Multi-cohort transcriptomics and donor-aware single-cell analyses prioritize S100A8 within an emergency-myeloid, HLA-DR-low state in sepsis.

Sepsis is heterogeneous, and transcripts that distinguish sepsis from health may lack clinical specificity. We tested whether S100A8 is reproducibly associated with sepsis and maps to a donor-aware myeloid state defined independently of S100A8. A discovery whole-blood cohort was analyzed on the log2 robust multi-array average scale using maximum-probe aggregation, limma, weighted gene co-expression network analysis (WGCNA), and leakage-controlled feature prioritization. The analysis identified 1,508 differentially expressed genes; 265 overlapped the strongest sepsis-positive WGCNA module. The

SAPHO-like bone scintigraphy in diffuse idiopathic skeletal hyperostosis: a case report.

BACKGROUND: Bone scintigraphy is sensitive to increased osteoblastic activity but is not disease specific. Diffuse idiopathic skeletal hyperostosis (DISH) may therefore be mistaken for synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome when uptake involves the anterior chest wall and spine. CASE PRESENTATION: A 50-year-old woman presented with a 1-year history of painless left sternoclavicular prominence. She reported that her daughter had SAPHO syndrome with anterior chest wall pain and palmoplantar pustulosis. Whole-body 99mTc-MDP scintigraphy and single-photon emissio

Sepsis-associated endothelial dysfunction: clinical phenotypes, biomarkers, bedside evidence, and translational implications.

Sepsis is a systemic vascular disorder in which endothelial activation, dysfunction, and structural injury contribute to shock, microcirculatory failure, immunothrombosis, and multiorgan dysfunction. Sepsis-associated endotheliopathy is not a uniform process but a dynamic trajectory that evolves from adaptive endothelial activation toward functional disruption, structural injury, and organ-specific vascular failure. In this Review, we propose a phenotype- and trajectory-oriented framework that integrates clinical manifestations, molecular mechanisms, and translational assessment of endothelial

Nanotherapeutic macrophage reprogramming through immunometabolic and microbial modulation for sepsis treatment.

Sepsis is a life-threatening syndrome characterized by dysregulated immune responses and metabolic dysfunction, and remains a major cause of global mortality. While nanomedicine offers new opportunities for sepsis therapy, current strategies rarely achieve comprehensive modulation of macrophage immunometabolism and microenvironmental signaling. Here, we develop a glutathione-responsive nanotherapeutic platform (EONPs) by conjugating the stimulator of interferon genes (STING) inhibitor 4-octyl itaconate to polyethylene glycol, followed by encapsulation of the dihydroorotate dehydrogenase (DHODH

Early hemodynamic changes after the use of push-dose noradrenaline in patients presenting to the emergency department with septic shock: a descriptive study.

BACKGROUND: Although continuous noradrenaline infusion (CNI) is standard vasopressor therapy during the initial resuscitation of patients with septic shock, hemodynamic stabilization may not be immediate. Push-dose noradrenaline (PDNA) has been proposed as a temporary measure while CNI is being established. However, its clinical use, dosing, early hemodynamic response, and short-term safety in the emergency department (ED) are not well described. METHODS: To address this gap, we conducted a single-center retrospective descriptive study at a tertiary care hospital in Japan in 2019. Adults with

[Mechanisms by which interleukin-18 binding protein regulates the NLRP3 inflammasome in Kawasaki disease-related coronary artery lesions].

Kawasaki disease (KD) is an acute febrile illness characterized by systemic vasculitis, with coronary artery lesion (CAL) representing its most serious complication. The NOD-like receptor family pyrin domain containing 3 (NLRP3) inflammasome plays a pivotal role in KD-associated CAL. Upon activation, it cleaves and releases mature interleukin-18 (IL-18) and other proinflammatory cytokines, which propagates downstream inflammatory cascades. IL‑18 binding protein (IL‑18BP) is an endogenous high-affinity antagonist. Studies have shown that dysregulation of the IL‑18/IL‑18BP axis, together with ab

Log-transformed systemic inflammation response index improves prediction of 28-day ICU mortality in critically ill patients with cirrhosis and atrial fibrillation: development in MIMIC-IV and external validation in eICU-CRD.

BACKGROUND: Short-term mortality is high among critically ill patients with cirrhosis and atrial fibrillation (AF). Existing scores such as SOFA, MELD-Na, and CHA₂DS₂-VASc do not directly reflect systemic inflammation. We investigated whether blood count-derived inflammatory indices could improve prediction of 28-day ICU mortality in this high-risk population. METHODS: Adult ICU patients with cirrhosis and AF were identified from MIMIC-IV v3.1 (single-center development cohort) and eICU-CRD v2.0 (multicenter external validation cohort). Five inflammatory indices, including SIRI, log-SIRI, SII

Controlling nutritional status score as a novel prognostic tool for hospital mortality risk stratification in sepsis: a large-scale retrospective cohort analysis.

