Targeting host ferroptosis in cancer cachexia: Tumor-immune triggers, tissue wasting, and the therapeutic paradox.

Cancer cachexia is a systemic wasting syndrome driven by tumor burden, chronic inflammation, disrupted nutrient handling, and neuroendocrine stress. Supportive nutrition, exercise, and symptom management rarely restore lean mass, function, or treatment tolerance once wasting is established. Ferroptosis, iron-dependent cell death caused by phospholipid peroxidation, may connect inflammatory iron redistribution, lipid remodeling, mitochondrial stress, and impaired antioxidant defenses. This review critically evaluates ferroptosis in cachexia from a tumor-host pharmacology perspective, distinguis

TRIM22 in cancer: a context-dependent E3 ligase at the interface of interferon, p53, and tumor plasticity.

TRIM22 is a TRIM family E3 ubiquitin ligase whose expression is strongly induced by interferons. For many years, it has been identified primarily as an antiviral effector, a role that remains well validated. A growing body of cancer-related work now forces us to expand that view considerably. Depending on tumor type, upstream signaling context, and the specific substrates available in a given cell, TRIM22 can either constrain malignancy or actively promote it. In breast, colorectal, ovarian, endometrial, hepatocellular, cervical, and gastric cancers, as well as in certain lung tumor subsets, T

Childhood-onset parkinsonism, dystonia and severe muscle wasting in a Sudanese girl caused by a novel homozygous DnaJ heat shock protein family (heat shock protein 40) member C6 frameshift variant, with an incidental pathogenic breast cancer 1 gene secondary finding: a case report.

BACKGROUND: DNAJC6-related juvenile parkinsonism is a rare autosomal recessive disorder of auxilin, a co-chaperone essential for clathrin-mediated synaptic vesicle recycling. Fewer than 50 kindreds have been reported worldwide, and the condition is easily missed when first-line investigations come back normal. We describe a Sudanese adolescent in whom a previously unreported homozygous DNAJC6 frameshift variant was identified only after a prolonged diagnostic journey that included a period during which her illness was mistakenly attributed to a functional neurological disorder. Clinical whole-

Targeting Muscle-Specific E3 Ligases in Cancer Cachexia Through Structure-Based Virtual Screening of Small-Molecule Inhibitors.

Cachexia is a multifactorial metabolic syndrome characterized by severe skeletal muscle loss that cannot be reversed by nutritional support. Common in cancer and other chronic diseases, it significantly increases morbidity and mortality. Muscle wasting is primarily driven by activation of the ubiquitin-proteasome pathway through the E3 ubiquitin ligases MuRF1 and Atrogin-1, which promote protein degradation and muscle atrophy. This study used computational modeling and docking to identify small-molecule inhibitors targeting these ligases, including repurposed drugs with anti-cachectic potentia

Integrated Single-Myofibre Multi-Omics Correlates Inflammatory State to Myosin Energetics in ICU-Acquired Weakness.

BACKGROUND: Skeletal muscle atrophy is a pervasive complication of critical illness that worsens survival and recovery, yet remains poorly explained by current clinical or molecular markers. METHODS: To directly connect disease-associated molecular states to the contractile machinery, this study combined sequential functional, transcriptomic and proteomic profiling of the same single human skeletal myofibres from critically ill patients in the intensive care unit with acquired weakness (ICU-AW) and controls. This integrated approach provides a framework for validating molecular signals direct

Spatiotemporal metabolic networks in a preclinical model of pancreatic cancer and associated cachexia.

Cachexia or body wasting is a significant contributing factor to the morbidity and mortality of pancreatic cancer. Despite an unmet need, its metabolic underpinnings remain poorly understood. Here, we characterize a mouse model that recaptures the longitudinal progression of human cachexia during pancreatic cancer development. We profile the metabolomes of pre-, early- and late-stage cancer-associated cachexia in male and female pancreas, its interstitial fluid (IF), plasma, liver, adipose tissue and skeletal muscle. We find that each tissue has a unique metabolome and trajectory across stages

Emerging roles of OSM and EDA2R-NIK signalling in cancer-associated muscle wasting.

