Association of hypothermic temperature-control duration at 33 °C with coagulation parameters after out-of-hospital cardiac arrest: a single-center retrospective cohort study.

BACKGROUND: The optimal duration of temperature control after out-of-hospital cardiac arrest remains uncertain. Prolonged hypothermic temperature control may affect coagulation, but clinical data remain limited. We evaluated the association between different durations of hypothermic temperature control at 33 °C and coagulation parameters, recorded clinical safety outcomes, and 6-month neurological outcomes in patients after out-of-hospital cardiac arrest. METHODS: This single-center retrospective cohort study included 112 adult patients with out-of-hospital cardiac arrest who achieved return

Clinical Trials for Necrotizing Enterocolitis.

Necrotizing enterocolitis (NEC) remains one of the most devastating diseases affecting preterm infants, with high mortality and significant long-term morbidity among survivors. Earlier trials have demonstrated broadly comparable outcomes between drainage and laparotomy overall, although recent evidence suggests a high probability of benefit of laparotomy for infants with NEC. Novel therapies, including pentoxifylline, therapeutic hypothermia, remote ischemic conditioning and regenerative biologics, show promise but remain in the early stages of clinical evaluation. Future progress will depend

Impact of hypothermia level on neurological and renal outcomes in emergency aortic surgery: a single-centre experience.

BackgroundTemperature management during emergency surgery for acute aortic syndrome is critical for neurological, renal, and systemic protection. Recent European Association for Cardio-Thoracic Surgery (EACTS) guidelines recommend moderate hypothermia with antegrade cerebral perfusion for hemiarch procedures. Nevertheless, deep hypothermic circulatory arrest (DHCA) remains common. This study evaluates neurological and other postoperative outcomes across different intraoperative temperature ranges.MethodsA single-centre retrospective study of consecutive patients undergoing surgery for acute ao

Severe Suspected Metformin-Associated Lactic Acidosis With Relatively Preserved Serum Creatinine at Presentation: Sequential Intermittent Hemodialysis and Continuous Venovenous Hemodialysis.

Metformin-associated lactic acidosis (MALA) is an uncommon but potentially fatal complication of metformin therapy in which extracorporeal treatment may be required for severe acidemia or markedly elevated lactate levels. We report the case of a 47-year-old man with type 2 diabetes mellitus treated with metformin who presented with encephalopathy, profound hypoglycemia, hypothermia, and severe high-anion-gap metabolic acidosis. Lactate was 24.1 mmol/L and peaked at 25.5 mmol/L, while serum total carbon dioxide was <5 mmol/L. Despite the severity of the metabolic disturbance, serum creatinine w

Assessing Medication Confounders Before Determination of Brain Death/Death by Neurologic Criteria: A Primer, with Opioids as a Worked Example.

PURPOSE: To provide a primer on how clinicians, particularly pharmacists, can systematically assess whether a medication may confound determination of brain death/death by neurologic criteria (BD/DNC), using pharmacokinetic and pharmacodynamic principles that can be applied to any agent with the aid of tertiary references, and using opioids as a worked example. SUMMARY: All major guidelines require exclusion of central nervous system (CNS) depressant effect before BD/DNC determination, yet none provides a general method for deciding when a given medication has cleared. Rather than cataloging

Renin-Aldosterone Activation and Enhanced Urinary Potassium Excretion in Asphyxiated Newborns Treated with Therapeutic Hypothermia: A Prospective Study.

Hypoxic-ischemic encephalopathy (HIE) is a critical neonatal condition for which therapeutic hypothermia (TH) remains the cornerstone of neuroprotective intervention. Clinical observations and prior studies note that electrolyte imbalances-particularly hyponatremia and hypokalemia-frequently emerge during TH. Therefore, this study explores how activation of the renin-angiotensin-aldosterone system (RAAS) may contribute to sodium (Na) and potassium (K) disturbances in asphyxiated neonates receiving TH. In this prospective study, 38 newborns with HIE treated with TH and 21 healthy term newborns

Hospital resuscitation in patients with primary hypothermic cardiac arrest.

Hypothermia is recognised as a reversible cause of cardiac arrest along with hypoxia, hypovolaemia and hypo or hyperkalaemia. However, hypothermia is the rarer one of these 'four Hs'. Because cardiac arrest caused primarily by hypothermia is a rare occurrence, resuscitation teams may lack real-world experience of it. This article sets out essential information for emergency department staff to understand this presentation and its management. It provides background information on hypothermia, describes the usual pattern of deterioration leading to cardiac arrest, and explains the key interventi

Comparative efficacy of interventions for preventing chemotherapy-induced alopecia: a systematic review and network meta-analysis.

