Ewing's sarcoma, which is regarded as one of the most lethal primary bone tumors, lies in the domain of the orthopedic surgeon because it occurs most commonly in the shaft of the long bones, especially in the lower extremities. Pain, leukocytosis, fever, anemia, and an elevated erythrocyte sedimentation rate are commonly seen. We are presenting a case of Ewing's sarcoma of the left greater trochanter with metastasis to the clivus producing a bilateral 6th nerve palsy. The presence of fever, nuchal rigidity, and photophobia simulated meningitis. The rapid evolution of radiological signs will be

Five cases of pyogenic vertebral osteomyelitis diagnosed clinically and with radioisotope studies are reported. All patients presented with symptoms of back pain, an elevated erythrocyte sedimentation rate, and a normal leukocyte count. Three had positive blood cultures and four had changes on plain radiographs. All five had positive phosphate bone and Ga-67 imaging, and two showed paravertebral uptake of gallium giving a "butterfly"-like appearance.

An elderly female with laboratory evidence of rheumatoid arthritis presented with malnutrition, elevated erythrocyte sedimentation rate, peripheral eosinophilia with eosinophilic infiltration of the pleura, uterus, skin and muscle without granuloma formation. She developed a necrotizing vasculitis of the mesenteric vessels without granuloma and died. This case illustrates that polyarteritis nodosa, even of the overlap type, may produce severe widespread mesenteric vasculitis.

Women with laparoscopically verified acute salpingitis (AS) were studied, and 151 were classified as having: chlamydia-associated AS (C-AS), gonococcal-associated AS (G-AS), and nonchlamydial, nongonococcal-associated AS (NCNG-AS). Patients with G-AS were more often febrile (rectal temperature greater than 38 degrees C) and more often had a moderately elevated erythrocyte sedimentation rate (ESR) (16 to 30 mm/hr) compared to other patients. Women with NCNG-AS were more likely to have a normal ESR and a mild inflammatory reaction laparoscopically. C-AS women were more likely to have had pelvic

Over a 16 year period, 47 patients with Wegener's granulomatosis were treated at the National Institutes of Health. Age at the onset of disease ranged from 15 to 75 years with a mean of 42.2 years. The group was evenly divided between male and female subjects. With seven exceptions, all patients had the classic form of Wegener's granulomatosis, with upper airway and pulmonary disease and glomerulitis. The most consistently abnormal laboratory test was an elevated erythrocyte sedimentation rate. Most of the patients were treated with cyclophosphamide alone or in combination with prednisone. In

A 51-yr-old, nonobese, male patient presented with hyperglycemia and a recent 40-pound weight loss. Severe insulin resistance was documented in studies in which high amounts of insulin were infused using the Biostator GCIIS. Diabetic control was finally achieved with subcutaneous injections of 470 U of insulin per day. Positive laboratory findings included a mild pancytopenia, elevated erythrocyte sedimentation rate, decreased C3 and properdin, and increased IgA. Antinuclear or other autoantibodies were not present. Insulin antibody levels were within the range usually present in insulin-treat

Four patients with active seronegative psoriatic arthropathy were treated with retinoid (Ro 10-9359) in a daily dose of 30 mg for at least four months. Within the first 4--6 weeks of treatment all patients showed marked improvement of arthritis as measured by the number of swollen joints, the Ritchie joint index, morning stiffness and the pain experienced. One patient stopped taking her concomitant medication of nonsteroidal anti-inflammatory agent, while the others were able to reduce the intake of such drugs substantially. None of the patients were taking steroids. The elevated erythrocyte s

Two elderly patients presented with generalized aches and pains (particularly in the shoulders and the pelvic girdle), stiffness, fatigue, anemia, and an elevated erythrocyte sedimentation rate, but there were no signs or symptoms directly referable to the joints. Two and five months later respectively, pain, swelling, and signs of synovitis appeared in several joints in a symmetrical pattern, and a diagnosis of rheumatoid arthritis was made. Rheumatoid arthritis in the elderly may resemble polymyalgia rheumatica. On the other hand, synovitis in many patients with polymyalgia rheumatica may re

Vasculitis should be suspected as a cause of stroke in younger patients or older patients with an elevated erythrocyte sedimentation rate, especially in the presence of systemic disease and mononeuritis multiplex. Corticosteroids and cyclophosphamide show promise of producing remission.

