From dam to neonate: microbial, metabolic, and functional pathways shaping early-life development in dogs.

Within minutes, the canine neonate transitions from placental support to respiration, thermoregulation, glucose homeostasis, feeding, and microbial colonization. These events are often considered separately, yet they form a connected developmental process that begins in the dam and continues through birth and lactation. This narrative review integrates evidence on maternal, microbial, metabolic, and functional determinants of early-life development in dogs. During gestation, maternal nutrition, metabolism, endocrine adaptation, and the maternal microbiota can influence fetal growth and maturat

Sixty golden minutes: a quality improvement bundle reduces morbidities in at-risk preterm infants.

OBJECTIVE: Timely interventions during the "Golden Hour" (GH) reduce neonatal morbidity and mortality. We aimed to decrease hypothermia, time to antibiotics and intravenous fluids (IVF) initiation and invasive ventilation by 25%, bronchopulmonary dysplasia (BPD) by 10% and achieve 50% adherence to a GH bundle in our NICU. STUDY DESIGN: We conducted a quality improvement initiative using seven Plan-Do-Study-Act cycles (December 2021-October 2023) for infants <32 weeks gestation and/or ≤ 1500 grams. Outcomes were analyzed using statistical process control charts. RESULTS: Over the study period

High Altitude Hypothermia Reversal in the Field: A Case from Northern Pakistan, Karakoram Range.

Severe accidental hypothermia is a well-recognized risk in high altitude environments, where sudden weather changes, prolonged exposure to unstable temperatures, and limited medical access can rapidly become life threatening. In remote areas such as the Karakoram Range of northern Pakistan, improvised rewarming techniques may be the only viable option until evacuation is possible. We report a case of a 32-y-old healthy male trekker who was found unresponsive by his companions at ∼4200 m in the Karakoram Range with a recorded axillary temperature of 27°C, measured using a portable low-reading t

Ginsenoside Ra1 alleviates endothelial barrier dysfunction and multi-organ injury in experimental sepsis via NMMHC IIA inhibition.

BACKGROUND AND PURPOSE: Non-muscle myosin heavy chain IIA (NMMHC IIA) has emerged as a significant therapeutic target in vascular diseases and sepsis. However, effective interventions targeting NMMHC IIA in sepsis-induced endothelial dysfunction remain unelucidated. Here, we have assessed ginsenoside Ra1 (Ra1) as a potent NMMHC IIA inhibitor and evaluate its efficacy and underlying mechanisms in sepsis. EXPERIMENTAL APPROACH: We used the caecal ligation and puncture model of polymicrobial sepsis in mice. Survival rate, body temperature, pathological changes, vascular permeability and plasma b

Rethinking circulatory management in thoracoabdominal aortic repair: split-body perfusion for continuous end-organ flow.

Thoracoabdominal aortic aneurysm (TAAA) repair involving the proximal descending thoracic aorta remains among the most technically demanding operations in cardiovascular surgery. Conventional approaches may require systemic hypothermia and periods of circulatory arrest to facilitate proximal aortic reconstruction, potentially exposing the brain, spinal cord, and abdominal viscera to ischemic injury. Strategies that permit complex proximal reconstruction while maintaining continuous perfusion to multiple vascular territories remain limited. We describe a split-body circulation technique designe

Development and validation of a machine learning model for preoperative prediction of intraoperative hypothermia in gynecological laparoscopic surgery: a two-center cohort study.

BACKGROUND: Intraoperative hypothermia (IOH, temperature <36.0 °C) is a common complication in gynecological laparoscopic surgery that leads to significant adverse outcomes that would benefit from preoperative risk prediction. METHODS: We developed and compared two predictive models-a conventional multivariable logistic regression (LR) model and a gradient boosting machine (XGBoost) model-to preoperatively predict IOH based on 20 objectively measurable preoperative clinical variables that could be directly extracted from electronic health records (EHRs). The derivation cohort consisted of 460

Extraction of atrial tumor thrombus in renal cell carcinoma via a complete transabdominal approach.

Surgical resection is the mainstay treatment of renal cell carcinoma (RCC) offering a 5-year survival rate of 70-80%. Associated vena cava tumor thrombus adds complexity to surgical planning especially when the thrombus extends above the diaphragm. Those who do not undergo treatment of the primary cancer as well as the associated thrombus have poor mortality outcomes. Cardiopulmonary bypass (CPB) has been the mainstay for level IV thrombectomy since first introduced. While it has advantages, it is also associated with multisystem failure, hypothermia, and coagulopathy. We sought to describe ou

Intelligent thermoregulation strategy versus routine thermal management on myocardial injury after noncardiac surgery during gastrointestinal tumor resection in elderly patients (SAGE): a multicenter, parallel-group, superiority trial.

BACKGROUND: Inadvertent perioperative hypothermia is associated with adverse cardiovascular outcomes, but adequately powered trials using high-sensitivity troponin endpoints is lacking. The SAGE trial evaluates whether an intelligent, closed-loop thermoregulation strategy reduces myocardial injury after noncardiac surgery (MINS) in high-risk elderly patients. METHODS: This prospective, multicenter, parallel-group, randomized controlled superiority trial conducted at seven tertiary hospitals in China. Participants aged ≥65 years scheduled for elective gastrointestinal tumor resection under gen

Heat-Shock and Cold-Shock Proteins in Energy-Based Aesthetic Medicine.