Cancer-associated cachexia (CAC) is a multifactorial metabolic syndrome characterized by progressive skeletal muscle wasting that reduces physical function, compromises treatment tolerance, and contributes to cancer-related mortality. Although systemic inflammation is a major driver of CAC, the mechanisms through which circulating inflammatory signals are translated into muscle-specific catabolic programmes remain incompletely understood. Recent studies have identified oncostatin M (OSM), a member of the IL-6 cytokine family, as an important mediator of cancer-induced muscle wasting through OS

Preoperative circulating musclin as a candidate biomarker of postoperative skeletal muscle vulnerability in colorectal cancer.

INTRODUCTION: Postoperative skeletal muscle deterioration is associated with impaired recovery and adverse outcomes after colorectal cancer (CRC) surgery, but preoperative circulating markers of subsequent muscle loss remain insufficiently defined. We evaluated whether circulating musclin was associated with postoperative skeletal muscle status and longitudinal skeletal muscle index (SMI) change. MATERIALS AND METHODS: This retrospective single-center study included 106 adults undergoing CRC surgery, of whom 95 had postoperative computed tomography (CT) data. Postoperative CT was obtained at

Aberrant Myelopoiesis and Haematopoietic Stem Cell Niche Remodelling Precedes Cancer Cachexia Onset in a Sex-Dependent Manner.

BACKGROUND: Male cancer patients are 30% more likely to develop cachexia compared to females; however, the underlying aetiology remains unknown. Myelopoiesis induces inflammation in cancer cachexia, which contributes to muscle atrophy. Whether differences in myelopoiesis during cancer cachexia initiation contribute to sex-based differences in cachexia incidence remains unknown. METHODS: Male and female C57BL/6 and B6(C)-Ccr2tm1.1Cln/J mice were randomized into sham (PBS) or were inoculated subcutaneously with Lewis lung carcinoma (LLC) to induce cachexia. Mice were sacrificed 2 (14 days), 2.5

Central Neuroendocrine Governance as a Unifying Framework for Host-Tumor Interaction: From Afferent Sensing to Therapeutic Modulation.

Cancer is not biologically confined to the tumor bed. Cachexia, fatigue, metabolic dysfunction, circadian disruption, and immune dysfunction reflect disturbances that local models alone cannot fully explain. The brain is well positioned to participate in these processes because it continuously samples circulating signals and coordinates endocrine and autonomic responses. Yet the afferent side of this communication has received less attention than its downstream consequences. Here, we synthesize evidence showing that cytokines, hormones, metabolites, and cargo carried by extracellular vesicles

Body Weight, Body Composition, Nutrition, and Sex in Gastric Cancer: Epidemiological Associations and Surgical Outcomes-A Narrative Review.

Gastric cancer remains a major cause of cancer-related mortality worldwide, while the roles of excess body weight, body composition, nutrition, and biological sex remain incompletely defined. This narrative review separately evaluates (i) epidemiological associations with gastric cancer risk, with explicit attention to cardia versus non-cardia tumors and Helicobacter pylori as a major confounder, and (ii) perioperative and long-term outcomes after gastrectomy, including postoperative complications, treatment completion, recurrence, and survival. NMA plus antitumor therapy versus antitumor therapy alone in patients with BCLC stage B or C hepatocellular carcinoma and pre-cachexia or cachexia: protocol for a prospective, multicenter, randomized, open-label phase II trial.

BACKGROUND: Cancer anorexia-cachexia syndrome (CACS) is prevalent among patients with Barcelona Clinic Liver Cancer (BCLC) stage B or C hepatocellular carcinoma (HCC) and is strongly associated with poor prognosis, reduced treatment tolerance, and impaired quality of life. Megestrol acetate (MA) has demonstrated efficacy in improving appetite and body weight; however, conventional formulations are limited by variable bioavailability. This trial will evaluate the efficacy and safety of nanocrystalline megestrol acetate (NMA) in patients with BCLC stage B or C HCC and pre-cachexia or cachexia re

A Proposed Consensus Definition of Severe COPD: a modified Delphi Study.

