The diagnostic efficacy of venous-phase spectral CT combined with Node-RADS for differentiating enlarged lymph nodes in common solid tumors: a preliminary exploratory study.

OBJECTIVE: To investigate the value of combining quantitative parameters from spectral CT with the Node Reporting and Data System 1.0 (Node-RADS) in distinguishing between benign and malignant lymph nodes enlarged due to solid tumors, and to explore a combined multiparametric approach. METHODS: We retrospectively analyzed data from patients with primary solid tumors accompanied by lymphadenopathy who underwent venous-phase spectral CT scans with energy-resolved imaging. A total of 243 lymph nodes were included in this study. Based on pathological findings, clinical observations, and CT follow

Organ-Confined Giant Renal Cell Carcinoma: A Case Report and Literature Review.

Giant renal cell carcinoma (RCC) is a rare clinical entity with no universally accepted definition, typically used to describe exceptionally large renal tumors. With the widespread use of cross-sectional imaging, such massive tumors are increasingly uncommon. Tumor size is generally considered a prognostic factor; however, its correlation with biological behavior remains variable. We report the case of a 53-year-old female who presented with progressive abdominal distension over two years, associated with recent-onset abdominal discomfort and constipation. Clinical examination revealed a large

Successful Rescue Therapy for Talaromycosis Complicated by Severe Acute Hepatorenal Failure with Hepatic Encephalopathy in an HIV/HCV-Coinfected Patient: A Case Report.

BACKGROUND: Treatment of critically ill patients with Talaromyces marneffei (T. marneffei, TM) infection involving multiple organ dysfunction remains a major clinical challenge. As an important contributor to mortality among individuals with Acquired Immunodeficiency Syndrome (AIDS), talaromycosis remains associated with mortality rates of up to 30% even with appropriate antifungal therapy. Severe hepatic and renal impairment may restrict antifungal dosing and thus further compromise therapeutic efficacy. However, current clinical evidence regarding the management of Talaromyces marneffei infe

Corticosteroid-responsive portal lymphadenopathy mimicking malignant progression induced by furmonertinib in EGFR-mutated NSCLC: a case report.

BACKGROUND: Third-generation epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) are widely used in the treatment of NSCLC with EGFR mutations due to their efficacy and favourable safety profile. However, drug-induced liver injury (DILI) associated with targeted therapy may occasionally present with atypical clinical and radiological features, posing diagnostic challenges. Here, we present a rare case of severe hepatotoxicity accompanied by pseudo-progressive portal lymphadenopathy that occurred following furmonertinib treatment. CASE PRESENTATION: A 59-year-old female pat

Case Report: Metagenomic next-generation sequencing diagnosed a rare case of sternal tuberculosis mimicking a malignant tumour.

This is a case report of a 17-year-old female patient who presented with a painless, palpable swelling on the anterior chest wall. Imaging studies revealed osteolytic lesions involving the manubrium and adjacent ribs, along with multiple enlarged lymph nodes, raising a high suspicion of malignant tumour with metastasis. An ultrasound-guided needle biopsy revealed the pathological finding of "granulomatous inflammation." Multidisciplinary consultation and clinical indicators, including a strongly positive purified protein derivative (PPD) test and markedly elevated erythrocyte sedimentation rat

A Case of Mediastinal Abscess Diagnosed by Endoscopic Ultrasound.

Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) plays an important role in the diagnosis of benign and malignant mediastinal lesions. However, it is mostly used for the biopsy of consolidated lesions and enlarged lymph nodes. This paper presents a case of mediastinal abscess diagnosed by endobronchial ultrasound-guided puncture.

A Case of Systemic Lupus Erythematosus Initially Presenting With Generalized Lymphadenopathy: A Diagnostic Challenge.

Lymphadenopathy (LAD) can emerge as an initial manifestation of systemic lupus erythematosus (SLE); however, establishing an accurate differential diagnosis remains challenging due to the wide spectrum of infectious, malignant, and autoimmune conditions associated with this presentation. We report the case of a 47-year-old man admitted with a two-month history of persistent fever (approximately 38 °C), night sweats, chills, and diffuse generalized lymphadenopathy. Physical examination revealed enlarged, mobile, non-tender, soft cervical, axillary, and inguinal lymph nodes measuring approximate

Molecularly Variant Synovial Sarcoma of the Lower Lip: MN1::SSX1 Fusion as a Diagnostic Pitfall in the Head and Neck.

Synovial sarcoma is a malignant mesenchymal neoplasm typically characterized by SS18::SSX gene fusions. Although most commonly arising in the extremities, it rarely occurs in the head and neck region. We report a case of monophasic synovial sarcoma of the lower lip harboring a rare MN1::SSX1 fusion that presented a diagnostic challenge due to negative standard molecular testing. A 23-year-old woman presented with a slowly enlarging right lower lip mass. Imaging revealed a well-defined subcutaneous lesion without mandibular invasion and mildly prominent bilateral cervical lymph nodes. Core biop

Post-operative liver metastasis from gastric cancer with HER2-positive areas newly identified upon re-evaluation, leading to a favorable response to trastuzumab-based therapy: A case report.

Recent advances in molecular targeted agents and immune checkpoint inhibitors have markedly improved outcomes in patients with unresectable or recurrent advanced-stage gastric cancer. As treatment options continue to expand, biomarker testing has become essential for determining first-line treatment strategies. Among these, human epidermal growth factor receptor 2 (HER2) plays a key role in treatment selection; however, the intratumoral heterogeneity of HER2 expression in gastric cancer has long been recognized and may influence therapeutic decision-making. The present study describes the case

Salvage Surgery Following Definitive Chemoradiotherapy and Immune Checkpoint Inhibitor Therapy for Locally Advanced Thymic Carcinoma.

Surgical resection improves prognosis for thymic carcinoma. Recent phase II trials have indicated the efficacy of immune checkpoint inhibitor therapy in unresectable disease. Here, we report a case of salvage surgery following immune checkpoint inhibitor therapy for an initially unresectable, locally advanced thymic carcinoma in a 67-year-old woman. Computed tomography revealed an anterior mediastinal mass and enlarged anterior mediastinal lymph nodes, diagnosed as an unresectable thymic epithelial tumour, due to suspected invasion into the left main pulmonary artery. Late Cervical Recurrence of Invasive Lobular Carcinoma Ten Years After Primary Breast Cancer: A Case Report and Review of the Literature.

Invasive lobular carcinoma (ILC) accounts for approximately 15% of breast cancers and the most common neoplasm in the female population. Cervical involvement is exceptionally rare and often underrecognized. This relationship is well-defined in the context of breast and ovarian cancer syndrome related to BRCA gene mutations. However, it is also observed in rare but underreported cases of cervical metastases originating from breast cancer. The objective of this manuscript is to describe a rare case of cervical recurrence of invasive lobular carcinoma and summarize comparable case to guide future

Hypercalcemia associated with kidney disease: it is not always what you think-a case report.

BACKGROUND: Hypercalcemia is a disorder resulting either from impaired calcium renal reabsorption, excessive bone resorption or intestinal absorption, or a combination of these conditions. It can lead to both acute kidney injury and chronic kidney disease, which may develop in case of prolonged or recurrent hypercalcemia. Therefore, the coexistence of hypercalcemia and kidney damage requires prompt recognition of predisposing factors. CASE REPORTS: Case 1 shows a 63-year-old white male with renal impairment and hypercalcemia who presented with weight loss, general malaise, and muscle pain. Re
