Catamenial diabetic ketoacidosis in a rural area of Kananga, Democratic Republic of the Congo: a case report.

Diabetic ketoacidosis (DKA) is a life-threatening acute complication of diabetes mellitus most commonly triggered by infection, metabolic stress, or inadequate insulin therapy. We report an unusual case of a young woman with type 1 diabetes who experienced recurrent episodes of ketoacidosis during the premenstrual period, suggesting a catamenial form of DKA. This case highlights the importance of considering hormonal fluctuations related to the menstrual cycle in diabetes management and in identifying factors of imbalance. The patient presented with abdominal pain, severe fatigue, and polyuria

[Clinical presentation and diagnostic procedures in canine hypoadrenocorticism].

ABSTRACT: Hypoadrenocorticism (HA) is a rare endocrinopathy in dogs that may occur in a primary form, characterized by a combined deficiency of glucocorticoids and mineralocorticoids or isolated glucocorticoid deficiency, or in a secondary form with an isolated glucocorticoid deficiency. In addition, iatrogenic HA may also occur. The primary form represents the most frequent type of the disease. Clinical signs are variable and often nonspecific, ranging from lethargy, inappetence, vomiting, and weight loss to polyuria/polydipsia or acute, life-threatening adrenal crises associated with hypovol

Primary hyperparathyroidism in a 4-month-old German Shepherd mixed breed dog.

A 4-month-old female-intact German Shepherd mixed breed dog was evaluated for polyuria, polydipsia, oral discomfort, tachypnea, and pyrexia. Blood testing documented total and ionized hypercalcemia with an increased parathyroid hormone concentration. Computed tomography identified bone abnormalities of the mandibule and mild, diffuse pulmonary changes. Ultrasonography of the neck documented one mildly enlarged parathyroid gland that was surgically removed. Histopathology indicated diffuse chief cell hyperplasia. The ionized calcium concentration normalized within 7 days postoperatively.

Hyperglycemic Hyperosmolar State As the Initial Presentation of Type 1 Diabetes Mellitus in an Adolescent: A Case Report.

Hyperglycemic hyperosmolar state (HHS) is a rare but life-threatening hyperglycemic emergency in children and adolescents. It is characterized by severe hyperglycemia, hyperosmolality, and dehydration, with absent or limited ketoacidosis. Although more commonly associated with type 2 diabetes mellitus, HHS may also occur as the initial presentation of type 1 diabetes mellitus in pediatric patients. We report the case of a previously healthy 13-year-old boy presenting with a three-week history of polydipsia and nocturnal polyuria. Initial evaluation revealed profound hyperglycemia (>1,000 mg/dL

Risk of Suboptimal Serum Sodium Correction According to Etiology in Severe Hyponatremia.

INTRODUCTION: Achieving an appropriate serum sodium correction rate in hyponatremia is essential but remains challenging. Given that the rate of correction in hyponatremia may vary by underlying etiology, understanding this relationship could help optimize clinical management, but it remains insufficiently explored. To address this, we aimed to evaluate whether the risk of suboptimal correction varies across different etiological categories. METHODS: This retrospective observational study included patients with severe hyponatremia (serum sodium level ≤120 mEq/L) at admission or during hospita

Disappearance of Posterior Pituitary Hyperintensity in Arginine Vasopressin Resistance: A Case Report.

The evaluation of arginine vasopressin resistance (AVP-R) and arginine vasopressin deficiency (AVP-D), respectively, previously referred to as nephrogenic and central diabetes insipidus, often includes brain magnetic resonance imaging (MRI). Its purpose is to assess pituitary structural changes and identify the physiological T1 hyperintensity of the posterior pituitary, present in most individuals. This hyperintensity reflects vasopressin storage, which shortens the T1 relaxation time. Its absence supports a diagnosis of AVP-D over AVP-R. A 13-year-old boy was referred to endocrinology in his

A standardized fraction of Cajanus scarabaeoides attenuates diabetic nephropathy via antiglycation, anti-inflammatory, and anti-fibrotic mechanisms.

Diabetic nephropathy is the critical microvascular complication of diabetes and a leading cause of end-stage renal disease. Its pathophysiology incriminates complex and interconnected mechanisms driven by hemodynamic and metabolic alterations, accompanied with glycation, inflammation, oxidative stress, and fibrosis. Targeting these cues through natural interventions may preserve kidney functions. The study aimed to investigate the renoprotective potential of a standardized ethyl acetate fraction of C. scarabaeoides (EAFCS), enriched with flavonoids including isoorientin, orientin, vitexin, iso

Diabetes Insipidus as a Rare Complication of Metastatic Breast Cancer With Pituitary Metastases.