A review of the medical records of 123 persons with Legionnaires' disease hospitalized in the 1976 Philadelphia epidemic showed that the manifestations of infection ranged from mild grippe to a severe pneumonia that also involved other organ systems. Early in the illness, constitutional symptoms predominated. Fever, malaise, myalgia, rigors, confusion, headache, and diarrhea were usually followed by nonproductive cough and dyspnea. Physical examination showed few abnormalities other than rales. Moderate leukocytosis with left shift, elevated erythrocyte sedimentation rate, elevation of serum l

Discitis, intervertebral disc-space infection, and vertebral osteomyelitis form a spectrum of disorders with a probably common bacterial etiology. We studied forty-one children who had symptomatic narrowing of the disc space associated with fever and an elevated erythrocyte sedimentation rate and found that the diagnosis of infection in the spine was usually delayed. Technetium 99m polyphosphate bone-scanning proved to be an accurate, rapid, and safe method of establishing an early diagnosis of infection. A positive culture of blood or biopsy material was found in 34 per cent of all the patien

A 39-year-old man is described who presented with the unspecific signs of systemic disease (elevated erythrocyte sedimentation rate, increased gamma-globulins, positive rheumatoid factor; clubbing, splenomegaly) and with fever whose origin remained undiagnosed for 8 years despite numerous investigations. Later in the course of the disease, the signs of mitral stenosis appeared, suggesting left atrial myxoma even in the absence of arterial embolization. This diagnosis was established by echocardiography and confirmed by angiocardiography as well as at operation, after which all the systemic sig

The association of inflammatory diseases with disciform macular detachment is described in three patients. The first patient with seropositive syphilis developed juxtapapillary choroiditis, disciform detachment of the left macula progressing to a wide-spread area with atrophy of the choriocapillaris and pigment epithelium, corpuscular aggregations of retinal pigment, and white fibrous tissue between the choroid and retina. The second patient with fever, anorexia, fatigue, elevated erythrocyte sedimentation rate and pulmonary changes developed choroiditis with disciform detachment of the left m

Chorea associated with systemic lupus erythematosus (SLE) has been reported in only 28 patients. The clinical and laboratory features of these cases are reviewed here, along with those of a 7-year-old boy who, we believe, represents the youngest child reported to date. In approximately half of these 29 individuals, most of whom were children, chorea preceded other manifestations of SLE. The age range and clinical characteristics of lupus-associated chorea were not appreciably different from those of Sydenham's chorea and most of the patients in whom chorea developed before other manifestations

To reevaluate the diagnostic significance of a markedly elevated erythrocyte sedimentation rate (ESR), the clinical diagnosis associated with an ESR of 100 mm/hr or greater was retrospectively analyzed in 200 patients at the Massachusetts General Hospital. In contrast to previously reported experiences in the the American literature suggesting a high frequency (58 percent) of malignant disease in such patients, the present study found infections to be the most frequently associated diseases (35 percent), while malignant disease accounted for only 15 percent of the patients. In a group of patients with a syndrome consisting of recurrent episodes of urticaria, arthralgia, abdominal pain, and (rarely) glomerulonephritis, examination of skin biopsy specimens showed necrotizing venulitis. An elevated erythrocyte sedimentation rate was the most common laboratory abnormality. Analyses of serum immunoglobulins revealed random abnormalities of immunoglobulin levels, and assessment of the complement system showed two groups of patients--some with hypocomplementemia and others with a normal complement system. In those with hypocomplementemia, there were low levels of C1q, C

Five patients had persistent, pulling pain in the sternum, clavicles, and upper ribs that was exacerbated by cold and dampness. Clublike, symmetrical enlargement of the clavicles was seen and, in two patients, venous congestion of the upper half of the body. All patients had a constantly elevated erythrocyte sedimentation rate. Radiologically there was symmetrical hyperostosis of the sternal and middle portions of the clavicles, synostosis of the sternoclavicular joints, a widened and thickened sternum, and varying degrees of involvement of the upper ribs. X-ray findings did not change over se

Serum lipoprotein (LP) concentrations were determined and LP patterns were classified in 261 middle-aged men, recruited from a health examination survey, with serum lipid values above the 80th percentile of the same population. Individuals with hyperlipoproteinaemia (HLP) and normolipidaemic controls were characterized also regarding family history of cardiovascular disease, socio-economic factors and clinical and laboratory variables. Subjects with HLP type IV-V and IIB were overweight and showed hyperuricaemia and hyperinsulinaemia compared with normolipidaemic controls and subjects with HLP

Polymyalgia rheumatica should be considered when a syndrome of constitutional symptoms, especially weight loss, low-grade fever, weakness, wasting proximal muscles, fatigue, malaise and depression, is seen in the elderly. Giant-cell arteritis plays a part later in the course. Thus the need for biopsy of a long segment of the temporal artery to help in determining diagnosis and therapy. An elevated erythrocyte sedimentation rate (ESR) is an important clue. The usual high value is about 80 mm/hour; if it is over 100 mm/hour, giant-cell arteritis should be suspected. Clinical features and validity of a duodenofiberscopic examination for the diagnosis of carcinoma of the papilla of Vater were evaluated on 13 cases preoperatively diagnosed endoscopically. Followings are the conclusions derived. 1) Average age was 59.8 years. Incidence was equal among men and women. 2) As initial symptoms, general fatigue and easy fatigability or symptoms of cholangitis should be emphasized. 3) Other important symptoms include jaundice, fever with chills and weight loss. Abdominal pain, when present, was noted as one of symptoms of cholangitis in most cases. 4) Important labo
