A Case of Arginine Vasopressin Deficiency with Anti-rabphilin-3A Antibody Positivity Following an Influenza Infection.

Lymphocytic infundibulo-neurohypophysitis (LINH) presents with arginine vasopressin deficiency (AVP-D), also known as central diabetes insipidus, caused by autoimmune mechanisms in the infundibulum and posterior pituitary. A 32-year-old man developed polydipsia and polyuria two months after influenza infection. A hypertonic saline test revealed no AVP responses. Magnetic resonance imaging demonstrated the loss of posterior pituitary hyperintensity. The patient tested positive for serum anti-rabphilin-3A antibodies, a diagnostic marker for LINH. This is the first report of a case of probable LI

Identification of a novel large deletion mutation in the AVPR2 gene responsible for hereditary nephrogenic diabetes insipidus in an infant: a case report.

BACKGROUND: Hereditary nephrogenic diabetes insipidus (HNDI) type1 is a rare genetic disorder that results from mutations in the AVPR2 gene, which encodes the arginine vasopressin receptor 2. The primary clinical manifestations of this disorder encompass polydipsia, polyuria and urine with low specific gravity. At present, there is no curative treatment, and the principal treatment goal is to manage symptoms in order to prevent complications such as dehydration, brain injury, and hydroureteronephrosis. CASE DESCRIPTION: This case report presents a 7-month-old boy who presented with persistent

TGFβ Blockade by Inhibition of Enolase-1-Mediated Plasmin Targets Tumor-Associated Macrophages in Pancreatic Ductal Adenocarcinoma.

Background: Pancreatic ductal adenocarcinoma (PDAC) is characterized by an immunosuppressive and metabolically rewired tumor microenvironment (TME). Although α-enolase (ENO1) is frequently overexpressed in PDAC and associated with poor prognosis, its functional role in TME remodeling remains unclear. This study investigated the role of ENO1 in plasmin-dependent transforming growth factor β (TGFβ) activation and metabolic adaptation in PDAC and evaluated the therapeutic potential of HuL001, a first-in-class humanized anti-ENO1 monoclonal antibody. Methods: ENO1 expression and clinical relevance

Refractory Rickets: Evaluation and Management.

Refractory rickets refers to a set of diseases which are identified by a lack of response to therapeutic doses used to treat vitamin D deficiency. A child presenting with refractory rickets can pose a diagnostic dilemma as many kidney diseases have been identified as possible causes. Inherited (e.g., distal renal tubular acidosis, Fanconi syndrome, hypophosphatemic rickets, vitamin D-dependent rickets, nephronophthisis) and acquired tubular disorders [e.g., posterior urethral valves, reflux nephropathy leading to chronic kidney disease (CKD)-mineral bone disorder] may be complicated by refract

Unveiling the unexpected: refractory rickets as an uncommon presentation of tyrosinemia type I.

BACKGROUND: Tyrosinemia type I is a rare autosomal recessive inborn error of metabolism caused by deficiency of fumarylacetoacetate hydrolase (FAH), an enzyme essential for the final breakdown of tyrosine. The enzyme defect leads to accumulation of toxic metabolites, primarily affecting the liver, kidneys, and brain. Typically, diagnosed in infancy due to acute or chronic liver dysfunction, refractory rickets is an uncommon presentation. We report a child with tyrosinemia type I presenting with refractory rickets and hepatosplenomegaly but with preserved liver function. CASE PRESENTATION: A th

Inhibition of compulsive behavior by dextromethorphan on schedule-induced polydipsia in rats: Role of NMDA, sigma-1, and 5-HT receptors.

Dextromethorphan (DM) is an FDA-approved treatment for major depressive disorder when combined with bupropion. DM functions as an inhibitor for 5-HT membrane transporters and antagonist for sigma-1 receptors (S1Rs) and NMDA receptors, which are pharmacological actions potentially therapeutically relevant for obsessive-compulsive disorder (OCD). The present study examined DM and compounds selective for these receptor mechanisms using the schedule-induced polydipsia (SIP) procedure, a preclinical model of compulsive behavior. Rats were food-restricted and given a fixed-time 60-s schedule of food

Analysis of Dual-Energy Computed Tomography Imaging for the Evaluation of Lymph Node Metastasis in Canine Anal Sac Gland Adenocarcinoma: A Case Report.

