shock criteria and outcomes in acute myocardial infarction-related cardiogenic shock treated with Impella: the J-PVAD registry.

BACKGROUND AND AIMS: The DanGer shock trial showed a survival benefit of Impella in highly selected patients with ST-elevation myocardial infarction-related cardiogenic shock (STEMI-CS). In real-world settings, however, Impella is often used in broader populations not meeting trial criteria. This study aimed to assess the distribution and outcomes of DanGer shock-eligible and -ineligible acute myocardial infarction-related cardiogenic shock (AMI-CS) patients in a nationwide Japanese registry. METHODS: A total of 3975 AMI-CS patients treated with Impella between 2020 and 2023 were identified f

Traditional Chinese Medicine in the Treatment of Diabetic Cardiomyopathy: A Comprehensive Review.

Diabetic cardiomyopathy (DCM), a global cardiovascular complication of diabetes, is characterized by concurrent diastolic and systolic ventricular dysfunction that progressively leads to heart failure, arrhythmias, and cardiogenic shock. Despite advancements in modern therapeutics, DCM continues to exhibit high mortality rates, underscoring the critical need for novel preventive and therapeutic strategies. In recent years, traditional Chinese medicine (TCM) has gained prominence in DCM management due to its established safety profile and emerging evidence of clinical efficacy. Current research

Rationale for and approach to establishing a multidisciplinary acute pulmonary embolism expert care team.

Patients with acute pulmonary embolism (PE) may present with cardiac arrest, overt or impending cardiogenic shock and/or severe respiratory insufficiency. Immediate evaluation and management of these patients require high clinical suspicion along with (bedside) imaging to confirm the diagnosis, targeted haemodynamic and/or respiratory support, appropriate anticoagulant treatment, and in many cases reperfusion therapy. The immediate treatment decision-making is largely driven by local expertise and resources and should be guided by the individual patient's characteristics such as cardiopulmonar

Hospital outcomes of patients with acute myocardial infarction-related cardiogenic shock with and without revascularization.

OBJECTIVES: An early invasive strategy is recommended for patients with acute myocardial infarction-related cardiogenic shock (AMICS). However, data on outcomes of patients undergoing early coronary angiography (CA) without subsequent percutaneous coronary intervention (PCI) are limited. The authors examined the characteristics and outcomes of patients with AMICS who underwent early CA and were treated with or without acute PCI. METHODS: The authors analyzed data from the prospective Arbeitsgemeinschaft Leitende Kardiologische Krankenhausärzte (ALKK) CA registry. Patient characteristics, indi
