[Hypovolaemic shock due to increased capillary permeability (author's transl)].

In a 69-year-old man admitted to hospital for shock with symptoms suggesting acute cor pulmonale, laboratory tests showed low proteinaemia associated with haemoconcentration. Vascular filling led to recovery, but the condition relapsed 6 months later, heralded by weight increase, swelling of the face and profuse swelling; the patient again recovered. On this last occasion, a monoclonal peak of IgG kappa thought to have antibody activity was discovered. These attacks of hypovolaemic shock seem to be due to sudden increases in capillary permeability.

[Fat embolism as a complication in shock. Clinical and therapeutic aspects (author's transl)].

In 36 patients the changes of the haemostasis in relation to hypovolaemic shock period were studied after polytrauma and or surgical operations. It was shown that a correlation between the hypovolaemic shock period and the degree of haemostatic disorder exists. The significance of this interaction for the manifestation of the fat embolism syndrome was demonstrated in two casuistics. It is concluded that the haemostatic disorder in shock enhances the development of the fat embolism syndrome. As a therapeutic consequence adequate volume substitution beside specific anticoagulation seems to be of

Blood transfusion therapy in the newborn.

This review deals with the various indications, the choice of blood products and the main aspects of their administration for transfusing neonates. Some special problems peculiar to neonatal age, that both neonatologists and blood transfusion services have to take into account, are emphasized. Exchange transfusion in the procedure most frequently used in blood transfusion therapy of neonatal hyperbilirubinaemia of various aetiology, severe anaemia and hyperviscosity due to polycythaemia. The procedure also represents a rational therapeutic approach in the bleeding thrombocytopenic newborn. Mor

Hypovolaemic shock with particular reference to the myocardial and pulmonary lesions.

Amongst 207 post mortem examinations of patiens dying as a result of haemorrhage, there were 102 with histological manifestations of the shock syndrome. Shock lesions in the lung were found in 50% of cases, and in the heart shock lesions were found in 39.2%. Shock lesions in the heart were the commonest cause of shock that proved refractory to restoration of the blood volume. The pulmonary lesions of shock developed later than the cardiac lesions and in this series were the commonest cause of death.

Spontaneous rupture of the liver in pregnancy.

A case of spontaneous rupture of the liver in pregnancy, fatal to mother and child, is described. The patient, a multiparous, 36-year-old diabetic, presented in the 27th week of pregnancy with upper abdominal pains resembling biliary dyskinesia. After 8 hours' observation in the hospital she suddenly developed hypovolaemic shock and fetal heart tones disappeared. On acute cesarean section a dead fetus was delivered. Hemoperiteoneum was present, arising from a huge subcapsular hematoma of the right lobe of the liver, with rupture of Glisson's capsule. The hematoma was evaluated and tears in the

Thalamonal, droperidol and fentanyl in induced hypovolaemic shock in the conscious dog.

Previous studies had shown that Thalamonal exerts a protective activity against hypovolaemic shock in experimental animals as well as in patients. The purpose of the present study was to make a long-term evaluation of the value of single doses of Thalamonal fentanyl and droperidol as shock-protecting agents in conscious dogs which received no replacement of blood loss. The study was conducted in 20 animals in which devices were implanted for measuring heart rate, arterial and venous blood pressure, left ventricular and left atrial blood pressure. The dogs were divided into 4 groups, receiving

Extensive gangrene of the bowel after haemorrhagic shock: a case report.

A 20-year-old man presented with an acute abdominal condition within 15 hours after having sustained severe extra-abdominal injuries with associated hypovolaemic shock. At laparotomy virtually the whole bowel was found to be ischaemic and perforation had occurred at the site of maximum ischaemia. No trauma to the bowel, mesenteric vessels or any other abdominal organ was noted, and it was concluded that the ischaemia had resulted from a prolonged splanchic vasospasm in an attempt to compensate for hypovolaemic shock. The condition is well recognized in animals, but has rarely been recorded in

Treatment of refractory hypovolaemic shock by 7.5% sodium chloride injections.

Injections of hyperosmotic (7.5%) sodium chloride (100-400 ml) were given to 12 patients in terminal hypovolaemic shock who had not responded to vigorous volume replacement and corticosteroid and dopamine infusions. Hyperosmotic sodium chloride promptly reversed the shock in 11 of these patients. The immediate effects of the NaCl injections were a moderate rise in arterial pressure, the resumption of urine flow, and recovery of consciousness. These effects tended to persist for a few hours. The hyperosmotic infusion also reduced isosmotic fluid requirement by 90%.

Reactivity of the cerebrocortical vasculature and energy metabolism to direct cortical stimulation in haemorrhagic shock.

