Subcutaneous palisading granulomas (SPG), also known as benign rheumatoid nodules, occur in otherwise healthy children. Although these lesions have the same histology as rheumatoid nodules in adults, subcutaneous granuloma annulare, and diabetic lipoidicum, they appear to represent a distinct clinicopathologic entity. We report the case of a 38-year old woman with massive ascites. No cause could be demonstrated despite multiple investigations. The patient was febrile and had an elevated erythrocyte sedimentation rate. Systemic lupus erythematosus was subsequently diagnosed, when malar rash, arthritis, leucopenia, anti-native DNA and anti-Sm antibodies appeared. Ascites and lupus manifestations resolved simultaneously on steroid therapy. This unusual cause of ascites should be recognized, since it responds favorably to steroid therapy.

We describe a 72-year-old woman with classic presentation of polymyalgia rheumatica and an elevated erythrocyte sedimentation rate. After treatment with low-dose corticosteroids and normalization of the sedimentation rate, hematocrit, and serum alkaline phosphatase, she developed symptoms and biopsy proved temporal arteritis. In patients with polymyalgia rheumatica and an initially elevated sedimentation rate, normalization of the sedimentation rate with corticosteroid therapy does not exclude the need for temporal artery biopsy if the symptoms warrant it.

A prospective study in 80 exfoliative dermatitis patients over a period of 5 years establishes 41.9 years as its mean age at onset. Eighty-two patients were investigated on their first visit to the outpatient department of St. Mark's Hospital, London, for the assessment of abdominal symptoms. One hundred eighty-nine patients with rheumatoid arthritis were entered into a prospective, controlled, double-blind multicenter trial comparing placebo and methotrexate (MTX). Single-agent intravenous vinblastine, 4 to 8 mg/week, was used to treat 38 patients with Kaposi's sarcoma related to the acquired immunodeficiency syndrome. The operative records of 2816 patients undergoing repair for abdominal aortic aneurysm (AAA) from 1955 to 1985 were reviewed. Inflammatory aortic or iliac aneurysms were present in 127 patients (4.5%), 123 men and four women. Most patients were heavy smokers (92.1%). A 71-year-old asymptomatic man was investigated because of anemia and an elevated erythrocyte sedimentation rate. Temporal artery biopsy showed that he had active giant cell arteritis. To our knowledge, this is the first report of giant cell arteritis in an asymptomatic patient. Giant cell arteritis may remain undiagnosed during life in many cases and may be an important cause of unexplained anemia in the elderly.

A retrospective study was carried out on patients with histologically proven gastric carcinoma diagnosed at the Gastro-intestinal Clinic, Tygerberg Hospital, over a 5-year period--1979-1983. Nine children over 20 months of age had acute osteomyelitis of the epiphysis of a long bone.
