Safety of Autologous Infusion of CER T Cells in Nonhuman Primates.

Cellular immunotherapies have shown great promise in the treatment of various diseases. Chimeric engulfment receptor (CER)-engineered T cells were developed to utilize the extracellular T-cell immunoglobulin and mucin domain-containing 4 (TIM-4) domain and intracellular engulfment signaling domains to target and enhance phagocytosis and T-cell cytotoxic function. These processes are activated through TIM-4 binding with phosphatidylserine (PS), a pro-phagocytic "eat me" signal. HIV infection has been shown to induce extracellular PS exposure shortly after infection, and engineered CER T cells h

Beyond cervicovaginal limitations: potential bidirectional interactions between HPV infection and mucosal microbiota from oral, anorectal and penile sites - a mini-review.

Most existing studies focusing on human papillomavirus (HPV)-microbiota interactions are confined to the cervicovaginal tract, while mucosal microecological crosstalk at oral, anorectal and penile sites remains insufficiently summarized. This mini-review provides a narrative synthesis of current sequencing and epidemiological evidence regarding potential bidirectional interactions between HPV infection and site-specific mucosal microbiota across extra-cervicovaginal niches in both males and females. Oral HPV reshapes microbial composition in a sex-, race- and population-dependent manner; perio

Viral triggers of Guillain-Barré syndrome: implications for laboratory diagnostics.

Guillain-Barré syndrome (GBS) is a leading cause of acquired acute neuromuscular paralysis and is an immune-mediated disorder of the peripheral nervous system (PNS). Accumulating evidence indi-cates that GBS is often preceded by infectious events that trigger autoimmune responses through mechanisms such as molecular mimicry and autoantibody production. Campylobacter jejuni remains the most common bacterial agent, but viral pathogens also contribute to the development of GBS. This review summarizes the current evidence regarding viruses as triggers of GBS, including herpes-viruses, hepatotropic

World Gastroenterology Organisation Practice Guidelines.

Significant progress on hepatitis B treatment pathways has been made since the last World Gastroenterology Organisation (WGO) guidelines in 2015. Global elimination targets have been set, prioritizing the prevention of new infections and optimizing screening and linkage to care in resource-limited settings. This has been led by the World Health Organization (WHO), which first published hepatitis B treatment and care guidelines in 2015 and updated its recommendations in 2024. The critical importance of preventing new infections was supported in 2020 by the WHO policy brief guidelines on antivir

Ultrasensitive single-genome sequencing reveals strong purifying selection in acute HIV- 1 infection.

UNLABELLED: HIV transmission from one individual to another occurs by one or a small number of virions followed by spread and genetic diversification into a complex quasispecies. To understand the early events in this process, we investigated how HIV-1 genomes diversify within the first two to three weeks after transmission by use of ultra-deep single subgenomic sequencing of over 10,000 plasma RNA genomes in each of a cohort of 15 individuals in acute infection. This approach confirmed transmission of one or a few transmitted/founder (TF) viral lineages and very limited early divergence from

Epigenetic regulation of HIV-1 transcription: insights into latency mechanisms and therapeutic strategies.

HIV-1 is one of the most serious pathogens currently threatening human health worldwide. Although antiretroviral therapy (ART) effectively suppresses HIV-1 replication, it fails to eliminate the latent reservoir of transcriptionally silent proviruses that persist in long-lived cells, making HIV-1 latency the major obstacle to achieving a functional cure for AIDS. The establishment and reversal of HIV-1 latency are largely regulated by epigenetic regulatory mechanisms involving the viral long terminal repeat (LTR) promoter. Following proviral integration into the host genome, the viral DNA is r

Effect of health-system continuous quality improvement on long-term retention in HIV care among pregnant and breastfeeding women in the Democratic Republic of Congo: A cluster-randomized trial.

BACKGROUND: Despite relatively high coverage of interventions to prevent vertical transmission, HIV incidence among children born to women living with HIV (WLH) remains above elimination targets, partly because of substantial disengagement from antiretroviral therapy (ART) during the postpartum period. We evaluated the effectiveness of a data-driven continuous quality improvement (CQI) intervention in improving retention in care, virological suppression, and prevention of vertical transmission among pregnant and breastfeeding WLH receiving ART in maternal and child health (MCH) clinics. The Potential Impact of Undisclosed Use of HIV Treatment and Prevention Antiretrovirals on Blood Safety.

The use of antiretroviral (ARV) medications for HIV treatment and prevention is a potential blood safety concern. HIV screening algorithms rely on the predictable appearance of viral RNA, p24 antigen, and anti-HIV antibodies during acute and early infection. However, exposure to ARVs can alter these biomarker dynamics and potentially compromise HIV detection. Evidence indicates that early initiation of antiretroviral therapy (ART) can suppress viremia, leading to incomplete seroconversion, reduced serologic reactivity, and seroreversion. HIV acquisition may also occur in individuals receiving

"We were caught sleeping": formulation of national policy to decentralise management of drug-resistant tuberculosis in South Africa - a qualitative policy analysis study.

