Young adults in the cardiac intensive care unit: insights from the Critical Care Cardiology Trials Network registry.

AIMS: While the aging cardiac intensive care unit (CICU) population has been well studied, young adults represent a distinct and underexplored subgroup. We aimed to characterize the demographics, clinical presentations, resource utilization, and outcomes of young adults admitted to contemporary CICUs. METHODS AND RESULTS: We analysed consecutive adult CICU admissions from 2018 to 2023 within the Critical Care Cardiology Trials Network, a multicentre registry of advanced CICUs in North America. Patients aged 18-39 years were classified as 'young adults' and compared with those aged ≥40 years.

Renal Recovery after Heart Transplantation in Patients With Type 1 Cardiorenal Syndrome Requiring Dialysis: Insights From a Cohort Without Mechanical Circulatory Support.

BACKGROUND: Type 1 cardiorenal syndrome requiring renal replacement therapy (RRT) commonly occurs in patients with advanced heart failure and cardiogenic shock. However, the prognostic impact of RRT in heart transplant (HT) candidates remains unclear, particularly in resource-limited settings without routine access to durable mechanical circulatory support (MCS). METHODS: We retrospectively analyzed a prospective cohort of adult HT recipients at a single Brazilian university hospital between April 2014 and November 2021. Patients with irreversible chronic kidney disease were excluded. Outcome

Cardiogenic Shock.

Cardiogenic shock is characterized by depression of cardiac function that leads to low blood pressure, coronary ischemia, and further decreased cardiac contractility resulting in tissue hypoxemia. The condition is associated with high early mortality, approaching 50%, which is largely influenced by the underlying etiologic factors. In infarct-related cardiogenic shock, rapid restoration of coronary blood flow substantially reduces mortality. Mechanical circulatory support devices offer hemodynamic stabilization and improved outcomes in carefully selected patients, although optimal patient sele

Association of Glycemic Gap with In-Hospital Adverse Outcome after First Attack of Acute ST Elevation Myocardial Infarction in Thrombolysed Patients.

Stress-induced hyperglycemia (SIH), reflected by an elevated glycemic gap, has been associated with poor outcomes in acute coronary syndromes. This study aimed to evaluate the association between glycemic gap and in-hospital adverse outcomes in patients experiencing their first attack of acute ST-elevation myocardial infarction (STEMI) who were managed with thrombolytic therapy. This cross-sectional comparative study was conducted at Mymensingh Medical College Hospital, Mymensingh, Bangladesh from October 2019 to March 2021. In this study 287 first-time STEMI patients were included who underwe

The rise of point-of-care ultrasound in cardiopulmonary diagnostics.

Point-of-care ultrasound (POCUS) has rapidly evolved from a diagnostic adjunct into an essential extension of bedside clinical reasoning in acute cardiovascular care. By providing immediate, physiologically grounded, and non-invasive information, POCUS enhances diagnostic accuracy, risk stratification, and therapeutic guidance in real time. Among its core applications, lung ultrasound enables reliable detection and monitoring of pulmonary congestion, outperforming traditional methods such as chest X-ray and physical examination. The Venous Excess Ultrasound Score offers a structured assessment

Heart-gut interaction in veno-arterial extracorporeal membrane oxygenation: a forgotten axis in critical care perfusion.

The expanding use of veno-arterial extracorporeal membrane oxygenation (VA-ECMO) for cardiogenic shock and post-cardiotomy support necessitates a focus on perfusion beyond traditional targets. The gastrointestinal tract (GIT) is particularly vulnerable, yet its dysfunction is often overlooked in ECMO management. This editorial synthesizes current literature and pathophysiological principles to analyze the mechanisms of GIT injury during VA-ECMO support. We examine the impact of VA-ECMO on hemodynamics, microcirculation, and inflammation, with a focus on the heart-gut axis. VA-ECMO compromises

Extracorporeal membrane oxygenation for acute decompensated heart failure: Encouraging long term outcomes.

