OBJECTIVE: To elucidate the prognosis of patients with primary Sjögren's syndrome (pSS) accompanied by hypergammaglobulinemia (HG), with a particular focus on the impact of various treatments.
METHODS: Patients were divided into the HG group and the non-HG group, based on the presence or absence of HG [serum immunoglobulin G (IgG) level above 16 g/L]. The demographics, clinical manifestations, laboratory findings, medications and outcomes were compared between the two groups. The adverse outcome was defined as death or the increase of Sjögren's Syndrome Disease Damage Index (SSDDI) score.

A 4-year-old child presented with a recent history of loss of appetite, weight loss, lower limb myalgias, fatigue, and new-onset hearing loss. These symptoms were associated with persistent microcytic anemia, leukocytosis, thrombocytosis, hypergammaglobulinemia, and elevated erythrocyte sedimentation rate. Major oncological, hematological, gastrointestinal, rheumatological, immunological, and infectious conditions were ruled out. Suspecting an autoinflammatory disease, high-dose corticosteroids were initiated but without clinical improvement. Due to the severe inflammatory state in the absence

Hypergammaglobulinemic purpura of Waldenström (HGPW) is a rare chronic purpuric eruption, usually on the lower legs, associated with polyclonal hypergammaglobulinemia, elevated erythrocyte sedimentation rate, and IgG and IgA rheumatoid factor, and can be primary or secondary to autoimmune disease. In this article, we present 2 cases of HGPW. Thorough evaluation and rheumatology follow-up are recommended, with treatment from conservative measures to systemic agents for frequent or painful flares.

Eosinophilic cellulitis, also known as Wells syndrome, is a rare inflammatory dermatosis that may clinically mimic bacterial cellulitis and contribute to diagnostic delay. We report the case of a 72-year-old woman with a history of systemic lupus erythematosus and other autoimmune comorbidities who initially presented with bilateral lower extremity erythema and received antibiotic therapy for presumed cellulitis, with transient improvement. Three days after completing treatment, she developed recurrent pruritic erythematous plaques involving both upper and lower extremities, accompanied by ede

We report the case of a 77-year-old woman who presented with progressive, painless bilateral visual loss over three weeks. Ophthalmologic evaluation demonstrated bilateral anterior ischemic optic neuropathy and right central retinal artery occlusion. Notably, she lacked classical systemic features of giant cell arteritis (GCA), including headache, scalp tenderness, or jaw claudication. Laboratory evaluation revealed an elevated erythrocyte sedimentation rate (ESR 64 mm/h). Temporal artery ultrasound demonstrated bilateral halo signs, supporting the diagnosis of GCA. High-dose corticosteroid th

INTRODUCTION: Tuberculosis (TB) can affect the eye through direct infection or immune mechanisms. While ocular TB most often presents as uveitis or scleritis, episcleritis is an uncommon manifestation and can be mistaken for an idiopathic or autoimmune disease.
CASE PRESENTATIONS: A 17-year-old female patient presented at our hospital with recurrent episcleritis. The first two flare-ups were mild and responded well to conservative treatment; therefore, no further investigation was pursued at the time. However, during the third episode, routine baseline workup revealed a mildly elevated erythr

Infective endocarditis (IE) is an infection of the endocardial surface of the heart, with Staphylococcus aureus as the leading cause in the United States. Identified risk factors include advanced age, male sex, intravenous drug use (IVDU), prior IE, structural or valvular heart disease, HIV infection, and chronic hemodialysis. In IVDU, IE most commonly affects the right-sided valves, while bivalvular involvement is uncommon but associated with more severe disease and increased risk of rapid deterioration. Multiple complications arise from IE, including variable vascular phenomena and systemic

