Clozapine-associated systemic inflammatory reaction with adult-onset Still's disease-like features and marked interleukin-18 elevation in a patient with schizophrenia: a case report.

BACKGROUND: Clozapine can trigger early inflammatory adverse reactions ranging from transient fever to organ-dominant syndromes. It remains unclear whether this spectrum can also present as a systemic adult-onset Still's disease (AOSD)-like phenotype. CASE PRESENTATION: A 54-year-old Japanese woman with long-standing treatment-resistant schizophrenia, psychogenic polydipsia with water intoxication, and overweight/obesity underwent olanzapine-to-clozapine cross-titration. Under the standard Japanese Clozaril Patient Monitoring Service procedure, clozapine was increased to 75 mg/day by day 23.

Case report: From metabolic instability to remission: a case of pheochromocytoma presenting as atypical diabetes.

INTRODUCTION: Pheochromocytoma is a well-established cause of secondary hypertension and can lead to dysglycemia. However, its initial presentation as atypical diabetes, characterized by severe glycemic instability, is uncommon and poses a significant diagnostic challenge. CASE PRESENTATION: A 49-year-old non-obese Chinese woman presented with a 15-month history of progressive polyuria and polydipsia. Initially diagnosed with type 2 diabetes, she developed significant glycemic variability with recurrent hypoglycemia and profound hyperglycemia, alongside a rapid decline in β-cell function. Dur

Case of postoperative diabetes insipidus following cervical decompressive laminectomies.

BACKGROUND: Postoperative diabetes insipidus (DI) is rare, involving the hypothalamic-pituitary axis following cervical spine surgery. CASE DESCRIPTION: A 51-year-old male underwent an elective C3-C6 decompressive laminectomy for cervical spinal canal stenosis. Three hours postoperatively, he developed abrupt massive polyuria (1300-1800 mL/h) with polydipsia (specific gravity 1.002), consistent with DI. Treatment with fluid replacement, desmopressin, and vasopressin resulted in clinical improvement. The brain MR revealed a normal pituitary gland and stalk, whereas histology from the laminecto

Microscopic Polyangiitis With Pituitary Dysfunction and Spontaneous Renal Aneurysm Rupture.

Antineutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis (AAV) is a relatively uncommon autoimmune disease, predominantly causing kidney or lung injury. Pituitary dysfunction is exceedingly rare in microscopic polyangiitis (MPA). Rupture or hemorrhage of renal artery aneurysms is also scarcely reported in AAV, especially in MPA. In this study, we describe a 68-year-old woman who presented with shortness of breath at rest, polydipsia, and polyuria. In addition to an increased p-ANCA titer (1:10) and the presence of anti-myeloperoxidase antibodies, T1-weighted magnetic resonance imag

An Unusual Presentation of Multisystem Langerhans Cell Histiocytosis in a Child: A Case Report.

Langerhans cell histiocytosis (LCH) is a rare clonal myeloid disorder with a broad clinical spectrum ranging from isolated lesions to multisystem disease. We report the case of a two-year-five-month-old boy who presented with a six-month history of treatment-refractory bilateral otorrhea, progressive mandibular swelling with premature tooth loss, seborrheic skin lesions, and weight loss. Further evaluation revealed severe polyuria-polydipsia suggestive of central diabetes insipidus. Imaging demonstrated multifocal craniofacial osteolytic lesions with hypothalamic-pituitary involvement. Histopa

Beyond Classical Diabetes Insipidus: A Retrospective Case Series Illustrating the Diagnostic Complexity and Diverse Etiologies of Polyuria-Polydipsia Syndrome.

Background Polyuria-polydipsia syndrome encompasses a heterogeneous group of disorders, including central diabetes insipidus (CDI), nephrogenic diabetes insipidus (NDI), and primary polydipsia. Despite presenting with similar clinical manifestations, these conditions differ markedly in their underlying pathophysiology, prognosis, and management. Distinguishing between them remains clinically challenging, particularly in patients with partial disease and in resource-limited settings where newer diagnostic biomarkers such as copeptin are not routinely available. This study highlights the diagnos

Arginine vasopressin deficiency: towards a better characterization.

