Advances in tuberculosis diagnostics and biomarkers, 2020-26: scientific evidence, recommendations, gaps, and developmental priorities.

Major advances in tuberculosis diagnostics and biomarker research from 2020 to 2025 have informed updated WHO policy guidance, consolidating recommendations for the detection of tuberculosis disease and drug resistance within a single framework. These recommendations align screening, diagnosis, and drug-resistance testing across populations and specimen types. Digital chest radiography, increasingly supported by computer-aided detection, is now a core WHO-recommended screening and triage tool, improving case detection and referral for confirmatory testing at scale. Decentralised molecular diag

Prevalence and determinants of metabolic abnormalities among adult tuberculosis patients at Gondar Town, Northwest Ethiopia: an institution-based comparative cross-sectional study.

BACKGROUND: Metabolic abnormalities, such as diabetes mellitus, hypertension, dyslipidemia, and central obesity, are significant contributors to the global burden of non-communicable diseases. Tuberculosis (TB) patients may be at increased risk of these conditions due to chronic inflammation, nutritional depletion, and anti-TB drug effects; however, locally grounded comparative evidence from resource-limited settings such as northwest Ethiopia remains scarce. This study aimed to assess the prevalence and determinants of metabolic abnormalities among TB patients in Gondar Town, Ethiopia. METHO

Nonoptimal Virological Response Among Adults Living With HIV After Initiation of Combined Antiretroviral Therapy at a Tertiary Hospital in Oman: A Retrospective Cohort Study, 1992-2024.

BACKGROUND: Combined antiretroviral therapy (cART) has transformed HIV infection from a life-threatening condition into a manageable chronic disease by reducing viral replication, preserving immune function, and lowering HIV-related morbidity and mortality. Nevertheless, achieving and maintaining viral suppression remains an important challenge. Evidence regarding factors associated with nonoptimal virological response among people living with HIV in Oman remains limited. AIM: This study investigated factors associated with nonoptimal virological response among adults living with HIV who init

The association between social capital and mental health among older people living with HIV: a systematic review.

BACKGROUND: Older people living with human immunodeficiency virus (HIV) constitute a vulnerable group in society and are at high risk of developing mental health problems. Although strengthening social capital is a common recommendation to reduce such mental health risks, no systematic review has examined the link between social capital and mental health among older people living with HIV. METHODS: We conducted a systematic review of peer-reviewed studies examining the association between social capital and mental health among adults aged ≥50 years living with HIV. Seven databases were search

Bedaquiline pharmacokinetics in drug-resistant tuberculosis: from exposure variability to individualized therapy.

Bedaquiline has transformed the treatment of multidrug- and rifampicin-resistant tuberculosis (MDR/RR-TB) and is now a core component of contemporary all-oral regimens. However, translating its distinctive pharmacokinetic properties into individualized clinical management remains challenging. Bedaquiline exhibits food-enhanced absorption, extensive plasma protein binding, broad but heterogeneous tissue distribution, cytochrome P450 3A4-mediated metabolism to the N-monodesmethyl metabolite M2, and prolonged persistence caused by tissue sequestration and slow redistribution. These features contr

Clinical and serological outcomes for successive syphilis episodes in people with HIV.

BACKGROUND: Syphilis incidence continues to rise globally among people with HIV (PWH), in whom repeated episodes are common and contribute to ongoing transmission. Whether prior infections modify clinical presentation, serological response, or treatment outcomes remains poorly understood. Partial immunity against Treponema pallidum has been proposed, but its clinical relevance across successive episodes has not been evaluated in a prospective cohort. The objective was to compare clinical presentation, time to serological response, serofast status, and seroreversion across successive syphilis e

CYP3A and CYP2D6 variability in antimalarial pharmacokinetics and pharmacogenetics across populations: a narrative review.

Malaria remains a global health burden, requiring optimised antimalarial therapies. This review examines the role of CYP450 3A family and 2D6 alleles in antimalarial drug metabolism, highlighting pharmacogenetics and distribution across populations. A narrative literature review was conducted using a structured literature search of PubMed, Web of Science, and Google Scholar to evaluate how CYP3A and CYP2D6 alleles affect the metabolism of quinine, artemether-lumefantrine, primaquine and tafenoquine. The review analysed genetic variations and interactions with other drugs. CYP3A alleles influen

Formulation-Specific Safety Issues of HIV Pre-exposure Prophylaxis Drugs: A Disproportionality Analysis of the FAERS Database.