A 9-year-old neutered male miniature schnauzer was referred to our facility with polyuria, polydipsia, persistent hypercalcemia, hypophosphatemia, and elevated parathyroid hormone-related protein. Dual-energy computed tomography (DECT) imaging revealed a mass in the left anal gland (LAG) and enlargement of the left medial iliac lymph node (LML) and the left internal iliac lymph node (LIL). The mass was surgically removed, and the histopathological diagnosis was anal sac gland adenocarcinoma, with metastasis to the LML and LIL. The pathological findings were concordant with those obtained from

Arginine Vasopressin Deficiency in Children - A Practical Guide to Etiological Diagnosis and Long-Term Surveillance.

Arginine vasopressin deficiency (AVP-D) is an uncommon but clinically important cause of the polyuria-polydipsia syndrome. Establishing the diagnosis extends beyond confirming hypotonic polyuria and requires differentiation from primary polydipsia and arginine vasopressin resistance, together with identification of the underlying etiology. Unlike adults, children and adolescents with AVP-D frequently present with the earliest manifestation of an evolving neoplastic, infiltrative, inflammatory, congenital, autoimmune, or genetic disorder, making longitudinal clinical, endocrine, and magnetic re

Genetic Insights Into AVP Deficiency: Identification of a Novel AVP Variant and Compilation of a Curated Catalogue of Pathogenic Variants.

Central diabetes insipidus, or arginine vasopressin deficiency (AVP-D), is a rare disorder caused by insufficient release of antidiuretic hormone from the pituitary gland. The disease is characterised by severe polyuria and polydipsia, which, in rare cases, can be caused by variants in the AVP gene. The aim of the present study was to identify the genetic cause of AVP-D in two families and compile and discuss an updated and complete catalogue of variants in AVP associated with AVP-D. Two Danish families with autosomal dominant inheritance of symptoms indicating AVP-D were included, and proband

Presumed partial aplasia of the hypothalamus and pituitary gland causing central diabetes insipidus in an adult cat.

CASE SUMMARY: A 2-year-old castrated male domestic shorthair cat presented with severe lethargy, hyporexia, dehydration and poor body condition. The incidence of polyuria/polydipsia was unknown because of the presence of multiple other cats in the household. Laboratory findings revealed severe hypernatraemia, isosthenuria, haemoconcentration and moderate azotaemia. MRI of the head demonstrated partial hypoplasia/aplasia of the hypothalamus and pituitary gland, confirming congenital central diabetes insipidus; additional endocrine testing excluded other causes. Initial fluid therapy improved th

Malnutrition and hypernatremic dehydration as the initial presentation of nephrogenic diabetes insipidus in an infant.

Congenital nephrogenic diabetes insipidus (CNDI), also known as arginine vasopressin resistance, is a rare inherited disorder of water homeostasis in which the kidneys lose their ability to concentrate urine, leading to polyuria, polydipsia, and a risk of hypertonic dehydration. We report the case of a 3-month-old infant with symptoms present for approximately 2 months, characterized by episodes of fever, irritability, and poor growth. Upon admission, severe malnutrition and dehydration were noted. A diagnosis of CNDI was made based on vasopressin levels, plasma and urine osmolarity values, hy

Urothelial Carcinoma in a Cat with Bilateral Ectopic Ureters.

A 10-year-old neutered female Maine Coon was presented with haematuria, weight loss, polyuria/polydipsia and progressive azotaemia. Diagnostic imaging revealed bilateral renal pelvic dilation, ureteral distension and ectopic ureters. Bacterial pyelonephritis was diagnosed, but the azotaemia progressed despite antibiotic therapy, and suspected ureteral obstruction prompted surgical ureteral reimplantation. The visualisation of omental nodules and thickening of the ectopic right ureterovesicular junction intraoperatively raised suspicion of malignancy, and histopathology revealed urothelial carc

Clean intermittent catheterization reverses hydronephrosis in a child with congenital nephrogenic diabetes insipidus: a case report.