RATIONALE: Advanced interventions are available for patients with more severe COPD despite guideline-recommended therapies, yet this population is heterogeneously defined and inconsistently recognized, and a standardized work-up is lacking, which could lead to mismanagement. OBJECTIVES: To formulate a new, expert-based consensus definition of severe COPD and a minimal set of diagnostics and characteristics to be assessed in the work-up of these patients. METHODS: A modified Delphi study with two online rounds was conducted between June 2024 and January 2025 with 55 international COPD experts

From stress cytokine to drug target: The translational landscape of growth differentiation factor 15.

Growth differentiation factor 15 (GDF15) is a stress-responsive cytokine that translates cellular injury, metabolic imbalance, and disease burden into systemic physiological responses. Discovery of its hindbrain-restricted receptor, glial cell line-derived neurotrophic factor family receptor α-like (GFRAL) and cosignaling partner rearranged during transfection (RET), established a dedicated neuroendocrine axis (GDF15-GFRAL-RET) linking stress to appetite suppression, nausea, and autonomic effects on energy expenditure. Initially regarded as a passive disease biomarker, GDF15 is now recognized

Complement C3 is associated with cancer-induced frailty: genetic and clinical evidence.

BACKGROUND: Frailty is a degenerative geriatric syndrome characterized by declining muscle mass and function, unintentional weight loss, and fatigue. It is a key risk factor for many geriatric diseases and poor postoperative outcomes. Currently, pathogenesis remains unclear, and apart from nutritional support and exercise, no pharmacological interventions are available. Notably, the frailty phenotype is also a major feature in patients with advanced-stage cancer, and both frailty and advanced cancer are characterized by dysregulation of nutritional and energy metabolism. Consequently, cancer s

An NAD+-responsive adipose signal coordinates inter-organ defense against muscle atrophy.

Loss of skeletal muscle mass is a major driver of frailty and disability, yet how systemic metabolic cues protect muscle integrity remains unclear. We identify Serpina3n, an adipose-derived adipokine, as a key mediator of the protective effects of nicotinamide adenine dinucleotide (NAD+) supplementation against muscle atrophy. Elevation of NAD+ in adipose tissue activates signal transducer and activator of transcription (STAT)1-dependent transcription of Serpina3n, leading to its systemic release and muscle protection across models of aging, disuse, and cancer cachexia. In muscle, Serpina3n pr

CPT1A-driven metabolic reprogramming serves as a compensatory adaptation to sustain energy homeostasis in obstructive cholestasis.

BACKGROUND: Cholestatic liver disease is characterized by severe weight loss that remains a major clinical challenge due to limited therapeutic efficacy. This metabolic dysregulation may stem from impaired energy homeostasis, yet the underlying mechanisms remain poorly understood. Fatty acid oxidation (FAO) is a critical energy-producing pathway in the liver, and its adaptation under cholestatic stress may determine whether patients experience metabolic resilience or progressive cachexia. METHODS: Adult male Sprague-Dawley rats and New Zealand rabbits underwent common bile duct ligation (BDL)

[The gut-muscle axis and the role of Roseburia inulinivorans in age-related muscle loss: a narrative review].

Age-related muscle loss (sarcopenia) represents one of the most significant health burdens in a globally ageing population. The relationship between the gut microbiome and skeletal muscle, referred to as the gut-muscle axis, is the subject of increasing research, yet the identification of specific responsible microbial species and mechanisms has remained limited. This review aims to summarize the current evidence on the gut-muscle axis, with particular focus on the association between the intestinal bacterium Roseburia inulinivorans and muscle performance reported in a 2026 Gut publication, an