BACKGROUND: Energy-based aesthetic devices alter tissue temperature across defined spatial and temporal distributions, producing biological effects commonly described as collagen contraction, neocollagenesis, elastogenesis, adipocyte injury, or tissue tightening. These clinical labels encompass distinct processes including protein damage, cellular stress adaptation, inflammation, cell death, extracellular matrix turnover, and long-term remodeling. Heat-shock proteins (HSPs) and mammalian cold-shock proteins provide a mechanistic framework for interpreting these responses. OBJECTIVE: To evalua

Clinical practice guidelines for neonatal therapeutic hypothermia across European Society for Paediatric and Neonatal Intensive Care (ESPNIC) member countries: a multidisciplinary blind comparison.

BACKGROUND: Therapeutic hypothermia (TH) decreases mortality and neuro-developmental disability in neonates greater or equal than 36 weeks' gestation with moderate-to-severe hypoxic-ischaemic encephalopathy (HIE) due to perinatal asphyxia. TH is often applied in different ways, as national guidelines and target populations may significantly differ. We aimed to compare TH guidelines from various European countries as we hypothesised that guidelines are significantly different and often of insufficient quality. METHODS: Multidisciplinary, blind, critical appraisal of clinical practice guideline

Anterior cingulate cortex glutamatergic projection neurons modulate motion sickness susceptibility via a cortico-vestibular circuit.

Motion sickness (MS) is a prevalent and debilitating vestibular disorder with limited therapeutic options, yet the cortical mechanisms governing individual susceptibility remain elusive. Using a biaxial rotation mouse model, we investigated the role of the anterior cingulate cortex (ACC) in MS by integrating morphology, calcium imaging, electrophysiology, cell ablation, chemogenetics, and optogenetics. Biaxial rotation induced hypothermia, anorexia, taste avoidance, locomotor hypoactivity, and gait incoordination. ACC pyramidal neurons exhibited enhanced activation and increased intrinsic exci

Corridor-based relative severity assessment of the canine anaesthetic course.

The Relative Severity Assessment (RELSA) score grades welfare severity based on the magnitude of multivariate deviations from a reference value. Many variables monitored during anaesthesia are homeostatic and become harmful when they leave the normal range in either direction. Building on the corridor concept of the PVS from human intensive care, we conducted a feasibility study to assess the application and behaviour of a corridor extension of RELSA in two canine anaesthesia datasets. Each homeostatic variable was given a normal band, and only its deviation was folded into the standard RELSA

Temporal profile of serum orexin-A in neonatal hypoxic-ischemic encephalopathy: a prospective longitudinal study.

BACKGROUND: No serum biomarker has yet been validated for clinical use in neonatal hypoxic-ischemic encephalopathy (HIE). Orexin-A is a hypothalamic neuropeptide that has demonstrated neuroprotective effects in experimental models of cerebral hypoxia-ischemia. We aimed to describe the early temporal profile of serum orexin-A in neonates with HIE, to compare its longitudinal change according to therapeutic hypothermia (TH) exposure, and to examine its relationship with clinical severity, electrophysiological findings and brain imaging. METHODS: This prospective longitudinal observational study

Lidocaine-ketamine-propofol total intravenous anesthesia in two chinchillas (Chinchilla lanigera).

This case report describes the use of a lidocaine-ketamine-propofol total intravenous anesthesia (TIVA) protocol in two chinchillas (Chinchilla lanigera). Two 4-year-old, 0.60 and 0.63 kg, intact female chinchillas underwent general anesthesia for dentistry. The first chinchilla presented for occlusal adjustment, but mucopurulent vaginal discharge was identified following sedation. Diagnostic findings were most consistent with an open pyometra, and an exploratory laparotomy with ovariohysterectomy was performed. Histopathology confirmed uterine adenocarcinoma. The second chinchilla underwent i

Perfusion imaging predicts therapeutic cooling response and tracks cis-p-tau pathology in porcine brain injuries.

Traumatic brain injury (TBI) and ischemic stroke cause substantial morbidity and mortality, yet effective neuroprotective therapies remain elusive. Although therapeutic hypothermia shows strong neuroprotection in lissencephalic rodent models, adult clinical trials have repeatedly failed, largely due to poor preclinical translatability and uniform cooling protocols that overlook patient heterogeneity. Using a large cohort of gyrencephalic porcine models (n = 89) of TBI and stroke, subjects were allocated to sham, injury, or therapeutic cooling groups administered via a custom-designed, non-inva

The relative contribution of anaesthetists and post-anaesthesia care unit nurses to patient-reported postoperative quality improvement indices.

Patient-reported outcomes in the post-anaesthesia care unit (PACU), such as pain, postoperative nausea and vomiting (PONV) and hypothermia, are often used for quality improvement (QI) purposes and to benchmark hospitals and anaesthetists. However, it is unclear whether variations in these measures reflect differences between clinicians or are driven by factors beyond clinician control. The aim of this study was to quantify how much of the variation in patient outcomes of severe pain, PONV and hypothermia in PACU, was associated with individual anaesthetists and PACU nurses, after adjusting for