OBJECTIVE: In response to growing numbers of people with drug-resistant tuberculosis (DR-TB), South Africa was, in 2011, among the first countries to launch a national policy on the decentralisation of DR-TB management, preceding normative guidance from the World Health Organization. The objective was to understand the process of formulation of the South African policy on DR-TB service decentralisation. DESIGN: Qualitative policy analysis. We reviewed policy-related documents to create a timeline, then conducted in-depth interviews with key individuals who had influence over or insights into

Optimal Timing for Surgical Intervention in Orbital Fracture Management: A Systematic Review and Meta-Analysis.

BACKGROUND: The optimal timing for surgical repair of orbital floor and combined orbital wall fractures remains a subject of clinical debate. While early surgery may prevent fibrosis, muscle entrapment, and enophthalmos, delayed intervention after edema resolution can sometimes yield comparable outcomes. This systematic review and meta-analysis aimed to evaluate the influence of surgical timing on functional and esthetic outcomes following orbital fracture repair. METHODS: The present systematic review followed PRISMA 2020 guidelines and was prospectively registered on PROSPERO (CRD4202450279

Evidence-Informed Provision of Doxycycline Postexposure Prophylaxis for Prevention of Bacterial Sexually Transmitted Infections.

Doxycycline postexposure prophylaxis (doxy-PEP) reduces the risk of bacterial sexually transmitted infections among men who have sex with men and transgender women. In the United States, doxy-PEP is in an early stage of integration into clinical practice, and national guidelines for its use were recently released. The goal of this manuscript is to provide practical guidance for clinicians who are considering or currently prescribing doxy-PEP. We address 5 clinical questions using post hoc analyses of data from the DoxyPEP randomized controlled trial and discuss the potential implications and l

Validating and leveraging non-SARS-CoV-2 respiratory infection as a negative control outcome in a phase 3 COVID-19 vaccine trial with extended observational follow-up.

Negative control outcomes (NCOs) are useful tools for hidden bias detection, but empirical evidence validating NCOs for COVID-19 is lacking. To address this gap, we examined the blinded phase of the randomized, placebo-controlled Coronavirus Vaccine Efficacy (COVE; NCT04470427) trial of the mRNA-1273 COVID-19 vaccine. We confirmed that acute respiratory illness with a positive test for a non-SARS-CoV-2 respiratory pathogen on a multiplex PCR panel was a valid NCO for COVID-19, considering that it was unaffected by vaccination (vaccine efficacy, VE = 3.3% (95% CI, -22.3 to 23.6)) yet strongly a

Functional roles, mechanistic insights, and therapeutic potential of the IGF2BP family in viral infections and virus-associated cancers.

The insulin-like growth factor 2 mRNA-binding protein (IGF2BP) family, comprises highly conserved RNA-binding proteins that play crucial roles in post-transcriptional gene regulation. Recent evidence suggests that IGF2BP proteins contribute to the pathogenesis and progression of multiple cancers and also participate in the regulation of viral infections and virus-associated tumors. This review provides a systematic synthesis of the biological functions of the IGF2BP family and its regulatory roles in viral life cycles. Particular emphasis is placed on oncogenic viruses, including hepatitis B a

The prognosis characteristics of HIV/HBV co-infection after antiviral therapy: a comprehensive review.

Co-infection of HIV and HBV is still a major global public health concern. Co-infected people have a nearly threefold increased risk of end-stage liver disease, with a noticeably faster development to cirrhosis and hepatocellular carcinoma than those with HIV mono-infection. Based on the information that is now available, antiretroviral regimens that combine lamivudine or emtricitabine with tenofovir (TDF or TAF) can effectively block both viruses, maintain or even partially reverse liver fibrosis, and improve immunological reconstitution. However, HBsAg decrease indicates that a functional cu

Portable mass spectrometry systems for point-of-care testing: technologies, applications, and clinical implementation.

Portable mass spectrometry systems have shown great potential in point-of-care testing and field-based diagnostics, as they fill important gaps in decentralized clinical analysis. Conventional centralized laboratory testing may delay clinical decision making due to transport and analysis times. For conditions where transportation is limited, these delays disproportionately affect resource-poor settings. Advances in technology allow the design and assembly of lightweight, portable MS systems. Fundamental issues such as size reduction, power consumption, and vacuum requirements have been address

An immunofluorescence microscopy assay to discriminate distinct expression patterns of HIV-1 Gag and Nef proteins in HIV-1 provirus-harboring cells.

Over 95% of HIV-1 proviruses are defective and were once considered clinically irrelevant. However, growing evidence shows that these defective proviruses can still be transcribed and translated into viral proteins. Here, we developed an improved immunofluorescence protocol that combines two anti-Nef antibodies with one anti-Gag antibody, along with membrane and nuclear staining, enabling direct visualization of protein expression and localization. This method allows detailed characterization of the expression patterns and subcellular distribution of Gag and Nef proteins derived from defective