PURPOSE: Chemotherapy-induced alopecia (CIA) negatively affects patients' appearance and emotional well-being. This network meta-analysis aimed to compare the efficacy of different preventive interventions for CIA. METHODS: We searched PubMed, Embase, CENTRAL, and EBSCO Open Dissertations from inception to March 2025. Randomized controlled trials evaluating interventions to prevent CIA in cancer patients and reporting hair preservation outcomes (as defined by individual trials, including CTCAE grade < 2, < 30 to < 50% hair loss, or WHO grade 0-2) were included. Evidence-Based Nursing Practices to Prevent Perioperative Unintentional Hypothermia in Elective Surgeries: An Observational Study.

Although evidence-based recommendations for preventing perioperative unintentional hypothermia have long been available, observational studies evaluating their implementation in hospital practice and the effectiveness of preventive interventions in at-risk patients are limited. This study aimed to determine the scope of nursing interventions for preventing perioperative unintentional hypothermia and examine the relationship between patient characteristics and these practices. This study employed a descriptive observational design. Patients undergoing elective surgery in the surgical units (ort

Development and validation of a predictive model for delayed emergence from anesthesia in elderly patients undergoing video-assisted thoracoscopic surgery: a retrospective study.

BACKGROUND: Delayed emergence from anesthesia is a common perioperative complication in elderly patients undergoing video-assisted thoracoscopic surgery (VATS), which may lead to prolonged recovery and increased healthcare burden. However, predictive tools specifically developed for this population remain limited. This study aimed to develop and validate a practical model to identify patients at increased risk of delayed emergence from anesthesia. METHODS: This retrospective study included 1,400 elderly patients who underwent video-assisted thoracoscopic surgery (VATS) at a tertiary hospital

Comparison of Long-Term Outcomes of Combined Hypothermia and Whole Body Cooling Therapy in Newborns with Hypoxic-Ischemic Encephalopathy.

Therapeutic hypothermia is widely used as a neuroprotective intervention in infants with hypoxic-ischemic encephalopathy (HIE); however, limited data exist regarding the long-term neurodevelopmental outcomes of different cooling strategies. The aim of this study is to compare the long-term neurodevelopmental and cognitive outcomes of combined hypothermia (CH) and whole-body cooling (WBC) treatments in children diagnosed with HIE with each other and with a healthy control group without an HIE diagnosis. This retrospective study included a total of 50 children diagnosed with HIE and treated with

Association between post-intravenous immunoglobulin hypothermia and lower receipt of second-line therapy in Kawasaki disease: a single-center retrospective cohort study.

INTRODUCTION/OBJECTIVES: Post-IVIG temperature patterns may be associated with the subsequent treatment course in Kawasaki disease (KD), but evidence regarding outcomes among patients who develop hypothermia after treatment remains limited. We evaluated the association between hypothermia following IVIG therapy and the receipt of second-line treatment. METHODS: We retrospectively reviewed patients with KD admitted to Matsudo City General Hospital between January 2018 and December 2023. Patients who achieved defervescence (body temperature < 37.5 °C) within 48 h after IVIG administration were

TRIM36 catalyzes K63-linked ubiquitination of TAK1 to drive septic inflammation.

BACKGROUND: The Toll-like receptor 4 (TLR4) pathway is critical for initiating innate immunity; however, its dysregulation drives pathological inflammation in diseases such as sepsis. METHODS: In the present study, we utilized septic patients, lipopolysaccharide (LPS)-induced mouse models, and LPS-treated mouse macrophages as well as human THP-1-derived macrophages to investigate the potential functions and molecular mechanisms of TRIM36 in sepsis. RESULTS: Here, we identify tripartite motif protein 36 (TRIM36) as a key positive regulator that amplifies TLR4 signaling. TRIM36 expression was

Transient Cold Hemagglutinin Disease (CHAD) During Cardiac Surgery: An Unexpected Complication from Hypothermia.

A 74-year-old male with mild non-specific anaemia underwent elective mitral and tricuspid valve surgery. Following initiation of cardiopulmonary bypass (CPB), systemic cooling to 34℃ and cardioplegia at 4℃, upon cardiotomy, clumps of blood were noted in the left atrium and left ventricle. ACT was confirmed to be in range, therefore a blood sample from CPB machine was collected and when exposed to ice demonstrated clumps, while they disappeared once at normal temperature. Hence, the patient was rewarmed to 37℃, subsequent cardioplegia doses were delivered warm and the heart chambers were washed

Feasibility and Safety of Intraoperative Abdominal Warming During Spinal Surgery in the Prone Position: A Pilot Study.

BACKGROUND: Inadvertent perioperative hypothermia caused by general anesthesia leads to several complications, including shivering, coagulopathy, surgical site infections, and delayed wound healing. Prone positioning for spine surgery severely restricts the available body surface area for warming, complicating intraoperative temperature management. This study investigated a novel approach utilizing the anatomical space beneath the chest and abdomen created by a standard four-point bolster support to maintain intraoperative core body temperature using standard pediatric underbody forced-air war
