Non-invasive preoperative MRI-based deep transfer learning radiomics model for predicting meningioma brain invasion.

BACKGROUND: Current preoperative assessment of meningioma brain invasion (BI) demands imaging analytical frameworks that optimize computational efficiency while capturing multidimensional tumor-pathology correlations. Building upon the inherent advantages of 2.5D MRI in synthesizing multiplanar biological data with manageable processing loads, this study innovatively integrates deep transfer learning (DTL), a technique bridging pretrained network knowledge with target domain adaptation, to systematically identify tumor-invasive biological signatures and develop an optimized predictive system u

Reference-signature projection framework for exercise transcriptomic directionality based on the IAV-DI reference axis.

BACKGROUND: Exercise is often described as medicine, but exercise and pharmacological interventions cannot be treated as clinically equivalent. This study developed a Reference Signature Projection Framework (RSPF) to evaluate the directional relationship of external disease and exercise transcriptomic responses relative to a predefined disease-treatment reference axis. The empirical analysis in this study was restricted to the IAV-DI reference axis defined by dimethyl itaconate (DI) treatment in the context of influenza A virus (IAV) infection. METHODS: (1) Reference-signature construction:

Quadrant training: a simple typology-based approach to improve strength and endurance in youth soccer players.

INTRODUCTION: Youth soccer players show heterogeneous performance profiles, yet individualized conditioning is often impractical outside professional settings. We examined whether a typology-based training allocation into four quadrants ("Quadrant Training"; QT) improves endurance- and strength-related performance compared with regular soccer training alone. METHODS: Male U16-U17 players from two youth academies completed a non-randomized, longitudinal controlled intervention (academy-level and intervention effects are confounded); one academy served as intervention group (n = 38), the other

A rare collision tumor of paraganglioma and intravascular large B-cell lymphoma: a case report and literature review.

Intravascular large B-cell lymphoma (IVLBCL) is a rare and aggressive subtype of extranodal diffuse large B-cell lymphoma. It is characterized by the selective proliferation of lymphoma cells within the lumina of small blood vessels. Collision tumors involving IVLBCL are exceedingly rare, and no cases involving paraganglioma as a collision component have been reported to date. We report the case of an 80-year-old man who presented with nonspecific gastrointestinal symptoms and intermittent fever. Imaging studies suggested biliary and intra-abdominal malignancies. Surgical resection revealed a

Retrospective evaluation of sirolimus (mTOR blocker) for lymphatic malformations presenting in the neonatal period: a single-center experience.

BACKGROUND: Lymphatic malformations are rare vascular anomalies. Currently, there is no uniform approach to the treatment of these patients. Neonates with extensive disease often suffer from multiple complications, organ dysfunction, and various tissue lesions, which may be life-threatening and lead to aesthetic deformity. Treatment with sirolimus has the potential to improve symptoms and downsize the lesion, up to complete regression. This retrospective single-center study presents our experience with oral sirolimus in the treatment of newborns with lymphatic malformations, evaluating its eff

Biomimetic sequential targeted nanoplatform for synergistic thrombolysis in ischemic stroke.

Conventional thrombolytic drugs for vascular blockage such as tissue plasminogen activator (tPA) are challenged by the low bioavailability, off-target side effects and limited penetration in thrombi, leading to delayed recanalization. Herein, we developed a sequential targeted nanoplatform for efficient thrombolysis based on the thrombolysis-neuroprotection integrated strategy. The mesoporous silica layer-modified upconversion nanoparticle (UCNP) was loaded with the nitric oxide (NO) donors (BNN6), further coating platelet membranes (PMs) on the surface of UCNP. tPA was conjugated onto PMs usi

Femoral-to-Carotid Shunting During Carotid-Access TAVR in Moyamoya-Pattern Vasculopathy.

BACKGROUND: Carotid access is an alternative for transcatheter aortic valve replacement (TAVR) in patients with prohibitive transfemoral anatomy, but it may cause cerebral hypoperfusion with impaired intracranial collateral circulation. CASE SUMMARY: A 67-year-old woman with symptomatic severe aortic stenosis, extensive comorbidities, and unsuitable transfemoral access underwent TAVR. Neuroimaging demonstrated moyamoya-pattern intracranial vasculopathy with limited collateral reserve. During left carotid exposure, test occlusion caused a >50% decline in cerebral near-infrared spectroscopy des

Thermal convective and flow dynamics of blood trihybrid nanofluid inside a stenosis artery for cardiovascular disease treatment using artificial intelligence approach.

