Passive cutaneous anaphylaxis and vasoactive amine challenge in nude (nu/nu) mice. A comparison with nu/+, axenic and young normal animals.

Recent suggestions of a thymic origin or thymo-dependent differentiation of tissue mast cells prompted an investigation in the athymic nude (nu/nu) mouse. The outbred nu/nu examined were found to possess mast cells in at least comparable numbers to the phenotypically normal nu/+. These nu/nu were superior to nu/+ as recipient for mast cell-dependent, long latent period (IgE-type), passive cutaneous anaphylactic (PCA) reactions. A variety of studies performed, including competition with nu/+ serum, thymic restoration, quantitation of nu/+ homocytotropic antibody responses, comparison with axeni

[Effects of experimental hypovolaemic shock on platelets in the pig (author's transl)].

The effects of standardized blood loss on the platelets, as well as on coagulation, fibrinolysis and complement parameters were studied in the pig. A simultaneous fall in arterial and venous platelet counts was demonstrated during the initial phase of blood loss, with a concomitat rise in pulmonary vascular resistance. After exsanguination to the extent of about 40% of the blood volume, the arterial platelet counts increased significantly, whilst the pulmonary vascular resistance, which was negatively correlated with the arterial platelet counts throughout the experiment, fell. Venous platelet

Tissue oxygenation in hypovolaemic shock.

Tissue hypoxia is an essential feature in the pathophysiology of hypovolaemic shock. In traumatized patients gross injury and haemorrhage may induce both general and local oxygen deficiency. Prolonged hypoxia can induce irreversible changes in tissues, inhibit wound healing, and increase susceptibility to infection. Correction of cardiac index and arterial PO2 does not necessarily ensure a normal tissue PO2, and therefore, arterial oxygen tension provides an inadequate index of peripheral tissue oxygenation. There are two methods currently available for clinical measurements of tissue gases: 1

[Stress ulcer-an experimental study (author's transl)].

This experimental study demonstrates by means of electromagnetic flow measurements that hypovolaemic shock results in a significant decrease in gastric flow. 14 mongrel dogs were subjected to haemorrhagic shock for 3 or 4 hours. Significant histological changes are seen in the gastric mucosa as a result of the haemodynamic changes, especially when the duration of shock was 4 hours. All stages of stress ulcer from the superficial erosion to deep ulcers were documented. The possibility that the mast cell plays a pathogenetic role as mediator in the origination of a stress ulcer is considered.

Management of bleeding disorders in traumatic-haemorrhagic shock states with deep frozen fresh plasma.

Coagulation disorders in traumatic-haemorrhagic shock need not represent a simple coagulation problem. They may also occur as a complex of local and disseminated intravascular consumption, dilution, extravascular loss and depressed synthesis of coagulation factors. In the severely bleeding patient with a haemorrhagic diathesis heparin is contrainedicated because it does not normalize coagulability immediately. Therefore, it fails to stop haemorrhage and the shock becomes untreatable. Fresh frozen plasma, however, has proved to be suitable as a simultaneous substitution therapy for the coagulat

[Non-ketosic hyperosmolar hyperglycemic coma. Case reports and therapeutic consideration].

Non-ketonic hyperosmolar hyperglycaemic coma (N.K.H.H.C.) is by no means uncommon in diabetes. Its picture includes sensorial depression, hyperglycaemia, hyperazotemia, marked dehydration and plasma hyperosmolarity. It is mostly found in elderly subjects with non-serious diabetes. Reference is made to 6 personal cases observed during a period of 14 months. The incidence of N.K.H.H.C. noted during this period was 2.2%; this was higher than that of ketoacidotic coma. Two patients died from hypovolaemic shock and one from septic complications. Three survived the episode. Treatment was based on th

[The effect of hypovolaemic shock on the pancreas (author's transl)].

Morphological changes similar to those found in acute pancreatitis were found at autopsy of ten patients who had died following temporary hypovolaemic shock. In an experimental study it was demonstrated by means of electromagnetic flow measurements that hypovolaemic shock results in a significant decrease in pancreatic flow, which is paralleled by a significant decrease in oxygen consumption of the organ in spite of a compensatory increase in the arterio-venous oxyhaemoglobin difference. These haemodynamic changes are followed by hypoxic cell damage mainly in the periphereal areas of the pancr

Surface electron microscopical changes of the gastric mucosa in experimental porcine stress ulceration.

Gastric mucosal changes in 8 piglets subjected to a transient hypovolaemic shock were studied using scanning electron microscopy (SEM). The mucosal lesions during the shock were characterized by progressive degenerative changes in the mucous membrane of the stomach. These surface changes included flattening or swelling of the individual cells. The flattening of the mucous membrane was later connected with red cell diapedesis through it. These changes were most typical in the areas of haemorrhagic gastritis and preceded the ulcer formation. Following retransfusion, the affected epithelial linin

The decisive period in the primary infection of muscle by Escherichia coli.

Escherichia coli superinfected with a non-replicating phage was used to study the course of infection in the adductor muscle of mice in terms of the content of viable bacilli and the in vivo multiplication rate. The infection was characterized by a steady decrease of bacterial numbers to 5% of the inoculum within 4 hours without any compensating multiplication. Within the next 3 hours there was slight multiplication (2 generation or less) followed by a slow decrease of numbers to nil in 72 hours. In terms of the viable count, the infection was temporarily enhanced between three- and eight-fold

Sympatho-adrenal hyperactivity--the key to irreversible shock?

There would appear sound evidence on which to incriminate sustained sympatho-adrenal hyperactivity as a deleterious factor of prime importance in hypovolaemic shock. The use of sympathomimetics in the treatment of shock of this type is irrational and deleterious. Their rational use in those few cases of clinical shock where they are indicated will have to await knowledge of plasma catecholamine levels in such cases. Experimental studies involving α- and β-adrenergic blockade, unilaterally and in combination, have revealed that, contrary to previous theories, deterioration is mediated through b

Volume expansion and flow promotion in shock.

Volume replacement is the most important step in treating hypovolaemic shock. Blood is needed when the oxygen carrying capacity threatens to fall below a critical level, but has the disadvantage of transmitting virus hepatitis. Anicteric hepatitis is about four times more frequent than the icteric form. Pasteurized plasma protein solution and albumin are free from the risk of transmitting hepatitis virus, and are good volume restorers. Dextran 70 represents the best artificial colloid with additional anti-thrombotic properties. Dextran 40 is indicated in special situations to promote flow. The
