Adult onset multisystem Langerhans cell histiocytosis initially presenting with arginine vasopressin deficiency: a case report.

BACKGROUND: Langerhans cell histiocytosis (LCH) is a rare hematopoietic disorder with diverse clinical presentations and sequential or concurrent multi-organ involvement. The adult onset is particularly uncommon and challenging to diagnose early. CASE PRESENTATION: A 36-year-old female patient initially presented with polydipsia, polyuria, and dry mouth, gradually developing bone pain, facial erythema, and lymph node enlargement. Cranial MRI showed pituitary stalk thickening with the absence of a posterior pituitary signal. Iliac bone biopsy with immunohistochemistry demonstrated S-100(+), CD

Hypersomatotropism and Hypercortisolism Caused by a Plurihormonal Pituitary Adenoma in a Dog.

A 12-year-old, male Labrador Retriever was presented because of polyuria, polydipsia, polyphagia, joint pain, and physical features consistent with acromegaly. Circulating insulin-like growth factor-1 (IGF-1) concentration was increased (> 1000 ng/mL; reference interval [RI], 42-449), suggestive of hypersomatotropism. An abnormal low-dose dexamethasone suppression test and increased circulating adrenocorticotropic (ACTH) concentration indicated pituitary-dependent hypercortisolism. Computed tomography identified an enlarged pituitary gland. Treatment with cabergoline initially decreased circul

WFS1 gene mutation associated with pediatric diabetes mellitus and congenital deafness: A case report.

BACKGROUND: Wolfram syndrome is a rare autosomal recessive genetic disorder characterized by early-onset diabetes and progressive neurodegeneration, most notably sensorineural hearing loss and optic atrophy. Because its initial manifestations are usually similar to those of type 1 diabetes, the diagnosis may be delayed until other manifestations appear. Pathogenic variations of the WFS1 gene can disrupt endoplasmic reticulum function and cellular homeostasis, but the complete mutation spectrum of WFS1 has not been fully determined. Early identification of monogenic diabetes caused by Wolfram s

Type 2 Diabetes Mellitus Diagnosed at the Age of Eight Years: A Case Report.

The onset of type 2 diabetes mellitus (T2DM) in prepubertal age is not uncommon nowadays. Here we are reporting on a young boy who was diagnosed with diabetes at the age of eight years. Diagnosis of diabetes was made based on symptoms of hyperglycemia (new onset bed wetting, also polyphagia and polydipsia) and laboratory evidence of high plasma glucose. The patient was obese (body mass index, BMI > 95th percentile) and had acanthosis nigricans during presentation. The type of diabetes was not confirmed at the time of diagnosis based on the supporting laboratory investigations. His initial high

Sudden cardiac death due to intussusception of a coronary artery: a case report.

In this case report, sudden cardiac death caused by intussusception of a coronary artery is discussed. A 47-year-old man was found dead in the nursing home where he lived, following an episode of polyphagia and two of vomiting. Upon cadaveric dissection, an overdistention of the large intestine was noted. Re-evaluation of the formalin-fixed whole heart revealed occlusion of the circumflex branch of the left coronary artery, which was not macroscopically attributable to vascular thrombosis or an atheromatous plaque. Histological investigations revealed ischaemic-type histological changes of the

LncPrep + 96kb inhibits ovarian fibrosis by upregulating prolyl oligopeptidase expression.

BACKGROUND: Cemented total hip arthroplasty (THA) is a safe and common procedure. In rare cases life threatening bone cement implantation syndrome (BCIS) may occur, which is commonly caused by pulmonary embolism (PE). CASE PRESENTATION: We describe the rare case of a 70-year old patient who underwent an elective total hip replacement. Before surgery he was diagnosed with underlying systemic indolent mastocytosis, a rare pathological disorder that may result in anaphylaxis after massive systemic mast cell activation. Triggers may be IgE-mediated, direct mast cell activation, or unclear. Some p

Risk factors and treatment strategies for adjacent segment disease following spinal fusion (Review).

Anaphylaxis is a severe multisystem reaction that occurs rapidly after the introduction of an antigen that would otherwise be a harmless substance. It is characterized by airway and respiratory problems, cardiovascular collapse, mucosal inflammation, and other complications, all severe symptoms that can cause death. IgE-dependent anaphylaxis involves mast cells (MCs) which are the main sources of biologically active mediators that contribute to the pathological and lethal phenomena that can occur in anaphylaxis. Antibody-mediated anaphylaxis can follow multiple pathways such as that mediated b

Feline Comorbidities: Hypersomatotropism-induced diabetes in cats.