This article briefly discusses the influence of heat radiation and heat source on the convective flow of a blood-based trihybrid nanofluid inside a stenotic artery with chemical reaction. It can assist in investigating and predicting blood flow dynamics and heat transmission in constricted arteries for thermal therapy, targeted drug delivery, and cardiovascular disease treatment. By replicating the intricate interactions of many nanoparticles with blood, the model also aids in the design and optimization of biomedical devices like stents and artificial organs. The AI technique also makes it po

Cyclosporine A and Romiplostim as a Bridging Therapy to Curative Surgery in Aplastic Anemia Complicated by Gastric Cancer: A Case Report.

We present a case of aplastic anemia (AA) complicated with gastric cancer that was successfully treated with cyclosporine A (CsA) and romiplostim (ROMI) as a bridging therapy before curative surgery. This patient had untreated gastric cancer at the time of AA diagnosis, and surgery was difficult because of thrombocytopenia. However, CsA and ROMI were effective, resulting in a rapid increase in the platelet count, thereby making surgery for gastric cancer possible. CsA and ROMI as a bridging therapy to curative surgery may therefore represent a feasible treatment option in patients with AA comp

Transformers for fMRI analysis: from spatiotemporal modeling to brain foundation models.

Functional magnetic resonance imaging (fMRI) serves as a pivotal tool for investigating the functional organization and dynamic connectivity of the human brain, indirectly reflecting neural activity through blood oxygen level-dependent (BOLD) signals. However, the high-dimensional spatiotemporal structure, prominent noise interference, and inter-individual variability of fMRI data pose significant challenges for traditional modeling methods in characterizing complex brain functional patterns. In recent years, the Transformer architecture has gained significant traction in fMRI data analysis, l

Diagnostic yield of reflex multiplex PCR after a negative rapid antigen test in children hospitalized with presumed viral acute respiratory tract infection: a prospective cohort study.

BACKGROUND: Acute respiratory infections, mostly viral, are a leading cause of childhood hospitalization. Rapid antigen tests are fast but cover few viruses; multiplex PCR detects many more. The value of reflex PCR after a negative antigen test is uncertain. AIMS: To compare the diagnostic yield of antigen testing alone and with reflex multiplex PCR, and to describe the viral spectrum, age associations, and antibiotic exposure. METHODS: In this single-center prospective cohort, 245 children hospitalized with presumed viral acute respiratory infection (October 2022-October 2023) underwent a f

Renal artery imaging through long-term follow-up after radiofrequency renal denervation: Results from the SPYRAL Clinical Program.

Background: Renal denervation (RDN) safely reduces blood pressure (BP) in patients with uncontrolled hypertension. A potential long-term concern is the development of de novo renal artery stenosis (RAS) secondary to vascular injury. This study assesses the long-term vascular safety and incidence of RAS in patients treated with radiofrequency RDN from the SPYRAL HTN-OFF MED and -ON MED trials. Patients and methods: All patients underwent renal angiograms for assessment of the renal anatomy prior to randomization. Patients with RAS >50% were excluded from enrollment. At long-term follow-up (439+

Conjugate electrospun fully inorganic SiO2/kaolin 3D nanofiber sponges for rapid hemostasis.

Uncontrolled bleeding remains a major challenge in trauma and surgical care. Herein, fully inorganic SiO2/kaolin (SiO2/K) nanofiber sponges were fabricated by conjugate electrospinning followed by high-temperature calcination. The three-dimensional architecture provided an interconnected porous network, low bulk density, and high water uptake. The optimized S-SiO2-5K sponge exhibited favorable cytocompatibility and hemocompatibility, with hemolysis below 5% under the tested conditions. Whole-blood coagulation assays and clotting time measurements supported its in vitro hemostatic performance,

Regional blood-brain barrier dysfunction maps to lower performance in cognitive ageing.

Age-related cognitive decline is a common and growing public health challenge, associated with loss of independence, reduced quality of life, increased caregiver burden and substantial healthcare and societal costs. Yet cognitive decline in ageing is heterogeneous, and current clinical constructs do not reliably capture the underlying biological mechanisms or predict progression to dementia. This cross-sectional study examined whether blood-brain barrier dysfunction (BBBD) provides a biologically supported correlate of cognitive performance beyond clinical diagnosis. The study included 46 pati
