Structural Characterization and Hypoglycemic Effects of a Purified Polysaccharide From Cinnamomum cassia.

A purified polysaccharide, designated CCP1A, was extracted and isolated from Cinnamomum cassia bark using ultrasonic-assisted composite enzymatic extraction followed by DEAE-52 and Sephadex G-100 column chromatography. CCP1A had a molecular weight of 8.161 kDa and was primarily composed of glucose and galacturonic acid, with minor amounts of rhamnose, arabinose, galactose, xylose, and mannose. Spectroscopic analyses (FT-IR and 1H NMR) confirmed the coexistence of α- and β-glycosidic linkages and a pyranose ring structure. The Congo red assay indicated a stable helical conformation, and transmi

Mitigation of Hepatic and Pancreatic Injury by Spondias tuberosa Extract Through Oxidative Enzyme Modulation and TGF-β Downregulation.

Diabetes mellitus (DM) is a chronic metabolic disorder marked by hyperglycemia due to impaired insulin secretion and/or action. Conventional therapies are often limited by side effects, highlighting the need for alternative treatments. Natural products have gained interest as promising therapeutic agents. Spondias tuberosa Arruda (umbu), traditionally used in Brazilian folk medicine, exhibits pharmacological potential against metabolic diseases. This study evaluated organoprotective and antioxidant effects of S. tuberosa extract (ExSt) in streptozotocin (STZ)-induced diabetic rats through clin

HNF1A-Maturity-Onset Diabetes of the Young (MODY) in a Family With a Rare Silent Variant: Clinical Challenges and Variable Expressivity.

Maturity-onset diabetes of the young (MODY) is a monogenic form of diabetes characterized by autosomal dominant inheritance, typically occurring during adolescence or early adulthood. Mutations in the hepatocyte nuclear factor 1-α (HNF1A) gene are among the most common causes of MODY and are frequently misdiagnosed as type 1 (T1D) or type 2 diabetes (T2D), leading to inappropriate management. We report the case of a preschool-aged boy who presented with polyuria and polydipsia, fasting hyperglycemia, negative pancreatic autoantibodies, and an elevated C-peptide level. A multi-generational fami

Youth with type 2-diabetes: clinical findings at onset and impact of continuous glucose monitoring (CGM) on glycemic control and disease management.

OBJECTIVES: The global obesity pandemic is associated with a higher prevalence of youth-onset type 2 diabetes (T2D). Continuous glucose monitoring (CGM) is established in type 1 diabetes, however, evidence regarding its impact in pediatric T2D is limited. This study has analyzed (a) clinical and metabolic characteristics in adolescents at T2D onset and (b) the impact of CGM on glycemic control, patient satisfaction and disease management in youths with T2D. METHODS: a) Clinical and biochemical parameters at diagnosis were assessed in adolescents with T2D. b) The impact of 8 weeks of CGM on Hb

An illustrative summary of receptor-mediated mechanisms involved in the therapeutic regulation of type 2 diabetes.

Type 2 diabetes (T2D) is a chronic metabolic disorder primarily affecting the metabolism of glucose in the body. It is characterized by polyphagia, polydipsia, polyuria, fatigue, and persistent hyperglycemia. It occurs mainly because of two major factors, namely, dysregulated insulin secretion from pancreatic β-cells and/or reduced sensitivity of insulin-responsive tissues to insulin, i.e., insulin resistance. The process of insulin secretion and insulin signaling is complex, requiring the involvement of distinct membrane receptors. Sulfonylureas are a class of antidiabetic drugs which exert t

Degradation Products of Guangdong Finger Citron Water-Soluble Polysaccharides by Gut Microbiota Ameliorate Type 2 Diabetes Mellitus via the Cyclic Adenosine Monophosphate Pathway.

Type 2 diabetes mellitus (T2DM) is a chronic metabolic disease defined by persistent hyperglycemia, primarily caused by insulin (INS) resistance and β-cell dysfunction. However, current pharmacological therapies are limited by adverse effects, highlighting the need for safe adjunctive strategies. Plant polysaccharides and their fermentation-derived degradation products exhibit multiple bioactivities and may be promising nutraceutical candidates. This study used degradation products of a water-soluble polysaccharide of finger citron from Guangdong Province (FCP-2-1) by gut micro biota fermentat

Familial hypomagnesemia with hypercalciuria and nephrocalcinosis (FHHNC) with ocular involvement due to a novel CLDN19 variant.