BACKGROUND: Congenital nephrogenic diabetes insipidus (CNDI) is most frequently caused by mutations in the AVPR2 gene. Patients exhibit persistent polyuria due to renal insensitivity to antidiuretic hormone. Chronic high urine output predisposes to bladder dysfunction and upper urinary-tract dilatation, notably hydronephrosis. Although pharmacotherapy can partially reduce urine volume, its capacity to reverse established hydronephrosis is limited. Clean intermittent catheterization (CIC), a mainstay in managing neurogenic bladder, warrants investigation regarding its utility in CNDI-associated

Navigating the Therapeutic Tightrope: Refractory Lithium-Induced Nephrogenic Diabetes Insipidus in a Patient With Advanced Chronic Kidney Disease.

Lithium is the most common and effective mood stabiliser in the management of bipolar disorder. Lithium-induced nephrogenic diabetes insipidus (NDI) is a well-recognised complication of long-term mood stabiliser therapy. However, management becomes exponentially more complex when therapeutic options, such as thiazides or amiloride, are contraindicated due to advanced chronic kidney disease (CKD) or electrolyte instability. We are reporting the case of a man in his 60s with a 40-year history of lithium use who presented with acute-on-chronic kidney injury (AKI) and severe hypernatraemia (164 mm

Isolated nonparathyroid hormone-mediated hypercalcemia: a rare presentation of retroperitoneal sarcoidosis.

Non-parathyroid hormone (PTH)-mediated hypercalcemia has diverse etiologies, including granulomatous disorders such as sarcoidosis, in which extrarenal 1-α-hydroxylase activity leads to excess production of 1,25-dihydroxyvitamin D (1,25(OH)₂D). We report a rare case of sarcoidosis presenting as isolated hypercalcemia without pulmonary involvement, complicated by initially normal 1,25(OH)₂D levels and a false-negative core biopsy. A 44-year-old man with hypertension and type 2 diabetes presented with malaise, polyuria, and polydipsia. Laboratory evaluation showed severe hypercalcemia with suppr

Analysis of energy expenditure and behavioral characteristics in different mouse strains under normal and disease conditions.

BACKGROUND: Metabolic and behavioral traits in mice exhibit substantial variability across strains, ages, and disease states, which can confound preclinical study outcomes. However, systematic characterization of these parameters (e.g., energy metabolism, activity, and feeding behavior) across diverse experimental conditions remains limited. METHODS: Core metabolic and behavioral parameters were assessed via metabolic cages in five 8-week-old mouse strains (ICR, C57BL/6J, BALB/c, BALB/c-nude, NOD-SCID), six C57BL/6J age groups (3 weeks to 18 months), and four mouse disease models (LPS-induced

A Diagnostic Pitfall: Carbamazepine-Induced Drug Reaction With Eosinophilia and Systemic Symptoms (DRESS) Syndrome Masquerading As Angiotensin-Converting Enzyme Inhibitor Angioedema.

Drug reaction with eosinophilia and systemic symptoms (DRESS), also known as drug-induced hypersensitivity syndrome (DIHS), is a severe, multiorgan, delayed hypersensitivity reaction with significant morbidity. Diagnosis is often delayed due to its mimicry of other conditions. We present the case of a 69-year-old Hispanic male with hypertension and stage two chronic kidney disease who developed trigeminal neuralgia and was started on carbamazepine. Approximately seven weeks later, he presented with a generalized rash, angioedema, and mucosal swelling, initially attributed to his concurrent lis

Central Diabetes Insipidus as a Rare Cause of Polyuria.

Central diabetes insipidus (CDI) is a rare condition characterized by arginine vasopressin deficiency, leading to persistent polyuria and polydipsia. Although it can present at any age, idiopathic CDI in young adults is uncommon and diagnostically challenging. Inflammatory conditions such as infundibuloneurohypophysitis must be considered, particularly when magnetic resonance imaging (MRI) reveals a thickened pituitary stalk and absence of the posterior pituitary bright spot. Early recognition and differentiation from diabetes mellitus, nephrogenic diabetes insipidus, and primary polydipsia ar
