Current Trends in Duchenne Muscular Dystrophy Research and Therapy: 3D Cardiac Modelling.

Duchenne muscular dystrophy (DMD), caused by dystrophin deficiency, presents a multifaceted challenge that affects both skeletal muscle function and cardiomyocyte homeostasis, causing progressive degeneration and life-threatening cardiac complications by adolescence. Current treatments fail to prevent poor prognoses, and while FDA-approved therapies show promise in targeting dystrophin restoration, including RNA-based approaches and microdystrophin gene therapy, clinical evidence supporting their efficacy remains limited. Substantial challenges persist, particularly in achieving effective card

The FGL1-LAG-3 axis attenuates melanoma-induced cachexia in mice.

Cachexia is a debilitating condition characterized by the wasting of adipose and skeletal muscle tissues, resulting in significant weight loss and frailty. It is responsible for 20% of cancer-related mortalities. Despite its clinical significance, the precise mechanisms underlying cachexia remain inadequately understood. Our gene expression analysis of white adipose tissues has identified an upregulation of fibrinogen-like protein 1 (FGL1) in B16F10 tumor-bearing mice. In this study, we demonstrate that the intraperitoneal administration of FGL1 protein mitigates tumor-induced wasting of adipo

Mitochondrial DNA Maintenance Defects: Clinical, Imaging, and Genetic Spectrum of Four Patients from a Single Tertiary Care Centre.

Mitochondrial DNA maintenance defects (MDMD) are rare genetic disorders that typically present in infancy but can manifest later with multi-organ involvement. We describe four MDMD cases (age 19-25) with distinct clinical and genetic profiles and delayed diagnosis. Two patients with mitochondrial neurogastrointestinal encephalomyopathy (homozygous TYMP variants: c.454G>T, c.866A>C) exhibited cachexia, ptosis, neuropathy, and confluent white matter hyperintensities leukodystrophy. Two others with MPV17 (c.293C>T) presented with neuromyopathy and hepatosplenomegaly; one showed novel concentric r

A catecholamine-independent pathway controlling adaptive adipocyte lipolysis.

Several adipose depots, including constitutive bone marrow adipose tissue, resist conventional lipolytic cues. However, under starvation, wasting or cachexia, the body eventually catabolizes stable adipocytes through unknown mechanisms. Here we developed a mouse model of brain-evoked depletion of all fat, including stable constitutive bone marrow adipose tissue, independent of food intake, to study this phenomenon. Genetic, surgical and chemical approaches demonstrated that catabolism of stable adipocytes required adipose triglyceride lipase-dependent lipolysis but was independent of local ner

Survival and determinants of mortality in nursing home residents with young onset dementia.

OBJECTIVES: People with young-onset dementia (YOD), defined as symptom onset before the age of 65, have mortality rates five to eight times higher than those of the general population of similar age. However, survival studies focused on individuals living in the community rather than those residing in nursing homes. This study aimed to estimate survival rates, its determinants, and causes of death in nursing home residents with YOD. DESIGN: Survival data from the BEYOnD (2005-2018) and Care4Youngdem (2016-2021) cohort studies. SETTING: YOD special care units of 20 nursing homes in the Nether

Sex differences in cachexia outcomes and branched-chain amino acid metabolism following chemotherapy in aged mice.

Cachexia is a complex muscle wasting syndrome that affects the majority of hospitalized cancer patients receiving chemotherapy. It is often unresponsive to nutritional interventions, including provision of branched-chain amino acids (BCAAs: leucine, isoleucine and valine). BCAAs are anabolic for skeletal muscle. We wondered whether their ineffectiveness in managing cachexia might be related to altered metabolism of these amino acids, a subject that has received minimal attention. Because estrogen limits BCAA catabolism, we hypothesized that the effects of chemotherapy on cachexia in old mice w

Association of adipose tissue activity in cervical regions with weight loss and prognostic survival in gastric cancer patients.

BACKGROUND: With the application of positron emission tomography-computed tomography (PET-CT) in recent years, studies have found that brown adipose tissue is not only present in infants and hibernating mammals but also persists in the neck regions of adults. Cancer-associated cachexia, a complex metabolic syndrome, is characterized by adipose tissue loss and heightened metabolic activity. Despite the profound impact of cachexia on patient outcomes, research into the metabolic dynamics of brown adipose tissue has been sparse. The objective of this study is to quantify the metabolic activity of

Cancer cachexia: molecular basis and therapeutic advances.

The dynamic interplay between neoplastic cells and the host has been increasingly recognized as important players in the pathogenesis of cancer cachexia, a syndrome affecting ~50-80% of cancer patients with various incidences of different types of malignancies. Despite its prevalence, a comprehensive understanding of cancer cachexia progression, with a holistic view at the cross-organismal, cellular and molecular levels, remains elusive. In this review, we undertake an in-depth exploration of the relevant target organs and their regulatory roles in cancer cachexia, with a particular focus on m

Tucatinib plus trastuzumab for chemotherapy-refractory, HER2 + , RAS wild-type metastatic colorectal cancer (MOUNTAINEER): final analysis.

MOUNTAINEER was a multicenter, open-label, phase 2 trial (NCT03043313) that evaluated the efficacy and safety of tucatinib plus trastuzumab, a dual HER2-targeted chemotherapy-free regimen. Patients were included if they had chemotherapy-refractory, HER2+, RAS wild-type unresectable or metastatic colorectal cancer. This final analysis reports updated efficacy and safety after a median follow-up of 32.4 months. Of the 84 patients who received tucatinib plus trastuzumab, the confirmed objective response rate was 39.3%; median duration of response was 15.2 months. Median progression-free survival

Appetite for change - the need to revisit malnutrition screening and assessment in oncology.