BACKGROUND: Cardiogenic shock (CS) secondary to acute decompensated heart failure (ADHF) is a life-threatening condition that may require the use of mechanical circulatory support (MCS). Venoarterial (VA) extracorporeal membrane oxygenation (ECMO) represents one of the first-line temporary MCS options. METHODS: We conducted a single-center, retrospective observational cohort study of adult patients with CS secondary to ADHF that required VA ECMO at our institution from 2014 to 2023. Patients requiring extracorporeal cardiopulmonary resuscitation (ECPR) or initially placed on venovenous (VV) E

Clinical outcomes associated with failed thrombus aspiration during primary percutaneous coronary intervention.

Thrombus aspiration is performed during primary percutaneous coronary intervention (PCI) to improve outcomes in patients with ST-elevation myocardial infarction (STEMI). However, its success varies, and failed aspiration may impact prognosis. This study aimed to determine the predictors and clinical outcomes associated with failed thrombus aspiration. This retrospective study included 5323 patients who underwent primary PCI for STEMI. Patients were categorized into failed and successful thrombus aspiration groups. Baseline characteristics, angiographic findings, and procedural details were com

Fluoroscopy-Free Axillary Impella 5.5 Implantation, Earlier Support With Just Transesophageal Echocardiography Images.

Delays in mechanical circulatory support (MCS) for cardiogenic shock can be critical. However, timely access to a hybrid operating room or C-arm fluoroscopy for urgent axillary Impella 5.5 (Abiomed, Danvers, MA) implantation remains a barrier. To address this, we developed a fluoroscopy-free axillary Impella 5.5 implantation technique using transesophageal echocardiography (TEE) guidance. From January 2023 to November 2024, 19 adults underwent the procedure. A preoperative flowchart ensured appropriate selection, with the right axillary artery as the preferred access. Transesophageal echocardi

Investigating biomarkers associated with mortality in patients receiving VA-ECMO for cardiogenic shock: a systematic review.

CONTEXT: Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is a life-sustaining therapy for severe refractory cardiogenic shock. Although VA-ECMO provides various degrees of cardiopulmonary support, mortality rates remain high. Serum biomarkers have potential to identify the rising risk of mortality in patients receiving ECMO, but evidence supporting their prognostic value is inconsistent. OBJECTIVES: This study aims to systematically investigate existing evidence on the relationship between biomarkers and mortality in adult patients with refractory cardiogenic shock undergoing VA-E

Left Ventricular Sarcoma Causing Dynamic Outflow Tract Obstruction and Cardiogenic Shock: A Case Report.

BACKGROUND: Primary cardiac sarcomas are rare and aggressive tumors that often present with nonspecific symptoms, typically diagnosed at advanced stages. Obstruction of the left ventricular outflow tract (LVOT) due to these tumors is an exceptionally rare and life-threatening complication. CASE PRESENTATION: We describe the case of a previously healthy 33-year-old male who presented with progressive exertional dyspnea, orthopnea, and hemoptysis. Upon admission, he quickly developed hemodynamic instability and respiratory failure. Point-of-care transthoracic echocardiography revealed a large i

Takotsubo Cardiomyopathy and Cardiogenic Shock in a 16-Year-Old With Vaping-Induced Pneumonitis.

BACKGROUND: Takotsubo cardiomyopathy (TC) is an uncommon diagnosis, particularly in the pediatric population. The midventricular subtype and presentation with cardiogenic shock are even more unique; the recognition of TC is critical for appropriate management. CASE SUMMARY: We present the case of a 16-year-old young woman who presented with cardiogenic shock. Cardiac magnetic resonance (CMR) confirmed left ventricular dilation, reduced systolic function, and myocardial edema, which are highly consistent with a diagnosis of TC, likely as a consequence of e-cigarette or vaping product use-assoc