Claudin-16 (CLDN16) and claudin-19 (CLDN19) are essential tight junction proteins in the kidney that are critical for magnesium homeostasis. Mutations in CLDN16 and CLDN19 cause Familial Hypomagnesemia with Hypercalciuria and Nephrocalcinosis (FHHNC), a rare autosomal recessive tubular disorder. The disorder can progressively lead to severe complications, including end-stage renal disease. Affected individuals often present with polyuria, polydipsia, nephrolithiasis, hematuria, muscular tetany, seizures, and failure to thrive. Notably, CLDN19 mutations have also been associated with ocular abn

Remission of severe familial proteinuria in a Bracco Italiano dog.

The objective of this report was to document clinical remission of suspected familial proteinuria in a Bracco Italiano dog. A 4-year-old intact female Bracco Italiano with a familial history of amyloidosis was presented because of unilateral chemosis, polyuria, and polydipsia. Hypoalbuminemia and severe proteinuria were detected via initial diagnostics. Although no renal histopathologic assessment was done, there was a strong clinical suspicion for renal amyloidosis. The dog was treated with telmisartan and colchicine (in addition to other therapies) for proteinuria and achieved clinical remis

[Pregnancy and thirst, a remarkable duo].

Establishing a differential diagnosis for polyuria and polydipsia during pregnancy can be challenging. We describe a pregnant woman with severe polyuria (urine output 9L/24h) and polydipsia without pre-existing symptoms. Gestational diabetes and electrolyte disturbances were excluded; HbA1c and daily glucose profiles were normal. Due to pregnancy, no water deprivation or arginine stimulation test was performed. Copeptin, a surrogate marker for AVP/ADH, was <2 pmol/L (reference: 0-9 pmol/L), ruling out AVP resistance. Because of disabling symptoms, a trial of oral desmopressin was initiated dur

Pregnancy-unmasked latent autoimmune arginine vasopressin deficiency with anti-rabphilin-3a antibody positivity.

Although pregnancy-associated arginine vasopressin (AVP) disorder is rare, it is often attributed to excessive placental vasopressinase activity. However, vasopressinase-independent mechanisms, including gestational unmasking of latent AVP deficiency (AVP-D), may occur. We report a 37-year-old primigravid female who developed severe polydipsia and polyuria in the third trimester. She had hypotonic polyuria with inappropriately low plasma AVP levels and loss of the posterior pituitary bright spot on T1-weighted magnetic resonance imaging (MRI). AVP-D was suspected; desmopressin promptly relieve

Resolved GH excess and arginine vasopressin deficiency following pituitary apoplexy.

A 10-year, 8-month-old male presented with polyuria, polydipsia, and secondary nocturnal enuresis for 4 months following a severe headache. Initial fasting studies raised concern for partial arginine vasopressin deficiency (AVP-D). Brain magnetic resonance imaging revealed a possible pituitary adenoma vs artifact. His height was >99th percentile; he was prepubertal and growth charts demonstrated growth velocity >7.5 cm/year for the past 2 years. Additional testing showed markedly elevated IGF-1 and IGF-binding protein 3. He passed a water-deprivation test, ruling out AVP-D. He underwent a GH s

Ultrasonographic and CT Features of a Neoplasia-Associated Visceral Arteriovenous Fistula in a Cat.

An 11-year-old neutered male European Shorthair cat presented with progressive weight loss, polyuria/polydipsia, lethargy, and recurrent hemorrhagic cystitis. Abdominal ultrasound revealed tortuous and dilated vessels with spontaneous echo-contrast within the right renal parenchyma. Pulsed-wave Doppler demonstrated continuous flow with pulsatile components within the abnormal vessels, consistent with an arteriovenous communication. Computed tomography (CT) angiography confirmed marked intrarenal vascular dilation, strong arterial-phase enhancement, aneurysmal dilation of the right dorsal renal
