Increasing evidence-based care practices for patients with Staphylococcus aureus bacteraemia through required infectious diseases consultation in a tertiary care hospital: a quality improvement initiative.

BACKGROUND: Staphylococcus aureus bacteraemia had a higher mortality rate than average at Kingston Health Sciences Centre (KHSC). Infectious diseases specialist consultation has been shown to improve outcomes for S. aureus bacteraemia by increasing adherence to evidence-based care practices. Yet, infectious disease specialists were not involved in many cases at KHSC. AIM: To improve adherence to evidence-based care practices by increasing the proportion of patients with S. aureus bacteraemia who receive a formal infectious diseases consultation. INTERVENTIONS: A multimodal intervention consi

Shortening antibiotic therapy duration for hospital-acquired bloodstream infections in critically ill patients: a causal inference model from the international EUROBACT-2 database.

INTRODUCTION: Hospital-acquired bloodstream infections (HA-BSIs) are severe and require antibiotic therapy. In non-complicated BSIs, shortened therapy reduces side effects without compromising efficacy. The impact of shortened antibiotic therapy in HA-BSI critically ill patients without indication of prolonged therapy requires further evaluation. METHODS: Using the international prospective EUROBACT-2 cohort, we compared shortened (7-10 days) versus long (14-21 days) treatment durations in ICU patients eligible for shortened therapy. Patients without antibiotic therapy within 3 days after HA-

Antibiotic therapy and clinical outcomes of penicillin-susceptible Staphylococcus aureus (PSSA) bloodstream infection (BSI): a ten-year retrospective cohort study.

In recent years, the incidence of penicillin-susceptible S. aureus (PSSA) bloodstream infection (BSI) has increased worldwide. However, the preferred antibiotic remains uncertain due to concerns of inducible resistance to benzylpenicillin. We compared outcomes associated with benzylpenicillin versus other antibiotics and investigated risk factors influencing treatment failure. Patients were grouped into benzylpenicillin and non-benzylpenicillin beta-lactam treatment groups (including anti-staphylococcal penicillins and cephalosporins). The primary outcome was overall treatment failure (30-day

Novel Aryl Sulfonium Modification on Vancomycin to Tackle MRSA and VRE In Vitro and In Vivo through Dual Enhanced Cell-Wall and Membrane Inhibition.

Gram-positive superbugs resistant to methicillin and vancomycin pose a severe threat to global public health, urgently demanding novel therapeutic strategies. Herein, we rationally designed and synthesized vancomycin derivatives modified with diverse aryl sulfonium moieties to reactivate its antibacterial potency. By optimizing the sulfonium-based SAR, we got derivatives 2-3 orders of magnitude more active in vitro than vancomycin. Subsequently, preliminary toxicity evaluations for the optimal derivative, 7e, indicated a favorable therapeutic index, while pharmacokinetic assays revealed its go

Bacterial Specific Recognition of Sulfonium Poly(Amino Acid) Adsorbents for Ultrafast MRSA Capture Against Bloodstream Infection.

Methicillin-resistant Staphylococcus aureus (MRSA) bloodstream infections pose significant health risks, potentially leading to severe conditions such as bacteremia. Developing effective treatments to eliminate resistant bacteria from the bloodstream, simultaneously mitigate infection-related complications, and reduce mortality remains challenging. Herein, microspheres are synthesized with bacterial elimination and inflammation prevention by crosslinked sulfonium poly(amino acids). As-synthesized microsphere, PM1 0.6B MS, exhibits an ultrafast adsorption efficiency of 0.41 × 108 CFU mg-1 min-1

The Importance of Systemic Inflammatory Response Syndrome in Diagnosing Occult Infections: A Case Report.

Systemic inflammatory response syndrome (SIRS) criteria are among the several screening tools for sepsis. While SIRS has been widely utilized in clinical practice, its reliability and construct validity have come under scrutiny in recent years. Research has established that the quick Sequential Organ Failure Score (qSOFA) and Sequential Organ Failure Assessment (SOFA) criteria serve as better predictors of sepsis-related outcomes. We report a case of a patient who was admitted for diabetic ketoacidosis and atrial fibrillation. Despite being afebrile with stable white blood cell counts, he met

Dual-membrane bioinspired nanocarriers for targeted therapy of MRSA-induced acute lung injury and bacteremia.

Bioinspired nanoparticles enhance the targeting of specific organs by facilitating interactions and communication at the nano-bio interface. Combining human neutrophil and lung epithelial cell membranes for nanoparticle cloaking offers distinct advantages in binding to bacteria and pulmonary epithelium, thus targeting infection-induced inflammatory areas. This study aimed to develop rifampicin-loaded biomimetic nanocarriers by wrapping a polymeric core with dual membranes derived from neutrophils and A549 cells, inheriting the membrane characteristics of the native cells. To evaluate the thera

Red Blood Cell-Induced Bacterial Margination Improves Microbial Hemoadsorption on Engineered Cell-Depleted Thrombi, Restoring Severe Bacteremia in Rats.

Extracorporeal hemoadsorption for treating bacteremia has exhibited limited success due to the lack of a clear strategy for effectively bringing bacterial cells into contact with the surface and universal bacteria-capturing substances. Here, a novel extracorporeal device is reported that can eliminate various intact bacteria from whole blood, employing microfluidic bacterial margination and engineered cell-depleted thrombus (CDT) presenting bacterial adhesin receptors. The critical strain rate of red blood cells (RBCs) (0.83 × 10-2) and the flow path height within about 300 µm required for RBC

Genomic and phenotypic adaptations of methicillin resistant Staphylococcus aureus during vancomycin therapy.

Methicillin-resistant Staphylococcus aureus (MRSA) poses a significant global health challenge, particularly associated with serious infections such as bacteremia. Lipoglycopeptide antibiotics, including vancomycin, dalbavancin, and daptomycin, are critical in MRSA treatment. In this study, we analyzed two MRSA isolates (XF1 and XF2) from a bacteremia patient treated with vancomycin. Antimicrobial susceptibility testing revealed that XF1 was sensitive to vancomycin, dalbavancin, and daptomycin, whereas XF2 exhibited 8- to 16-fold higher minimum inhibitory concentrations for these antibiotics,