Arginine vasopressin (AVP) deficiency, previously termed central diabetes insipidus, arises from impaired AVP synthesis or secretion by the hypothalamus and/or the posterior pituitary gland and presents with hypotonic polyuria and polydipsia. To differentiate AVP deficiency from AVP resistance and primary polydipsia, a stepwise diagnostic workup is required. In recent years, copeptin, as a reliable surrogate marker of AVP secretion, has been incorporated into diagnostic algorithms, and copeptin-based stimulation tests have substantially improved diagnostic accuracy. Once AVP deficiency has bee

[From syndrome of inappropriate antidiuretic hormone secretion deficiency to excess - Challenges and solutions in a unique case].

Introduction: the sequential transition from central diabetes insipidus (CDI) to the syndrome of inappropriate antidiuretic hormone secretion (SIADH) is an exceptionally rare condition and represents a major diagnostic and therapeutic challenge. Case report: we report the case of a 47-year-old man with Burkitt lymphoma involving the hypothalamic-pituitary region who initially presented with polyuria, polydipsia, and hypernatremia. CDI was confirmed by biochemical findings and a positive response to desmopressin. Following desmopressin treatment and intensive chemotherapy, rapid tumor regressio

Severe recurrent hypernatremia and probable lithium-associated nephrogenic diabetes insipidus following pancreatectomy in a patient with schizophrenia: a case report.

BACKGROUND: Nephrogenic diabetes insipidus (NDI) is characterized by impaired renal responsiveness to arginine vasopressin, causing defective urinary concentration and large volumes of dilute urine. Long-term lithium exposure is an important acquired cause. Perioperative fasting, restricted free-water intake, and ongoing fluid losses may unmask compensated lithium-associated urinary concentrating dysfunction and precipitate severe hypernatremia. CASE PRESENTATION: A 61-year-old man underwent laparoscopic resection of a pancreatic tail lesion. His history included schizophrenia, type 2 diabete

Disorders of Water and Tissue Perfusion (Diabetes Insipidus, Renin-Angiotensin-Aldosterone Axis, and Anhidrosis).

This review highlights disorders of water and tissue perfusion in equids, specifically diabetes insipidus, the renin-angiotensin-aldosterone axis, and anhidrosis. The differences in renal versus central diabetes insipidus is discussed, as well as diagnosis and treatment of diabetes insipidus in equids. The physiology of the renin-angiotensin-aldosterone axis is reviewed, and recent research in the role of the renin-angiotensin-aldosterone axis in equids in certain states is emphasized. The proposed pathophysiology of anhidrosis in horses is discussed, as well as potential therapies.

Apparent mineralocorticoid excess with a novel HSD11B2 variant: longitudinal clinical and biochemical follow-Up.

Apparent mineralocorticoid excess (AME) is a rare autosomal recessive disorder caused by loss-of-function mutations in HSD11B2, leading to impaired conversion of cortisol to cortisone and inappropriate mineralocorticoid receptor activation. Early recognition is critical, as untreated disease results in progressive cardiovascular and renal complications. We describe a 2-year-old boy with severe early-onset hypertension, polyuria, polydipsia, abdominal distension, and growth retardation. Biochemistry revealed hypokalemic metabolic alkalosis with suppressed renin and low aldosterone. Renal ultras

BAZ2B contributes to endometrial fibrosis by promoting fibroblast-to-myofibroblast differentiation.

Endometrial fibrosis is a repair process of the endometrium that is commonly associated with trauma to the endometrium from surgical procedures, primarily curettage, and these dysfunctions lead to embryo implantation dysfunction and consequent infertility or spontaneous abortion. However, the underlying molecular mechanisms and genetic determinants driving endometrial fibrosis are largely unknown. Here, we conducted a systematic drug screen using an epigenetic compound library to investigate the epigenetic mechanism essential for endometrial fibrosis, and identified an epigenetic reader, bromo

Recognizing Euglycemic Diabetic Ketoacidosis in the Era of SGLT2 Inhibitors: A Case Report.