BACKGROUND: Acute rheumatic fever (ARF) is caused by an autoimmune response to throat infection with group A Streptococcus. ARF and its sequel, chronic rheumatic heart disease, remain significant public health problems in many low-income countries. Rapid diagnosis and timely treatment of ARF are essential to avoiding serious adverse outcomes. CASE PRESENTATION: A 16-year-old adolescent with hyperthyroidism and treatment noncompliance presented with symptoms such as fever, difficulty lying flat at night, cough with expectoration, palpitations, and tachypnea. On the basis of the patient’s sympto

Visceral leishmaniasis is a rare parasitic infection in immunocompetent individuals, being more frequent in children and immunocompromised individuals. Despite being endemic in several regions of the world, including Portugal, it continues to be underdiagnosed. The most characteristic clinical manifestations include constitutional symptoms, pancytopenia, polyclonal hypergammaglobulinemia, and splenomegaly. Direct identification of the parasite is the preferred diagnostic method; however, this approach is not always feasible. Consequently, serological testing becomes essential, particularly in

BACKGROUND: Systemic lupus erythematosus is a chronic autoimmune disease with heterogeneous clinical manifestations. Neuropsychiatric involvement is relatively common in systemic lupus erythematosus; however, isolated dysarthria or slurred speech is an exceedingly rare presentation. Reporting such atypical manifestations may improve clinical awareness and facilitate early diagnosis of neuropsychiatric systemic lupus erythematosus.
CASE PRESENTATION: A 14-year-old Iranian girl with no family history of autoimmune disease was referred to the rheumatology department with persistent fatigue, weak

UNLABELLED: Methotrexate (MTX) is a class of antifolate chemotherapeutics and immunosuppressants that are effectively used in the treatment regimen of cancer and autoimmune diseases. In the current MASLD era, methotrexate-induced hepatotoxicity must be understood in the context of host metabolic dysfunction, obesity, diabetes, and alcohol use that amplify oxidative and inflammatory liver injury. But its administration is being restricted due to hepatic toxicity, which increases asymptomatic in transaminases and progresses to fibrosis. This review examines the complex mechanisms underlying MTX-

INTRODUCTION: Poncet's disease (PD) is a rare form of TB-associated reactive arthritis, presents a major diagnostic challenge due to its close resemblance to rheumatoid arthritis and tuberculous septic arthritis. A clear diagnostic workflow is essential to support clinical decision-making to prevent misdiagnosis and inappropriate immunosuppressive treatment. This review synthesizes current evidence on the diagnostic characteristics of PD and propose a structured diagnostic workflow to improve clinical decision-making and reduce misdiagnosis.

BACKGROUND: The management of pediatric tuberculosis involves several challenges, as illustrated in these three cases. Challenges are numerous and each case demonstrates the difficulty and delay in diagnosis, social constraints that delay the diagnosis, and drug dosing schedules that cause serious side effects in children. All three cases received the Bacillus Calmette-Guérin vaccine at birth, and each has documented confirmation of vaccination.

Miliary tuberculosis (TB) represents a potentially fatal form of disseminated TB resulting from the lymphohematogenous dissemination of Mycobacterium tuberculosis. Although accounting for approximately 1-2% of all TB cases in immunocompetent adults, its mortality rate exceeds 30%. This review screened peer-reviewed, original research studies via four databases (PubMed, EMBASE, MEDLINE, and CENTRAL) to synthesize up-to-date evidence. Key risk factors include human immunodeficiency virus (HIV) co-infection, anti-tumor necrosis factor-α (TNF-α) agents and chronic malnutrition. The clinical presen

Osteoarthritis (OA) is condition which poses a main concern to the aging population and its severity is expected to increase with the increasing life expectancy. In the future, several possible targets for OA treatment need to be defined. Dickkopf‑related protein 3 (DKK3) is an atypical member of the Wnt‑antagonistic dickkopf‑related protein (DKK) family. The availability of research into the role of DKK3 in the abnormal remodeling of subchondral bone in human knee joints is currently limited. Thus, the aim of the present study was the evaluation of DKK3 expression in the abnormal bone remodel
