Management of acute type A aortic dissection in the Nordic countries.

BACKGROUND: Several aspects of acute type A aortic dissection (ATAAD) management remain debated. The strategies for arterial cannulation, cerebral protection, temperature management and extent of aortic resection are all contested. We aimed to explore and describe variations in the current management of patients with ATAAD in the Nordic countries, which form a limited geographic region with similar healthcare systems. METHODS: A 32-item questionnaire regarding the organizational and periprocedural aspects of care for patients with ATAAD was sent to 17 Nordic cardiac surgery units, prospective

Gradual proactive regulation of body state by reinforcement learning of homeostasis.

Living systems maintain physiological variables such as temperature, blood pressure, and glucose within narrow ranges-a process known as homeostasis. Homeostasis involves not only reactive feedback but also anticipatory adjustments shaped by experience. Prior homeostatic reinforcement learning (HRL) models have provided a computational account of anticipatory regulation under homeostatic challenges. However, existing formulations lack mechanisms for gradual, trial-by-trial adjustment and for extinction learning. To address this issue, we developed a continuous HRL framework that enables trial-

Alpha-2 Adrenergic Agonists Reduce Heavy Alcohol Drinking and Improve Cognitive Performance in Mice.

Alcohol use disorder (AUD) is one of the top behavioral causes of global disease burden in the United States. Repeated cycles of alcohol intoxication and abstinence induce neuroplastic alterations which induce excessive drinking and cognitive impairments. A system deeply dysregulated by chronic drinking is norepinephrine (NE). At moderate levels, NE has beneficial effects on cognition and behavior, mediated by the α2 adrenergic receptor (AR) subtype. Whether α2 AR activation blunts alcohol consumption in models of heavy drinking has not been determined, and whether α2 AR activation improves co

DGAT-driven futile lipid cycling has a pronounced, yet concealed, thermogenic function.

Thermoregulation is an essential yet incompletely understood homeostatic process in mammals. UCP1-mediated thermogenesis, while efficient, is dispensable, suggesting the existence of alternative mechanisms. Using a pharmacogenetic approach, we show that the adipose tissue futile lipid cycling (FLC) contributes to UCP1-independent thermogenesis, with DGATs being involved in the regulation of FLC. The loss of DGAT-driven FLC-mediated thermogenesis is compensated for by the hierarchical recruitment of alternative mechanisms such as shivering and enhanced lipid catabolism mediated by AMPK activati

Additive effect of leptin and palm-LEAP2(1-14) ameliorates obesity-induced metabolic stress in ob/ob mice.

Liver-enriched antimicrobial peptide 2 (LEAP2) can act as a natural antagonist or inverse agonist of the growth hormone secretagogue receptor, also known as the ghrelin receptor. In our previous studies, palm-LEAP2(1-14), a truncated palmitoylated analogue of LEAP2, proved highly stable and exerted an acute anorexigenic effect. Ghrelin, the endogenous agonist of the ghrelin receptor, and the anorexigenic hormone leptin have opposing effects on appetite. We hypothesized that leptin supplementation combined with long-term antagonism of the ghrelin receptor using palm-LEAP2(1-14) might have a ben

Glycyrrhizin's Potential to Modulate Feeding Behaviour by Neutralizing Reduced Feed Intake in Inflamed Arian Broilers.

BACKGROUND: The Arian broiler, an important native Iranian breed, shows heightened sensitivity to metabolic disturbances caused by lipopolysaccharide (LPS). This study evaluated whether glycyrrhizin could mitigate LPS-associated physiological disruptions. METHODS: In a controlled trial, 100 Arian broilers (21-day-old, mean weight 750 g) were randomly allocated into five experimental groups: (1) ICV glycyrrhizin dose-response assessment (1, 2 and 4 µg) evaluating feeding behaviour; (2) ICV LPS challenge (12.5, 25 and 50 ng) with 8-h monitoring of feed intake and cloacal temperature; (3) IP gly

Prevalence and associated factors of neonatal hypothermia in the neonatal intensive care unit at St. Peter's specialized Hospital, Ethiopia.

Neonatal hypothermia, defined as a body temperature below 36.5 °C, is a global issue that significantly increases neonatal morbidity and mortality, particularly in low- and middle-income nations. Despite global guidelines, the prevalence remains high in many resource-limited settings. This study aimed to assess the prevalence and associated factors of hypothermia among neonates admitted to the intensive care unit of St. Peter's Specialized Hospital. An institution-based cross-sectional study was conducted from July to September 2024 among 222 neonates admitted to the intensive care unit of St

Selective brain cooling via cerebrospinal fluid exchange: Proof of concept in a porcine model.