PRACTICAL RELEVANCE: Diabetes mellitus is the second-most common feline endocrinopathy, affecting an estimated 1/200 cats. While the underlying causes vary, around 15-25% of cats with diabetes mellitus develop the condition secondarily to progressive growth hormone (GH)-induced insulin resistance. This typically results in a form of diabetes that is challenging to manage, whereby the response to insulin is very variable or high doses are required to achieve even minimal diabetic control. CLINICAL CHALLENGES: Although uncontrolled chronic excessive GH may result in phenotypic changes that rais

Protective Effect of Black Rice Cyanidin-3-Glucoside on Testicular Damage in STZ-Induced Type 1 Diabetic Rats.

Diabetic testicular damage is quite a common and significant complication in diabetic men, which could result in infertility. The natural fertility rate of type 1 diabetes men is only 50% because of testicular damage. This research first aimed to explore the intervention effect of C3G on testicular tissue damage induced by diabetes. Here, a streptozotocin-induced type 1 diabetic rat model was established, and then C3G was administered. After 8 weeks of C3G supplementation, the symptoms of diabetes (e.g., high blood glucose, lower body weight, polydipsia, polyphagia) were relieved, and at the s

Uncommon dermatologic manifestation (paronychia) in a cat with diabetes mellitus.

A 3.5-year-old male intact domestic short hair cat presented for a chronic wound and crusts over the claw and claw folds over several months. The cat was diagnosed with diabetes mellitus based on the presence of persistent hyperglycemia, glucosuria, and compatible clinical signs which consist of polyuria, polydipsia, polyphagia, and weight loss. Glipizide (Glucotrol XL, Pfizer, Indonesia) 2.5 mg orally twice daily was prescribed. By the seventeenth day, the patient's claws and skin around the paw had normalized and the abnormal claw sloughed off, revealing a normal claw underneath. Blood gluco

Therapeutic Potential of Rosa davurica Pall. Root Extract as an Antidiabetic Agent: A Comprehensive Analysis from Molecular Mechanisms to In Vivo Efficacy.

Rosa davurica Pall. is widely used in traditional oriental herbal therapy, but its components and molecular mechanisms of action remain unclear. This study investigates the antidiabetic potential of Rosa davurica Pall. root extract (RDR) and elucidates its underlying molecular mechanisms with in vitro and in vivo models. Data from the current study show that RDR exhibits strong antioxidant activity and glucose homeostasis regulatory effects. It significantly impacts glucose homeostasis in C2C12 skeletal muscle cells by inhibiting α-glucosidase activity. Further molecular mechanistic studies re

Thousands of Years of Pastoralism Don't Count: Coprophagous Beetles Prefer Exotic Alpaca Dung to That of Cattle.

Dung beetles mostly feed on mammal dung. Throughout the European Alps, the dung produced by local domestic ungulates attracts many species of dung beetles, giving rise to rich and diversified communities that play an important role in the Alpine agricultural ecosystem. There is, therefore, understandable concern about the introduction of exotic livestock, such as alpacas (Vicugna pacos (Linnaeus, 1758)), into the region. This research studied dung beetle assemblages in an Alpine valley where both cattle and alpacas are raised. We used standardized pitfall traps baited with alpaca or cow dung a

Avian Diabetes Mellitus: A Review.

Diabetes mellitus (DM) is an uncommon, poorly documented metabolic disorder of birds. Extrapolating knowledge from DM in mammals is challenging because of marked differences in avian physiology and metabolism. A literature review from December 1991 to January 2022 identified 14 publications covering 16 diabetic birds, 63% (10/16) of which belonged to the order Psittaciformes with Ara as the predominant genus. No sex predilection was noted, but males generally presented at a younger age. Commonly reported clinical signs included polyuria 94% (15/16), polydipsia 88% (14/16), weight loss 75% (12/

Genotype-phenotype characteristics of 57 patients with Prader-Willi syndrome: a single-center experience from Turkey.

OBJECTIVES: Prader-Willi syndrome (PWS) is a rare and complex genetic disorder caused by the loss of expression of the paternal copy of the imprinted genes on chromosome 15q11-q13. A variety of findings have been reported on the phenotypic differences between the genetic subtypes of PWS. This article compares the clinical findings of 57 PWS patients by genetic subtype and explores possible associations in this context. METHODS: Methylation‑specific multiplex ligation-dependent probe amplification and single nucleotide polymorphism microarrays were used to diagnose deletion and uniparental dis

Xerophthalmia secondary to bowel malabsorption after Roux-en-Y gastric bypass.

INTRODUCTION: Vitamin A is a fat-soluble vitamin, obtained through diet. Vitamin A deficiency is the leading cause of preventable blindness in children in developing countries due to impaired intake (Phanachet et al. 2018). Nevertheless, it is uncommon in the developed world where malabsorption takes a prominent role. CASE DESCRIPTION: A fifty-one-year-old female presented complaining of foreign body sensation, pain, tearing, fluctuating visual acuity, nyctalopia, diarrhea, polyphagia and weight loss. She had history of Roux-en-Y gastro-jejunal bypass, Lynch syndrome and right hemicolectomy w