Direct electrical stimulation of the cerebral cortex was used to determine the changes of cortical carbohydrate and oxidative metabolism and of vascular reactivity during haemorrhagic shock. The results were as follows. 1. Electrical stimulation of the brain cortex applied in the control period led to a marked vasodilation and NAD reduction that was preceded in part of the experiments by a transient NADH oxidation. It is suggested that the increase in cortical NADH fluorescence observed during direct stimulation is due to the fact that the rate of cytoplasmic NADH production exceeded the rate

[Arterial blood supply of the liver].

We examined (dogs) the influence of hypovolaemic shock and reversible interruption of the portal vein on the arterial blood flow and metabolism of the liver. Depending on the arterial mean-pressure hypovolaemic shock decreases the flowrate of the hepatic artery, while it is increases to 30 to 40% during interruption of the portal vein. Whether hypoxy will cause liver damage or not is only dependant from the resulting oxygen supply of the liver. This critical range is below 6 to 7 ml/min per 100 g liver tissue.

Sequential patterns of haemodynamic and metabolic changes in experimental hypovolaemic shock. I. Responses to acute haemorrhage.

Little is known about cardiorespiratory changes during the development of hypovolaemia. This study attempts to provide such information and compares the period of bleeding with that of established hypovolaemia. Eleven anaesthetized and ventilated greyhounds were bled and analyses of cardiopulmonary function made at fixed intervals both during and after haemorrhage. Six sequential patterns of cardiopulmonary and metabolic change were recognized. It was apparent that bleeding caused the first three phases of change, recovery from the effects of bleeding the next two and steady hypovolaemia the l

Sequential patterns of haemodynamic and metabolic changes in experimental hypovolaemic shock. II. Responses to reinfusion of shed blood.

The haemodynamic and metabolic effects which follow the infusion of blood in experimental hypovolaemia have not been studied in detail. The 10 dogs that survived 90 min of hypovolaemia in a study of bleeding (Pardy and Dudley, 1979) were investigated during and shortly after the reinfusion of shed blood using the same techniques. Data from 9 animals were suitable for analysis. As with bleeding, 6 sequential phases were identified; reinfusion of blood was completed in the fifth phase. Initial reinfusion was associated with a rapid improvement in haemodynamic and metabolic status, although mean

Hypovolaemic shock with oedema due to increased capillary permeability.

A case of severe hypovolaemic shock related to idiopathic oedema was observed in a 37 year old woman. Large plasma volume expansion (nearly 12 1 over 9 hours) did not change the clinical status. Haemodynamic studies showed low cardiac index (1.1 1/min/m2), decreased left ventricular stroke work index (6.7 gm/m2), and high systemic arterial resistance (52 mmHg/1/min/m2). Dopamine infusion improved the haemodynamic condition which returned to normal 30 hours after the beginning of shock. After recovery, capillary permeability measured by a modification of Landis' method was markedly increased. A

[Gastro-duodenal ulcers with pregnancy. Condition of shock in labour due to intestinal haemorrhage from an ulcer (author's transl)].

We present the case of a multiparous woman of 32 years of age who, at term after a more or less normal pregnancy, went into severe shock in labour. The patient in this case was very difficult to diagnose. Her life was only saved after massive blood transfusions and after delivering a fresh stillborn infant. It was not a case of uterine rupture nor of retro-placental haemorrhage. The site of origin of the hypovolaemic shock was intestinal. The patient had an ulcer of the pylorus which was diagnosed after fibroscopy. Some authors state that it appears that the few ulcers that have been diagnosed

Neonatal anaemia secondary to blood loss.

The newborn has a limited capacity to tolerate acute haemorrhage. Prompt diagnosis and therapy are essential for survival. Blood loss can occur in the newborn due to occult haemorrhage, prior to birth or during delivery, obstetric accidents, maternal haemorrhage, or secondary to recurrent blood sampling. Special precautions must be taken when events known to cause fetal haemorrhage have occurred during pregnancy. These include a maternal history of transfusion reaction in the absence of a transfusion, third trimester bleeding with placenta praevia, placenta abruptio and vasa praevia, emergency

Intramyocardial gas tensions in the canine heart.

Intramyocardial gas tensions were determined in the left ventricular wall of the canine heart by means of chronically implanted silastic tonometers. In the central zone of the myocardial wall the mean baseline PO2 was 22 mmHg and the mean baseline PCO2 28 mmHg. Breathing of pure oxygen elevated the myocardial PO2 to a level of 60 to 90 mmHg. Hypovolaemic shock induced by shedding 30% of the blood volume resulted in a sharp decrease of the myocardial PO2 and a marked increase of the myocardial PCO2. During hypovolaemia, ventilation with pure oxygen elevated the oxygen tension to the control lev
