Human immunodeficiency virus infection mimicking very early onset inflammatory bowel disease.

Very early onset inflammatory bowel disease (VEO-IBD), defined as diagnosis before 6 years of age, is often associated with immune dysregulation and monogenic etiologies; however, infectious mimics may be overlooked in low-prevalence regions. We report a 4-year-old male with poor weight gain, intermittent diarrhea, elevated fecal calprotectin, and persistently elevated erythrocyte sedimentation rate. Endoscopy showed diffuse friable colonic mucosa, and biopsies demonstrated focal active colitis with mild crypt distortion. He was diagnosed with VEO-IBD and treated with mesalamine, infliximab, a

Adult-Onset Still Disease Triggered by Recent Cytomegalovirus Infection: A Diagnosis of Exclusion and a Review of the Literature.

Adult-onset Still disease (AOSD) is a rare systemic autoinflammatory disorder characterized by quotidian fever, inflammatory arthritis, evanescent rash, neutrophilic leukocytosis, and hyperferritinemia. Because no pathognomonic diagnostic test exists, diagnosis relies on characteristic clinical features and exclusion of infectious, malignant, and autoimmune disorders. Viral infections, including cytomegalovirus (CMV), have been proposed as potential immunologic triggers in genetically susceptible individuals, although causality remains unproven. We report a 47-year-old Hispanic woman with hype

From Heart Murmur to Bone Involvement: Tricuspid Valve Endocarditis With First-Rib Osteomyelitis in a Patient With Methicillin-Resistant Staphylococcus aureus (MRSA) Bacteremia.

Tricuspid valve infective endocarditis (TVIE) is most commonly associated with intravenous drug use and Staphylococcus aureus bacteremia. While right-sided endocarditis may present with septic pulmonary emboli, hematogenous spread to the bone is a rare but serious complication. Among osseous sites, first-rib osteomyelitis is particularly uncommon and may be underrecognized in patients presenting with localized chest pain or swelling. We present the case of a 37-year-old woman with intravenous substance use who presented to the emergency department with left-sided upper chest pain, swelling, an

Case report: Incidental finding of a duplicated right vertebral artery in a 17-year-old female with suspected systemic lupus erythematosus.

We present the case of a 17-year-old female with suspected systemic lupus erythematosus (SLE), receiving prednisolone and hydroxychloroquine, who was evaluated for a difference in blood pressure between the upper extremities and an elevated erythrocyte sedimentation rate (ESR). CT angiography (CTA) of the cervical vessels, performed because of clinical suspicion of vasculitis, incidentally demonstrated duplication of the right vertebral artery (VA), with both limbs originating from the right subclavian artery. The medial limb coursed posterior to the common carotid artery and anterolateral to

Predictive Modeling of Clinically Apparent Shoulder Periprosthetic Joint Infection (PJI).

INTRODUCTION: The first International Consensus Meeting (ICM) definition of shoulder periprosthetic joint infection (PJI) introduced a tiered framework of PJI categories based on weighting of minor criteria for those cases not having major criteria of Definite infection. The selection of minor criteria and weightings was based on expert opinion and limited available evidence. The purpose of this study was to use prospective, multicenter data to develop a predictive model for clinically apparent shoulder PJI to inform a more evidence-based understanding of minor criteria and PJI categories. Proceedings from the 2025 International Consensus Meeting on Orthopedic Infections: the definition of Shoulder Periprosthetic Joint infection.

BACKGROUND: Periprosthetic joint infection (PJI) remains one of the greatest diagnostic and treatment challenges facing shoulder surgeons and their patients. A definition for shoulder PJI was established by the 2018 International Consensus Meeting (ICM) on Orthopedic Infections, but the selection and weighting of its minor criteria relied largely on expert opinion. The purpose of this study was to present an updated, evidence-based definition of shoulder PJI generated by the 2025 ICM Shoulder Group. METHODS: The 2025 ICM Shoulder Group, an international panel of shoulder surgeons and infectio

Immunoglobulin G4 (IgG4)-Related Lung Disease Mimicking Pulmonary Malignancy: A Case Report With Histopathological Characterisation and Review of Diagnostic Criteria.

Immunoglobulin G4 (IgG4)-related disease (IgG4-RD) is a systemic fibroinflammatory condition with highly variable pulmonary manifestations that frequently overlaps clinically, radiologically, and metabolically with primary malignancy, granulomatous disease, and vasculitis. Tissue diagnosis with immunohistochemical quantification of IgG4-positive plasma cells remains the diagnostic cornerstone. A 50-year-old male ex-smoker presented with a two-month history of progressive cough, dyspnoea, chest pain, and 8 kg of unintentional weight loss. Examination revealed digital clubbing and bilateral crep

The dual role of activating transcription factor 4: from cellular stress sentinel to cardiovascular disease intervention.