Therapeutic hypothermia is neuroprotective after brain injury, reducing metabolic demand, inflammation, and secondary injury cascades. However, clinical application is limited by systemic side effects, and selective brain cooling without systemic hypothermia has not been demonstrated in large mammals. We aimed to demonstrate the feasibility of selective brain cooling using cerebrospinal fluid (CSF) exchange with cooled saline via a double-lumen external ventricular drain (EVD) system in a porcine model. Using 12 pigs under general anesthesia, we placed a double-lumen EVD in the lateral ventric

Metabolic positron emission tomography imaging and tumor growth inhibition during the Q neuron-induced hibernation-like state in mice.

To the best of our knowledge, this is the first positron emission tomography (PET) study using a Q neuron-induced hypothermia and hypometabolism (QIH) mouse model, which is a synthetic hibernation-like state triggered by neural stimulation. We investigated changes in the in vivo dynamics of various essential nutrients and bioactive substances during hibernation using three types of metabolic probes: the glucose derivative [18F]FDG (fluorodeoxyglucose), amino acid derivative L-m-[11C]tolylalanine, and vitamin B1 derivative [11C]thiamine. We found that cardiac glucose metabolism was significantl

Advanced bilateral EEG monitoring for neuroprotection in high-risk cardiac surgery: a successful case report of emergency ascending aortic replacement.

BACKGROUND: Aortic surgery involving circulatory arrest and unilateral cerebral perfusion may be associated with hemispheric differences in scalp-recorded electroencephalographic (EEG) activity that are not detected by standard intraoperative monitoring. Bilateral frontal EEG analysis may provide additional descriptive information on cerebral electrical patterns during these procedures. CASE PRESENTATION: A 62-year-old woman underwent emergency repair for type A aortic dissection with selective right-sided antegrade cerebral perfusion under deep hypothermia. Bilateral frontal EEG monitoring sh

Ogonto, a traditional Japanese herbal medicine, attenuates food allergy symptoms in mice through the induction of regulatory T cells.

Despite the rising prevalence of food allergies (FAs) in recent decades, existing therapeutic drugs remain inadequate. Therefore, the development of novel therapeutic strategies for FAs is urgently needed. Baicalein, a flavonoid from Scutellariae baicalensis, has been reported to ameliorate ovalbumin (OVA)-induced FA responses by inducing regulatory T cells (Tregs) in a mouse model. Here, we examined the effects of five traditional Japanese herbal medicines (Kampo medicines) containing Scutellariae radix on Treg induction and FA responses. All five Kampo medicines significantly enhanced the tr

Pharmacologic and toxicologic confounders in brain death determination: a multidisciplinary guide.

BACKGROUND: The assessment of comatose ICU patients presents several challenges with respect to the etiology, depth and ultimate outcome. The acceptance in 1959 of the worst-outcome scenarios of coma, i.e. brain death, and the publication of the Harvard Brain Death Criteria in 1968, were key developments in the management of irreversible coma. Pharmacologic confounders often complicate coma assessments, including brain-death determination. Moreover, associated clinical factors during coma, such as organ failure, hypothermia, prolonged continuous infusions, intoxication, and extreme obesity oft

Improved Outcomes with Hypothermic Machine Perfusion in Donation after Circulatory Death Liver Transplantation.

BACKGROUND: Donation after circulatory death (DCD) allografts were not widely used in liver transplantation (LT) due to higher rates of biliary complications and retransplantation. Although European trials have shown improvement in early allograft function and biliary complications with hypothermic-oxygenated machine perfusion (HMP-O2) compared with traditional static cold storage (SCS) on ice, no US clinical series have demonstrated a benefit. STUDY DESIGN: All DCD LTs performed at a single center from April 6, 2015, to December 24, 2024, were evaluated. DCD-HMP-O2 transplants were performed

Prolonged stable hypothermia during a 10-hour cold open-water marathon swim.

Marathon open-water swimming presents extreme thermophysiological challenges, particularly in cold environments. This case report describes continuous core temperature data from a solo crossing of New Zealand's Foveaux Strait, an infamously cold (13°C -14°C), turbulent open-water swim. A 52-year-old male swimmer (body mass index, 27.9 kg m-2; body fat, 18%) completed the 37 km swim in 9 h 52 min under standard marathon swimming rules (no wetsuit). Core temperature (measured via an ingestible thermometer pill) decreased rapidly after immersion, falling from 37.9°C to <35.0°C within 50 min. The