Malnutrition is highly prevalent among oncology patients, with large-scale studies reporting involuntary weight loss in 31-87%, depending on tumour site and disease stage. A combination of nutrition-impact symptoms, reduced oral intake and systemic inflammation lead to poor tolerance to treatment, diminished quality of life and reduced survival. Systemic inflammation is a hallmark of cancer-associated malnutrition and contributes to loss of lean mass and abnormal body composition phenotypes (sarcopenia, cachexia and low muscle density) which may coexist with overweight and obesity. Malnutritio

Impact of parenteral nutrition on oral intake and eating pleasure in advanced cancer patients: A sub-analysis of a multicenter randomized controlled trial.

BACKGROUND: Malnutrition is highly prevalent in advanced cancer patients, contributing to reduced quality of life, impaired physical function, and decreased survival. Although parenteral nutrition (PN) is used to supplement oral intake, its effect on oral feeding behaviors and eating pleasure remains unclear. We therefore aimed to evaluate the impact of supplemental PN on oral food intake and eating pleasure in malnourished advanced cancer patients with an intact bowel function. METHODS: This study presents a sub-analysis of data from a larger, multicenter, randomized controlled trial. In thi

TNF-α-induced contractile dysfunction in three-dimensional engineered muscle.

Three-dimensional engineered muscle (3D-EM) provides a physiologically relevant model for examining skeletal muscle function. Tumor necrosis factor-α (TNF-α), a pro-inflammatory cytokine elevated in chronic conditions such as sarcopenia and cachexia, has been linked to muscle weakness. However, the mechanism underlying this effect remains unclear. In this study, we used a 3D-EM system to evaluate the direct impact of TNF-α on muscle contractile force. 3D-EM was produced by seeding C2C12 myoblasts with type I collagen on a culture device, followed by 15 days of differentiation. Constructs were

Skeletal Muscle Mass Modifies the Prognostic Impact of LDL Cholesterol in Chronic Heart Failure.

BACKGROUND: Dyslipidaemia is among the major risk factors for atherosclerotic cardiovascular disease. Paradoxically, higher cholesterol levels are associated with better survival in heart failure (HF) of any aetiology. Because cholesterol is an integral component of skeletal muscle structure, one possible explanation involves the interplay between lipid metabolism and skeletal muscle health. Using data from the Studies Investigating Comorbidities Aggravating Heart Failure, we investigated whether an association exists between low-density lipoprotein cholesterol (LDL-C) levels and all-cause mor

A randomized controlled trial evaluating the effects of anamorelin in postoperative patients with upper gastrointestinal cancer: protocol for the CLEAR-UP study.

Patients undergoing curative surgery for upper gastrointestinal (UGI) cancers often experience substantial postoperative weight loss and reduction in muscle mass, one contributing factor being decreased ghrelin secretion following surgery. Anamorelin, a ghrelin receptor agonist, has been shown to improve appetite and increase lean body mass in patients with cancer cachexia; however, its efficacy in the postoperative setting has not been established. The CLEAR-UP study is a multicenter, open-label, randomized controlled trial designed to investigate the clinical effects of anamorelin in patient

Albumin Levels Are Predictive of Cachexia-Induced Time-Dependent Clearance of Therapeutic Antibodies: A Physiologically Based Pharmacokinetic Model of Durvalumab.

Cachexia is a metabolic condition that accelerates the clearance of monoclonal antibodies in cancer patients and is a known mechanism causing time-dependent clearance. Successful anticancer treatment often ameliorates symptoms of cachexia, reducing the drug clearance over time especially in patients who respond. Serum albumin level is a common biomarker of cachexia that is frequently associated with antibody drug clearance. Physiologically based pharmacokinetic (PBPK) models of antibody drugs have incorporated albumin metabolism but have not been applied to describe time-varying clearance due

Mitochondrial Transporter ABCB10 Protects Against Doxorubicin-Induced Respiratory Muscle Dysfunction Independent of Changes to Diaphragm Accumulation.

BACKGROUND: Doxorubicin (DOX) is a highly effective chemotherapeutic agent whose use can cause respiratory toxicity, increasing patient fatigue and negatively impacting quality of life and survival. These adverse effects occur due to diaphragm muscle mitochondrial accumulation of DOX, where it causes reactive oxygen species production and iron dysregulation. ABCB10 is a mitochondria-localized ATP-binding cassette transporter hypothesized to play a role in the maintenance of mitochondrial redox balance and iron homeostasis, and potentially the mitochondrial export of DOX. This study investigate

Associations of nutritional risk, pre-treatment malnutrition, and cachexia with mortality, local recurrence, and metastasis in head and neck cancer: a five-year prospective cohort study.

BACKGROUND: Malnutrition is a recognized risk factor for reduced survival in patients with head and neck cancer (HNC). However, evidence remains limited regarding whether, and to what extent, pre-treatment nutritional parameters contribute to local recurrence and metastasis. So, the aim was to estimate the association between pre-treatment nutritional parameters and these outcomes such as mortality, local recurrence, and metastasis over a five-year follow-up in patients with HNC, and to assess the prognostic validity of the nutritional parameters. METHODS: A prospective cohort study was condu