As the master orchestrator of integrated stress response, activating transcription factor 4 (ATF4) operates as a central molecular switch that directs cellular fate toward survival or death by regulating genes associated with oxidative stress, endoplasmic reticulum stress, apoptosis, ferroptosis and metabolism. The functional outcome of ATF4 activation is critically dependent on the context: it usually contributes to cellular adaption and survival under mild or transient stress, yet triggers cell death when stress is severe or prolonged. Dysregulation of this dichotomous function has been impl

First-Year IgG Dynamics in IgG4-Related Disease: A Critical Window for Predicting Long-Term Outcomes.

OBJECTIVE: To investigate longitudinal serum IgG dynamics in IgG4-related disease (IgG4-RD) after treatment and assess their prognostic value for relapse. METHODS: A retrospective cohort of 274 newly treated patients with IgG4-RD was stratified into elevated IgG (n = 186) and normal IgG (n = 88) groups. Treatment responses were evaluated by covariance analysis adjusted for baseline IgG. Longitudinal IgG trends and relapse risk were analyzed using Kaplan-Meier curves and Cox regression. RESULTS: Elevated baseline IgG level was associated with more severe phenotypes, including male predominanc

Incidental Diagnosis of Erdheim-Chester Disease in a 42-Year-Old Man With Persistent Elevated ESR and Peri-Ocular Xanthomas: A Case Report.

Erdheim-Chester disease (ECD) is a rare non-Langerhans cell histiocytosis characterized by multisystem involvement and frequent diagnostic delay due to nonspecific clinical manifestations. We report a 42-year-old asymptomatic man with persistently elevated erythrocyte sedimentation rate (ESR) detected during routine laboratory evaluation. Physical examination revealed bilateral peri-ocular xanthomas. Multimodal imaging demonstrated bilateral perirenal soft-tissue infiltration with a characteristic "hairy kidney" appearance on computed tomography, symmetric skeletal involvement on bone scintigr

Posterior ischemic optic neuropathy as the main manifestation of giant cell arteritis: a case-based literature review.

This case-based literature review aims to challenge a common misconception that elevated erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) levels, halo sign on temporal artery ultrasound, and arteritic anterior ischemic optic neuropathy are essential to make a diagnosis of giant cell arteritis (GCA). Here, we present a case of a 70-year-old woman with a biopsy-proven GCA who met the 2022 American College of Rheumatology/European Alliance of Associations for Rheumatology Classification Criteria for Giant Cell Arteritis. On admission she complained of left temporal headache, left

Anti-SSA/Ro60kd Antibodies Are Associated With Positive Salivary Gland Biopsy in Sjögren's Syndrome: A Retrospective Exploratory Study.

INTRODUCTION/OBJECTIVES: Sjögren's syndrome (SS) is a systemic autoimmune disease with heterogeneous clinical, laboratory, and histopathological manifestations, which complicate timely diagnosis. Minor salivary gland biopsy (MSGB) is a key component of SS classification, although its relationship with clinical and serological features remains incompletely characterized. This study aimed to evaluate the association between MSGB findings and clinical-laboratory variables in patients with SS and to identify variables associated with a positive biopsy. METHODS: We conducted a retrospective observ

Does an Isolated Elevated Erythrocyte Sedimentation Rate Warrant Further Work-Up for Periprosthetic Joint Infection after Total Joint Arthroplasty?

BACKGROUND: Erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) are widely used as screening tools for periprosthetic joint infection (PJI) following total knee and hip arthroplasty (TKA and THA, respectively). However, the significance of an isolated elevated ESR with a normal CRP, especially in cases with low clinical suspicion, remains unclear. METHODS: We retrospectively reviewed 19,872 primary TKAs and 12,143 primary THAs performed between March 2012 and March 2023 at a high-volume academic orthopaedic hospital. Of these, 656 TKA and 253 THA patients ≥ 18 years old who unde

Analysis of risk factors for hypoproteinemia after total hip arthroplasty and construction of a nomogram: A retrospective study.

Postoperative hypoalbuminemia (POHA) remains one of the major complications following total hip arthroplasty (THA) in patients with end-stasge hip disease. Identifying factors that can reduce the incidence of POHA is crucial for improving clinical outcomes in these patients. Our study aimed to develop and validate a nomogram that can pre-operatively quantify an individual patient's risk of POHA. We retrospectively reviewed patients aged ≥65 years who underwent primary unilateral THA between January 2021 and December 2022. Inclusion criteria were: age ≥ 65 years, primary unilateral THA, pre-ope