INTRODUCTION: Malnutrition is associated with poor outcomes in critically ill patients. However, the prognostic value of the Controlling Nutritional Status (CONUT) score in sepsis remains unclear. This study aimed to investigate the association between CONUT score and hospital death in sepsis patients. METHODS: In this retrospective cohort study, 2,143 sepsis patients from the eICU Collaborative Research Database were categorized into Mild (2-4 points), Moderate (5-8 points), and Severe (9-12 points) malnutrition groups based on the CONUT score. Multivariable logistic regression analysis was

Respiratory Viral Positivity Is Associated with Decreased Risk of Serious Bacterial Infections in Febrile Infants.

INTRODUCTION: Febrile infants ≤ 60 days of age are at increased risk for serious bacterial infections (SBIs), often necessitating extensive diagnostic evaluation and hospitalization. Although respiratory viral infections are common in this population and may influence the likelihood of concurrent bacterial infection, the relationship between respiratory viral panel (RVP) results and SBI risk remains an important consideration in clinical decision-making. The objective of this study was to evaluate the association between RVP results and the occurrence of SBIs among febrile infants ≤ 60 days of

Changes in the microbial spectrum and antimicrobial resistance of bloodstream infections in intensive care units before and during the COVID-19 pandemic.

BACKGROUND: Bloodstream infections (BSIs) are major infectious conditions associated with high morbidity and mortality, particularly in intensive care units (ICUs). In critically ill patients, early and appropriate empirical antimicrobial therapy plays an important role in preventing adverse clinical outcomes. However, the increasing burden of antimicrobial resistance necessitates that empirical treatment strategies be continuously reassessed based on up-to-date local resistance data. This study aimed to compare the distribution of bacterial pathogens and antimicrobial resistance profiles of B

Development, Validation, and Deployment of an Interpretable Machine Learning Model for In-Hospital Mortality Prediction in Critically Ill Patients with COPD and Sepsis: A Multicenter Cohort Study.

BACKGROUND: Critically ill patients with chronic obstructive pulmonary disease (COPD) and sepsis are at high risk of in-hospital mortality because of respiratory and organ dysfunction. Early risk assessment may be useful for clinical decision-making. This study aimed to develop an interpretable machine learning model for this population. METHODS: Data were extracted from MIMIC-IV and the eICU Collaborative Research Database (eICU-CRD). The MIMIC-IV cohort was split in a 7:3 ratio into training and internal test sets. Features were selected using least absolute shrinkage and selection operator

TRIM36 catalyzes K63-linked ubiquitination of TAK1 to drive septic inflammation.

BACKGROUND: The Toll-like receptor 4 (TLR4) pathway is critical for initiating innate immunity; however, its dysregulation drives pathological inflammation in diseases such as sepsis. METHODS: In the present study, we utilized septic patients, lipopolysaccharide (LPS)-induced mouse models, and LPS-treated mouse macrophages as well as human THP-1-derived macrophages to investigate the potential functions and molecular mechanisms of TRIM36 in sepsis. RESULTS: Here, we identify tripartite motif protein 36 (TRIM36) as a key positive regulator that amplifies TLR4 signaling. TRIM36 expression was

UNLABELLED: Carbapenem-resistant (CR) Acinetobacter spp. bloodstream infections (BSIs) are difficult to treat in children, yet pediatric data on factors associated with resistance and mortality remain limited. We aimed to identify factors associated with carbapenem resistance and 30-day mortality in pediatric Acinetobacter BSIs. We retrospectively analyzed children with monomicrobial Acinetobacter BSIs at a tertiary-care university hospital between January 2014 and September 2025. Factors associated with carbapenem resistance were assessed using co-primary logistic regression analyses in the f

The Immunological Consequences of Pediatric Adenotonsillectomy: Myths, Evidence, and Unsolved Questions.

Tonsillectomy is one of the most common surgical procedures in pediatric populations, indicated primarily for recurrent tonsillitis and upper airway obstruction. Palatine tonsils represent lymphoepithelial structures crucial for local and systemic immune surveillance, with a key role in lymphocyte maturation and the production of immunoglobulins, particularly secretory IgA. This review summarizes the available evidence on the immunological impact of tonsillectomy, including short- and long-term studies. Data show that tonsillar removal can result in transient changes in immunoglobulin and lymp

Correlation of Neuron-Specific Enolase and S100ß Protein Levels, Inflammatory Cytokines, and Neurological Injury in Children Undergoing Cardiopulmonary Bypass.

BACKGROUND: The systemic inflammatory response syndrome to cardiopulmonary bypass (CPB) affects all organ systems including the brain. Perioperative brain injury in children with congenital heart disease is often clinically silent in the acute perioperative period and may not be detected until patients are older. The plasma biomarkers neuron-specific enolase (NSE) and S100ß protein show potential for detecting neurological injury in the perioperative period. OBJECTIVES: The three objectives of this study were to examine the correlation of NSE and S100ß protein levels with inflammatory cytokin
