Sijing Pill modulates PGE2/EP4/PI3K-AKT pathway via gut-bone axis to treat postmenopausal osteoporosis.

INTRODUCTION: Postmenopausal osteoporosis (PMOP) represents a substantial clinical burden for aging women worldwide. Existing pharmacotherapies are frequently constrained by suboptimal efficacy, poor adherence, and adverse effects, underscoring the need for superior treatment alternatives. The Sijing Pill (SJP) has shown potential in alleviating bone loss in PMOP. However, the fundamental mechanisms underlying its therapeutic effects remain incompletely elucidated, which has impeded its clinical translation and rational application. METHODS: The therapeutic effect of SJP on PMOP was first eva

Comparative Analysis of Breast Implant Illness Symptoms in Implant-Based, Autologous, and No-Reconstruction Mastectomy Patients: A TriNetX Database Study.

BACKGROUND: Breast implant illness (BII) refers to a cluster of nonspecific systemic symptoms that some patients attribute to breast implants. This study compared long-term rates of "BII-compatible symptoms" among mastectomy patients who received implant-based reconstruction, autologous flap reconstruction, or no reconstruction. METHODS: A retrospective, propensity-matched cohort study was conducted in the TriNetX Global Collaborative Network. Women ≥18 years with breast cancer diagnosed between 2005 and 2025 who underwent mastectomy entered 3 mutually exclusive groups: implant-based reconstr

Preclinical Safety Evaluation of a Replication-Defective Canine Adenovirus Type 2 Vector-Based SARS-CoV-2 Vaccine Candidate in Murine Models.

Background: Adenoviral vectors are widely used in vaccine development; however, pre-existing immunity to common human adenovirus serotypes can limit their effectiveness. Canine adenovirus type 2 (CAV-2) is a non-human adenovirus with low seroprevalence in humans, making it a suitable alternative vector. Despite its promise, comprehensive preclinical safety data for CAV-2 vector-based vaccine platforms remain limited. In this study, we evaluated the safety profile of a replication-defective CAV-2 vector expressing the Omicron BA.4 SARS-CoV-2 spike immunogen in BALB/c mouse models. Methods: The

Treatment of patients with tumor/treatment-related hypothalamic obesity in the first two years following surgical treatment or radiotherapy.

Survivors of sellar/suprasellar tumors involving hypothalamic structures face a risk of impaired quality of life, including tumor- and/or treatment-related hypothalamic obesity (TTR-HO) defined as abnormal weight gain resulting in severe persistent obesity due to physical, tumor- and/or treatment related damage of the hypothalamus. We analyze German claims data to better understand treatment pathways for patients living TTR-HO during the two years following the index surgical treatment. A database algorithm identified patients with TTR-HO in a representative German payer claims database betwee

Overweight and glucose/lipid metabolism abnormality associated with SSRIs: a pharmacovigilance study based on the FDA adverse event reporting system.

BACKGROUND: In the past few decades, selective serotonin reuptake inhibitors (SSRIs) became widely used antidepressants worldwide. Therefore, the adverse reactions of patients after SSRI administration became a public and clinical concern. In this study, we conducted a pharmacovigilance study using the Adverse Event Reporting System (FAERS) database of the US Food and Drug Administration. Our main goal was to evaluate adverse events related to SSRIs, with a particular focus on abnormal weight gain and glucose/lipid metabolism disorders. METHOD: The adverse event data for representative SSRIs

Improving dietary energy and antioxidative properties benefit early maternal BMI and further manage adverse pregnancy outcomes with better weight gain.

Dietary characteristics affect maternal status in early pregnancy, which is important for later outcomes. However, Chinese dietary guidelines for pregnant women are not specific to obesity, overweight, and underweight. Moreover, since pregnancy is a prolonged process, an intermediate factor is needed to connect early maternal BMI with pregnancy outcomes. In this cohort of 1785 Chinese pregnant women from 2020 to 2022, 37.98% of participants had abnormal BMI in early pregnancy. A lower energy intake from carbohydrates (<50%) but higher intake from protein (>20%) and fat (>30%) resulted in exces

Economics of hypothalamic obesity in patients with craniopharyngioma and other rare sellar/suprasellar tumors.

BACKGROUND: Rapid and abnormal weight gain resulting in severe persistent obesity due to physical, tumor- and/or treatment-related damage to the hypothalamus, is called acquired hypothalamic obesity (aHO), and is often linked to craniopharyngioma and/or sellar/suprasellar tumors. Here, we examine the healthcare resource use (HCRU) and costs of aHO following treatment of these tumors. METHODS: We used a retrospective matched cohort design with German statutory health insurance data on 5.42 million people from 2010 to 2021. We applied a novel three-step approach using diagnostic and prescriptio

Associations of serum choline, betaine and trimethylamine N-oxide with gestational diabetes mellitus among Chinese pregnant women.