Diabetes insipidus (DI) is a rare but significant and life-threatening complication of metastatic breast cancer, typically arising when cancer spreads to the pituitary gland. This condition results from impaired secretion or action of antidiuretic hormone (ADH) or arginine vasopressin (AVP), leading to symptoms including excessive urination and thirst. The posterior lobe of the pituitary and the infundibulum are most susceptible to metastatic involvement due to their direct arterial blood supply, making them more accessible to circulating tumor cells. This phenomenon explains why DI can be a m

Arginine vasopressin deficiency revealing a disseminated human chorionic gonadotropin-secreting germinoma.

Arginine vasopressin deficiency (AVP-D) is an uncommon disorder caused by deficient vasopressin production due to hypothalamic or pituitary gland dysfunction resulting in hypotonic polyuria, whose etiology oftentimes remains elusive. We present the case of a 20-year-old male patient evaluated for polyuria and polydipsia. Laboratory evaluation demonstrated hypernatremia, elevated serum osmolality, and inappropriately dilute urine with symptomatic improvement following initiation of desmopressin, consistent with AVP-D. Initial pituitary magnetic resonance imaging (MRI) showed a small enhancing l

Characterizing Longitudinal Molecular Changes in ctDNA in Patients with Metastatic Castration-Resistant Prostate Cancer.

Polydipsia and xerostomia are the most common complications that seriously affect oral health in patients with diabetes. However, to date, there is no effective treatment for diabetic xerostomia. Recent studies have reported that artesunate (ART) and metformin (Met) improve salivary gland (SG) hypofunction in murine Sjögren's syndrome. Therefore, aim of this study was to investigate the effect and underlying mechanism of artesunate (ART) alone and in combination with metformin (Met) on hyposalivation in type 2 diabetes mellitus (T2DM) rats. T2DM rats were induced using a high-fat diet and stre

The conundrum of drug development in higher-risk MDS: lessons learned from recently failed phase 3 clinical trials.

Aside from allogeneic transplantation, the current standard-of-care approach for higher-risk myelodysplastic syndromes/neoplasms (HR-MDS) remains monotherapy with a hypomethylating agent (HMA), including azacitidine, decitabine, or oral decitabine/cedazuridine. Many attempts using HMA-based combinations have failed to improve upon HMA monotherapy. Although promising efficacy was observed in early-phase clinical trials with several agents, subsequent randomized phase 3 trials failed to confirm improvements in complete response rates or overall survival. In this review, we discuss lessons learne

Copeptin as a Marker for Vasopressin Dysregulation and Diagnosis.

BACKGROUND: Arginine vasopressin (AVP) is a neuroendocrine hormone essential for fluid balance, vascular tone, and the endocrine stress response. However, its clinical measurement is limited by instability and technical challenges. Copeptin, the C-terminal segment of the AVP precursor, is secreted in equimolar amounts with AVP and serves as a stable, measurable surrogate. CONTENT: This mini-review outlines the synthesis, release, and physiological roles of AVP and copeptin, emphasizing their relevance in endocrine regulation. It highlights copeptin's diagnostic utility in differentiating AVP-

A review of the neuropharmacological profile of schedule-induced polydipsia.

Schedule-induced polydipsia (SIP) arises when subjects consume excessive amounts of water during interval responding on operant tasks, a paradigm introduced and mostly developed in rats. Neuropharmacological studies conducted on SIP have mainly shown a prominent role of dopamine and 5HT in this adjunctive behavior. In particular, D1/D2 receptor antagonists decrease SIP, whereas low doses of dopamine reuptake blockers increase SIP. There is more specific evidence of a decline in SIP after lesions of the mesolimbic dopamine pathway. A decline in SIP is likewise observed after the injection of 5H

Lactobacillus paracasei fermentation enhances the aroma profile and antidiabetic efficacy of goji berry juice.

Goji berry (Lycium barbarum L.) is rich in bioactive compounds, and its functional efficacy can be further enhanced through fermentation. This study evaluated the impact of Lactobacillus paracasei-fermentation on the quality attributes of goji berry juice and its in vivo antihyperglycemic efficacy. Fermentation significantly increased total flavonoids (+31.8%) and polysaccharides (+5.4%), enhanced antioxidant activity, and enriched the volatile aroma profile relative to unfermented juice. In a type II diabetic mouse model, four weeks of gavage with fermented goji berry juice alleviated weight

Effects of GLP-1 receptor agonists on copeptin in euvolemic participants.

OBJECTIVE: Glucagon-like peptide-1 (GLP-1) plays an important modulatory role in sodium and water homeostasis. Recent studies demonstrated that long-term treatment with GLP-1 receptor agonists (RAs) reduces fluid intake and urine output. To the best of our knowledge, no direct effect of GLP-1 on vasopressin has been observed. This secondary analysis aimed to investigate changes in copeptin levels in euvolemic participants treated with the GLP-1 RA dulaglutide versus placebo. DESIGN: Secondary analysis of two randomized, double-blind, placebo-controlled, cross-over trials in 34 patients with p