Euglycemic diabetic ketoacidosis (euDKA) is an increasingly recognized complication associated with SGLT2 inhibitors. Because serum glucose levels may remain normal or only mildly elevated, diagnosis can be delayed, potentially increasing morbidity. We present the case of a 51-year-old male with long-standing poorly controlled type 2 diabetes mellitus who developed euDKA shortly after initiation of empagliflozin/metformin therapy. The patient presented with generalized weakness, gastrointestinal symptoms, reduced oral intake, polyuria, polydipsia, and weight loss. Initial laboratory evaluation

Primary Care Recognition of Rabson-Mendenhall Syndrome Despite Absence of Classical Diabetic Symptoms.

BACKGROUND Rabson-Mendenhall syndrome (RMS) is an extremely rare autosomal recessive disorder caused by pathogenic variants in the insulin receptor gene, leading to severe insulin resistance and compensatory hyperinsulinemia. Classical features include acanthosis nigricans, non-obese or underweight body habitus, hirsutism, dental abnormalities, dysmorphic features, and variable growth abnormalities. Early recognition may be difficult when the initial presentation is dominated by non-specific symptoms rather than classical metabolic complaints. CASE REPORT A 10-year-old Saudi girl presented to

Dual Aetiology of Diabetes Insipidus in Pregnancy: Vasopressinase-Mediated and Central Mechanisms.

Diabetes insipidus (DI) is a heterogeneous disorder characterised by polyuria and polydipsia due to impaired arginine vasopressin (AVP) secretion or action. We describe a 36-year-old pregnant woman who presented at 28 weeks' gestation with weakness, severe hypokalaemia from renal potassium wasting, hypernatraemia and polyuria with dilute urine. Vasopressinase-mediated DI was suspected; there was no response to AVP, but desmopressin (dDAVP) produced a rapid clinical improvement, confirming the diagnosis. However, postpartum recurrence of symptoms prompted further evaluation, with imaging sugges

Disrupted fluid homeostasis in patients with post-Covid-19 syndrome - a case series.

The post-covid-syndrome (PCS) is characterized by severe persisting fatigue and other long haul symptoms following COVID-19 infection. Disease-driving mechanisms are elusive, biomarkers are missing, treatment options few, and yet millions of people worldwide are affected. We hypothesized that the common complaint of polydipsia/polyuria and its attendant effect on osmolality can be quantified and identified as an unrecognized clinical feature of the PCS. We conducted a clinical case-series of 10 consecutive patients with PCS (5 females, 5 males; mean age 44 ± 14 y) who completed questionnaires

Paralytic Ileus as the Initial Manifestation of New-Onset Type 1 Diabetes Without Diabetic Ketoacidosis: A Case Report.

Type 1 diabetes (T1D) is an autoimmune disease characterized by pancreatic β-cell destruction and absolute insulin deficiency. In pediatric patients, T1D typically presents with classic symptoms such as polydipsia, polyuria, weight loss, or diabetic ketoacidosis (DKA). Although gastrointestinal symptoms may occur at disease onset, paralytic ileus as an initial manifestation of T1D, particularly in the absence of DKA, is extremely uncommon. A 15-year-old boy presented with severe abdominal pain and vomiting and was diagnosed with paralytic ileus based on abdominal radiographic findings. Mild hy

Bardet-Biedl syndrome in two sibling pairs: a case series.

BACKGROUND: Bardet-Biedl syndrome is a rare multisystem ciliopathy defined by retinal dystrophy, obesity, polydactyly, hypogonadism, and renal anomalies, often accompanied by neurodevelopmental and behavioral issues. Diagnosis relies heavily on clinical features, particularly in resource-limited settings where genetic testing is unavailable. CASE PRESENTATION: We describe two sibling pairs, all of South Asian, Pakistani ethnicity, clinically diagnosed with Bardet-Biedl syndrome using contemporary diagnostic frameworks. The first pair, two male brothers aged 17 and 21 years, came from a consan