OBJECTIVE: Mechanistic studies indicated beneficial effects of choline and betaine on glucose homeostasis during pregnancy. However, limited human studies explored the associations of biomarkers of choline and its related metabolites with gestational diabetes mellitus (GDM) and results remained inconsistent. This study aimed to explore associations of serum choline, betaine and trimethylamine N-oxide (TMAO) with GDM odds among Han Chinese women. RESEARCH DESIGN AND METHODS: Pregnant women with singleton gestation were enrolled during GDM screening between 24 and 28 weeks of gestation at the S

From Mycobiome Remodeling to Therapeutic Candidate: Aspergillus ruber and 3-methyldioxyindole, Acknowledged as a Novel Modulator Via BPIS Treatment and Alleviating MASLD.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is a leading significant public health concern, affecting 25-30% of the population globally. Recent advances in gut microbiome research are increasingly recognized. Studies exploring the gut-liver axis have mostly focused on bacteriomes. There are limited studies about commensal fungi. Our earlier research on the anti-inflammatory properties of bound polyphenols of the inner shell (BPIS) extracted from Foxtail millet in the mouse colitis model revealed that BPIS significantly increased the abundance of Aspergillus ruber (A. ruber

Failure of GLP-1 Agonist Therapy to Improve Weight in a 3-Year-Old Patient With Tumor-Related Obesity.

A 19-month-old female patient presented due to rapid weight gain starting at age 5 months. Due to continued abnormal weight gain after 1 year of age, an MRI of the brain was performed, revealing a 5.5 × 2.6 × 2.3 cm mass centered within the medulla oblongata with extension to the C3 vertebral body. Biopsy confirmed a diagnosis of ganglioglioma. A trial of topiramate was attempted, but there was no improvement in weight gain after 2 months. The patient started on 0.3 mg liraglutide daily, and the dose was titrated up to 3 mg daily over 3 months. The patient did not experience abdominal pain, na

Locally adapting digital health technologies to address the global challenge of Parkinson's disease.

OBJECTIVE: To examine how 1Hz and 10Hz rTMS temporarily influence ratings of tinnitus loudness, annoyance, and awareness. The thalamocortical dysrhythmia (TCD) model of tinnitus was tested by examining changes in spectral power and coherence of resting state EEGs from baseline to each phase of treatment and correlating these data with change in tinnitus. METHODS: Nineteen participants completed a double-blind, placebo (sham rTMS) controlled, within-subjects study with crossover between the two active rTMS treatment conditions. An imposed order effect, sham rTMS first, eliminated drift of acti

Risk of longer-term endocrine and metabolic conditions in the Deepwater Horizon Oil Spill Coast Guard cohort study - five years of follow-up.

INTRODUCTION: Long-term endocrine and metabolic health risks associated with oil spill cleanup exposures are largely unknown, despite the endocrine-disrupting potential of crude oil and oil dispersant constituents. We aimed to investigate risks of longer-term endocrine and metabolic conditions among U.S. Coast Guard (USCG) responders to the Deepwater Horizon (DWH) oil spill. METHODS: Our study population included all active duty DWH Oil Spill Coast Guard Cohort members (N = 45,224). Self-reported spill exposures were ascertained from post-deployment surveys. Associations between weight gain, integrase inhibitors antiretroviral agents, and gut microbiome in people living with HIV: a cross-sectional study.

Dolutegravir and bictegravir are second-generation HIV integrase strand transfer inhibitors (INSTIs) that were previously associated with abnormal weight gain. This monocentric cross-sectional study investigates associations between weight gain during the first year after initiation of dolutegravir, bictegravir or other anchor drugs and gut microbiome diversity as well as taxa composition. The study enrolled 79 participants receiving dolutegravir, 32 receiving bictegravir and 10 receiving non-INSTI based regimens. Most of them were treatment experienced at initiation of those anchor drugs agen

Gastro-esophageal diagnostic workup before bariatric surgery or endoscopic treatment for obesity: position statement of the International Society of Diseases of the Esophagus.

Obesity is a chronic and multifactorial condition characterized by abnormal weight gain due to excessive adipose tissue accumulation that represents a growing worldwide challenge for public health. In addition, obese patients have an increased risk of hiatal hernia, esophageal, and gastric dysfunction, as well as gastroesophageal reflux disease, which has a prevalence over 40% in those seeking endoscopic or surgical intervention. Surgery has been demonstrated to be the most effective treatment for severe obesity in terms of long-term weight loss, comorbidities, and quality of life improvements

The "crosstalk" between microbiota and metabolomic profile in high-fat-diet-induced obese mice supplemented with Bletilla striata polysaccharides and composite polysaccharides.

The potential prebiotic feature of Bletilla striata polysaccharides (BSP) has been widely accepted, while the beneficial effect of BSP on high-fat-diet-induced obesity is unclear. Moreover, the "crosstalk" between microbiota and metabolomic profile in high-fat-diet-induced obese mice supplemented with BSP still need to be further explored. The present study attempted to illustrate the effect of BSP and/or composite polysaccharides on high-fat-diet-induced obese mice by combining multi-matrix (feces, urine, liver) metabolomics and gut microbiome. The results showed that BSP and/or composite pol