BACKGROUND AND OBJECTIVE: As human immunodeficiency virus (HIV) pre-exposure prophylaxis (PrEP) use expands, real-world post-marketing safety evidence remains limited, particularly regarding potential formulation-specific adverse-event patterns across oral and long-acting agents. We aimed to characterise the post-marketing safety profiles of FDA-approved PrEP drugs using the FDA Adverse Event Reporting System (FAERS). METHODS: We conducted a disproportionality analysis using the FDA Adverse Event Reporting System (FAERS) data from 2012 to 2024. Individual Case Safety Reports (ICSRs) listing e

Caregiver burden and malnutrition in patients with HIV-associated lymphoma: indirect pathways via symptom distress and family closeness, and a test of resilience as a mediator.

BACKGROUND AND OBJECTIVE: Malnutrition is a frequent and clinically significant complication in patients with HIV-associated lymphoma, adversely affecting treatment tolerance, prognosis, and quality of life. Although disease-related and immunological determinants of malnutrition have been widely reported, the potential psychosocial pathways-particularly the role of caregiver burden-in patients with HIV-associated lymphoma remain insufficiently characterized. This study aimed to identify key determinants of malnutrition and to examine whether caregiver burden exerts indirect effects through sym

Vitamin D deficiency among Ethiopian adults: current evidence, determinants, and policy implications.

Vitamin D deficiency, commonly defined as a serum 25-hydroxyvitamin D [25(OH)D] level below 20 ng/mL (50 nmol/L), is a hidden but potentially high-impact public health problem in Ethiopian adults, despite the country's abundant year-round sunlight. Emerging studies indicate that certain populations, including pregnant women, patients with tuberculosis or HIV, and urban residents, experience high rates of deficiency, yet nationally representative data are virtually nonexistent. The lack of robust prevalence data, combined with limited laboratory capacity and low clinical awareness, has left thi

Digital health innovation in HIV prevention and sexual health promotion: current applications and future prospects of mobile health and telehealth.

Digital health is now a delivery strategy for HIV prevention and sexual health promotion, not simply a channel for health education. Mobile messaging, web-based testing, HIV self-testing (HIVST) support, mobile applications, social media recruitment, telehealth, tele-pre-exposure prophylaxis (PrEP), and chatbot-assisted workflows can reduce barriers related to stigma, distance, clinic capacity, and confidentiality. These tools also create an opportunity for primary HIV and sexually transmitted infection (STI) prevention before high-risk patterns are established, using positive, age-appropriate

Workplace HIV and TB Programmes for Laboratory Health Workers: A Rapid Review.

INTRODUCTION: Human immunodeficiency virus (HIV) and tuberculosis (TB) remain global health challenges. Health workers (HWs), particularly laboratory HWs who handle infectious samples, face increased occupational risks of infection. Laboratory HWs are essential for disease diagnosis and monitoring, yet their health is endangered by workplace exposures to HIV and TB, potentially leading to workforce depletion and compromised healthcare delivery. Despite existing guidelines and policies for access and implementation of effective occupational health services (OHS), including hazard identification

Incidence rates of non-Hodgkin lymphoma subtypes among Black Africans with and without HIV in South Africa: a national registry-based study.

BACKGROUND: The impact of HIV on incident non-Hodgkin lymphoma (NHL) varies by subtype. We compared the incidence rates of NHL subtypes (aggressive and indolent) between Black Africans with and without HIV in South Africa from 2010 to 2021. METHODS: We identified incident NHL among children and adults from the South African National Cancer Registry and stratified yearly population by HIV status, sex, age group and health care sector. We applied Poisson regression models, adjusting for sex, age, and calendar period, to estimate adjusted incidence rate ratios (aIRRs) for NHL and its subtypes am

Near point-of-care nucleic acid amplification tests (NPOC-NAATs) using respiratory specimens for detection of pulmonary tuberculosis.

RATIONALE: Near point-of-care nucleic acid amplification tests (NPOC-NAATs) are a new class of diagnostics that can be used for tuberculosis diagnosis. NPOC-NAATs are suitable for use in peripheral levels of healthcare systems as they are battery-operated instrument-based tests without special infrastructure requirements and can be performed by health care workers with basic technical skills. We evaluated the accuracy of NPOC-NAATs in diagnosing pulmonary tuberculosis using respiratory samples. OBJECTIVES: To assess the diagnostic accuracy of NPOC-NAATs for detecting pulmonary tuberculosis us
