Trends in mechanical circulatory support utilization, left ventricular assist device implantation and transplant during cardiogenic shock hospitalizations, after the new heart allocation policy.

BACKGROUND: In October 2018, a new heart allocation policy was implemented to risk stratify patients listed for transplant, prioritizing patients supported with temporary mechanical circulatory support (MCS). The policy changes may have had an impact on the management of cardiogenic shock (CS). We sought to determine the changes in use of temporary MCS, durable left ventricular assist device (LVAD) and transplant in patients hospitalized before and after the new policy. METHODS: A retrospective analysis was conducted using the National Inpatient Sample (NIS) between 2017 and 2020. Hospitaliza

Predictors and regional prevalence of food insecurity in Ethiopia during COVID-19: a multilevel analysis.

INTRODUCTION: Food insecurity is one of the most serious issues, especially in developing countries, that harm many public health outcomes through increased under nutrition, mental health problem, and premature mortality. It is widespread socio-economic problem of Ethiopia, with unequal distribution among its regions, during COVID-19 and other shock event manifestations for the last three years. This study aimed to analyse country-wise and region-specific food insecurity prevalence; assess its variation among regions; and identify predictors that influenced households' food insecurity in Ethio

Shock wave-driven phase transition of iron sulphide for enhanced photocatalytic application: a combined experimental and DFT approach.

In the present investigation, we systematically investigate how acoustic shock waves induce a phase transition from FeS to α-Fe2O3 and how this transition influences the material's structural, morphological, optical, and photocatalytic properties. The findings from X-ray diffraction (XRD), scanning electron microscopy (SEM), ultraviolet differential reflectance spectroscopy (UV-DRS), and X-ray photoelectron spectroscopy (XPS) studies unequivocally demonstrate a complete phase transition from FeS to α-Fe2O3 at 600 shock pulses. The control sample has an uneven morphology. The material exhibits

Polarity-Dependent, Reduced or No-Output Shocks in Implantable Cardioverter-Defibrillators.

Recently, polarity-dependent shock failures were reported in implantable cardioverter-defibrillators caused by structural failure in the high-voltage feedthrough. Short circuits may occur when the right ventricular coil is cathodal for phase 1 of biphasic shocks (cathodal shock). This viewpoint proposes a mechanism for observed polarity dependence and considers whether the same mechanism may apply in other shock-induced, short circuits. Implantable cardioverter-defibrillator connections to the lead traverse feedthroughs into the hermetically sealed housing ("Can"). The feedthrough comprises 2

Discriminant ability of the shock index, modified shock index, and reverse shock index multiplied by the Glasgow coma scale on mortality in adult trauma patients: a PATOS retrospective cohort study.

BACKGROUND: The shock index (SI) predicts short-term mortality in trauma patients. Other shock indices have been developed to improve discriminant accuracy. The authors examined the discriminant ability of the SI, modified SI (MSI), and reverse SI multiplied by the Glasgow Coma Scale (rSIG) on short-term mortality and functional outcomes. METHODS: The authors evaluated a cohort of adult trauma patients transported to emergency departments. The first vital signs were used to calculate the SI, MSI, and rSIG. The areas under the receiver operating characteristic curves and test results were used

Vasopressin in Sepsis and Other Shock States: State of the Art.

This review of the use of vasopressin aims to be comprehensive and highly practical, based on the available scientific evidence and our extensive clinical experience with the drug. It summarizes controversies about vasopressin use in septic shock and other vasodilatory states. Vasopressin is a natural hormone with powerful vasoconstrictive effects and is responsible for the regulation of plasma osmolality by maintaining fluid homeostasis. Septic shock is defined by the need for vasopressors to correct hypotension and lactic acidosis secondary to infection, with a high mortality rate. The Survi

Serum Metabolomic Profiles in Critically Ill Patients with Shock on Admission to the Intensive Care Unit.

Inflammatory processes are common in intensive care (ICU) patients and can induce multiple changes in metabolism, leading to increased risks of morbidity and mortality. Metabolomics enables these modifications to be studied and identifies a patient's metabolic profile. The objective is to precise if the use of metabolomics at ICU admission can help in prognostication. This is a prospective ex-vivo study, realized in a university laboratory and a medico-surgical ICU. Metabolic profiles were analyzed by proton nuclear magnetic resonance. Using multivariable analysis, we compared metabolic profil

Wavelet-Based Transmissibility for Structural Damage Detection.

Short-time, abrupt events-such as earthquakes and other shock loadings-often lead to damage that is difficult to detect in structures using output-only vibration measurements. The time-variant transmissibility is proposed to tackle this problem. The approach is based on two-dimensional wavelet power spectra. The time-frequency transmissibility and relevant coherence function are used for structural damage detection in structural elements in buildings. Numerical simulations and experimental tests are used in these investigations. The results are compared with the classical transmissibility and

Natural killer cells induce HIV-1 latency reversal after treatment with pan-caspase inhibitors.

The establishment of a latency reservoir is the major obstacle for a cure of HIV-1. The shock-and-kill strategy aims to reactivate HIV-1 replication in HIV -1 latently infected cells, exposing the HIV-1-infected cells to cytotoxic lymphocytes. However, none of the latency reversal agents (LRAs) tested so far have shown the desired effect in people living with HIV-1. We observed that NK cells stimulated with a pan-caspase inhibitor induced latency reversal in co-cultures with HIV-1 latently infected cells. Synergy in HIV-1 reactivation was observed with LRAs prostratin and JQ1. The supernatants

Identifying cardiogenic shock in the emergency department.

INTRODUCTION: Cardiogenic shock is difficult to diagnose due to diverse presentations, overlap with other shock states (i.e. sepsis), poorly understood pathophysiology, complex and multifactorial causes, and varied hemodynamic parameters. Despite advances in interventions, mortality in patients with cardiogenic shock remains high. Emergency clinicians must be ready to recognize and start appropriate therapy for cardiogenic shock early. OBJECTIVE: This review will discuss the clinical evaluation and diagnosis of cardiogenic shock in the emergency department with a focus on the emergency clinic

Thiamine (vitamin B1) in septic shock: a targeted therapy.

Thiamine (vitamin B1) is a water-soluble vitamin essential for human health. Thiamine deficiency is causal and/or contributory in a number of debilitating diseases including beri-beri, the Wernicke-Korsakoff syndrome, optic neuropathy, and others. While thiamine deficiency is relatively rare in developed nations as a result of dietary supplementation, thiamine deficiency is more common in nutritionally compromised populations. Thiamine pyrophosphate, a thiamine derivative, is essential to the citric acid cycle and thiamine deficiency can result in impaired aerobic respiration and cellular ener

Simulation of nonlinear trans-skull focusing and formation of shocks in brain using a fully populated ultrasound array with aberration correction.

Multi-element high-intensity focused ultrasound phased arrays in the shape of hemispheres are currently used in clinics for thermal lesioning in deep brain structures. Certain side effects of overheating non-targeted tissues and skull bones have been revealed. Here, an approach is developed to mitigate these effects. A specific design of a fully populated 256-element 1-MHz array shaped as a spherical segment (F-number, F# = 1) and filled by randomly distributed equal-area polygonal elements is proposed. Capability of the array to generate high-amplitude shock fronts at the focus is tested in s
