Refractory ventricular fibrillation caused by caffeine intoxication.

A 30-year-old female with no history of cardiac or systemic disease presented with incessant ventricular fibrillation (VF) after an intake of 12 g of over-the-counter caffeine tablets for a suicidal purpose. Her VF was refractory, and repeated defibrillations were needed to attain a recovery of sinus rhythms. We then performed percutaneous cardiopulmonary support and therapeutic hypothermia to stabilize her circulation and prevent anoxic brain damage, respectively. A blood examination revealed an extremely high concentration of caffeine (172 mg/L). She fully recovered 16 days after her admissi

CB1 positive allosteric modulation attenuates Δ9-THC withdrawal and NSAID-induced gastric inflammation.

Recently, multiple compounds have been synthesized that target the allosteric binding site(s) of CB1. These CB1 positive allosteric modulators may capture the benefits of cannabinoid receptor activation without unwanted psychoactive effects, such as sedation. For example, ZCZ011 blocks neuropathic pain, absent the catalepsy, sedation, and hypothermia caused by CB1 orthosteric modulators, including Δ9-tetrahydrocannabinol (THC). The primary goal of the present study was to evaluate the potential of ZCZ011 to attenuate somatic signs of cannabinoid withdrawal in mice. Mice were repeatedly adminis

Transient temperature fluctuations severely decrease P. falciparum susceptibility to artemisinin in vitro.

Clinical studies suggest that outcomes for hospitalised malaria patients can be improved by managed hypothermia during treatment. We examined the impact of short pulses of low temperature on ring-stage susceptibility of Plasmodium falciparum to artemisinin in vitro. The usually artemisinin-sensitive clone 3D7 exhibited substantially reduced ring-stage susceptibility to a 4-h pulse of 700 nM dihydro-artemisinin administered during a 5-h pulse of low temperature down to 17 °C. Parasite growth through the subsequent asexual cycle was not affected by the temperature pulse. Chloroquine and pyronari

Cerebral Circulation During Retrograde Cerebral Perfusion: Evaluation Using Laser Speckle Flowgraphy.

BACKGROUND: The cerebroprotective effect of retrograde cerebral perfusion (RCP) and selective antegrade cerebral perfusion (SCP) still remains controversial. Laser speckle flowgraphy has shown much promise for novel perioperative neuromonitoring by assessing blood flow of the optic nerve head. This study aimed to evaluate the cerebral microcirculation in humans using laser speckle flowgraphy during simple circulatory arrest, RCP, and SCP under moderate hypothermia and to investigate whether RCP under moderate hypothermia is a reliable method of cerebral protection. METHODS: A total of 23 cons

[Thermoregulatory disorders and their significance in the elderly].

Our treatise is aimed at providing a brief survey on ethiology, pathogenesis, risks of inception, clinical picture, treatment pitfalls and prognosis of hypothermia and hyperthermia and possible interactions of these thermoregulation disorders with some other co-morbidities. It deals also with drugs and medicines including their potential unwanted side effects in these thermally abnormal conditions. The problems of hypothermia and hyperthermia in the senium (especially the late one) will be closely connected with labile homeostasis and the occurence of frailty syndrome which increases with the

Faster Hypothermia Induced by Esophageal Cooling Improves Early Markers of Cardiac and Neurological Injury After Cardiac Arrest in Swine.

Background After cardiopulmonary resuscitation, the protective effects of therapeutic hypothermia induced by conventional cooling are limited. Recently, esophageal cooling ( EC ) has been shown to be an effective, easily performed approach to induce therapeutic hypothermia. In this study we investigated the efficacy of EC and its effects on early markers of postresuscitation cardiac and neurological injury in a porcine model of cardiac arrest. Methods and Results Thirty-two male domestic swine were randomized into 4 groups: sham control, normothermia, surface cooling, and EC . Sham animals und

[SOME PHYSIOLOGICAL PROCESSES IN ISCHEMIC PENUMBRA OF THE BRAIN (REVIEW)].

The zone of penumbra in ischemic brain tissue lies below the local cerebral blood flow threshold for electrocortical silence but above that for anoxic depolarization and massive ionic dyshomeostasis. Conflicting data concerning the width of therapeutic window in which successful reperfusion of blood still is possible, are cited in this paper. The role of temperature in development of ischemic stroke is also considered. There are data that even small degrees of brain temperature alterations crucially affect the outcome of ischemic insults. The lowering of intraischemic brain temperature by just

Bystander use of automated external defibrillators in the Spanish autonomous community of the Principality of Asturias in 2012-2014.

On-site bystander use of automated external defibrillators (AEDs) was analized in Asturias, Spain, between January 2012 and December 2014. Hospital management and neurologic state on discharge were also studied. Our retrospective observational population-based design had 3 phases comrpising: 1) a telephone survey of facilities with onsite public-access AEDsc, 2) analysis of relevant case records held by the Asturian emergency medical service, and 3) analysis of relevant hospital case records. Thirteen cases of AED use by bystanders were found. Eleven patients (84.5%) had initial shockable rhyt

Osborne waves in the hot summer.

Osborn waves are produced when the J-point deviates from baseline. While there are many known causes of Osborne waves, hypothermia remains the most common. Previous studies have been inconsistent about the risk of Osborne waves progressing to a deadly arrhythmia. Commonly, once patients are rewarmed, they no longer exhibit Osborne waves or experience cardiac arrhythmias. This patient presented with hypothermia on a hot, humid August day demonstrating two factors known to cause Osborne waves - hypothermia and hypocalcemia. While replenishing the calcium was beneficial, providing ventilator supp

Slow as Compared to Rapid Rewarming After Mild Hypothermia Improves Survival in Experimental Shock.

BACKGROUND: Accidental hypothermia following trauma is an independent risk factor for mortality. However, in most experimental studies, hypothermia clearly improves outcome. We hypothesized that slow rewarming is beneficial over rapid rewarming following mild hypothermia in a rodent model of hemorrhagic shock. MATERIALS AND METHODS: We subjected 32 male Wistar rats to severe hemorrhagic shock (25-30 mmHg for 30 min). Rats were assigned to four experimental groups (normothermia, hypothermia, rapid rewarming [RW], and slow RW). During induction of severe shock, all but the normothermia group we
